The Effects of a Shift Improvement Pilot Project on Nurses' Shift Adjustment, Work-Life Balance, and Turnover Intention: A Mixed-Methods Study (교대제 개선 시범사업이 간호사의 교대근무 적응, 일과 삶의 균형 및 이직의도에 미치는 효과: 혼합연구방법)

  • Purpose: This study aimed to investigate the impact of a nurse shift improvement pilot project on nurses' shift adjustment, work-life balance, and turnover intention, as well as nurses' experiences of participating in the pilot project. Methods: An explanatory sequential mixed-methods study was performed. Survey data was collected from a total of 1,051 nurses both in participant and non-participant groups. For the qualitative study, focus group interviews were conducted with 22 nurses who participated in the shift improvement pilot, including 12 staff nurses, 7 nurse managers, and 3 substitute nurses. Results: There were significant positive effects on the key variables of nurses' shift adjustment, work-life balance, and turnover intention in the group that participated in the nurse shift improvement pilot project. Based on the qualitative study, participants in the shift improvement pilot project indicated that it enabled nurses to recover their circadian rhythm, and achieve a better work-life balance, and they wanted to continue working as a nurse, and hoped expanded application of the pilot project. Conclusion: In order for expanded implementation of the pilot project and continued improvement in the shift system, we recommend establishing compensation standard, systematizing the use of substitute nurses, developing guidelines for shift schedules, and expanding the shift improvement pilot project to other departments such as the emergency room.

  • Among the countless elements for healing environment in healthcare facility, 'privacy' has been rather neglected in general hospital designs in Korea. The fact may be due either to present situation of excessively crowded patients or to cultural origin which more stresses on the mass than on individuals. However, as patient amenity has increasingly become important, privacy should be viewed as one of the key elements in the general hospital design, in order to improve the quality of patient service as well as to compete with other hospitals. Also it is very important that those methods for privacy improvement should be not only theoretically correct but also practical enough to be compatible with medical efficiency, space availability or construction cost within a particular hospital. The purpose of this case study is to present an actual examples to improve patients' privacy adopted in the schematic design of the Seoul Asan Hospital renovation project, along with comparisons with other hospital cases.

Post Occupancy Evaluation of the Central Hospital in Quang Nam Province of Vietnam Funded by Korean ODA (ODA 지원 베트남 중부 꽝남성 종합병원의 거주후 평가)

  • Purpose: This is the ex-post evaluation on "the Central General Hospital in Quang Nam Province in Vietnam" funded by Korean ODA. Through this evaluation, this study tries to explore useful ways to improve the effectiveness of Korean ODA projects related to hospital establishment. Methods: Field survey and analysis of questionaire to the staffs and patients of Quang Nam Province General Hospital were conducted twice in 2012 and 2014 after the completion of the Hospital. Two sets of surveys and questionaire outcomes have been comparatively analysed by statistics in order to seek the trend of satisfaction level of hospital users and find out the improvement way in hospital design and construction. Results: The satisfaction level on the hospital was relatively high and that of 2014 is higher than that of 2012. The satisfaction level of the users was recorded in Likert 5 scales. Continuous facility complement and maintenance is considered as the main contributor in increasing the satisfaction level of users. As far as the satisfaction level of outpatient is concerned, that of 2012 was recorded as 3.65 and that of 2014 was 4.05. In case of inpatient, the satisfaction level on the hospital in 2012 was 3.76, and 4.06 in 2014. Implications: In overseas hospital projects, proper hospital project management system including medical service plan, strategic plan, brief, design & construction and maintenance have to be put together effectively and efficiently for the success of the hospital project. At the same time local climate, culture, building material and local code have to be considered in order to build sustainable hospital.

  • The recent project has been progressed from focusing on the establishment of having an international sense and making a symbolic building in Cheju Island. Its purpose is to provide the best medical services to the inhabitants, and to reflect the local feelings and the entire design concept based upon the role of the health promotion. This study is to explain the process of a traditional architecture method of Cheju Island applied to the modern design element and re-composition into the layout plan, the composition diagram of the exterior, the elevations, etc. of the drawings of the project.

Characterization of Invading Glioma Cells Using Molecular Analysis of Leading-Edge Tissue

  • Objective : We have introduced a method of characterization of invading glioma cells by using molecular analysis of marginal invading tumor cells and molecular profiles of glioma tumor margin. Methods : Each of tumor core and marginal tissues was obtained in 22 glioma patients. Tumor core cells and marginal cells from each glial tumor were collected by laser capture microdissection or intraoperative microdissection under the operating microscope. Expression of MMP-2, MMP-9, CD44 and RHAMM mRNA by invading glioma cells compared with tumor core was confirmed by realtime-PCR of twenty-four glioma specimens. Clinical data also were reviewed for invasion and recurrence pattern of the gliomas radiologically and invasive rim pattern microscopically. Results : Overall results of the molecular analysis showed that relative overexpression of MMP-2, MMP-9 and RHAMM were noted at the invasive edge of human glioma specimens comparing to the tumor core but CD44 was highly expressed in the tumor core comparing to the margin. High marginal expression of MMP-2 and MMP-9 were noted in poorly ill-defined margin on the pathological finding. High marginal expression of CD44 and MMP-2 were demonstrated in the midline cross group on the radiological review, and that of RHAMM and MMP-2 were showed in the aggressive recurrence group. High expression of MMP-2 seems to be involved in the various invasion-related phenomenons. Conclusion : Up-regulation of MMP-2, MMP-9, CD44 and RHAMM was noted in invasive edge of gliomas according to the various clinical situations.

  • I. Objective and Importance of the Project We have been using MC-50 cyclotron and NT-50 neutron therapy machine for treating cancer patients since 1986 at Korea Cancer Center Hospital. It is mandatory to measure accurately the dose distribution and the total absorbed dose of fast neutron for putting it to the clinical use. At present the methods of measurement of fast neutron are proposed largely by American Associations of Physicists in Medicine (Task Group 18), European Clinical Neutron Dosimetry Group, and International Commission on Radiation Units and Measurements. The complexity of measurement, however, induce the methodological differences between them. In our study, therefore, we tried to establish a unique technique of measurement by means of measuring the emitted doses and the dose distribution of fast neutron beam from neutron therapy machine, and to invent a standard method of measurement adequate to our situation. II. Scope and Contents of the Project For establishing a unique technique of measurement and inventing a standard method of measurement of fast neutron beam, 1. to grasp the physical characteristics of neutron therapy machine 2. to study the principles for measrement of fast neutron beam 3. to get the dose distribution (dose rate, percent-depth dose, flatness etc) throught the actual measurement 4. to compare our data with those being cited world-widely.

Job Experiences of Nurses Participating in Pilot Project of Fixed Night Shift Nursing by a Tertiary Hospital (상급종합병원의 야간전담 간호제도 시범사업에 참여하는 간호사의 직무 경험)

  • Purpose: This study aimed to explore the meaning of job experiences of fixed night shift nurses participating in a pilot project by a tertiary hospital, and to provide basic information for improving their night shift working environment. Methods: A descriptive qualitative study was conducted using individual interviews in September 2022. Three female nurses who worked fixed night shifts were recruited from a tertiary hospital. Data were analyzed using content analysis. Results: By analyzing the meaning of job experience as a fixed night shift nurse, four categories and nine subcategories were identified. The four categories of job experience of fixed night shift nurses were: expecting an improvement physical health due to predictable working hours, enjoying leisure time due to long off-duty hours, regretting for loosened relationships, and considering a successful settlement of fixed night shift nursing. Conclusion: As night shift work is essential for nurses to provide continuous nursing to patients, it is necessary to develop various strategies to improve the physical health of night shift nurses through fatigue management and good sleep quality, mental health through work-life harmony, and social health through intimate relationships between patients and coworkers.

  • The construction management has been applied for the first time in South Korea on the New Catholic Medical Center. The construction of the project has been controlled and managed by the CMr, as Client's agent from the very beginning stage of construction when the block plan is being established by the Contractor's design consultant. As CMr generally gets involved in the project throughout the entire construction phases on behalf of the Client, Parsons Brinckerhoff has been delivering high quality solutions that satisfy the Client's requirements in Pre-construction stage and also brought practical and innovative engineering solutions in a time and cost effective method. The objective of this case study is to describe works executed by CMr in Pre-construction stage ; and, analyze the positive effects by applying CM to the project. Therefore, it is anticipated that CM procedure might be introduced and adapted to the up-coming hospital construction projects.

  • To provide high-quality training to residents in a rapidly changing medical environment, it is very important to improve the annual training curriculum centered on competency and ensure that training hospitals maintain an environment suitable for training. The Korean Society of Radiology (KSR) has been steadily improving the training system and has suggested the improvement of the training system by strengthening the competency-based evaluation and faculty development. Currently, KSR was selected for the second annual training curriculum systematization construction project in July 2021, and developed entrustable professional activities, core competencies, and assessment guidelines required by the construction project. Therefore, the development process and assessment guidelines will be introduced to residents and the faculty.
