중환자실 간호사의 환자안전간호활동 교육요구도 조사 : IPA 활용 (Study on Educational Needs of Patient Safety Nursing Activities in Intensive Care Units: Utilizing Importance-Performance Analysis)

  • Purpose : The purpose of this study was to not only examine the importance and performance status of patient safety nursing activities in the intensive care unit but also to understand nurses' educational needs in this respect. Methods : In this descriptive study, 196 nurses working in the intensive care units of university hospitals located at B City and G Province were enrolled. They completed the Importance-Performance Analysis (IPA) questionnaire, and the data were statistically analyzed. Results : Mean±standard deviation item scores of importance and performance of patient safety nursing activities were 4.75±0.33 and 4.48±0.38, respectively. In the IPA results, a total of 10 items were included in the second quadrant, including 1 item related to 'transfer,' 1 to 'infection,' and 1 to 'firefighting surveillance.' As the second quadrant contains the high-importance, low-performance items, these 10 items resemble the nurses' educational needs that should be prioritized. Conclusion : The results of this study can be used as a basis for the development of nursing educational programs for patient safety in the intensive care unit.

병원 간호등급에 따른 간호수행 정도 (Evaluation on the Performance of Nursing in according to the Nursing grade of Hospitals)

  • As a cross-sectional study, this study was aimed to investigate and compare the job efficiency and satisfaction of nurses according to the hospital grade. Survey was conducted by mail on June 2009, and the respondents were 1,016 nurses working in 15 hospitals which are 9 high-grade general hospitals and 6 general hospitals. The percent of nurses acknowledging their hospital grades is 34.5%, and that is 20.5% at high-grade general hospitals. As the result of review of studies, it is concluded that under the circumstance that differential rates are contracted to calculate fees for hospital services and copayment of patients are according to nursing grades and hospital grades, the degree of nurses' awareness of insurance fees impact on their performance like recording of care and prescription. In order to improve nurses' performance, they need to be educated about the national insurance fee system. In hospitals with higher nursing grade and more beds, the levels of nursing quality and faithfulnes and their job satisfaction were higher. Nurses' awareness of their hospital nursing grade was related to the quality of nursing but not the faithfulness. Nurses working in higher nursing-grade hospital are more self-respect and satisfied at their jobs, and their job efficiencies are not significantly different. The current nursing fees based on the proper number of nurses per beds of nursing units should be changed to be based on the amount of job per nurse by their nursing protocol, and the nurse staffing standard should be differentiated between nursing grades. As the aspect of nursing, 24-hours patient care, it is difficult to improve nurses' job satisfaction, and in the other hand, that tends to depend on their income level. In the current circumstance, comprehensive research is required to investigate the propriety of 25% of the inpatient fees as the nursing management charge.

임상간호사의 의료 질 향상 활동에 관한 연구: 병원 간호병동 수준으로 (A Study on the Quality Improvement Activities of Clinical Nurses: Nursing Care Unit level)

  • Purposes: This study analyzed activities for improvement of quality of care in nursing units to identify group-level success factors of quality improving efforts. Methodology/Approach: Research subjects were 31 general wards of C university hospital, which has 1,200 beds. Data were collected through survey and focus group interview. The mean value of nurses in a ward was converted to unit-level variable value of the ward. The SPSS 24.0 version was used to analyse the data. The units were classified into two groups, high performing and low performing, by the subjective level of quality improvement performance. Findings: The main findings are as follows: 1. The high performing groups participated more in education related to quality improvement and showed more quality improvement cases in progress than that of their counterpart. 2. The high performing group's nurses show more positive perception and attitude on quality improvement activities, and they have positive assessment on the necessity, effect, satisfaction about the quality improvement activities. 3. Middle managers' ability, attitude, motivation, and effective communication across members of the ward played pivotal roles in boosting the quality improvement activities of wards.

  • Purpose: This study to describe experiences of nurses who work in intensive care units (ICUs) where they frequently encounter patients with attempted suicide by drug ingestion. Methods: Data were collected by 2 focus group interviews with 9 ICU nurses. The interview were recorded and transcribed, and data were analyzed using qualitative content analysis. Results: The results of data analysis, five themes were drawn: 'Confusion about for the attempted suicidal patient care', 'Helplessness for dying with attempted suicidal patients', 'Guilty for insufficient care', 'Ambivalence for the attempted suicidal patients', 'Recognition of need for professional approach'. Conclusion: The results of this study help us to understand patients who attempted suicide through the ICU nurses' experience. It would be useful to develop effective education programs for ICU nurses to preventive strategies for patients who are high risk of recurrence of suicidal crisis situation.

병동형 호스피스 환자를 위한 원예요법의 개발 및 수면의 질과 면역기능에 미치는 효과 (Development and Effects of Horticulture Therapy on Quality of Sleep and Immune Function in Patients in Hospice Units)

  • Purpose: The study was done to examine the development and effects of horticulture therapy on quality of sleep and immune function in patients in hospice units. Methods: The participants in this study were hospice patients in D hospital in D city. Thirty hospice patients were assigned to the experimental group, thirty to the control group. Data were collected from April 29 to July 26, 2009. The horticulture therapy program consisted of indoor and outdoor horticultural activities. The horticulture therapy was conducted for 30 minutes, 6 times a week for 3 weeks (a total 18 times). Measures were quality of sleep, and immune function by serum T-cell, NK-cell count. Data were analyzed using descriptive statistics, chi-square test and t-test with SPSS/WIN 13.0 version. Results: Patients in the experimental group receiving horticulture therapy had a significant difference in changes in the quality of sleep compared to the control group. There were also a significant difference in changes in the immune function (serum T cell and serum NK cell) between the experimental group and control group. Conclusion: The study results indicate that horticulture therapy developed for hospice patients is an effective, palliative intervention program to improve the quality of sleep and immune function of hospice patients.

  • Purpose: Neonatal nurses are expected to have clinical competency to provide qualified and safe care for high-risk infants. An educational intervention to enhance nurses' clinical competence is often a priority in the nursing field. This study was conducted to explore nurses' perceived importance and performance confidence of nursing care activities in neonatal intensive care units. Methods: One hundred forty-one neonatal nurses from seven hospitals across South Korea participated in the online survey study. The scale of neonatal nursing care activity consisted of 8 subdomains including professional practice (assessment, diagnosis, planning, intervention, evaluation, education, research, and leadership). The Importance-Performance Matrix was used to analyze the importance of and confident performance in each of the nursing subdomains. Results: Both importance and performance confidence increased as nurses' age (p=.042 and p<.001) and clinical experience (p=.004 and p<.001). Participants scored relatively higher in importance and performance confidence in the professional practice subdomains (assessment, intervention, evaluation), but scored lower in the education and research subdomains. Conclusion: To provide evidence-based nursing care for high-risk infants in neonatal intensive care units, educational interventions should be developed to support nurses based on the findings of the research.

병원전산화에 따른 직접 및 간접간호 양상의 변화 (Impact of Computerization on Direct and Indirect Nursing Time)

  • The purpose of this study was to examine impact of hospital computerization on the nursing practice. The study was conducted using the repeated measured design. The subjects were 166 nurses working at surgical nursing care units in a tertiary teaching hospital. A questionnaire was developed based on the Burke's study, and reviewed by a committee for content validity. Eighty-eight nurses responded the questionnaire throughout the study. Indirect nursing care time significantly decreased(p<0.05) after the computerization. These decrease related to time saving in checking physicians' order and writing nursing records. However, the change in direct nursing care time was not clear. In conclusion, the computerization led to the decrease in indirect nursing care time but it did not link to increase in direct nursing care time. Therefore, nurse managers should take into account redesign of the nursing works after computerization.

신생아집중치료실 간호사의 발달지지간호 수행 영향요인 (Factors Influencing Developmental Care Performance among Neonatal Intensive Care Units Nurses)

  • Purpose: This study was conducted to identify factors related to developmental care performance among neonatal intensive care units (NICU) nurses. Methods: The participants were 139 nurses who had provided care to premature infants for more than 6 months and were recruited from the NICU of 8 hospitals. Data were collected from September 1 to December 1, 2017 through questionnaires that encompassed developmental care performance, developmental care perceptions, and the nursing work environment. Results: More than half (51.8%) of the participants responded that they had never received developmental care education, and for 89.6% of those who had received developmental care education, it was a one-time event. The average developmental care performance of NICU nurses was 0.81, with a range of 0.5~1 point. Multiple regression analysis, demonstrated that the nursing work environment (β=.27, p=.001) and developmental care perceptions (β=.23, p=.004) influenced developmental care performance, with a total explanatory power of 14%. Conclusion: Based on these results, developmental care education for the NICU nurses must be provided systematically. In addition, strategies to improve nurses' perceptions of developmental care and to provide appropriate support for the nursing work environment can promote developmental care performance.

병원 간호사의 조직 잔류의도에 대한 질적 연구 (Qualitative Study on Clinical Nurses' Intention to Stay in Hospital)

  • Purpose: The purpose of this study was to evaluate the experience of hospital nurses regarding their intention to stay at hospital. Methods: Experiential data were collected from 10 experienced nurses through in-depth interviews. The main question was "Could you describe your experience and your work during your years at the hospital?" Qualitative data from the field and transcribed notes were analyzed using Strauss and Corbin's grounded theory methodology. Results: The core category of experience regarding hospital nurses' intention to stay was 'following a stable rather than a challenging path'. Participants used three interactional strategies: 'being encouraged via rapport with peers', 'accept reality', and 'find vitality in academic pursuits'. Conclusion: The retention of experienced nurses is critical to human resource management in nursing departments. This study found that experienced nurses have a vague uncertainty about their future in the hospital. Therefore, nursing managers should support experienced nurses by providing them with the opportunities needed to develop their careers, by managing conflicts in nursing units, and by implementing new programs to increase confidence.

중환자실 간호사의 간호업무내용에 따른 손씻기에 관한 연구 (A Study of Handwashing by Intensive Care Unit Nurses according to the Content of Nursing Faculty Practice)

  • Purpose: This study was done to determine the rate for handwashing by intensive care unit nurses according to the content of nursing tasks, to investigate the relationship between hand washing practice evaluated by nurses themselves and their actual practice observed, and finality to provide basic materials for strategy for hand washing education. Method: Data were collected by observing 27 nurses working in intensive care units of a hospital in Uijeongbu, Gyeonggi-do and by using observation and a structured self-assessment tool. Collected data were analyzed with SPSS and SAS. Results: The handwashing rate for the nurses was 4.3%. The handwashing rate was high in proportion to the risk of cross infection. In addition, the handwashing rate was highest in nurses working in the neurosurgery intensive care unit. The average score for self-assessment of handwashing was $49.42{\pm}3.78$ points and it was higher than their actual practice of handwashing. Conclusion: In order to improve handwashing by nurses, it is necessary to educate them on the importance of handwashing. In addition, there should be strategies for standardizing knowledge and attitudes to handwashing and inducing nurse:3 to practice hand washing in compliance with the policies and working conditions of the institution.
