자궁 체부에 발생한 중신 유사 선종의 영상 소견: 증례 보고 (Radiologic Findings of Mesonephric-Like Adenocarcinoma of the Uterine Corpus: A Case Report)

  • 중신 유사 선종은 2020년 World Health Organization 분류체계에서 자궁내막암의 한 아형으로 새롭게 분류되었고, 드물기 때문에 잘 알려지지 않은 질환이다. 저자들이 아는 한, 영어 문헌에서 영상의학적 소견은 아직 보고된 바 없다. 자궁의 중신 유사 선종은 일반적인 자궁내막암에 비해 예후가 좋지 않고 더 공격적인 생물학적 양상을 보인다. 저자들은 65세 여성의 자궁 체부에 생긴 중신 유사 선종의 영상의학적 소견에 대하여 보고하고자 한다. 자궁내막 고형 종괴가 심부 자궁근 침범을 보였고, 조영 증강은 잘되지 않았고 중등도의 확산제한을 보였다.

  • In today's hospital-management, information system and the information by it will be a necessary administrative materials of organization with man-power, funds, equipment and resources. This study is for figuring out steps to utilize information-system, an important strategy to grow the hospital applying to the change of surroundings, improving the service for patients, accelerating medical examination, improving it in quality and suggesting a basis be able to manage it effectively by putting information system in practice such as economic effect by shorting time and the effect of medical study by computerizing clinical data. And the result of it is as follows: From the Nolan's 6 step - model about the degree using information system in hospital-management, the 1st group of hospital-size came to 1st and 2nd step, the 2nd group to the 3rd and 4th step and the 3rd group to the 5th and 6th step, which correspond to the survey- results that information system increases with the size of the hospital to propagation, regulation and integration step and that the number of the hospital developing to the step managing information is increasing.

Dexamethasone Disrupts Cytoskeleton Organization and Migration of T47D Human Breast Cancer Cells by Modulating the AKT/mTOR/RhoA Pathway

  • Background: Glucocorticoids are commonly co-administered with chemotherapy to prevent drug-induced allergic reactions, nausea, and vomiting, and have anti-tumor functions clinically; however, the distinct effects of GC on subtypes of tumor cells, especially in breast cancer cells, are still not well understood. In this study, we aimed to clarify the effect of GC on subtypes of T47D breast cancer cells by focusing on apoptosis, cell organization and migration, and underluing molecular mechanisms. Materials and Methods: The cell scratch test was performed to observe the cell migration rate in T47D cells treated with dexamethasone (Dex). Hoechst and MTT assays were conducted to detect cell survival and rhodamine-labeled phalloidin staining to observe cytoskeleton dynamics. Related factors in the AKT/mTOR pathway were determined by Western blotting. Results: Dex treatment could effectively inhibit T47D breast cancer cell migration with disruption of the cytoskeletal dynamic organization. Moreover, the effect of Dex on cell migration and cytoskeleton may be mediated by AKT/mTOR/RhoA pathway. Although Dex inhibited T47D cell migration, it alone may not induce cell apoptosis in T47D cells. Conclusions: Dex in T47D human breast cancer cells could effectively inhibit cell migration by disrupting the cytoskeletal dynamic organization, which may be mediated by the AKT/mTOR/RhoA pathway. Our work suggests that glucocorticoid/Dex clinical use may prove helpful for the treatment of breast cancer metastasis.

리더십 교육훈련 프로그램 학습의 현장 전이 비교 연구 : 병원 의사와 기업 관리자를 중심으로 (Comparison Study for Learning Transfer Factors of the Leadership Training Program in Different Types of Job : Focused on Physicians in Hospitals and Managers in Firms)

  • This paper is a comparison study about leadership training transfer factors between physicians working in large scale hospitals and managers working in firms. To fulfill this purpose, this study conducted a regression analysis on 101 managers and 59 physicians who had attended similar leadership training programs more than 16 hours recently in order to identify the differences on the learning transfer factors. 6 factors such as Learner readiness, Performance self-efficacy, (so far as Trainee Characteristics group), Organization Culture, Supervisor's tangible incentives and Supervisor's intangible support, (so far as Work environment group), Content Validity & Transfer Design (so far Training Design group) were used as independent variables while the personal Managerial Capability Increase and Leadership Capability Increase were used as dependent variables. And also we used 5 factors as control variables ; Job style (Manager or Physician), Age, Gender, Working years and Organization size. Here are the summary of major findings ; first, there were statistically significant differences between the learning transfer factors in leadership training programs for managers and those of physicians. Second, there were also statistically significant differences among trainees' working years and their organization size factors while age and gender do not affect the learning transfer factors. Third, for the physician's leadership training the practitioners should focus on two factors ; Organization Culture and Learner readiness.

진단검사의학과의 검사기능과 공간구성에 관한 연구 - 1000병상급 종합병원을 중심으로 (A Study on the Laboratory Function and Spatial Organization for Laboratory Medicine - Focused on the Tertiary Level General Hospitals having more than 1000 Beds)

  • Purpose: As the structure of the disease has been changed and the infectious disease has been increased, the demand for diagnostic examination has been increasing. So, the department of laboratory medicine in hospital has playing the important role accounting for about nineteen percent of total medical expenses in korea. This study is to investigate the laboratory function and spatial composition of the department and figure out the space area and space organization. Methods: Explore the literature review to identify the laboratory function. Limit to five cases of tertiary level general hospital having about a thousand bed and analyze the space layout and floor area to confirm the spatial composition. Classify the exam function and check the spatial composition and spatial organization. Results: This study allows 5 conclusions to be summarized. Laboratory medicine divided into nine part in regulation, but in space allocated into core lab, emergency lab and six part lab. Total laboratory area is $2,036m^2$ in average, and is composited with 60% for lab, 17% for office and 20% for public. Lab area per a bed presents $1.88m^2$. Microbiolgy and molecular lab area are getting large. Laboratory space organized into the four zone, like an entrance zone, core zone, rear zone and peripheral zone. Emergency and transfusion lab are allocated in entrance zone, hematology chemistry and immunology in core lab, microbiology and molecular lab in rear, support offices in peripheral zone. The most important point was to check the spatial composition of the Laboratory Medicine according to the inspection function. Implications: This study can be used as a useful data in planning and designing a Laboratory Medicine Department.

서비스본질기반 조직의 직무만족 및 몰입에 관한 연구 : 치과병원에서 노사관계와 참여적 작업조직의 매개를 중심으로 (A Study on Job Satisfaction and Commitment of Service Nature Based Organizations: Centering on Labor Relations and Mediation of High-Involvement Work Organizations in Dental Hospital)

  • 본 연구는 의료기관인 치과병원의 서비스본질이 직무만족 및 직무몰입에 미치는 영향을 실증적으로 분석하였다. 최근 치과 병원이 의료에 대한 수요가 점차 증가하면서 신설과 확장을 통해 다양한 경쟁상황에 직면하고 있어 치과병원의 직무만족과 직무몰입의 중요성이 더욱 강조되고 있는 실정이다. 치과병원의 서비스본질은 직무만족과 직무몰입에 영향을 미치는 것으로 알려져 있는데, 이 두 요인은 치과 병원의 생존과 발전을 위해 요구되는 병원 본연의 기능뿐만 아니라 치과 병원의 경쟁력을 확보하기 위한 핵심 요인으로 평가되고 있다. 따라서 본 연구에서는 치과 병원의 서비스본질에 영향을 주는 조직만족과 조직몰입에 관한 요인들에 대하여 노사관계와 참여적 작업조직을 매개변수로 효과 분석을 수행하였다. 분석 결과 서비스본질기반 조직에서는 직무만족이 직무몰입에 유의한 영향을 끼치는 과정에서 노사관계와 참여적 작업조직 없이는 성립되지 않는 완전매개효과가 있는 것으로 나타났다. 따라서 인센티브나 외적 보상에만 집중하여 직원들의 직무만족을 높이려는 기존연구 결과와 달리, 본 연구에서는 서비스본질 요소들이 반영된 내적 지원체계를 강화할 필요가 결론을 도출하였다. 본 연구에서 투입된 변수인 직무만족과 직무몰입은 치과병원의 경쟁력 향상을 위한 기초가 될 수 있을 것으로 기대한다. 향후 본 연구를 중소 치과병원이나 치과 의원들을 대상으로 범위를 확장할 필요가 있다. 또한 관계성이나 쌍방향성과 같은 서비스본질을 조직차원에서 강화하는 것이 서비스조직의 경쟁력을 강화하는 효과적인 방법임에 대해 다양한 조직을 대상으로 입증하는 연구가 필요하다.

통원수술센터의 평면계획에 관한 연구 (A Study on the Planning of an Day Surgery Center)

  • As a functionally complicated facility, it requires rational strategy to accommodate multi-functional needs to plan a hospital. Ambulatory surgery units located in large hospitals have problems such as longer waiting time and congestion for patients due to complicated planning and organization within limited area, which raise needs for independent ambulatory surgery centers to enable smoother medical treatment for patients. To design an independent ambulatory surgery center, understanding of zoning and layout of each unit needs is particularly required. This study focuses on research of efficient zoning/organization of day surgery unit and circulation arrangements of ambulatory surgery centers by analyzing various examples abroad, which will eventually help finding appropriate way of planning domestic ambulatory surgery centers. Specific area of research includes ambulatory surgery process, programmatic requirements, spatial organization of the facility, layout of surgery unit and circulation arrangements of ambulatory surgery centers.

간호사의 감정노동과 소진, 이직의도와의 관계에서 자기효능감과 사회적 지지의 역할 (The Role of Self-Efficacy and Social Support in the Relationship between Emotional Labor and Burn out, Turn over Intention among Hospital Nurses)

  • Purpose: The purpose of this study was to investigate the role of Self-efficacy and Social Support in the emotional labor among hospital nurses. Method: Data was collected from 389 nurses in 3 general hospitals by means of structured questionnaires. Data was analyzed with descriptive statistics, Pearson's correlation coefficient and hierarchial multiple regression using the SPSS 14.0 program. Result: (a) self-efficacy of hospital nurses had no significant moderating and mediating effects between emotional labor and burn out, turn over intention. (b) Social support of hospital nurses had significant mediating effects between emotional labor and burn out, turn over intention. However, social support did not show moderating effects. Conclusion: Social support was a more important predictor of emotional labor than self-efficacy of hospital nurses. This finding suggests the importance of social support to increase hospital nurses psychological well-being and development of organization.

병원원가계산 실태의 국제적 동향과 시사점: 미국.영국.일본 사례를 중심으로 (A Comparative Study of Recent Development of Hospital Cost Accounting in UK, US, and Japan)

  • Given the pressures to promote the efficient utilization of medical resources, hospitals have developed cost accounting systems in several countries. This study discusses the recent development and problems of hospital cost accounting practices in three countries: UK, US, and Japan. first, we discuss a cost accounting structure and detailed pictures of costing practices. Second, problems of current systems arc reviewed and then possible remedies are discussed. Third, we provide implications for implementing the systems(especially ABC). finally, we assert that infrastructure(hospital information systems, database, etc.) must be established and the target level of costing has to be considered before organization-wide application.

병원조직의 경력관리활동이 행정직종사자들의 고용불안 심리에 미치는 영향 (Effect of Career Management Activity in Hospital Organization on the Job Instability for Hospital Administrative Staff's)

  • 본 연구는 병원의 조직차원 경력관리활동이 병원행정직 종사자들의 고용불안 심리에 어떠한 영향을 미치는지를 파악하여, 내 외부 고객과의 상호작용을 강화하기 위한 조직차원의 효율적인 경력관리방안을 제시하고자 하였다. 연구대상은 부산권역에 소재한 종합병원 7개, 병원 7개, 총 14개 병원의 행정직종사자 192명을 대상으로 하였으며, 연구방법은 SPSS통계프로그램을 이용하여 요인분석 및 신뢰도 검정, 회귀분석, 상관분석을 실시하였다. 조직차원의 경력관리활동은 타당성과 내적일관성 검증결과 직접적인 경력관리활동과 간접적인 경력관리활동으로 구분되었으며, 고용불안 심리는 직접적인 경력관리활동에 의해 유의한 영향을 받는다는 것을 알 수 있었다. 그리고 조직차원의 직접적인 경력관리활동 중 '조직구성원들의 경력을 위해 필요한 훈련을 받을 수 있는 대책강구'가 고용불안 심리와 가장 강한 상관관계가 있었으며, '새로운 직무기회에 관한 충분한 정보제공' 과 '직원들을 참여시켜 경력관리 계획과정 개발'도 상대적으로 높은 상관관계를 가지고 있었다. 따라서 고객과의 상호작용을 강화하기 위한 조직차원의 효율적인 경력관리방안은 경력관리 대책 수립 시 조직구성원들을 참여시켜는 것이 바람직함을 시사하였다.
