A Study on the Chest Radiography with Diseases in Consideration of Image Qualify and Patient Exposure (흉부질환의 화질과 피폭을 고려한 촬영조건의 연구)

  • To evaluated the image quality and the patient exposure for the chest radiography, its fundamental imaging properties were investigated. The basic imaging properties were evaluated by measuring characteristic curves, relative speeds, average gradient, and patient exposure dose. The image qualities were evaluated by using a rotating meter. It was found that the film gradient of SRO750/SRH system was 3.13 and that of SRO750/HR-C30 was 1.77. The ratio of SRO1000/TMH to FS/RP-1 was 1 to 18.2. It was possible to visualize the static image when the exposure time was less than 2.5 msec in patient that had respiratory excessive motion, heart beat and natural physical motion, and less than 8.5 msec in normal. The ratio of medical exposure dose compared with our method was 1 to 25 in the routine chest radiography and maximum was 1 to 70. In estimation of the image quality in same cases, we found that the image of SRO1000/SRH and TMH of super sensitive systems was the same results. We found that these super sensitive screen-film systems were available for the chest radiography considering the relationship between the image quality and patient exposure.

Relationships of Activities of Daily Living and Body Image with Quality of Life in Stroke Patients: Mediating Effects of Interpersonal Relations (뇌졸중 환자의 일상생활 수행능력, 신체상이 삶의 질에 미치는 영향: 대인관계 매개효과 중심)

  • Purpose: This study was conducted to identify factors associated with stroke patients' quality of life (QOL) and examine the mediating effects of interpersonal relations in the relationships of activities of daily living (ADL) and body image with QOL. Methods: In this study, 160 stroke patients were recruited from an outpatient clinic of a university hospital and rehabilitation clinic of a long-term care hospital. Participants completed a questionnaire which included sociodemographic characteristics, ADL, body image, interpersonal relation, and QOL. Descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, multiple regression analysis, and process macro mediation analysis were conducted using SPSS. Results: The mean scores were 91.01 (range 6~100) for ADL, 61.19 (range 40~79) for body image, 87.53 (range 29~123) for interpersonal relations, and 186.67 (range 71~243) for QOL. Multiple regression analyses showed that ADL, body image, interpersonal relations, and participation in group activities after stroke were significantly associated with QOL among stroke patients (p<.05). There were no mediating effects of interpersonal relations in the relationships of ADL and body image with QOL (p>.05). Conclusion: This study showed that there is a need for physical, psychological, and social recovery to improve the QOL of stroke patients.

Evaluation and Comparison of Signal to Noise Ratio According to Histogram Equalization of Heart Shadow on Chest Image (흉부영상에서 평활화 시 심장저부 음영의 신호 대 잡음비 비교평가)

  • The purpose of this study was to measure signal to noise ratio (SNR) according to change of equalization from region of interest (ROI) of heart shadow in chest image. We examined images of chest image of 87 patients in a University-affiliated hospital, Seoul, Korea. Chest images of each patient were calculated by using ImageJ. We have analysis socio-demographical variables, SNR according to images, 95% confidence according to SNR of difference in a mean of SNR. Differences of SNR among change of equalization were tested by SPSS Statistics21 ANOVA test for there was statistical significance 95%(p < 0.05). In SNR results, with the quality of distributions in the order of original chest image, original chest image heart shadow and equalization chest image, equalization chest image heart shadow(p < 0.001). In conclusion, this study would be that quantitative evaluation of heart shadow on chest image can be used as an adjunct to the histogram equalization chest image.

Feasibility of Free-Breathing, Non-ECG-Gated, Black-Blood Cine Magnetic Resonance Images With Multitasking in Measuring Left Ventricular Function Indices

  • Objective: To clinically validate the feasibility and accuracy of cine images acquired through the multitasking method, with no electrocardiogram gating and free-breathing, in measuring left ventricular (LV) function indices by comparing them with those acquired through the balanced steady-state free precession (bSSFP) method, with multiple breath-holds and electrocardiogram gating. Materials and Methods: Forty-three healthy volunteers (female:male, 30:13; mean age, 23.1 ± 2.3 years) and 36 patients requiring an assessment of LV function for various clinical indications (female:male, 22:14; 57.8 ± 11.3 years) were enrolled in this prospective study. Each participant underwent cardiac magnetic resonance imaging (MRI) using the multiple breath-hold bSSFP method and free-breathing multitasking method. LV function parameters were measured for both MRI methods. Image quality was assessed through subjective image quality scores (1 to 5) and calculation of the contrast-to-noise ratio (CNR) between the myocardium and blood pool. Differences between the two MRI methods were analyzed using the Bland-Altman plot, paired t-test, or Wilcoxon signed-rank test, as appropriate. Results: LV ejection fraction (LVEF) was not significantly different between the two MRI methods (P = 0.222 in healthy volunteers and P = 0.343 in patients). LV end-diastolic mass was slightly overestimated with multitasking in both healthy volunteers (multitasking vs. bSSFP, 60.5 ± 10.7 g vs. 58.0 ± 10.4 g, respectively; P < 0.001) and patients (69.4 ± 18.1 g vs. 66.8 ± 18.0 g, respectively; P = 0.003). Acceptable and comparable image quality was achieved for both MRI methods (multitasking vs. bSSFP, 4.5 ± 0.7 vs. 4.6 ± 0.6, respectively; P = 0.203). The CNR between the myocardium and blood pool showed no significant differences between the two MRI methods (18.89 ± 6.65 vs. 18.19 ± 5.83, respectively; P = 0.480). Conclusion: Multitasking-derived cine images obtained without electrocardiogram gating and breath-holding achieved similar image quality and accurate quantification of LVEF in healthy volunteers and patients.

Comparison and Review of GBEF% on the Anterior and Right Lateral Images of Nuclear Hepatobiliary Scan (핵의학 간담도 스캔 시, 전면상과 우측 측면상에서의 담낭박출률에 대한 비교 및 고찰)

  • Purpose In case of nuclear medical hepatobiliary scan, To quantitatively evaluate contractility of a gallbladder, gallbladder ejection fraction (GBEF%) is calculated from anterior images using fatty meal. However, when a gallbladder and other organs overlap on an anterior image, the gallbladder ejection fraction is not accurately evaluated. In order to reduce this error, the objective of our study was to figure out whether there is a significant difference in GBEF% calculated from the anterior and right lateral images. Materials and Methods After intravenous injection of 99mTc-Mebrofenin 370 MBq to randomly 50 patients who visited our hospital, we started to examine nuclear hepatobiliary scan. Using skylight(Philips, United States), we acquired anterior and right lateral image at 10 minutes, 20 minutes, 30 minutes, 60 minutes, 90minutes after injection. Using images at 60 and 90 minutes, gallbladder ejection fraction (GBEF%) was calculated from the anterior and right lateral images using JETstream workspace. For drawing more accurate ROI, CT images were referenced and 4 radiologists calculated the GBEF% in the same image and calculated the average value. We assessed whether there was a significant difference in GBEF% calculated from the anterior and right lateral images using SPSS program(Statistical Package for the Social Science, SPSS Ver.18 Inc. USA). Results About randomly 50 patients, the average value of the GBEF% calculated from the anterior image was 63.212 and the average value of the GBEF% calculated from the right lateral image was 62.666. GBEF% decreased 0.433% on the right lateral image compared with anterior image. Result of paired sample t-test, p value is over 0.05. So, there was no significant difference in GBEF% calculated from the anterior and right lateral images. Conclusion In the case that a gallbladder and other organs are not separated on an anteior image, Right lateral image would be better to acquire more accurate GBEF% than using anterior image.

  • In this study, we developed the protopype of QA phantom for image QA including an additional component for image based radiation treatment system. The new phantom considered two main parts: Image quality and fusion accuracy. Image quality part included for daily CT number linearity and spatial resolution, and fusion accuracy part designed to simulate a simple translation-rotation setting. The CT scans of the phantom obtained from conventional CT, MVCT of Tomotherapy unit, and both image sets were satisfied the recommendation of spatial resolution. This phantom was simple and efficient for daily imaging QA, and it is important to provide a new concept of verification of image registration.

A Novel Fundus Image Reading Tool for Efficient Generation of a Multi-dimensional Categorical Image Database for Machine Learning Algorithm Training

  • Background: We described a novel multi-step retinal fundus image reading system for providing high-quality large data for machine learning algorithms, and assessed the grader variability in the large-scale dataset generated with this system. Methods: A 5-step retinal fundus image reading tool was developed that rates image quality, presence of abnormality, findings with location information, diagnoses, and clinical significance. Each image was evaluated by 3 different graders. Agreements among graders for each decision were evaluated. Results: The 234,242 readings of 79,458 images were collected from 55 licensed ophthalmologists during 6 months. The 34,364 images were graded as abnormal by at-least one rater. Of these, all three raters agreed in 46.6% in abnormality, while 69.9% of the images were rated as abnormal by two or more raters. Agreement rate of at-least two raters on a certain finding was 26.7%-65.2%, and complete agreement rate of all-three raters was 5.7%-43.3%. As for diagnoses, agreement of at-least two raters was 35.6%-65.6%, and complete agreement rate was 11.0%-40.0%. Agreement of findings and diagnoses were higher when restricted to images with prior complete agreement on abnormality. Retinal/glaucoma specialists showed higher agreements on findings and diagnoses of their corresponding subspecialties. Conclusion: This novel reading tool for retinal fundus images generated a large-scale dataset with high level of information, which can be utilized in future development of machine learning-based algorithms for automated identification of abnormal conditions and clinical decision supporting system. These results emphasize the importance of addressing grader variability in algorithm developments.

Depression, sleep quality, and body image disturbances among pregnant women in India: a cross-sectional study

  • Background: Pregnancy is associated with a number of physical, emotional, and biological changes that can exacerbate maternal psychological disturbances, such as body image concerns and depression. Sleep disturbances during pregnancy can also have adverse impacts. This study aimed to determine the prevalence of depression, sleep disturbances, and body image concerns among pregnant women. The study also examined the relationship between these factors and pregnancy-related variables, such as bad obstetric history and whether the pregnancies were unplanned. Methods: A cross-sectional study of 146 pregnant patients was conducted at a tertiary care center over 15 months. The patients were administered the Beck Depression Inventory, Pittsburgh Sleep Quality Index, and Body Image Concern Inventory questionnaires. Contingency tables, Fisher exact test, and Spearman correlation were used to identify underlying relationships. Results: The prevalence of depression was 22.6%. Although body image disturbance was noted in only 2.7% of patients, 46.6% had poor sleep quality. Poor sleep was associated with primigravida status. Bad obstetric history and unplanned pregnancy were associated with depression. Depression was found to be significantly correlated with body image disturbances and poor sleep quality. Conclusion: Psychiatric disorders were prevalent during pregnancy. This study highlights the importance of screening for depression in pregnant patients. Counselling and caregiver education can be useful for mitigating psychological disturbances. Management of pregnancies by multidisciplinary teams that include psychiatrists could be immensely useful in improving the pregnancy experiences of patients.

Evaluation of compression ratios using JPEG 2000 on diagnostic images in dentistry (치과병원에서 사용되는 진단영상에 대한 JPEG2000 압축률에 대한 평가)

  • Purpose : To find out the proper compression ratios without degrading image quality and affecting lesion detectability on diagnostic images used in dentistry compressed with JPEG 2000 algorithm. Materials and Methods : Sixty Digora periapical images, sixty panoramic computed radiographic (CR) images, sixty computed tomographic (CT) images, and sixty magnetic resonance (MR) images were compressed into JPEG 2000 with ratios of 10 levels from 5:1 to 50:1. To evaluate the lesion detectability, the images were graded with 5 levels (1 : definitely absent; 2: probably absent; 3: equivocal; 4: probably present; 5: definitely present), and then receiver operating characteristic analysis was performed using the original image as a gold standard. Also to evaluate subjectively the image quality, the images were graded with 5 levels (1 definitely unacceptable; 2: probably unacceptable; 3: equivocal, 4: probably acceptable; 5· definitely acceptable), and then paired t-test was performed. Results : In Digora, CR panoramic and CT images, compressed images up to ratios of 15 : 1 showed nearly the same lesion detectability as original images, and in MR images, compressed images did up to ratios of 25 : 1. In Digora and CR panoramic Images, compressed images up to ratios of 5 : 1 showed little difference between the original and reconstructed images in subjective assessment of image quality In CT images, compressed images did up to ratios of 10: 1 and in MR images up to ratios of 15 : 1 Conclusion : We considered compression ratios up to 5 : 1 in Digora and CR panoramic images, up to 10 : 1 in CT images, up to 15 : 1 in MR images as clinically applicable compression ratios.
