  • 병원 경쟁의 심화와 의료시장 개방에 시달리고 있는 한방병원이 논리적이고 체계적인 한방 병원 시스템 구축은 매우 중요한 일이다. 디지털 한방 병원 구축은 의료 서비스 향상과 병원 관리의 효율성을 최대화시키기 일이다. 본 논문은 한방의료 분야와 IT기술의 접목하여 디지털 한방병원 구축을 위한 유무선 기반의 한방의료 CRM 시스템을 개발한다.

여성전문병원의 고객관계관리(CRM)가 고객충성도에 미치는 영향 (Impact of Customer Relationship Management on Customer Loyalty among Patients Visiting a Woman's Hospital)

  • Although a variety of customer relationship management (CRM) activities have been provided by many hospitals in Korea, there lacks empirical evidence on the effect of CRM. The present study was conducted to examine the effect of CRM in terms of the customer response to CRM in a woman's hospital setting. A total of 380 patients receiving inpatient or outpatient care from woman's hospital between October 25 and November 4, 2005 were surveyed for the degree of their experience of and preference for CRM activities of the hospital by 5-point Likert-type scale. Patients were also asked about the level of customer loyalty to the hospital. Eighteen CRM activities offered by the hospital was classified into 4 types of CRM strategies according to Berry and Parasuranman: price, social, structural, and relationship recovery strategy. There's a significant positive correlation between the degree of experience of CRM and preference for CRM(r=0.49, p<0.001). Regression analysis results showed the significant positive relationship between the degree of experience of CRM and customer loyalty(${\beta}$=0.448, p<0.05). Among the 4 CRM strategies, only social(${\beta}$=0.127, p<0.05) and structural strategy(${\beta}$=0.266, p<0.05) showed signifiant positive relationship with customer loyalty. Overall, the favorable customer response to CRM in terms of preference for CRM and customer loyalty indicates that there's a positive effect of CRM on the continuity of the relationship between patients and health care providers.

  • 본 연구는 병원고객관리를 위한 의료용 CRM 시스템의 설계를 목적으로 한다. 본 CRM 시스템은 병원에서 제공하는 정보 및 세미나를 필요로 하는 환자들의 우선순위 검색과 이를 통해 검색 결과 내의 인원별, 지역별 등의 연관 검색이 가능하도록 하였다. 고객은 병원에서 발송한 메시지를 받고, 세미나에 참여의사를 밝히는 메시지를 병원에서 실시간으로 출력하고 이를 관리한다. 이를 통해 병원에서는 고객이 참가할 세미나의 참여도를 파악하고, 세미나에 적극적으로 참여하는 환자들을 별도로 관리하여 향후 세미나 개최 시 우선적으로 서비스를 제공할 수 있도록 설계하였다.

  • Decision Support Systems (DSS) is an Information Systems (IS) application that aids in decision-making processes for many business concepts and Customer Relationship Management (CRM) is one of them and it depends on the firm's tasks for developing and retaining customers while achieving their satisfaction and enhancing the sense of belongingness for their products and services. Profit maximization, the process of customer value, and building strategic values for the firm are the three empirical benefits of CRM that are achieved through analytical, operational, and direction (AOD) capabilities respectively. This research focuses on the application of DSS models of what-if analysis (WIA) for CRM at (AOD) and also shows the dependence on the Information Success model (ISM). Hypothetical data are analyzed for (AOD) by three types of (WIA) to attain CRM and profit maximization and this analytical method can be used by any customer-oriented firm as a general model and for the purpose of the study we have compared the CRM between patients and hospital management.

  • 병원 산업계의 경영관리 환경 변화에서 인터넷의 정보 제공 수준이 높아짐으로써 고객의 의료 지능이 높아지고, 병원선택(Hospital Shopping)과 의사선택(Doctor Shopping)을 스스로 하고 있어서, 병원의 경영도 고객(환자)접점 관리가 사업성과를 중요한 결정 요인으로 작용하고 있다. 이러한 결과로 환자와의 접점관리를 위한 고객관계관리(CRM) 시스템을 구축하고 운영하는 병원이 점차 늘어가고 있다. 그러나 이러한 병원CRM시스템 투자 노력에도 불구하고 그 성과는 미미한 수준에 머물고 있거나 대부분의 CRM 시스템이 실패함에 따라 CRM의 적극적인 도입의 지가 줄어들고 있다. 본 연구에서는 병원CRM시스템의 도입 의도에 어떤 영향 변수들이 주요한 변수로 작용하고 있는지에 대하여 연구하였다. 이러한 문제를 해결하기 위해 병원CRM시스템의 구축에 있어서 영향을 많이 끼치는 분야로 병원정보시스템을 선택했고, 이를 위해 구조방정식모형을 이용하여 연구 가설을 검증하고 연구 모형의 유효성을 증명한 독립변수의 측정 변수를 도출하여 향후 CRM 도입을 하고자 하는 병원에게 지식정보를 제공하고자 하였다.

  • 최근 들어서 병원에서도 일반 기업에서와 같이 CRM 시스템의 구축 및 운용 사례가 늘고 있다. 이러한 현상은 병원의 경영환경에도 경쟁구도가 자리 잡기 시작했고, 의료서비스 시장에서의 중심축이 공급자 중심에서 수요자중심으로 옮겨가고 있기 때문이다. 이런 환경에서 병원의 CRM 시스템의 구축 및 운용 현황에 관심을 갖는 것은 당연하다 할 것이다. 본 논문에서는 이와 같이 구축운영 중인 병원 CRM 시스템의 성과를 측정하기 위한 분석 방법에 대해서 연구하였다. 이를 위해 선행 연구를 통한 CRM의 성공 요인을 도출하고, 실증분석을 통해 요인 분석의 타당성을 분석하였다. 요인의 타당성 분석을 위해 AMOS 구조모형 분석 방법을 실시하여, 요인 분석 결과의 유의성을 높혔다. 요인 분석 결과 도출된 요인의 중요성의 측정을 위해서는 요인 간 중요도를 측정할 수 있는 AHP 기법을 적용하였다. 이는 요인의 분류 외에는 측정변수의 가중치 등의 주요 정보를 제공하지 못한다는 요인 분석 방법론의 단점을 보완하기 위함이다. 결과적으로 본 연구를 통해서 병원의 CRM 시스템의 운용 성과를 측정하기 위한 주요요인 도출과 가중치 분석을 통해 병원의 고객관리시스템의 운용 성과를 합리적으로 측정할 수 있는 지표를 제공하였다.

  • As the information from today's knowledge-based society alters the awareness of patients and provides a wider selection of medical institutions, the reality of medical institution management is struggling compared to the past. CRM was introduced to domestic hospitals as marketing strategy to dominate the competition under varying medical environment. Thus, this research invests on CRM activity types used in hospitals and the effects of CRM activities toward loyalty and switching intention. Futhermore, we investigate the mediating role of customer relationship commitment and termination in CRM activities. The subjects in this study included three hundred customers who are using hospital settings or who had experience with using the hospital settings. The researcher collected 262 survey questionnaires from May 23th, 2011 to June first. In order to evaluate the hypothesis in this study, structural equation model was analyzed by using AMOS 18.0.

Diagnostic Accuracy of CT for Evaluating Circumferential Resection Margin Status in Resectable or Borderline Resectable Pancreatic Head Cancer: A Prospective Study Using Axially Sliced Surgical Pathologic Correlation

  • Objective: CT plays a central role in determining the resectability of pancreatic cancer, which directs the use of neoadjuvant therapy. This study aimed to assess the diagnostic accuracy of CT in predicting circumferential resection margin (CRM) involvement in patients with resectable or borderline resectable pancreatic head cancer. Materials and Methods: Seventy-seven patients who were scheduled for upfront surgery for resectable or borderline resectable pancreatic head cancer were prospectively enrolled, and 75 patients (38 male and 37 female; mean age ± standard deviation, 68 ± 11 years) were finally analyzed. The CRM status was evaluated separately for the superior mesenteric artery (SMA) and posterior and superior mesenteric vein/portal vein (SMV/PV) margins. Three independent radiologists reviewed the preoperative CT images and evaluated the resection margin status. The reference standard for CRM status was pathologic examination of pancreaticoduodenectomy specimens in an axial plane perpendicular to the axis of the second portion of the duodenum. The diagnostic accuracy of CT was assessed for overall CRM involvement, defined as involvement of the SMA or posterior margins (per-patient analysis), and involvement of each of the three resection margins (per-margin analysis). The data were pooled using a crossed random effects model. Results: Forty patients had pathologically confirmed overall CRM involvement in pancreatic cancer, while CRM involvement was not seen in 35 patients. For overall CRM involvement, the pooled sensitivity and specificity were 15% (95% confidence interval: 7%-49%) and 99% (96%-100%), respectively. For each of the resection margins, the pooled sensitivity and specificity were 14% (9%-54%) and 99% (38%-100%) for the SMA margin, 12% (8%-46%) and 99% (97%-100%) for the posterior margin; and 37% (29%-53%) and 96% (31%-100%) for the SMV/PV margin, respectively. Conclusion: CT showed very high specificity but low sensitivity in predicting pathological CRM involvement in pancreatic cancer.

포스트 코로나시대 의료기관 CRM시스템 구축모형 : 의원급 의료기관을 중심으로 (A Study of Establishment of Medical CRM Model in the Post-Corona Era : Focusing on the Primary-Level Hospital)

  • The purpose of this study is to analyze the medical ecosystem in the post-corona era. In addition, this study introduces a new medical CRM model that allows primary-level hospitals to overcome the economic difficulties and to occupy a competitive advantage in the post-corona era. The medical environment in the post-corona era is expected to be changed by non-face-to-face treatment, reinforcement of public medical care, the transformation of a medical system centered on the primary-level hospitals, and the use of AI and big data technologies. The medical CRM model presented in this study emphasizes the establishment of mutual customer relationships through close information exchange between patients, primary-level hospital, and the government. In the post-corona era, primary-level hospitals should not simply be approached as private hospital pursuing profitability. These should be reestablished as the hospitals that can provide public health care services while ensuring stable profitability.

Inhibition of chromosomal region maintenance 1 suppresses the migration and invasion of glioma cells via inactivation of the STAT3/MMP2 signaling pathway

  • Chromosomal region maintenance 1 (CRM1) is associated with an adverse prognosis in glioma. We previously reported that CRM1 inhibition suppressed glioma cell proliferation both in vitro and in vivo. In this study, we investigated the role of CRM1 in the migration and invasion of glioma cells. S109, a novel reversible selective inhibitor of CRM1, was used to treat Human glioma U87 and U251 cells. Cell migration and invasion were evaluated by wound-healing and transwell invasion assays. The results showed that S109 significantly inhibited the migration and invasion of U87 and U251 cells. However, mutation of Cys528 in CRM1 abolished the inhibitory activity of S109 in glioma cells. Furthermore, we found that S109 treatment decreased the expression level and activity of MMP2 and reduced the level of phosphorylated STAT3 but not total STAT3. Therefore, the inhibition of migration and invasion induced by S109 may be associated with the downregulation of MMP2 activity and expression, and inactivation of the STAT3 signaling pathway. These results support our previous conclusion that inhibition of CRM1 is an attractive strategy for the treatment of glioma.
