The effect of the improvement of nursing productivity in Hospital Information System;A Case study on Kwangju Patriots' and Veterans' Hospital (병원정보시스템내의 간호생산성향상효과에 관한 연구)

  • The purpose of this study is to suggest successful strategies through which the effect of the information system of a hospital can be forecasted at the nursing department. In order to set up successful strategies, in the first place, both the methods of CSF(Critical Success Factor: Rockart, 1979) and ULD(User-Led Development) method and the method suggested by the Korea Productivity Center were applied. In order to measure the improvement of nursing productivity, the Dissonance theory was used. The data were collected from 100 employees serving at the clinic department of Kwangju Patriots' and Veterans' Hospital from July 4 to July 25, 1998 with reference to all 222 cases, for sampling work; then the part of the efficiency of the treatment or management of hospital business - simplification of the process of the treatment of hospital business and reduction of the time of the treatment of hospital business were measured; and in order to forecast organizational behavior, 100 cases of organization behavior were analysed, based on the well structured, questionnaires. In order to forecast the user's organizational behavior, a tool(Ronald. 1988; Stephen, 1982: Senn, 1992: Olsen, 1980: Anderson, 1988: Kim. 1992: Cho. 1994) to measure the extent or degree of the user's recognition or understanding whose reliability coefficient is 0.63 was used: and regarding the items expected by the users concerning the convenience of the system, a tool created by Bernadett, Szajna and Richard W. Scamell(1993) whose reliability coefficient is 0.88 was used. And finally, those data were analysed, utilizing the statistical package of SPSS/PC 6.0. successful strategies are suggested as follows: 1. In order that the Kwangju Patriots' and Veterans' Hospital's purpose can be successful through its strategic, information system, the quality of its services should be elevated. and for elevating the quality of medical services, elevation of the quality of medical expertism or specialty is an important factor in determining such quality. 2. In order to make the hospital information system to be successful, the hospital's top manager should participate in the effort making it successful with helping hands of the members or personnel of the hospital. 3. In order to make users participate in the hospital information system, it is prerequisite that all nurses in a hospital should voluntarily participate in the system 4. In order to reduce the expense, the time in coping with business per duty should be reduced by 10${\sim}$33.23%. The time of the direct nursing care which added value is relatively high should be elongated in order to elevate the quality of hospital services. 5. Since the introduction and spread of the hospital information system are influenced by the duration in the experience of computer use, the user of the hospital information system should have a plan to receive well-planned computer education. Finally it is suggested that the forecast of long-term productivity through a review of the user's expectation of the system should be inspected and tested through continuous studies of its effectiveness.

The Financial Performance of Hospitals Belonging to Multi-hospital System : A Comparative Study (네트워크 시스템 병원의 경영성과 : 비교 연구)

  • The purpose of this study is to analyze the performance difference between multi-hospitals and free-standing hospitals. Scholars in industrial economics argue that, due to economies of scale and integration, multi-hospital system may have a better performance compared to freestanding hospitals. The study overturned the hypothesis based on a theory. By analyzing 425 acute-care hospitals in Korea, this research shows that multi-hospital systems and market factors, which have been perceived to be strengths to hospitals, are negatively related to hospitals' financial performance. Specifically, the results showed a better performance of freestanding hospitals compared to multi-hospital systems. Higher labor and administrative cost by multi-hospital system may be the reason for the difference, and it means they are not more effective at cost control. Managers in multi-hospital system, therefore, should pay attention on cost-reducing issues to regain managerial efficiency of organizations.

A Study on the Hospital Information System in Korea (우리나라 병원정보시스템 실태에 관한 연구)

  • As the hospital environment is changing rapidly, management is therefore obligated to use new ways to provide better service to patients. For example information system is introduced to some hospitals in Korea. Information system has several benefits such a soperational efficiency by on-line delivery and processing of data, accurate and rapid information production, continuous monitoring of performance and feedback, improvement of work process and better service quality. Still, many hospitals cannot be sure that information system is useful for improving organization performance. Because only one or two area of hospital information system were considered for development and with limited development hospital information system cannot be used effectively. The purpose of this study is to suggest the model of information system utilizaton and to analyze the hospital in formation system in Korea. The results are following; 1. In Korea, proportion of operating system of comuter is 31% of UNIX, 11% of IBM OS and 10% of PC LAN. 2. As we expect, scope of application software and amounts of heardware and software invest ments are affected by concern of CEO. 3. Many CIO(chief information officer) say that the biggest problems are lack of after-service and obsolescence of computer equipments in terms of hardware and shortage of application software for hospital in terms of software. 4. Personnel of information system department is so small that hospital information system can't be improved. 5. During the development of information system, full-time participation of end user is only 20% hospital with almost less than 12 person-month. This study was accomplished by survey through mail questionnaires. Response to the survey was only 55% and it was hard to generalize all the result obtained from this survey. However, We hope that this study would be helpful for helth care organization to acknowlege hospital information system in korea and to design the future architecture and frame of information system.

Utilization of Computer System for Outpatient's Dispensing Affairs in Hospital Pharmacy (병원약국의 외래조제업무에 대한 컴퓨터의 이용)

  • Hospital pharmacy services are divided into dispensing affairs for inpatients and outpatients, pharmaceutical service, stock control, intravenous admixture service, drug information service, pharmacokinetic consultation service, education and research work, etc. But among those affairs, dispensing affair for outpatient is perceived as the most important work in Korea, because it is linked directly with hospital service for patients. Therefore, total computer system for dispensing area was adopted from opening point of hospital in 1989 in Asan Medical Center. Utilization of computer system for outpatient dispensing area is as follows; 1) Order communication system of prescription by Total Hospital Information System, 2) Automatic print-out system of direction for use by sticker connected with on-line net work, 3) Use of automatic tablet counting and packaging machines connected with on-line net work. Those computer system resulted in curtailment of pharmacy manpower and shortening of waiting-time for outpatient.

  • The purpose of this thesis is to lay groundwork for the development of emergency care system in metropolitan area. It compares the performance and outcome of the current system with foreign counterparts and investigates the changing aspects of future medical environment. Emergency medical system can be divided into two parts of both pre-hospital care, which refers to the emergency measures taken before arriving at a hospital, and hospital care that is given within a hospital. Pre-hospital care includes on-the-spot expedients, information system and delivery system, whereas hospital care is related to the classification and specialization of medical care facilities. This research focuses on the evaluation of the performance of a rescue party, which is part of pre-historical care system. As a result, it provides valuable material for the development of the emergency medical system in Seoul.

  • The competition between hospitals has become an issue for many hospitals because of the sudden increased number of health providers. As the marketing or customer management have become important factors as means of competition, a number of hospital have been considered or already adopted the coordination system. A system dynamics model is developed to see the effect of the hospital coordination system. This paper after briefly explaining the structure of the system dynamics model, discusses the roles of the hospital coordination system with examples. The system dynamics model quantifies through simulations the process of effects of coordination system on the number of customers, productivities of employees (nurses and doctors), and finally the financial status. As a conclusions of the simulations and their analysis, it is concluded that coordination system could be more effective than the investment to the facilities. Since the study is based on only one hospital case, it is limited. However, it explains the mechanism of the coordination system quantitatively as well as qualitatively.

  • In this study, using an experiment, in certified an error between each system of SUV (standardized uptake value) that is one of the main analyses to diagnose a tumor in PET/CT, so, it would assure reliability and help to diagnose any lesions accurately. That is, a detailed analysis progressed. as all images reconstructed every setting time, then, clinical reliability between each system was expressed numerically at MBq/mL and SUV. Therefore, this study is considered that flexibility of follow-up using diverse system was insured, and it helps to offer accurate and beneficial information for diagnosis of various fields.

  • This study aims to define concepts on Hospital Design Focused on System to respond to the development and change by comparing general design methods of the General hospital architecture in Korea and analyze elements suitable to concepts on Hospital Design Focused on System in the General hospital architecture through examining the transformation of General hospitals in Korea. Essential architectural elements composing the Hospital Design Focused on System are as follows. First of all, the elements which can react to development of hospital architecture are HOSPITAL STREET and site situation. Secondly, core, equipment shaft, column, MAIN STREET and air handling unit room are elements of great importance as the location of these elements determines the dimensions and scale of the space which hospital functions are assigned to. Third, the area in regard to the rate of change is formed by MAIN STREET, which is the primary element, and envelope. The depth between MAIN STREET and envelope is defined as space depth. The flexible area is determined depending on this space depth and thus how to set up this flexible area determines the degree of readiness in responding to the change.

  • Purpose: Space planning to cope with the changing function of the hospital is essential in hospital architecture. In order to do so, it is vital that the paradigm shifts from hospital design focused on purpose toward hospital design focused on system. Not only space planning but also air-conditioning plan, which is most closely related to the operation and maintenance of hospital facilities; and the environment of hospital users, should be able to respond to changes with ease. Thus this study is to provide fundamental data of the air-conditioning plan for the hospital design focused on system by analyzing the concept and characteristics of the air-conditioning plan in the recent hospital architecture planning. Results: As a result of this study, in the air conditioning plan for the hospital architecture planning, the most important are the location relation among the departments the air handling unit room and the air handling unit manage and the air conditioning zoning setting according to the air conditioning system. Therefore, for the hospital architecture planning focused on system, it is necessary to establish the air conditioning setting that can accommodate changeable environment of departments and accordingly the appropriate area range of the air conditioning zoning and the plan for the location of the air handling unit should be considered. Implications: Thereby aims to provide fundamental data on air handling unit zoning planning in the hospital architecture planning.

The Effects of an Advanced Cardiac Life Support Training via Smartphone's Simulation Application on Nurses' Knowledge and Learning Satisfaction (스마트폰 어플리케이션을 활용한 전문심폐소생술 시뮬레이션 재학습이 간호사의 지식 및 교육 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to examine how an Advanced Cardiac Life Support (ACLS) simulation application via smartphone effects nurses' ACLS knowledge and learning satisfaction. Methods: The participants were selected from nurses in medical Intensive Care Unit (ICU), surgical ICU and emergency room. The experimental group consists of fifty nurses who were self-learned with ACLS simulation application via smartphone and the control group of seventy-one nurses who used traditional learning materials. Outcome variables included nurses' knowledge and learning satisfaction which were collected before and after the intervention. Results: The scores of ACLS knowledge were higher in the control group compared to the experimental group (p=.001) while learning satisfaction showed no statistical difference (p=.444). In learning satisfaction, the experimental group showed higher interest than the control group (p=.019) while the control group rated higher on the item, 'the contents of education was reliable' (p=.007). Conclusion: ACLS knowledge score was graded higher in control group that used traditional learning method than the experimental group that used the smartphone application. This study showed that training with the new material significantly increased nurses' interest in ACLS education. Hence, more applications for smartphones should be developed to provide self-learning environment for nurses and improve care quality.
