Determinants of Satisfaction in the Usage of Healthcare Information Systems by Hospital Workers in Hyderabad, India: Neural Network and SEM Approach

  • This study focuses on the adoption of Healthcare Information System (HIS) in India's healthcare services, which has led to an increased use of HIS software for managing patient information in hospitals. The study aims to evaluate the factors that influence hospital workers' satisfaction with HIS usage and its impact on their intention to continue in the use of HIS. Primary data was collected through a survey questionnaire from 265 hospital workers. A new framework was developed, and Structural Equation Modeling (SEM) was used for analysis. Sensitivity analysis was also conducted on demographic data using an Artificial Neural Network (ANN) approach. The results indicated that all hypotheses were significant (p < 0.05). Effort expectancy was the most significant factor influencing hospital workers' satisfaction (p < 0.01). Sensitivity analysis showed that education (Model-A) and experience in use of HIS (Model-B) were the most important factors. The study contributes by proposing a new theoretical framework and extending the previous research on HIS usage satisfaction. Overall, the study highlights the importance of easiness and usefulness in predicting HIS usage satisfaction.

  • The objectives of the study were to explore the effects of personal factors, organizational factors, and system factors on satisfaction on hospital information system among hospital nurses, and to provide suggestions for improving usability of information system and promoting business efficiency and system satisfaction by evaluating hospital information system. The study sample group consisted of 233 nurses from general hospitals. The collected data were analyzed using the Hierarchical Regression. The results suggested that system quality, information quality, and service quality showed positive and significant effects on hospital nurses' satisfaction. The organizational factors and personal factors were not statistically significant controlling for the system factors. This study was significant in that direct relationships between sub-dimensions of hospital information system and organization's performance, measured in nurses' perceived evaluation were empirically explored. Accordingly, the application of hospital information system could provide a direction for establishing a systematic strategy to improve the work performance of hospital nurses.

  • As the adoption of PACS and hospital information system among university hospitals and hospital level institutions grows bigger, the need of sharing and transferring medical information among medical institutions is rising. For the medical information, which is saved in the hospital medical system, to be transferred within the same hospital, domestic, or foreign medical institutions, a standard protocol is necessary. But realistically, most of the domestic hospitals do not abide by H7L which is the HIS standard and so, information transferring is not possible as of present. As such, the purpose of this research is to implement the information between HIS and PACS to an international standard by constructing HL7 messages through HL7 Interface. which will eventually make possible information transferring between different hospitals. Our research team has developed a method which will make the PACS equip hospitals that do not follow HL7 standard which will make possible to transfer information between HIS and PACS through HL7 Message. By constructing message files, which follow the form of HL7 Message in the HL7 Interface, they can be transferred to PACS through the ftp protocol. The realization of the HIS/OCS Interface through HL7 enables data transferring between domestic and foreign medical institutions possible by implementing the international standard in the PACS and HIS data transferring process. The HL7 that our research team has developed made patient data transfer between medical institutions possible. The Interface is for a specific system model and in order for the data transfer between different systems to be realized, interfaces that are fit for each system must be needed. If the interface is improvised and implemented to each hospital's information system, the data sharing among medical institutions can be broadened.

A Case Study of Implementation of Concurrent Drug Utilization Review System at a General Hospital (동시적 의약품 사용평가(cDUR) 시스템 구축 및 적용 사례 연구 : 국내 한 대학병원을 중심으로)

  • Medical errors such as adverse drug event, improper transfusion, wrong-site surgery, mistaken patient identity and so on commonly occur at health care practice. Information technology, like Drug Utilization Review(DUR) system which reviews, analyzes, and interprets medication data when prescribing, can play a key role in reducing such medical errors and improving patient safety. Korean Government has guided all hospitals to implement concurrent DUR(cDUR) system, which is the first case worldwide in that all healthcare providers have to use cDUR system when prescribing. This paper introduced a case study that a tertiary hospital has integrated the cDUR system into its comprehensive Hospital Information System(HIS) and analyzed the whole prescription data during a week right after adoption of cDUR system. Considering technical strength and weakness, the cDUR system was integrated into the HIS, using Broker Servers for minimizing doctors' anxiety. As the quantitative analysis of the whole prescription data, DUR conflict events, which mainly included duplicate medications and contra-indicated drug interactions for outpatients, were 2.77%. Although only 0.7% is for the contra-indicated drug interactions, it will be greatly devoted to achieve the purpose of DUR such as improving patient safety.

  • This study is a proposal of quality scale and measurement for the Mobile Hospital Information System Usability used to the Chungnam National University Hospital in Daejeon Korea. This study is to provide decision- making guideline for hospital managers and to provide feedback for the users' information needs in Mobile Hospital Information System. The target people of the study were medical doctors in Chungnam National University Hospital. The service contexts of usability were request of medical Care, Vital sign check of Patient, Care Agreement of Patient, Blood management, Check of care state, Sign of choice doctors, Query of doctor order and patient measurement result. The Usability factors were the efficiency, effectiveness and satisfaction of quality model ISO9241-11. This study shows that the Mobile HIS is used for the communication and education between doctor and patients. Especially, The Care Agreement of Patient is a part of the most utilized in the Mobile HIS.

  • Precision Medicine, which utilizes personal health information, genetic information, clinical information, etc., is growing as the next-generation medical industry. In Korea, medical institutions and information communication companies have coll aborated to provide cloud-based Precision Medicine Hospital Information Systems (P-HIS) to about 90 primary medical ins titutions over the past five years, and plan to continue promoting and expanding it to primary and secondary medical insti tutions for the next four years. Precision medicine is directly related to human health and life, making information protecti on and healthcare information protection very important. Therefore, this paper analyzes the preliminary research on inform ation protection models that can be utilized in cloud-based Precision Medicine Hospital Information Systems and ultimately proposes research on ways to improve information protection in P-HIS.

Factors Affecting Use Satisfaction with Hospital Information System of Hospital Administrator (병원행정직원의 병원정보시스템 이용만족도에 영향을 미치는 요인)

  • This study was designed to identify satisfaction with Hospital Information System(HIS) and the factors affecting them in hospital administrators. The data was collected for 24 days from Mar. 02 to 25, 2011. Among a total of 82 cases of the questionnaires, only 76 cases were used. From the survey, it was made known that there was statistically significant difference in speed, preciseness and stability of environmental factors depending on the type of hospitals while significant difference was found in stability by number of beds. Among the factors of system operation, the administrational method affected speed, preciseness, stability and convenience statistically significantly whereas, however, preciseness, stability and convenience became significantly different depending on the number of staff members. As for the factors that could exercise influence upon satisfaction with administrative service of the HIS, it was revealed that statistically considerable impact could be caused under management of own EDPS expert as well as under comanagement by the medical institution and professional business entity in case of hospital.

  • IT investments of hospitals are growing exponentially and various information systems such as OCS (Order Communication System), EMR (Electronic Medical Record), PACS (Picture Archiving and Communication System), ERP (Enterprise Resource Planning) have been implemented at a number of hospitals in Korea. We have surveyed and analyzed the current status of hospital information systems of major hospitals in Korea, and proposed an information strategy for e-Hospital implementation. Firstly, implementation status of major subsystems of HIS such as OCS, PACS, and EMR has been surveyed and types of IT personnel management have been examined. Based on the field survey result, an information strategy for e-Hospital implementation has been proposed, that can be referenced by hospitals to build their own information strategy. We expect that the study result can contribute to understanding the present status and issues of HIS and information strategy planning of hospitals.

  • Tn the modern integrated healthcare environment, digital technology plays a critical role in the success of its operation. For overcoming the deteriorated healthcare management environment is to improve business service ability, cost and management efficiency through information processing of the business service process and the decision support system. For a solution of these problems, the Hospital Information System(HIS) was introduced. In this paper we proposed HIS system through the HL7-based with legacy system that improves both the efficiencies of medical of office and medical treatments. The implementation of HIS system enables combination of legacy systems including HL7 based OCS and PACS. And we proved the effective HIS development method provided by analysis of total module.
