병원간호사의 재직의도에 영향을 미치는 요인: 진정성 리더십과 간호조직문화를 중심으로 (Factors Affecting the Intention of Hospital Nurses to Stay at Work: In Relation to Authentic Leadership and Nursing Organizational Culture)

  • Purpose: The purpose of this study was to examine the relationships of authentic leadership and nursing organizational culture to the intention of hospital nurses to stay in their current position. Methods: The participants of this study were 503 nurses in 8 hospitals. We collected data using questionnaires for assessing authentic leadership, nursing organization culture and intention to stay. For data analysis, t-test, ANOVA, Cronbach's ${\alpha}$, Pearson's correlation coefficient, Tukey test, Multiple regression were performed using SAS ver.9.4 (SAS Institute Inc., Cary, NC, USA). Results: Authentic leadership (${\beta}=.12$, p=.008) and relation-oriented culture (${\beta}=.13$, p=.009) affected the intention of the hospital nurses to stay. These variables accounted for 29% of the variance in the intention to stay among hospital nurses. Conclusion: The finding of this study shows that the authentic leadership and nursing organizational culture especially relation-focused can influence nurses' intention to stay in their current position. For retaining nurses, it is suggested to use an authentic leadership training program for nursing leaders and to make efforts to establish a relation-focused culture in the hospital.

  • 본 연구는 의료기관 동일조직 내에서 인적자원관리 개별 활동들을 묶음(bundles) 형태로 보는 포괄적 접근법으로 병원조직의 인적자원관리 효율화를 위한 관리방안을 파악해 보고자 하였다. 인적자원관리 개별제도의 합인 인적자원관리의 포괄적 접근법에 의한 평균을 기준으로 평균이상의 집단과 평균미만의 집단으로 구분하여 인적자원관리 개별제도를 이용하여 집단분류 한 결과, 소속집단 판별에 이용되는 인적자원관리 개별제도는 직종에 따라 다소 차이가 있었다. 그러나 인적자원관리 개별제도 중 '경력개발기회 제공 및 경영정보 공개' 제도는 모든 직종에서 유의하게 판별함수에 포함되는 변수이었다.

Allopregnanolone suppresses mechanical allodynia and internalization of neurokinin-1 receptors at the spinal dorsal horn in a rat postoperative pain model

  • Background: To identify a new strategy for postoperative pain management, we investigated the analgesic effects of allopregnanolone (Allo) in an incisional pain model, and also assessed its effects on the activities of the primary afferent fibers at the dorsal horn. Methods: In experiment 1, 45 rats were assigned to Control, Allo small-dose (0.16 mg/kg), and Allo large-dose (1.6 mg/kg) groups (n = 15 in each). The weight bearing and mechanical withdrawal thresholds of the hind limb were measured before and at 2, 24, 48, and 168 h after Brennan's surgery. In experiment 2, 16 rats were assigned to Control and Allo (0.16 mg/kg) groups (n = 8 in each). The degree of spontaneous pain was measured using the grimace scale after the surgery. Activities of the primary afferent fibers in the spinal cord (L6) were evaluated using immunohistochemical staining. Results: In experiment 1, the withdrawal threshold of the Allo small-dose group was significantly higher than that of the Control group at 2 h after surgery. Intergroup differences in weight bearing were not significant. In experiment 2, intergroup differences in the grimace scale scores were not significant. Substance P release in the Allo (0.16 mg/kg) group was significantly lower than that in the Control group. Conclusions: Systemic administration of Allo inhibited mechanical allodynia and activities of the primary afferent fibers at the dorsal horn in a rat postoperative pain model. Allo was proposed as a candidate for postoperative pain management.

상급 종합병원 간호사의 근거기반실무 준비도에 대한 영향요인 (Factors Influencing Evidence-Based Practice Readiness for Tertiary General Hospital Nurses)

  • 본 연구는 상급종합병원간호사의 비판적사고, 간호조직문화 유형, 근거기반실무 준비도를 파악하고, 근거기반실무 준비도(지식/기술, 태도, 조직의 지원)에 영향을 미치는 요인을 규명하기 위한 서술적 조사연구이다. 자료는 2013년 3월 21일부터 3월 28일까지 2개 상급종합병원에서 근무하는 간호사 160명을 대상으로 조사하였다. 수집된 자료는 t-test, ANOVA, 상관분석, 회귀분석으로 검정하였으며, 모든 검정의 유의수준은 p<.05이었다. 근거기반실무지식/기술의 영향요인은 비판적사고이었으며 34.7% 예측 설명하였고, 근거기반실무 태도의 영향요인은 근거기반실무지식/기술, 외과병동이었으며 14.2% 예측 설명하였다. 근거기반실무를 위한 조직의 지원에 대한 영향요인은 혁신지향문화, 관계지향문화이었으며 24.6% 예측 설명하였다. 따라서 근거기반실무 도입과 활성화를 위해서 간호부 차원에서의 비판적 사고와 근거기반실무 지식/기술을 높이기 위한 교육프로그램 개발 및 적용과 더불어 간호조직의 문화를 혁신지향문화로 조성하기 위한 체계적 지원구축이 요구된다.

간호조직문화가 신규간호사의 회복탄력성에 미치는 영향 (Influence of Nursing Organization Culture on Resilience in New Nurses)

  • Purpose: The purpose of this study was to identify the influence of nursing organization culture on resilience of new nurses. Methods: Participants were 1,033 new nurses with careers of less than 12 months and who were from 43 general hospitals located in Seoul, Incheon, other metropolitan cities and provinces in Korea. The data were analyzed using t-test, ANOVA, Pearson correlation coefficient and multiple regression with SPSS/WIN 24.0 program. Results: Relation-oriented culture was scored highest for perception of organizational culture by new nurses (3.35), followed by hierarchy-oriented culture (3.19), innovation-oriented culture (3.12), and task-oriented culture (2.73). The score for resilience of the new nurses was 3.49, and the first subcategories were interpersonal relationship ability 3.66, self-positivity 3.45, and self-control ability 3.36. Relation-oriented culture (${\beta}=.30$, p<.001) was the most significant factor influencing resilience of new nurses, followed by innovation-oriented culture (${\beta}=.14$, p=.001), hierarchy-oriented culture (${\beta}=.12$, p<.001), task-oriented culture (${\beta}=-.10$, p=.003) in that order ($R^2(adj. R^2)=.16$, F=47.04, p<.001). Conclusion: Findings show that for improved resilience in new nurses it is necessary to develop relation-oriented culture to provide a cooperative work environment(friendly interpersonal relation and dialogue) for new nurses and to encourage involvement of these members in the hospital culture.

Stereomicroscopic on-site evaluation in endoscopic ultrasound-guided tissue acquisition of upper gastrointestinal subepithelial lesions

  • Background/Aims: In stereomicroscopic sample isolation processing, the cutoff value (≥4 mm) of stereomicroscopically visible white cores indicates high diagnostic sensitivity. We aimed to evaluate endoscopic ultrasound-guided tissue acquisition (EUS-TA) using a simplified stereomicroscopic on-site evaluation of upper gastrointestinal subepithelial lesions (SELs). Methods: In this multicenter prospective trial, we performed EUS-TA using a 22-gauge Franseen needle in 34 participants with SELs derived from the upper gastrointestinal muscularis propria, requiring pathological diagnosis. The presence of stereomicroscopically visible white core (SVWC) in each specimen was assessed using stereomicroscopic on-site evaluation. The primary outcome was EUS-TA's diagnostic sensitivity with stereomicroscopic on-site evaluation based on the SVWC cutoff value (≥4 mm) for malignant upper gastrointestinal SELs. Results: The total number of punctures was 68; 61 specimens (89.7%) contained stereomicroscopically visible white cores ≥4 mm in size. The final diagnoses were gastrointestinal stromal tumor, leiomyoma, and schwannoma in 76.5%, 14.7%, and 8.8% of the cases, respectively. The sensitivity of EUS-TA with stereomicroscopic on-site evaluation based on the SVWC cutoff value for malignant SELs was 100%. The per-lesion accuracy of histological diagnosis reached the highest level (100%) at the second puncture. Conclusions: Stereomicroscopic on-site evaluation showed high diagnostic sensitivity and could be a new method for diagnosing upper gastrointestinal SELs using EUS-TA.

조직몰입에 대한 개념분석(간호조직을 중심으로) (Concept Analysis on the Organizational Commitment : Focused on the Nursing Organizations)

  • Purpose: The purpose of this study was to explore the clear concept of organizational commitment for effective nursing personnel management. Method: This study was conducted using Walker & Avant's conceptual analysis framework(2005). Results: Organizational commitment was identified with six attributes: acknowledgment, acceptance, trust, sacrifice, attachment, regulation. Antecedents of organizational commitment were personal characteristics, perceived organizational support, empowerment, fair compensation, job satisfaction, job autonomy, organizational culture, and leadership. Consequences of organizational commitment were turnover intention, job performance and organizational citizenship behavior. Conclusion: Organization commitment is a core concept for understanding the management of nursing personnel. Appropriate instruments to operationalize the concept of organizational commitment including six attributes need to be developed.

WHO Treatment Guidelines for Drug-Resistant Tuberculosis, 2016 Update: Applicability in South Korea

  • Despite progress made in tuberculosis control worldwide, the disease burden and treatment outcome of multidrug-resistant tuberculosis (MDR-TB) patients have remained virtually unchanged. In 2016, the World Health Organization released new guidelines for the management of MDR-TB. The guidelines are intended to improve detection rate and treatment outcome for MDR-TB through novel, rapid molecular testing and shorter treatment regimens. Key changes include the introduction of a new, shorter MDR-TB treatment regimen, a new classification of medicines and updated recommendations for the conventional MDR-TB regimen. This paper will review these key changes and discuss the potential issues with regard to the implementation of these guidelines in South Korea.

  • As a society advances and develops more rapidly, there is a corresponding increase in an aging population; and therefore, transportation and transportation needs develop in a complicated way. In these times, medical rehabilitation services are required and the demands for these services will increase as time passes. Moreover, the scope of disability as defined by the government has been expanded owing to the welfare policy for the disabled. Therefore, the number of disabled who require medical services is on the rise. Along with an increase in medical rehabilitation demands, the necessity for medical rehabilitation increases as well and its role becomes more diverse on a wider spectrum. However, architectural studies on rehabilitation facilities have been insufficient and there are many problems currently in facilities operations. This study aims to present a spatial organization system along with required space that would form the basis for the most effective spatial plan and facility operation for rehabilitation hospitals.

COVID-19 as a Recognized Work-Related Disease: The Current Situation Worldwide

  • Coronavirus disease 2019 (COVID-19), which is a newly emerging infectious disease worldwide, can be categorized as an occupational disease, because employees, particularly in the healthcare system, can be infected at the workplace. As of December 15, 2020, we summarized the occupational safety and health practices in selected countries on the recognition of COVID-19 as one of the occupational risks. The situation has differed among countries, including the recognition status and whether a specific regulation existed. International organizations, namely the International Labour Organization, World Health Organization, and European Union, should plan and conduct studies on the work-relatedness of COVID-19, propose criteria for recognition, and add the infection to the occupational disease list to provide a basis for specific country regulations. Stakeholders should also act to adjust country-level legislation.
