The development and evaluation of a pediatric nurse education program to improve nursing competency for newly graduated nurses in a children's hospital (어린이병원 신규간호사를 위한 간호역량 향상 교육프로그램 개발 및 효과검증)

  • Purpose: The purpose of this study is to identify the effects of a standardized educational program to improve nursing competency on newly graduated nurses in a children's hospital after developing and applying a pediatric nurse education program. The effectiveness of the program was confirmed by evaluating the clinical competency and field adaptation. Methods: In the first step, an education program was developed using the analysis, design, development, implementation and evaluation (ADDIE) model. As a second step, a similar experimental study of a single group repeat measures design was conducted to evaluate the clinical competency and field adaptation over time after application of the program. Additionally, a focus group interviews were conducted to collect subjective data on the effects and improvement points of the program. Results: As a result of applying the program, there was a significant change in the clinical competence and the field adaptation of newly graduated nurses in a children's hospital. The categories derived from the focus group interviews were "getting special guidance," "better care," "becoming a nurse at a children's hospital" and "winning together." Conclusion: It was confirmed that the education program enhances the clinical competency of new nurses in children's hospitals. In addition, it provided the necessary data to understand the experiences of new nurses, help them adapt effectively, and establish appropriate interventions.

Heavy concrete shielding properties for carbon therapy

  • As medical facilities are usually built at urban areas, special concrete aggregates and evaluation methods are needed to optimize the design of concrete walls by balancing density, thickness, material composition, cost, and other factors. Carbon treatment rooms require a high radiation shielding requirement, as the neutron yield from carbon therapy is much higher than the neutron yield of protons. In this case study, the maximum carbon energy is 430 MeV/u and the maximum current is 0.27 nA from a hybrid particle therapy system. Hospital or facility construction should consider this requirement to design a special heavy concrete. In this work, magnetite is adopted as the major aggregate. Density is determined mainly by the major aggregate content of magnetite, and a heavy concrete test block was constructed for structural tests. The compressive strength is 35.7 MPa. The density ranges from 3.65 g/cm3 to 4.14 g/cm3, and the iron mass content ranges from 53.78% to 60.38% from the 12 cored sample measurements. It was found that there is a linear relationship between density and iron content, and mixing impurities should be the major reason leading to the nonuniform element and density distribution. The effect of this nonuniformity on radiation shielding properties for a carbon treatment room is investigated by three groups of Monte Carlo simulations. Higher density dominates to reduce shielding thickness. However, a higher content of high-Z elements will weaken the shielding strength, especially at a lower dose rate threshold and vice versa. The weakened side effect of a high iron content on the shielding property is obvious at 2.5 µSv=h. Therefore, we should not blindly pursue high Z content in engineering. If the thickness is constrained to 2 m, then the density can be reduced to 3.3 g/cm3, which will save cost by reducing the magnetite composition with 50.44% iron content. If a higher density of 3.9 g/cm3 with 57.65% iron content is selected for construction, then the thickness of the wall can be reduced to 174.2 cm, which will save space for equipment installation.

  • The study is to identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing service has expanded nationwide from 2016 but 10 out of 34 public hospitals are currently unable to operate as nurses due to the shortage. Analyze each floor plan and space program of ward and provide the basic data for the construction of the architecture plans for Integrated nursing care service. For this study, selecting 14 wards of public hospital in Korea and then analyzes space organization, distance from NS to bedroom and NS to core(Elevator) and spatial hierarchy of them. Among the 14 public hospitals, there are 7 types of double-loaded corridor, 4 types of double corridor and 3 types of single corridor. If NS is placed centrally, the control of the outsider and the patient's control would be efficient, but most of NS are lean on one side of floor. the placement of NS should be independent of individual arrangements, but it should be smooth alignment with patient spaces, staff spaces, and public spaces. This study could serve as basic research for the architectural plan for future integrated nursing care in ward of public hospital.

Consumer Satisfaction with Medical Services and Hospital Patient Gowns (병.의원의 의료서비스와 환자복에 대한 소비자 만족)

  • This study identifies consumer satisfaction with medical services and the patient gowns of hospitals. Also analyzed are the elements that influence hospital satisfaction and the general satisfaction with patient gowns. A survey was conducted among Korean male and female ex-patients regardless of their age. A total of 513 responses were analyzed using descriptive statistics, paired t-test, correlations, regression, and factor analysis derived from data collected in April and May, 2009. The results are as follows. The general satisfaction with the hospital recently visited was higher than the general satisfaction with all of the hospitals visited. The satisfaction with medical services were high in good services, trustful medical examinations, easy access to utilities, and clean utilities. Hospital satisfaction was determined by good services, trustful medical examinations, easy access to utilities, patient gowns, and fresh indoor air. The satisfaction factors of general patient gowns were determined as functionality, fabric/design, sewing, and the management system factors. The most important factor explaining patient gown satisfaction was fabric/design, and this was the most unsatisfied factor at the same time.

A Study of Gender Differences in Inpatients' Privacy Concerns in Multi-bed Hospital Room (다인병실 입원환자의 성별에 따른 프라이버시 특성 연구)

  • This study aims to find gender differences in inpatients' privacy and stress concerns in multi-bed hospital room. In the literature review, we consider the characteristics of multi-bed hospital room and patients' privacy as a psychological environment factor. In the survey research, the questionnaires were used to understand the inpatients' general characteristics and privacy requirements. A total of 109 copies were analyzed through a cross-tabulations and T-test using the SPSS 18.0. Results and conclusions are as follows: (1) In the case of multi-bed room patients, there were some gender differences in privacy and stress, but the differences were limited. (2) There is a difference in the patient 's preference of the hospital room according to gender, and this is related to the privacy characteristic. (3) The social communication and strolling are commonly effective stress relief methods for both men and women, so space is needed to do this methods. The meaning of this study is to specify the privacy and stress of the inpatients in multi-bed patients' room.

A Study on Planning for Elderly Hospital's Atrium on the base of Users Demand (사용자요구기반에 의한 노인전문병원 아트리움계획 연구)

  • As the number of aged people increase, hospitals specializing in elderly cares have also increased very rapidly. Therefore it is necessary to improve the quality of these hospitals for the elderly people in order to enhance the function of these hospitals not only for conventional treatment but also for recovery and convalescence. To study such trend and needs, this study researched on planning for Elderly Hospital's Atrium that provides a space for the elderly people so that they may enjoy the natural environment while being supported from diverse aspects for long term cares and sustained living, and tried to find out design solutions based on users' demand through questionnaire survey and interviews both with the management of the elderly hospital and the inpatients or the users. As a result of this study, it was revealed that each user showed his or her own respective characteristics concerning his or her expectation about the environment of their using spaces, and according to this result the strategic direction and design plan could be drawn. Based on plans for this Elderly Hospital's Atrium specializing in cares for the elderly people, useful information will be provided for caring, treatment and recovering environment of the elderly hospitals in future considering not only the conventional medical and technical services but also the inpatients' or the users' physical, psychological and social factors.

  • Purpose: There have been more researches focused on eco-friendly construction practices in order to save energy consumed and these practices have been extensively applied for constructing buildings. However, researches on energy consumption saving and efficiency for hospital facilities have been still insufficient. This research aims to draw factors that affect energy efficiency of hospital buildings through literature reviews and perform an expert-survey using AHP(Analytical Hierarchy Process) method in order to analyze the importances and priorities of these energy efficient factors. Method: Using the AHP method, this paper suggests the importances and priorities of factors to affect energy efficiency in hospital buildings. The survey of experts at a architectural design and a construction management companies was conducted via e-mail and mail. Result: As a result, factors of window and door, insulation, ventilation, and natural lighting turned out relatively important as respectively 0.104, 0.102, 0.101, and 0.095 n the energy efficiency in hospital buildings, while factors of artificial lighting, geothermal, solar heat, and control did unimportant as 0.027, 0.033, 0.043, and 0.053.

  • Alloplastic total replacement of the temporomandibular joint (TMJ) was developed in recent decades. In some conditions, previous studies suggested the rationale behind alloplastic TMJ replacement rather than reconstruction with autogenous grafts. Currently, three prosthetic products are available and approved by the US Food and Drug Administration. Among these products, customized prostheses are manufactured, via computer aided design/computer aided manufacturing (CAD/CAM) system for customized design; stock-type prostheses are provided in various sizes and shapes. In this report, two patients (a 50-year-old female who had undergone condylectomy for the treatment of osteochondroma extending to the cranial base on the left condyle, and a 21-year-old male diagnosed with left temporomandibular ankylosis) were treated using the alloplastic total replacement of TMJ using stock prosthesis. The follow-up results of a favorable one-year, short-term therapeutic outcome were obtained for the alloplastic total TMJ replacement using a stock-type prosthesis.

Recent increase of human granulocytic anaplasmosis and co-infection with scrub typhus or korean hemorrhagic fever with renal syndrome in Korea

  • We report 17 patients with human granulocytic anaplasmosis between January 2015 and September 2018 at two tertiary university hospitals in Korea. Monthly incidence peaked in May and June. Among these patients, we identified three who were co-infected with scrub typhus, and one patient with hemorrhagic fever with renal syndrome.
