Effects and adverse-effects of growth hormone therapy in children with Prader-Willi syndrome: A two year study (Prader-Willi 증후군 소아에서 성장호르몬 치료의 효과와 부작용)

  • Purpose : The objective of this study was to evaluate the effects and adverse side-effects of growth hormone (GH) therapy in children with Prader-Willi syndrome (PWS). Methods : Forty-one patients who had been treated with GH for more than two years (24 boys and 17 girls, mean age $7.3{\pm}3.3$ years during treatment initiation) were enrolled for this study. Results : After 2 years of GH therapy, the height and weight standard deviation scores (SDS) increased significantly ($-1.19{\pm}1.37$ vs. $-0.02{\pm}1.45$, and $1.02{\pm}2.42$ vs. $1.63{\pm}2.22$, P<0.002); however the percentage body fat decreased ($44.6{\pm}9.9%$ vs. $38.1{\pm}10.5%$, P<0.001). Further, no change was observed in the thyroid and serum glucose levels, but the total cholesterol level decreased. GH therapy did not impact glucose control in the patients with diabetes. The most common adverse effects of GH therapy were the progression of scoliosis and adenoid hypertrophy. Conclusion : GH therapy improved the height SDS and body composition in patients with PWS. However, GH should be used with caution in patients with scoliosis and adenoid hypertrophy.

The Effect of Exogenous Hormone Treatment on Spermiation in Rhynchocypris oxycephalus (Sauvage and Dabry)

  • For the evaluation of hormonal control of spermiation in fish, a method to quanify the spermiation response or mature Rhynchocypris oxycephalus (Sauvage and Dabry) to hormonal therapy is described. Spermatocrit was determined after 7 min centrifugation at 18,000 $\times$g and sperm density was estimated by a standard hemocytormer method. Sperm density can be predicted from spermatocrit since their relationship is linear as described by the regression equation, Y=3.68X-27,18 ($R^2$=0.82, N=50), where Y is spermatocrit and X is sperm density. Milt production by mature R. oxycephalus was highest at 24 h after injection of 1,000 IU human chorionic gonadotropin (HCG) and 50 $\mu\textrm{g}$ luteinizing hormone-releasing hormone analogue (LHRHa) per kg body weight. Increased milt production coincided with low spermotocrit and sperm density levels. These results demonstrate that spermition in mature R. oxycephalus can be reliably evaluated by a spermatocrit method and that HCG and LHRHa are effecove in stimulating of spermiation in this species.

A Report on the Results of Korean Medicine Health Promotion Project 'Vitality UP Health UP Class for Climacteric Women' in Boeun-gun in 2022 (2022년도 보은군 한의약 건강증진사업 '갱년기 여성 활력UP 건강UP 교실'에 대한 결과 보고)

  • Objectives: The purpose of this health promotion project for Korean medicine is to promote the health of climacteric women in Boeun-gun. Methods: During the project, pressure needle acupuncture treatment, herbal medicine treatment, meditation pore therapy, and health education were conducted. To evaluate the results, basic health surveys (body height, body composition test, blood pressure etc.), female hormone tests, Kupperman's index (KI), Menopause-specific quality of life questionnaire (MENQOL), and Perceived stress scales (PSS) were conducted. Results: No significant change was observed in the body composition test and blood pressure after the project. Follicle stimulating hormone (FSH) was significantly increased after the project, but Luteinizing Hormone (LH) and total estrogen levels were not significantly changed. KI and MENQOL scores significantly decreased after the project, and significantly decreased in 4 out of 11 items of KI and 3 out of 4 domains of MEMQOL. No significant change was observed in the PSS score after the project. Conclusions: From the results of this project, it can be seen that the Korean medicine health promotion project can help alleviate symptoms of climacteric syndrome and postmenopausal syndrome.

Effects on the Laughter Score, Cortisol and Immunoglobulin of Laughter Therapy in Middle Aged Women (중년기 여성의 웃음치료가 웃음지수, Cortisol 및 면역글로불린에 미치는 효과)

  • Purpose: In this study, the effects of laughter therapy on a laughter index, cortisol and immunoglobulin of middle aged women were examined. Methods: A quasi-experimental nonequivalent control group pre/post-test design were used. The participants (n=54) included 27 in the experimental group and 27 in the control group. The data were collected from February through March of 2013. The experimental group participated in laughter therapy for 40 minutes per session, five times a week for a period of 2 weeks. The tool was used to measure pre/post-test laughter index, cortisol and immunoglobulin of middle aged women. Results: The results showed that laughter therapy was effective in increasing the laughter index (p<.001), and immunoglobulin IgA (p<.001), IgG (p<.001), and IgM (p=.010). Also, the stress hormone, cortisol of middle aged women decreased but non-significantly (p=.158). Conclusion: The result of the study indicates that laughter therapy may have an influence on nursing intervention and treatment effects to improve the laughter index and immunoglobulin of middle aged women.

Pattern of Reproductive Cancers in India

  • Background: Reproductive cancers are those that affect the human organs that are involved in producing offspring. An attempt is made in the present communication to assess the magnitude and pattern of reproductive cancers, including their treatment modalities, in India. The cancer incidence data related to reproductive cancers collected by five population-based urban registries, namely Bangalore, Bhopal, Chennai, Delhi and Mumbai, for the years 2006-08 were utilized. The reproductive cancers among females constituted around 25% of the total and around 9% among males. Among females, the three major contributors were cervix (55.5%), ovary (26.1%) and corpus uteri (12.4%). Similarly among males, the three major contributors were prostate (77.6%), penis (11.6%) and testis (10.5%). For females, the AAR of reproductive cancers varied between 30.5 in the registry of Mumbai to 37.3 in the registry of Delhi. In males, it ranged between 6.5 in the registry of Bhopal to 14.7 in the registry of Delhi. For both males and females, the individual reproductive cancer sites showed increasing trends with age. The leading treatment provided was: radio-therapy in combination with chemo-therapy for cancers of cervix (48.3%) and vagina (43.9%); surgery in combination with chemo-therapy (54.9%) for ovarian cancer; and surgery in combination with radio-therapy for the cancers of the corpus uteri (39.8%). In males, the leading treatment provided was hormone-therapy for prostate cancer (39.6%), surgery for penile cancer (81.3%) and surgery in combination with chemo-therapy for cancer of the testis (57.6%).

  • This study was to determine whether iontophoresis application would produce analgesic effect for clinical practice. Physical therapist controls pain produced by various causes and plays a role improving functional disability. I studied varieties of pain theories, mechanisms and iontoporosis principles which need for physical therapist. These were summarized as follwings; 1 . In the case of chronic patients, it is helpful to adapt iontophoresis treatment as well as generalized treatments which goals for pain releasc. 2. lontophoresis treatment should be positively examined to control pains safely. efficienently without sideeffects. 3. lontoporesis treatment suggests the foundations that hormone or anaesthetics should be incluided in the range of medicines physical therapist can deal with.

Lifestyle Components and Primary Breast Cancer Prevention

  • Breast cancer primary prevention is a high research priority due to the high psychological and economic costs. The disease is a multistep process and several risk factors have been recognized. Over the past three decades numerous studies have investigated the association of lifestyle with breast cancer, showing independent effects of various factors. We report here a summary of the present state of knowledge on the role of lifestyle patterns, such as physical activity, diet, smoking, hormone therapy, and experience of psychological stress in the modulation of breast cancer in women, and discuss commonly accepted biological mechanisms hypothesized as responsible for the associations. The findings indicate that regular physical activity of moderate to vigorous intensity is probably linked with the decreased breast cancer risk among postmenopausal females and suggestive for a decrease of the risk in premenopausal women. In contrast, the consumption of high-fat diet, alcohol intake, and use of combined estrogen and synthetic progestagen hormonal therapy may increase the risk. Epidemiological findings dealing with a role of smoking and experience of psychological stress are conflicting.

Anti-oxidant Effect and Enhancement of Menopausal Signs by Sophorae fructus-derived Isoflavones in Ovariectomized Rats and Cytoprotective Effect in Neuro-2a (난소절제 랫드에서 회화나무 이소플라본의 갱년기증상 개선효과 및 신경세포 보호효과)

  • Isoflavones are reported to playa role in menopausal women as a phytoestrogen, which can replace estradiols in hormone replacement therapy (HRT). Recently; due to the risk of breast cancer by HRT, phytoestrogens (e.g. isoflavones) have been focused as an alternative therapy in menopause. In the study, we investigated whether isoflavones derived from Sophorae fructus (SISO) have more benefit than that of soybean isoflavones in estrogen deficient rats. We found that SISO effectively controled $H_2O_2$ comparing with the baseline (p<0.01 vs. post value of OVX-Cont), and the blood sugar and weight were also controlled with decreasing patterns. Additionally, in LDH assay for cytoprotective effect in Neuro-2a cell line, SISO protected cells from the damage by SNAP (p<0.05). In conclusion, SISO may have more beneficial effect in enhancing the menopausal signs than that of soybean isoflavones and the cytoprotective effect in neuron cells suggests that SISO can play a certain role in neuroprotection after menopause.

Effect of therapeutic radioiodine activity on ablation response in differentiated thyroid cancer patients with cut-off serum thyroglobulin levels after 2 weeks of thyroid hormone withdrawal: a retrospective study

  • This retrospective study aimed to investigate whether there was a difference in the success rate of removal of residual thyroid tissue in patients with the same cutoff serum thyroglobulin (Tg) value-measured 2 weeks after thyroid hormone withdrawal (THW)-for different radioactive iodine (RAI) activities. We identified 132 patients with papillary thyroid cancer who were treated with total thyroidectomy and RAI therapy to evaluate the efficacy of three radioactivities of I-131: 1,110, 3,700, and 5,550 MBq. Serum Tg testing was performed 1 week before RAI treatment and 2 weeks after THW (pre-Tg); the cutoff pre-Tg level was below 10 ng/mL. Stimulated Tg levels were measured on the day of I-131 administration (off-Tg). After 6 months of treatment, we compared the groups for complete ablation, defined as no uptake on a diagnostic I-131 scan, stimulated Tg level of <1.0 ng/mL, and Tg antibody level of <100 ng/mL. Ninety-five patients (72.0%) achieved complete ablation, with 57.1% (8/14), 78.2% (68/87), and 61.3% (19/31) in the 1,110 MBq, 3,700 MBq, and 5,550 MBq groups, respectively. There was no significant difference in the complete ablation rates between the three groups. In the multivariate analysis, the off-Tg level was a significant predictor of complete ablation. RAI therapy with low radioactivity (1,110 MBq) seemed sufficient for ablation in patients with papillary thyroid cancer with a pre-Tg level below 10 ng/mL. The off-Tg level is a promising and useful predictor of complete ablation after initial RAI therapy.

Effects of Growth Hormone Therapy in Children with Idiopathic Short Stature (특발성 저신장증 소아에서 성장호르몬의 치료효과)

  • Purpose : The use of growth hormone(GH) to promote growth in normal short children without classical GH deficiency is controversial. Numerous foreign studies have shown the effects of GH therapy in children with idiopathic short stature(ISS) whereas few has been interested in Korea. Therefore, this study is designed to investigate the effects of GH therapy on ISS by observing correlations and changes among various growth parameters such as, insulin-like growth factor-I(IGF-I) and insulin-like growth factor binding protein-3(IGFBP-3). Methods : This study was conducted retrospectively with 15 children with ISS in Chungbuk National University Hospital in Korea. Mean age was $11.44{\pm}2.81$ and the children were treated with 0.66 IU/kg/wk dosage of GH for 1 or 2 years. Also, the growth parameters before and after the GH therapy were observed. Results : Height standard deviation score(HT-SDS) was increased from $-1.85{\pm}0.70$ to $-1.58{\pm}0.56$ at 1 year and to $-1.21{\pm}0.37$ at 2 years after GH therapy. Predicted adult height standard deviation score(PAH-SDS) was also increased from $-2.10{\pm}0.52$ to $-1.67{\pm}0.59$ at 1 year, and to $-0.96{\pm}0.60$ at 2 years. Serum IGF-I and IGFBP-3 levels were significantly increased after 1 year and marginally increased after 2 years of GH therapy. Conclusion : It is concluded that GH therapy has growth promoting effect. The significant increase in IGF-I and IGFBP-3 levels during the GH therapy suggests that IGF-I and IGFBP-3 are useful predictors of response to the use of GH therapy. It is expected that larger patient samples would provide more reliable information about the effect of GH therapy.
