Development of Electronic Portal Imaging Device and Treatment Position Verification for Fractionated Stereotatic Radiotherapy

  • The video based electronic portal imaging device (EPID), which could display the portal image in near real time, was implemented to verify treatment position error in FSRT(Fractionated Stereotatic Radiation Therapy) instead of a portal film. Also, Developed FSRT system was composed of the stereotactic frame, frame mounting system and collimator cones. The verification of treatment position is very crucial in special therapies like FSRT. In general, the FSRT uses high dpse rate at small field size for treating small intracranial lesions. To evaluate quantitative positioning errors in FSRT, we used the first FSRT image as reference image and obtained the second FSRT image that was moved 2mm intentionally and detected intracranial contours after image processing. The generated 2mm error could be verified by overlapping only contours of two images. Through this study, the radiation treatment efficiency could be improved by performing precise radiation therapy with a developed video based EPID and FSRT.

의복 음양, 명암 및 직업 상징성을 중심으로 한 간호사복의 범주화 (Categorizations of Nurses' Uniforms based on 3 Major Garment Characteristics)

  • The purposes of this study is to investigate the present condition of nurses' uniform design and to categorize the image of nurses' uniform design. To investigate the nurses' uniform design, total 241 data were collected from the pictures of nurses who work for private hospital and semi-general hospital in Deajeon and Seoul city and Chungnam province and the pictures of uniform for nurses in general hospital presented at nurses' uniform company web sites. This data was evaluated by 3 major garment characteristics, yin/yang, casual/professional, and lightness/ darkness-were used as criteria for categorizing the image of nurses' uniform design. Focus group consisting of 10 experts majored in Clothing and Textiles were evaluated by 7 point Likert type scales. Nurses' uniform design at the present time showed equal distribution in terms of yin/yang and casual/professional. However, in terms of lightness/darkness, nurses' uniform design were partial to light image. Therefore, dark image, specially dark and masculine image in nurses' uniform design were very rare. Nurses' uniform design had some differences by type of hospital, department, and geographical area. The most important meaning from the result through this study was that the nurse uniforms were classified into groups according to the present condition of nurses' uniform design.

  • Bioluminescence imaging (BLI) is the most sensitive animal imaging technique for molecular imaging research. Generally, highly sensitive CCD is used to detect an optical probe introduced in a living mouse. However, in many cases, the light signal emitted from a probe is too small to detect because it is scattered and attenuated by the tissue prior to being detected. The problem is that scattering and attenuation not only inhibit accurate measurement but also make image quality down. Thus we introduced a new method to reduce noise by using property of CCD and method to improve image quality of bioluminescence image by using two steps Gaussian blurring.

한방병원에 요통, 하지방사통으로 내원한 환자 236명에서 MRI상 추간판 탈출증과 적외선 체열 검사(DITI)와의 연관성 분석 (Correlation Analysis between HIVD of L-spine MRI and Digital Infrared Thermal Image (DITI) on the Patients of LBP Who Visit Korean Medicine Hospital)

  • Objectives This study is planned to classify correlation between HIVD of L-spine MRI and Digital Infrared Thermal Image (DITI). Methods We measured the temperature of both leg whose 120 men and 116 women patients with lumbar pain in Bucheon Jaseng Korean Medicine Hospital. And We use Magnetic Resonance Imaging (MRI) for classifying the patients who has lumbar intervertebral disc or not. Results 1) There was no statistical relation between difference of both leg's temperature and gender (p>0.05). 2) There was meaningful statistical relation between difference of both leg's temperature and age (p<0.05). 3) There was meaningful statistical relation between direction of HIVD of L-spine and direction of temperature reduction. 4) There was meaningful statistical relation between the severity of HIVD of L5/S1 and degree of temperature reduction. But there was no statistical relation between the severity of HIVD of L3/4, L4/5 and degree of temperature reduction. Conclusions We can use Digital Infrared thermal image (DITI) on low back pain patients for diagnosis. But we should not use DITI alone. DITI has limit in diagnosis.

Dosimetric Verification for Primary Focal Hypermetabolism of Nasopharyngeal Carcinoma Patients Treated with Dynamic Intensity-modulated Radiation Therapy

  • Objective: To make sure the feasibility with $^{18F}FDG$ PET/CT to guided dynamic intensity-modulated radiation therapy (IMRT) for nasopharyngeal carcinoma patients, by dosimetric verification before treatment. Methods: Chose 11 patients in III~IVA nasopharyngeal carcinoma treated with functional image-guided IMRT and absolute and relative dosimetric verification by Varian 23EX LA, ionization chamber, 2DICA of I'mRT Matrixx and IBA detachable phantom. Drawing outline and making treatment plan were by different imaging techniques (CT and $^{18F}FDG$ PET/CT). The dose distributions of the various regional were realized by SMART. Results: The absolute mean errors of interest area were $2.39%{\pm}0.66$ using 0.6cc ice chamber. Results using DTA method, the average relative dose measurements within our protocol (3%, 3 mm) were 87.64% at 300 MU/min in all filed. Conclusions: Dosimetric verification before IMRT is obligatory and necessary. Ionization chamber and 2DICA of I'mRT Matrixx was the effective dosimetric verification tool for primary focal hyper metabolism in functional image-guided dynamic IMRT for nasopharyngeal carcinoma. Our preliminary evidence indicates that functional image-guided dynamic IMRT is feasible.

Deep Learning-Based Computed Tomography Image Standardization to Improve Generalizability of Deep Learning-Based Hepatic Segmentation

  • Objective: We aimed to investigate whether image standardization using deep learning-based computed tomography (CT) image conversion would improve the performance of deep learning-based automated hepatic segmentation across various reconstruction methods. Materials and Methods: We collected contrast-enhanced dual-energy CT of the abdomen that was obtained using various reconstruction methods, including filtered back projection, iterative reconstruction, optimum contrast, and monoenergetic images with 40, 60, and 80 keV. A deep learning based image conversion algorithm was developed to standardize the CT images using 142 CT examinations (128 for training and 14 for tuning). A separate set of 43 CT examinations from 42 patients (mean age, 10.1 years) was used as the test data. A commercial software program (MEDIP PRO v2.0.0.0, MEDICALIP Co. Ltd.) based on 2D U-NET was used to create liver segmentation masks with liver volume. The original 80 keV images were used as the ground truth. We used the paired t-test to compare the segmentation performance in the Dice similarity coefficient (DSC) and difference ratio of the liver volume relative to the ground truth volume before and after image standardization. The concordance correlation coefficient (CCC) was used to assess the agreement between the segmented liver volume and ground-truth volume. Results: The original CT images showed variable and poor segmentation performances. The standardized images achieved significantly higher DSCs for liver segmentation than the original images (DSC [original, 5.40%-91.27%] vs. [standardized, 93.16%-96.74%], all P < 0.001). The difference ratio of liver volume also decreased significantly after image conversion (original, 9.84%-91.37% vs. standardized, 1.99%-4.41%). In all protocols, CCCs improved after image conversion (original, -0.006-0.964 vs. standardized, 0.990-0.998). Conclusion: Deep learning-based CT image standardization can improve the performance of automated hepatic segmentation using CT images reconstructed using various methods. Deep learning-based CT image conversion may have the potential to improve the generalizability of the segmentation network.

응급의료종사자가 인식하는 병원 내 응급구조사의 이미지 (대전, 충남 3차의료기관 중심으로) (Images of Hospital Emergency Medical Technicians Perceived by Emergency Medical Service Providers (Focusing on tertiary medical institutions in Daejeon and Chungcheongnam do))

  • 본 연구는 대전 충남 소재 3차 의료기관에서 근무하는 응급의료종사자(의사, 간호사) 122명을 대상으로 병원 내 응급구조사의 이미지를 조사하였다. 병원 내 응급구조사의 이미지를 향상시키기 위한 기초 자료를 제공하고자 연구를 실시한 결과, 이미지는 $3.27{\pm}0.34$점이었다. 영역별로는 전문적 이미지가 가장 높았고, 역할 이미지가 가장 낮았다. 문항별로는 '의사와 우호적 관계를 유지하고 있다.'가 가장 높았고, '잘난 체하고 업무 수행을 소홀히 한다.'가 가장 낮았다. 병원 내 응급구조사의 이미지를 더욱 향상시키기 위해서는 응급구조사가 환자와 동료에게 진정성 있는 태도로 일관해야하며, 전문직으로서 직업의식을 고취시키고, 응급구조사의 고유 업무와 역할을 충실히 해내야 할 것이다. 또한 전문 직업인으로서의 이미지를 구축할 수 있도록 협회 차원의 홍보와 대책이 마련되어져야 할 것이다.

동적 영상 획득 방식을 이용한 정적 영상의 움직임 보정 (The Application of Dynamic Acquisition with Motion Correction for Static Image)

  • 정적 영상 획득 시 기존의 정적 영상 획득 방식 대신 동적 영상 획득 방식을 이용하여 움직임에 의한 motion correction을 적용함으로써 정적 영상 획득 시 발생되는 움직임에 의한 문제점을 해결하고자 하였다. 실험은 capillary tube와 IEC body phantom을 이용하여 움직임이 없을 때 정적 영상 획득 방식으로 얻은 영상과 동적 영상 획득 방식으로 얻은 각각의 frame을 더한 영상에 대해 resolution, frequency, total counts, blind test를 비교 분석하였으며 임의로 최소한의 움직임과 과도한 움직임을 주어 motion correction 전후의 영상에 대해서도 resolution, frequency, total counts, blind test를 비교 분석 하였다. 기존의 정적 영상 획득 방식으로 얻은 영상과 동적 영상 획득 방식으로 얻은 각각의 frame을 더한 영상의 resolution, frequency, total counts, blind test의 결과 값의 차이가 없었다. 또한 최소한의 움직임과 과도한 움직임을 준 영상에 대해 motion correction 적용 전후의 비교 결과 값은 motion correction 후 resolution, frequency, blind test의 결과 값이 움직임이 없을 때의 정적 영상과 거의 차이가 없었다. 하지만 과도한 움직임에 대한 보정 시 frame당 흐림 현상이 많이 발생 하였으므로 좌표 보정이 어려워 frame을 제외하는 방법을 적용하였기 때문에 과도한 motion correction 후 삭제한 frame 수만큼 total counts에서 차이를 보였다. 정적 영상 획득 시 움직임이 예상되는 환자에게 기존의 정적 영상 획득방식이 아닌 동적 영상 획득 방식을 이용하여 움직임 발생시 좌표 보정과 흐림 현상이 심한 frame 제외 방법을 이용하여 정적 영상에서 움직임에 의해 발생되었던 영상의 질 저하와 정량적 분석의 신뢰도 감소, 재검사에 대한 문제점을 해결할 수 있을 것이라고 생각되며 motion correction에 제공되는 다양한 프로그램 개발과 임상 적용에 대한 광범위한 연구가 현실적으로 필요하며 향후 지속적인 연구가 기대되는 바이다.

비만이 소아의 삶의 질에 미치는 영향 (Impact of Obesity on Health-Related Quality of Life among Children)

  • Objectives To examine the relationship between health-related quality of life (HRQoL) and body mass index (BMI) of obese children. Methods This cross-sectional study included 387 children. HRQoL was measured with the PedsQLTM 4.0 Generic Core Scale. BMI was classified according to the World Health Organization Asia-Pacific obesity guidelines. Psychosocial factors (body image, self-esteem, and depression/anxiety) were also measured. Data were analyzed using one-way analysis of variance, Pearson's correlation and Path analysis. Results Obese participants reported lower score for physical evaluation, self-esteem, appearance evaluation component of body image, total HRQoL score, and physical/emotional/social function components of HRQoL ; they reported higher score for depression/anxiety. In addition, results indicated that as BMI increased, appearance evaluation, total HRQoL score, and social function component of HRQoL dropped. Path analysis revealed that BMI did not directly affect HRQoL ; however, BMI directly affects body image and self-esteem, indirectly mediates depression/anxiety, and thereby impacts on an individual's HRQoL. Conclusions Body image and self-esteem, rather than BMI itself, have more influence on HRQoL. This reinforces the importance of therapeutic intervention to enhance body image and self-esteem among obese children.

내부마케팅, 서비스지향성, 병원이미지간의 관계: 자기효능감의 조절효과 (The Relationship among Internal Marketing, Service Orientation, and Hospital Image: The Moderating Effect of Self-Efficacy)

  • 최근 의료기관에서도 내부마케팅 활동이 중요하다는 인식이 점차 확대되고 있지만 병원 내부마케팅에 관한 연구는 미흡한 상황이다. 따라서 본 연구의 목적은 내부마케팅과 서비스지향성이 병원이미지에 미치는 영향을 분석하고 내부마케팅과 서비스지향성 간의 영향관계에서 자기효능감의 조절효과를 검정하는 것이다. 본 연구의 주요결과는 다음과 같다. 첫째, 내부마케팅 요인 중 권한위임, 교육과 훈련, 리더십이 서비스 지향성에 긍정적 영향을 미치는 것으로 나타났으며 리더십이 가장 많은 영향을 주는 것으로 나타났고, 내부커뮤니케이션과 보상은 영향을 미치지 않는 것으로 나타났다. 둘째, 서비스지향성이 병원이미지에 긍정적인 영향을 미치는 것으로 나타나 두 요인 간에 영향관계가 형성되고 있음을 알 수 있었다. 셋째, 내부마케팅과 서비스지향성간에 자기효능감의 조절효과를 분석한 결과, 유의한 차이가 없는 것으로 나타났지만 대체적으로 효능감이 높은 집단이 좀 더 많은 영향을 미치는 것으로 나타났다. 따라서 관리자들은 높은 수준의 병원이미지를 구축하기 위해 내부마케팅과 서비스 지향성을 개선시켜야 하며, 종업원들이 자기효능감을 가질 수 있도록 동기를 부여해야 한다.
