A Study on the OPD Planning by the Introduction of Hospital Information System in General Hospital (병원정보시스템 도입에 따른 종합병원 외래진료부의 건축계획에 관한 연구)

  • In order to improve their hospital services in unlimited competitive situations, the general hospitals, particularly, have since the 1990s been concentrating more on hospital information processing systems and the automation of the patients' convenience. They also give priority to the advance of integrated hospital information systems centred around the patients. Introducing more progressive information technology to already established hospitals, the OPD has been most affected by HIS because there are a lot of patients and doctors. On this background, this study analyse the change of OPD and some related departments around the 1990s with the general hospitals as it's object. It investigates the way of building hospitals and presents some information about the planning of building hospitals.

  • The patient's waiting time and length of stay have been reported as a factor decreasing their satisfaction in the hospital, especially in developing countries. This paper focuses on modeling hospital's outpatient department workflow in a developing country and optimizing the patient waiting time as well as total length of stay. By using discrete-event simulation, many alternative scenarios have raised, such as adding more working time, altering human resources, and adjusting the staff's responsibility, those scenarios will be examined to explore better settings for the hospital. The results show that possible to achieve a 9.6% reduction in patient total length of stay and it could be accomplished without adding more resources to the hospital.

  • Purpose: Optical imaging is providing great advance and improvement in genetic and molecular imaging of animals and humans. Optical imaging system consists of optical imaging devices, which carry out major function for monitoring, tracing, and imaging in most of molecular in-vivo researches. In bio-luminescent imaging, small animals containing luciferase gene locally irradiate light, and emitted photons transmitted through skin of the small animals are imaged by using a high sensitive charged coupled device (CCD) camera. In this paper, we introduced optical imaging system for the image acquisition of bio-luminescent signals emitted from small animals. Materials and Methods: In the system, Nikon lens and four LED light sources were mounted at the inside of a dark box. A cooled CCD camera equipped with a control module was used. Results: We tested the performance of the optical imaging system using effendorf tube and light emitting bacteria which injected intravenously into CT26 tumor bearing nude mouse. The performance of implemented optical imaging system for bio-luminescence imaging was demonstrated and the feasibility of the system in small animal imaging application was proved. Conclusion: We anticipate this system could be a useful tool for the molecular imaging of small animals adaptable for various experimental conditions in future.

  • The productivity of a hospital foodservice system has a significant implication in hospital management as costs for labor and material increase, competitions among hospitals increase, and patients' expectations as to the quality of hospital services increase. The foodservice is characterized by its labor intensiveness. The objective of this study was to examine associations between operational and managerial factors and the productivity of dishwashing work in hospital foodservice systems. The labor productivity in 20 conventional food service systems was assessed and related to a number of influencing variables within the system. The productivity measurement was based upon the total dish equivalents as a ratio of the total direct and non-direct labor hours required to wash these dishes. 20 hospitals with more than 500 beds located in Seoul were surveyed to obtain data for study variables. Questionnaire and a survey form were mailed. Statistical methods used in this study were descriptive analysis and Pearson product moment correlation analysis. Hospital system characteristic which was found to correlate significantly with productivity was the ratio of dish loss. As this increased, the productivity level increased.

  • Objectives: This study evaluates the effect of Korean medicine in a patient with multiple system atrophy-C (MSA-C). Methods: A patient diagnosed with MSA-C was treated with herbal medicine (Boyangwhano-tang hap Yeokgan-san), acupuncture, and moxibustion for 8 weeks. Clinical improvements were evaluated by the unified MSA rating scale (UMSARS) and the scale for the assessment and rating of ataxia (SARA). Results: Improvements in the total scores of UMSARS Part I, II, and IV and SARA were observed after Korean medicine treatments. However, there were no improvements to the urinary and autonomic dysfunctions. Conclusions: Korean medicine treatment may be an effective treatment for a degenerative, progressive disease like MSA-C.

  • This is a clinical report of an aspect Soyangyangmyunghabbyung patient with acute pyelonephritis. One patient who had headache, fever, chilling sign, nausea and constipation was treat. The symptoms have relation to Soyangyangmyunghabbyung(少陽陽明合病) of Yugkyeung(六經) thee. On the basis of this thee, we performed the treatment on acute pyelonephritis. We treated her with Herbal medicine(Daeshihotanggami, Bojungikgitanggami) and acupuncture(Ganjeonggyeok). As the result of our treatment, we were experiencef an improved case of acute pyelonephritis patient.

  • Formalin is a water-soluble, colorless, pungent, irritating and highly reactive gas. A 40% solution of formaldehyde in water, also known as formalin, is used as a disinfectant, antiseptic, deodorant, tissue fixative and embalming fluid. Ingestion can lead to immediate deleterious effects on almost all systems of the body including gastrointestinal tract, central nervous system, cardiovsacular system and hepato-renal system, causing gastrointestinal hemorrhage, cardiovsacular collapse, unconsciousness or convulsions, severe metabolic acidosis and acute respiratory distress syndrome. We treated a 39-year-old woman who ingested 300 ml formalin in a suicidal attempt. Despite hemodialysis, death occurred after 23 h.

Treatment Using the SpyGlass Digital System in a Patient with Hepatolithiasis after a Whipple Procedure

  • An 89-year-old man was referred to our hospital for treatment of hepatolithiasis causing recurrent cholangitis. He had undergone a prior Whipple procedure. Computed tomography demonstrated left-sided hepatolithiasis. First, we conducted peroral direct cholangioscopy (PDCS) using an ultraslim endoscope. Although PDCS was successfully conducted, it was unsuccessful in removing all the stones. The stones located in the B2 segment were difficult to remove because the endoscope could not be inserted deeply into this segment due to the small size of the intrahepatic bile duct. Next, we substituted the endoscope with an upper gastrointestinal endoscope. After positioning the endoscope, the SpyGlass digital system (SPY-DS) was successfully inserted deep into the B2 segment. Upon visualizing the residual stones, we conducted SPY-DS-guided electrohydraulic lithotripsy. The stones were disintegrated and completely removed. In cases of PDCS failure, a treatment strategy using the SPY-DS can be considered for patients with hepatolithiasis after a Whipple procedure.

  • Purpose : Emergency medical services in China are increase in demand by people and under the greater pressure than ever before. So it is, necessary to advance the pre-hospital system in order to promote the development of emergency medical services. Methods : This is based on China-related articles, books, journals, reports, statistical data and other literature. Results : First, pre-hospital emergency medical care with the introduction of specialist training program should be established. Second, to strengthen pre-hospital emergency services and to develop the EMS guidelines. Third, the "120" reporting systems unification and awareness activation. Fourth, the preparation of the EMS facilities equipment system. Fifth, the rapid transport system establishment to the selected medical institutions. Conclusion : It is necessary to strengthen the emergency medical personnel at the scene, rapid transport, rapid patient triage and to improve the survival rate of the patients.

  • This study purposes to examine the relationship between high performance-HRM system of medical doctor and the effectiveness of hospital. The high performance-HRM practices were derived from reviewing the literature of SHRM(Strategic Human Resource Management) and investigating some cases of Korean leading hospitals. The result reveals that the high performance-HRM system of medical doctor is significantly related with subject measurement such as financial performance, employees' turnover, and customer(patient) satisfaction. Moreover, it is positively related to objective performance such as hospital's profit growth, yearly patient growth. Based on the result, the academic and practical implications are suggested and then the limitation and further research directions are discussed.
