This study was conducted to investigate whether treatment with Korean medicine might help to improve symptoms of patients diagnosed with Ramsay Hunt Syndrome. A patient diagnosed with Ramsay Hunt Syndrome was hospitalized for 15 days from October 4, 2018 to October 18, 2018 and treated using Korean medicine (Sinbaro3 pharmacopuncture, acupuncture, electroacupuncture, and herbal medicine). Therapeutic effects were evaluated using the Numerical Rating Scale (NRS), House-Brackmann grade, Yanagihara's score survey, and Digital Infrared Thermographic Imaging (DITI). After treatment, the Numerical Rating Scale score (6 at admission down to 2 points at discharge), House-Brackmann grade (Grade V down to Grade II), Yanagihara's score (6.0 up to 22.0), and Digital Infrared Thermographic Imaging showed improvement from admission to discharge. Treatment with traditional Korean medicine, may help improve symptoms of Ramsay Hunt Syndrome such as facial nerve paralysis, neuralgia after paraplegia, hearing difficulties, and dizziness.
Purpose: This study was conducted to examine the effects of tailored supportive education on physical, psychological status and quality of life in the patients with congestive heart failure (CHF) who had not participated previously in cardiac rehabilitation program. Methods: This study was used a non-equivalent control pre-post design, conducted on 64 CHF patients under medical treatment in a hospital. The experimental group (n=31) received the tailored supportive educational program (once 1~2 days before discharge and 6 times after discharge through outpatient visits or telephone contact: once every week for the first 4 weeks after discharge and once every 2 weeks for the remaining 4 weeks). The control group (n=33) received a traditional discharge education. Data were analyzed using descriptive statistics, ${\chi}^2$-test, Fisher's exact test, t-test and repeated measure ANOVA using the SPSS/ WIN 18.0 program. Results: Participants in the experimental group showed the significantly increased scores of the quality of life (F=16.01, p<.001), and the significantly decreased scores of physical function (F=7.27, p=.009), depression (F=8.25, p=.006) and anxiety (F=4.11, p=.047), when compared to those of the control group. Conclusion: The findings indicated tailored supportive education was an effective intervention care in physical, emotional status and quality of life for CHF patients.
Purpose: The present study was to investigate sleep disturbance and related variables in patients undergoing hemopoietic stem cell transplantation(HSCT) on the day of admission, before HSCT, and of discharge, respectively. Method: A total of 52 HSCT patients were recruited from an university hospital from August 2002 to August 2003. The patients were asked to complete a set of sleep disturbance questionnaires. Result: The scores of sleep disturbance-related physical and emotional subscales on the day before HSCT and on the day of discharge were significantly higher than that on the day of admission. However, the score of a emotional subscale was not significantly different during the hospitalization for HSCT. About 10% of the variance of sleep disturbance was explained by the length after diagnosis on the day of admission, and 59.4% was by the type of disease, religion, gender, educational level on the day before HSCT. About 40.7% was by family income, gender, and the frequency of administration on the day of discharge. Conclusion: Nursing staffs should care for HSCT patients with more interest to reduce variables disturbing sleep. Especially, more careful nursing interventions have to be considered at the time of the day before HSCT and the time of discharge.

Electronic health records (EHRs) enable us to use and re-use electronic data for various multiple purposes, such as public reporting, quality improvement, and patient outcomes research. Current hospital-acquired pressure injury (HAPI) risk assessment instruments have not been specifically developed for intensive care unit (ICU) patients and showed false positive rates in this specific populations. Previous research studies report a number of risk factors; however, it is still not clear what factors influence ICU HAPI in this population. As part of a larger research study, we performed an exploratory analysis by using a large electronic health record data. The aims of this study were to compare characteristics of patients who developed HAPIs during their ICU stay with those who did not, and to determine whether the two groups were different in the aspects of length of ICU stay, discharge disposition, and discharge destinations. We conducted chi-square test and t-test for group comparison. Association was examined by using bivariate analyses. Pearson correlation coefficients were used to examine correlation between LOS and number of medications. Our findings suggest a number of consistent and potentially modifiable risk factors, such as sedation, feeding tubes, and the number of medications administered. The mortality of the HAPI group was significantly higher than the non-HAPI group in our data. Discharge disposition was significantly different between the groups. 67% of the HAPI group transferred to intermediate or long-term care hospitals whereas 57.7% of the non-HAPI group went home after discharge. Awareness of these risk factors can lead to clinical interventions that can be preventative in the ICU setting.
Objectives: The purpose of this study was to assess the incidence of delivery and puerperium complications in South Korea and analyze the correlations between the patient's characteristics and delivery and complications before and after 10 years. Methods: This study used the data from an Korean National Hospital Discharge In-depth Injury Survey. Cases of which the principal diagnosis and second diagnoses were disease classification ICD code O00-O99(Pregnancy, childbirth and the puerperium) were defined as the study subjects, and the first study group was divided as the year of discharge from 2005 to 2007, and the second study group from 2015 to 2017. Results: The number of patients discharged whose principal diagnosis or second diagnoses was O00-O99 was 21,598(Weighted 423,306) from 2005 to 2007 and 19,028(Weighted 364,384) from 2015 to 2017, which decreased by 13.9% compared to 10 years ago. The average age of discharged patients increased by about 2 years and was statistically significant (p<.0001). Factors associating spontaneous delivery, caesarean section and puerperium complication were hospitalization route, bed size, maternal age, length of hospital stay, and the year of discharge. Conclusion: Based on the results of this study, health and education policies and economic support for medical care for high-risk pregnancy and delivery management would be necessary continuously. In addition, policies to strengthen the medical system for high-risk pregnancy management in non-metropolitan areas with high fertility rates would also be needed.
Background: This study investigated mortality and morbidity in patients requiring postcardiotomy extracorporeal membrane oxygenation (ECMO) support after operations for congenital heart disease (CHD). Methods: CHD patients requiring postoperative ECMO support between May 2011 and May 2021 were retrospectively reviewed. Patients were divided into non-survivors and survivors to hospital discharge. Survival outcomes and associations of various factors with in-hospital death were analyzed. Results: Fifty patients required postoperative ECMO support. Patients' median age and weight at the time of ECMO insertion were 1.85 months (interquartile range [IQR], 0.23-14.5 months) and 3.84 kg (IQR, 3.08-7.88 kg), respectively. Twenty-nine patients (58%) were male. The median duration of ECMO support was 6 days (IQR, 3-12 days). Twenty-nine patients (58%) died on ECMO support or after ECMO weaning, and 21 (42%) survived to hospital discharge. Postoperative complications included renal failure (n=33, 66%), bleeding (n=11, 22%), and sepsis (n=15, 30%). Prolonged ECMO support (p=0.017), renal failure (p=0.005), continuous renal replacement therapy (CRRT) application (p=0.001), sepsis (p=0.012), bleeding (p=0.032), and high serum lactate (p=0.002) and total bilirubin (p=0.017) levels during ECMO support were associated with higher mortality risk in a univariate analysis. A multivariable analysis identified CRRT application (p=0.013) and a high serum total bilirubin level (p=0.001) as independent risk factors for death. Conclusion: Postcardiotomy ECMO should be considered as an important therapeutic modality for patients unresponsive to conventional management. ECMO implementation strategies and management in appropriate patients without severe complications, particularly renal failure and/or liver failure, are crucial for achieving positive outcomes.
The purpose of this study is to provide a basic data of home health care nursing centered in the hospital by analyzing home health care needs. Data were collected from June 15. 2000 to June 23. 2000 through questionnaires taken by 208 patients to be discharge in a general hospital. The Home Health Care Need instruments used for collecting data was developed by the researcher. The data obtained were analyzed using frequency. percentage. mean. standard deviation. Chi-square test. t-test. one-way ANOVA. Pearson's correlation coefficients. Cronbach's alpha coefficients and Factor Analysis. The results of the study were as follows: 1. The perception of home health care nursing were 65.4% of subjects had never heard about home health care service and 2.4% of subjects knew about the methods & contents in detail. About the demands of home health care, 58.2% of the subjects were willing to use home health care and 41.8% weren't. 2. Regarding the areas of home health care needs. basic nursing care area was the highest. Education/ counselling, exercise/ hygiene. therapeutic nursing care was orderly. Injection and medication management of basic nursing care area were the highest. 3. In relation to characteristic-related diseases and home health care needs. as for the discharge type, the existence of sores, paralysis showed significant differences with home health care needs in the area of exercise/ hygiene(p<,05). Home health care needs in the area of therapeutic nursing care and exercise/ hygiene showed significant differences with the existence of pain. Home health care needs in the area of therapeutic nursing and basic nursing areas were significantly different in the use of catheter/ assistant instrument(p<.05). Home health care needs in the area of therapeutic nursing. exercise/ hygiene and basic nursing care showed significant difference with diagnosis(p<,05). In conclusion, awareness about home health care nursing were very low, home health care needs in the area of basic nursing was the highest. Home health care needs showed significant difference with discharge type, sores, paralysis, existence of pain, use of catheter/ assistant instrument and diagnosis.
Background: Adolescent mothers may find the transition to motherhood to be overwhelming when compared to adult mothers. The adaptiveness of mother and infant feeding behaviors may greatly influence the quality of adolescent mothers' feeding interactions when compared to adult mothers. Purpose: To examine the adaptiveness of early maternal and infant feeding behavior between adolescent mothers and adult mothers. Method: In this secondary analysis, six adolescent mothers (< 20 years old) and six adult mothers were videotaped while feeding. The mothers were observed 3 times: when the infants were in the hospital, 1 month old, and 4 months old. The videotapes were coded using four scales of the Parent-Child Early Relational Assessment (PCERA). The two maternal scales were Positive Affective Involvement and Sensitivity/Responsiveness (PAISR) and Regulation of Affect and Behavior (RAB). The two infant scales were Infant Positive Affect, Communication, and Social Skills (IPACS) and Infant Regulation of Affect and Behavior (IRAB). The dyads were videotaped just before hospital discharge and at one and four months corrected age. The Wilcoxon signed-rank test was used to examine the difference in the PCERA between the two groups. Results: Adolescent mothers had a significantly lower score on the PAISR before hospital discharge (p-value 0.005) and at one month corrected age (p-value 0.008) than adult mothers. Adolescent mothers also had a significantly lower score on the RAB before hospital discharge, 1 and 4 months corrected age than adult mothers. There were no significant differences in IPACS and IRAB of infants of adolescent and adult mothers. Conclusion: Clinicians need to be aware of the differences in sensitivity that occur.
Purpose: This was a nationwide population-based study conducted to investigate the epidemiology, treatment, disease outcomes, and associated factors of childhood intussusception in South Korea. Methods: Data from the Korean National Health Insurance Service database on all patients <18 years old diagnosed with intussusception from 2007 to 2017 were analyzed. Results: A total of 34,688 cases were identified among 30,444 patients. The overall incidence was 28.3/100,000 person-years with a male predominance. Most cases (83.1%) occurred in children <3 years old, with an annual incidence of 195.2, 200.1, and 118.6 cases per 100,000 children in their first, second, and third year of life, respectively. The median age at the first occurrence was 18.7 months, and it was higher in boys than in girls. The post-discharge recurrence rate was 10.6% (3,226/30,444) and the in-hospital recurrence rate was 6.1% (1,842/30,444). The total recurrence rate (post-discharge recurrence and/or in-hospital recurrence) was 15.0% (4,580/30,444). Enema reduction was successful in 90.0% of cases. Enema reduction was more successful in girls than in boys. A total of 3,296 (10.8%) patients underwent 3,481 surgeries, including 735 (21.1%) laparoscopic surgeries. Post-discharge recurrence and surgery were significantly affected by age, sex, and hospital type. Mortality was noted in nine cases (0.03%). Conclusion: Our study provides accurate epidemiologic data on the treatment and outcomes of intussusception through complete enumeration during an 11-year-period.
