The Effect of Aroma Inhalation Therapy on Nausea, Vomiting, Abdominal Discomfort, Compliance and Colon Cleansing in the Preparation of Patients Undergoing Colonoscopy (아로마 흡입요법이 대장내시경 검사 대상자의 오심, 구토, 복부 불편감, 순응도 및 대장 정결도에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of aroma inhalation therapy on nausea, vomiting, abdominal discomfort, compliance, and colon cleansing for patients having a colon lavage solution before colonoscopy. Methods: This study was performed using a nonequivalent control group post-test design. The study was conducted from 20 Jun to 29 Aug, 2011. The participants were 152 patients (experimental group: 77, control group: 75) who underwent colonoscopy at A hospital, Seoul. Results: The experimental group with the aroma inhalation therapy reported significantly lower nausea and vomiting than did the control group. There were no significant differences in abdominal discomfort, compliance, and colon cleansing between experimental and control group. Conclusion: Use of an aroma inhalation therapy was efficient in nausea and vomiting among patients having a colon lavage solution before colonoscopy. Therefore, aroma inhalation therapy is recommended as an effective nursing intervention for relieving nausea and vomiting among patients having a lavage solution before colonoscopy.

A study on the antihypertensive effect and mechanism of Siegesbeckia pubescens aqua-acupuncture treatment in spontaneously hypertensive rats (희염약침자극이 자발성(自發性) 고혈압(高血壓) 흰쥐의 혈압(血壓)에 미치는 영향(影響) 및 강압기전(降壓機轉)에 대(對)한 연구(硏究))

  • In order to investigate the antihypertentive effects and its mechanism of the Siegesbeckia pubescens aqua-acupuncture treatment, experiments were performed on immediate and continuous antihypertensive effects, vasodilatation-autonomic nerve block, diuretic activity, and angiotensin converting enzyme inhibitory activity. The results were as follows; 1. Acupuncture treatment group showed significantly immediate antihypertensive effects in 4 hours after treatment. Normal saline aqua-acupuncture treatment group showed significantly immediate antihypertensive effects in 2, 4 and 6 hours after treatment. Siegesbeckia pubescens aqua-acupuncture treatment group showed significantly immediate antihypertensive effects in 30 minutes, 1, 2, 4 and 6 hours after treatment. 2. Siegesbeckia pubescens aqua-acupuncture treatment group showed significantly continuous antihypertensive effects in 6, 8, 9 and 10 days after treatment. 3. Siegesbeckia pubescens aqua-acupuncture extract solution showed significantly vasodilatatory and sympathetic nerve block effects with concentration of $10^{-6},\;10^{-5},\;10^{-4},\;10^{-3}g/m{\ell}$. 4. Oral administration group of the Siegesbeckia pubescens aqua-acupuncture extract solution didn't show no significantly diuretic effects. 5. Siegesbeckia pubescens aqua-acupuncture extract solution showed 28.8% angiotensin converting enzyme inhibitory activity.

  • An experimental study was performed to evaluate the efficacy of nerve growth factor(NGF) on inferior alveolar nerve in a rabbit model. In 20 New Zealand white rabbits, 14mm of bilat eral alveolar nerve were resected and the defects were repaired with the 17mm silicone conduits. In group I, 5mm autologous nerve segment were located centrally in the left side after tubulization and NGF solution(Sigma chemical 0.1 mg/mL) was instilled into each conduit. In group II, nerve repair modality was the same but no NGF solution was instilled into the conduits. On 2 months and 6 months postoperatively, the results were evaluated by clinical and hist omorphometric assessment. The result was that autologous nerve segment group show the best nerve regeneration effect and NGF instilled group the worst.

Dynamic Research of a Potential Carrier for Hydrophobic Compound Model Pyrene Using Amphiphilic Peptide EYK

  • In recent years, the study of self-assembly peptide used in drug delivery system has been attracted great interest from scientists. In the category are self-assembly peptides in the structure either with one hydrophobic surface and another hydrophilic or a hydrophobic head and a hydrophilic tail. Here, we focus on a novel designed peptide EYK with double amphiphilic surfaces, investigating on the capability of peptide as a carrier for hydrophobic compound model pyrene. The fluorescence data presented the dynamic process of the transfer, showing that the pyrene was in the crystalline form in peptide solution, and molecularly migrated from its peptide encapsulations into the membrane bilayers when the peptide-pyrene suspension was mixed with liposome vesicles. The results indicated that the peptide EYK could stabilize hydrophobic pyrene in aqueous solution and delivered it into EPC liposome as a potential carrier.

Treatment for Acute Stage Complex Regional Pain Syndrome Type II with Polydeoxyribonucleotide Injection

  • Complex regional pain syndrome (CRPS) type II is a syndrome that develops after nerve injury. Symptoms may be severe, and vary depending on the degree of sympathetic nerve involvement. As yet, there is no satisfactory treatment. We report the case of a female patient who had an L5 left transverse process fracture and an S2 body fracture, who developed symptoms of CRPS type II in her left lower leg that were aggravated during ambulation in spite of absolute bed rest for one month after the trauma. Several treatments, including bed rest, medication, and numerous nerve blocks were attempted, but the pain persisted. We finally tried injection of polydeoxyribonucleotide (PDRN) solution at the left L5 transverse process fracture site because we knew of the anti-inflammatory effect of PDRN. One day after this treatment, her symptoms had almost disappeared and three days later, she was discharged. We will also further discuss the possibility of using PDRN solution for the treatment of CRPS.

A Comprehensive Study on Patient Flow Improvement Solutions and Their Implementation Strategies in an Outpatient System (대형 병원 외래 시스템의 환자 흐름 개선방안의 적용 전략에 관한 연구)

  • There are various ways to manage the patient flow of the hospital outpatient system. However, it is difficult to apply many implementation solutions to the real outpatient system at once. Because first, the expected effects of each different solution are very much depend on the real situation of the system and applied other solutions, and second, owing to the limited resources, each solution should be implemented according to the priority. In order to overcome these difficulties, this paper focuses on proposing the comprehensive subset of implementation solutions, which is one of the most effective among various kinds of subsets, and verifying the effects of it. The comprehensive subset of solutions is derived from conducting design of experiments and simulation which determine the optimum set of different solutions and analyze the particular interactions and priority order among them. This implementation strategy can solve the difficulties of applying different kinds of various solutions to the hospital outpatient system.

Comparison of Whole versus Split-Dose PEG Solution for Colonoscopy Preparation on Patient Compliance, Quality of Bowel Cleansing, and Endoscopist's Satisfaction (대장내시경 정결제 복용방법에 따른 대상자 순응도, 대장정결 효과, 및 시술자 만족도)

  • Purpose: This study was a randomized single-blind trial of whole versus split-dose PEG solutions for colonoscopy preparation to compare the patient compliance, quality of bowel cleansing, and endoscopist's satisfaction. Methods: The participants were recruited from outpatients who planned to receive colonoscopy of C hospital in Busan. Sixty participants were randomly assigned to receive either a spit-dose group(n=30) consuming $2{\ell}$ of PEG solution twice, or a whole-dose group(n=30), consuming $4{\ell}$ of PEG solution once. These participants completed the questionnaire to assess their compliance before colonoscopy. The quality of bowel cleansing was assessed using the Ottawa Scale with the endoscopist who was blinded to the type of preparation, and their satisfaction by using VAS. Results: The participants who did not completely consume $4{\ell}$ of PEG solution were less in split-dose than in whole-dose group (0% vs 13.3%). The split-dose group complained less about abdominal pain(t=2.644, p=0.009) and abdominal bloating(t=2.802, p=0.013) with a statistical significance. For the quality of bowel preparation, there were no significant differences in the bowel cleansing scores and the endoscopist's satisfaction between two groups. Conclusion: Colonic preparation with split-dose of PEG solution could be a more useful method for better patient compliance, with no significant impact on bowel cleansing quality.

A Case Report of Korean Medicine Treatment for A Patient with Variant Angina and Asthma Symptoms (천식 증상을 가진 변형 협심증 환자에 대한 한방치료 증례보고)

  • Objective: The purpose of this case study was to report the symptom improvement effect of integrative Korean medicine treatment based on Pyongsim Solution: Cheonwangbosim-dan in a patient with variant angina and asthma symptoms. Methods: This 68-year-old male patient was treated with Pyongsim Solution, Gamihyangsayukgunja-tang, Gwakhyangjunggi-san, acupuncture, pharmacopuncture, and cupping therapy for 24 days. The European Quality of Life-5 Dimensions-5 Levels profile, Numerical Rating Scale score, and pulse rate per minute were measured on admission, at two weeks of hospitalization, and upon discharge to assess the clinical outcomes. Results: After treatment, the "mobility" dimension profile of the European Quality of Life-5 Dimensions-5 Levels changed from 1 to 1, "self-care" changed from 1 to 1, "usual activity" changed from 3 to 2, "pain/discomfort" changed from 4 to 3, and "anxiety/depression" changed from 3 to 2. The Numerical Rating Scale score for chest pain decreased from 7 to 5, chest tightness decreased from 6 to 4, sleep disturbance decreased from 6 to 3, polydipsia decreased from 5 to 3, and stomach discomfort decreased from 5 to 3. All the profiles of these five dimensions were unchanged for 11 days in the period between the two weeks of hospitalization and discharge. There were also no changes in the profiles of either "mobility" or "self-care" throughout the entire 24-day hospitalization period. Conclusion: This case report shows that integrative Korean medicine treatment based on Pyongsim Solution can improve the symptoms of patients with variant angina and mild intermittent asthma, suggesting that Pyongsim Solution could be an important herbal medication for treatment.

Effect of Oxygenation of Cardioplegic Solution on Postischemic Recovery of Cardiac Function after Ischemic Arrest in Isolated Rat Heart[II] - Oxygenation of Cardioplegic Solution and its Consequent pH Change - (백서의 적출된 심장에서 심정지액의 산소화가 허혈성 심정지후 심기능 회복에 미치는 영향[II])

  • The hypothesis tested is that shifts in pH, induced when a cardioplegic solution is oxygenated, can be detrimental. The object of this study is to evaluate the effect of the pH of the oxygenating cardioplegic solution on postischemic recovery in the isolated rat heart. Either 100% oxygen or 95% oxygen: 5% carbon dioxide was added to the cardioplegic solution[St. Thomas` Hospital No. 2] and determined postischemic recovery of isolated rat hearts after 2 hours and 3 hours of 20oC cardioplegic protected ischemia. Heart were arrested and reinfused every 30 minutes throughout the ischemic period with cardioplegic solution. When 100% oxygen was added, the pH of the cardioplegic solution increased from 7.8[no oxygen] to 8.5[100% oxygen] without any change in postischemic functional recovery. But when 95% oxygen ; 5% carbon dioxide was added, the pH of the cardioplegic solution reversely decreased to 6.84 in the 2-hour ischemic group and 6.73 in the 3-hour ischemic group, associated with improved postischemic functional recovery. After 2-hour ischemia, systolic pressure improved from 88.2$\pm$3.7%[no oxygen] and 88.7$\pm$3.8%[100% oxygen] to 96.6$\pm$1.8%[95% oxygen : 5% carbon dioxide], p<0.05, aortic flow from 43.3$\pm$3.1% and 38.4$\pm$10.6% to 74.5$\pm$5.0%, p<0.001, cardiac output from 55.5$\pm$4.6% and 47.4%$\pm$10.6% to 73.1$\pm$4.6%, p<0.05, stroke volume from 62.7$\pm$4.6% and 52.0$\pm$10.1% to 77.2$\pm$4.6%, p<0.05, and dP/dT from 59.3$\pm$7.2% and 56.7$\pm$7.6% to 78.9$\pm$4.6%, p<0.05. The infused amount of the cardioplegic solution during 2-hour ischemic period was similar in three groups. After 3-hour ischemia, cardiac output improved from 17.0$\pm$3.8%[no oxygen] to 45.9$\pm$7.5%[95% oxygen: 5% carbon dioxide], p<0.05, and stroke volume from 21.0$\pm$3.9%[no oxygen] to 50.1$\pm$6.6%[95% oxygen: 5% carbon dioxide], p<0.01. In conclusion, the St. Thomas` Hospital No. 2 cardioplegic solution should be oxygenated but with 95% oxygen: 5% carbon dioxide and not 100% oxygen because of the additive effect of a relatively "Acidotic" pH.t; pH.

  • A real-time measurement and control system was developed, This system is used for nurses at hospitals to check the residual quantity and changing time of Ringer's solution in nurses' room. Load Cell is utilized as a sensor to check the residual quantity of Ringer's solution, This Load Cell detects the physical changes of Ringer's solution and transfers electronic signal to the amplifier. Amplified analog signal is converted into digital signal by NO converter. Developed Embedded system, which computes these data with microprocess(8052) then makes it possible to monitor the residual quantity of Ringer's solution real-time on a server computer.
