Effect of zinc supplementation on insulin resistance and metabolic risk factors in obese Korean women

  • Zinc deficiency is known to be associated with insulin resistance in obese individuals. This study was performed to evaluate the effect of zinc supplementation on insulin resistance and metabolic risk factors in obese Korean women. Forty obese women (body mass index (BMI) ${\geq}25kg/m^2$) aged 19-28 years were recruited for this study. Twenty women of the study group took 30 mg/day of supplemental zinc as zinc gluconate for 8 weeks and 20 women of control group took placebo. Usual dietary zinc intake was estimated from 3-day diet records. Insulin resistances were measured using Homeostasis model assessment (HOMA) indices, and insulin sensitivities Matsuda indices, which were calculated using oral glucose tolerance test data. Metabolic risk factors, such as waist circumference, blood pressure, fasting glucose, triglyceride, high density lipoprotein (HDL) cholesterol, and adipocyte hormones such as leptin, and adiponectin were also measured. At the beginning of study, dietary zinc averaged 7.31 mg/day and serum zinc averaged $12.98{\mu}mol/L$ in the study group. Zinc supplementation increased serum zinc by 15% and urinary zinc by 56% (P < 0.05). HOMA values tended to decrease and insulin sensitivity increased slightly in the study group, but not significantly so. BMI, waist circumference, blood pressure, blood glucose, triglyceride, HDL cholesterol, and adipocyte hormones did not change in either the study or control group. These results suggest that zinc status may not affect insulin resistance and metabolic risk factors in obese Korean women. Further research is required on a larger cohort with a longer follow-up to determine the effects of zinc status on insulin resistance and metabolic variables.

지방흡입술 후 초기 대사성 변화 및 고찰 (Early Metabolic Changes and Its Considerations after Liposuction)

  • Purpose: Advanced techniques now make it possible to remove considerable amounts of subcutaneous adipose tissue more safely with minimum blood loss. However, few have analyzed the metabolic consequences of liposuction. The purpose of this study was to identify the early effects of the surgical removal of subcutaneous fat on metabolic changes in patients who have undergone liposuction. Methods: Nineteen patients were evaluated from June 2005 to December 2005. Preoperative body weight, serums levels of lipids, apolipoprotein A1, dehydroepiandrosterone(DHEA), uric acid, insulin, and glucose were evaluated. Insulin resistance was determined using the homeostasis model assessment of insulin resistance (HOMA-IR), which is based on fasting glucose and insulin concentrations. All of these data were remeasured in 1 and 4 weeks postoperatively. Tumescent fluid was infiltrated using the superwet technique. The liposuction device used was a $Liposlim^{(R)}$ power-assisted unit. Results: Average volumes of infiltrate and aspirate were 3,268mL and 2,892mL, respectively. Results in 1 week postoperatively demonstrated a significant difference in high-density lipoprotein(HDL) cholesterol, apolipoprotein A1, insulin, and HOMA-IR levels. However, all values were within normal limits and returned to baseline in 4 weeks postoperatively.Conclusion: This study provides little to support the presumed therapeutic effect of liposuction. And, it is unclear whether liposuction can prevent or be used to treat the metabolic complications of obesity. However, the results of the present study lead us to believe that liposuction is a metabolically safe procedure.

Association Between Trp64arg Polymorphism of the β3 adrenoreceptor Gene and Female Sex in Obese Turkish Children and Adolescents

  • Purpose: The ${\beta}3-adrenergic$ receptor (ADRB3) is expressed in visceral adipose tissue and has been speculated to contribute to lipolysis, energy metabolism, and regulation of the metabolic rate. In this study, we aimed to investigate the association of polymorphism of the ADRB3 gene with the sex of children with obesity and related pathologies. Methods: ADRB3 gene trp64arg genotyping was conducted in 441 children aged 6-18 years. Among these subjects, 264 were obese (103 boys; 161 girls) and 179 were of normal weight (81 boys; 98 girls). In the obese group, fasting lipids, glucose and insulin levels, and blood pressure were measured. Metabolic syndrome (MS) was defined according to the modified World Health Organization criteria adapted for children. Results: The frequency of trp64arg genotype was similar in obese and normal weight children. In obese children, serum lipid, glucose, and insulin levels; homeostasis model assessment of insulin resistance (HOMA-IR) scores; and MS were not different between arg allele carriers (trp64arg) and noncarriers (trp64trp). In 264 obese children, genetic analysis results revealed that the arg allele carriers were significantly higher in girls than in boys (p=0.001). In the normal weight group, no statistically significant difference was found between genotypes of boys and girls (p=0.771). Conclusion: Trp64arg polymorphism of the ADRB3 gene was not associated with obesity and MS in Turkish children and adolescents. Although no relationships were observed between the genotypes and lipids, glucose/insulin levels, or HOMA-IR, the presence of trp64arg variant was frequent in obese girls, which can lead to weight gain as well as difficulty in losing weight in women.

Vitamin D Effect on Ultrasonography and Laboratory Indices and Biochemical Indicators in the Blood: an Interventional Study on 12 to 18-Year-Old Children with Fatty Liver

  • Purpose: The rising prevalence of childhood obesity in the past decades has caused non-alcoholic fatty liver disease (NAFLD) to become the most common cause of pediatric chronic liver disease worldwide. This study was aimed at determining the effect of vitamin D (Vit D) on ultrasonography and laboratory indices of NAFLD and some blood biochemical indicators in children. Methods: In this interventional study liver ultrasonography was performed in 200 children with overweight and obesity. A 108 had fatty liver among which 101 were randomly divided into two groups of study (n=51) and control (n=50). The study group was treated with Vit D, 50000 U once a week whereas the control group received placebo with the same dose and package, both for 12 weeks. At the end of the intervention lab tests and ultrasound study was performed once again to evaluate the response to treatment. Results: It was found out that Vit D supplementation improved the fatty liver grade in the study group. The mean changes in hemoglobin (Hb), uric acid, highdensity lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), insulin, albumin and alanine aminotransferase (ALT) was significantly higher in the study group compared to controls (p<0.05). After the intervention and means adjustment, a significant difference was obtained in HDL-C, insulin, LDL-C and homeostasis model assessment of insulin resistance (HOMA-IR) between the two groups. Conclusion: Vit D supplementation in addition to improving the fatty liver grade in ultrasonography and increasing the blood Vit D level, increases the HDL and Hb level besides decreasing uric acid, LDL, HOMA-IR, insulin and ALT levels.

Artemisia annua L. Extracts Improved Insulin Resistance via Changing Adiponectin, Leptin and Resistin Production in HFD/STZ Diabetic Mice

  • Objectives: Insulin resistance (IR) is major cause of type 2 diabetes (T2D), and adipokines (e.g., adiponectin, leptin, and resistin) play an important role in insulin sensitivity. Medicinal plants are frequently used for T2D treatment. This study investigates the effect of Artemisia annua L. (AA) extracts on adipokines in mice with high-fat-diet (HFD)/streptozotocin (STZ)-induced T2D. Methods: We divided 60 mice into 12 groups (n = 5 per group): control, untreated T2D, treated T2D, and 9 other groups. T2D was induced in all groups, except controls, by 8 weeks of HFD and STZ injection. The treated T2D group was administered 250 mg/kg of metformin (MTF), while the nine other groups were treated with 100, 200, and 400 mg/kg of hot-water extract (HWE), cold-water extract (CWE), and alcoholic extract (ALE) of AA (daily oral gavage) along with 250 mg/kg of MTF for 4 weeks. The intraperitoneal glucose tolerance test (IPGTT) was performed, and the homeostasis model assessment of adiponectin (HOMA-AD) index and blood glucose and serum insulin, leptin, adiponectin, and resistin levels were measured. Results: Similar to MTF, all three types of AA extracts (HWEs, CWEs, and ALEs) significantly (p < 0.0001) decreased the area under the curve (AUC) of glucose during the IPGTT, the HOMA-AD index, blood glucose levels, and serum insulin, leptin, and resistin levels and increased serum adiponectin levels in the MTF group compared to the T2D group (p < 0.0001). The HWEs affected adipokine release, while the CWEs and ALEs decreased leptin and resistin production. Conclusion: Water and alcoholic AA extracts have an antihyperglycemic and antihyperinsulinemic effect on HFD/STZ diabetic mice. In addition, they decrease IR by reducing leptin and resistin production and increasing adiponectin secretion from adipocytes.

쑥 (Artemisia princeps P.) 추출물이 고지방식이를 급여한 흰쥐의 대사장애 및 아디포카인 조절에 미치는 영향 (Amelioration of metabolic disturbances and adipokine dysregulation by mugwort (Artemisia princeps P.) extract in high-fat diet-induced obese rats)

  • 본 연구는 고지방식이와 쑥추출물이 흰쥐의 지질대사, 인슐린저항성, 아디포카인 분비에 미치는 효과를 측정하였다. 고지방식이에 의해 부고환지방과 복막후강지방의 축적이 증가되었고, 그 결과 혈장 항염증성 아디포카인은 감소하고 염증성 아디포카인은 증가하여 아디포넥틴/렙틴 및 아디포넥틴/PAI-1 비가 저하되는 불균형을 초래하였다. 내장지방 축적과 관련된 아디포카인의 불균형은 TG, TG/HDL-C, TC/HDL-C를 증가시키고 HOMA-IR의 증가를 유발한 것으로 추정된다. 이에 비해 쑥 에탄올추출물의 첨가는 부고환지방과 복막후강지방의 축적을 완화함으로써 PAI-1과 렙틴을 감소시켜 아디포넥틴/렙틴 및 아디포넥틴/PAI-1 비의 저하를 억제하고 아디포카인의 균형을 상대적으로 유지하였으며, TG, TG/HDL-C, TC/HDL-C 및 HOMA-IR 수치가 감소하였다. 따라서 쑥 에탄올추출물은 내장지방 축적을 억제하고 아디포카인의 분비, 특히 염증성물질인 렙틴과 PAI-1의 분비를 저하시켜 아디포카인의 균형을 조절함으로써 지질대사 및 인슐린저항성을 개선하는 효과가 있는 것으로 나타났다.

소아 비만에서 대사증후군의 고찰 (The Metabolic Syndrome in Obese Children)

  • 목적: 소아 비만클리닉에서 시행하는 선별검사로 대사증후군의 위험인자를 미리 인지하여 비만아가 치료를 지속적으로 할 수 있도록 지도하는데 도움을 주고자 하였다. 대상: 2000년 2월부터 2004년 6월까지 이대 목동병원 소아 비만클리닉을 방문한 비만아에서 의무기록이 충실하고 체질량지수(BMI)가 95 백분위수 이상인 88명(남 52명, 여 36명)을 대상으로 하였다. 방법: 비만아에서 BMI를 계산하고 혈압은 안정 상태에서 두 번 측정하여 평균을 구하였다. 12시간 공복 후에 혈청 지질(중성지방, 총콜레스테롤, 고밀도 콜레스테롤, 저밀도 콜레스테롤), 혈당과 혈중 인슐린 농도를 측정하였다. 인슐린 저항성 지표로 fasting insulin to glucose ratio (FIGR), homeostasis model assessment of insulin resistance (HOMA-IR), quantitative insulin sensitivity check index (QUICKI)를 이용하였다. 결 과: 비만아 88명 중 53명(60.2%)에서 하나 이상의 대사증후군의 위험 인자를 보였다. 고혈압은 13명(14.8%), 중성지방 ${\geq}150mg/dL$ 13명(14.8%), 총콜레스테롤 ${\geq}200mg/dL$ 23명(26.1%), 고밀도 콜레스테롤 ${\leq}40mg/dL$ 13명(14.8%), 저밀도 콜레스테롤 ${\geq}130mg/dL$ 11명(12.5%)이었으며 고인슐린 혈증은 11명(12.5%)이었다. BMI는 수축기 혈압(r=0.535), 고밀도 콜레스테롤(r=-0.214), 인슐린 농도(r=0.342), HOMA-IR (r=0.346), FIGR (r=0.329), QUICKI (r=-0.22)와 유의한 상관 관계를 보였다. 인슐린 농도(r=0.233), HOMA-IR (r=0.234)은 수축기 혈압과, FIGR은 수축기 혈압(r=0.227) 및 저밀도 콜레스테롤(r=0.223)과, QUICKI는 혈당(r=-0.308)과 각각 유의한 상관관계를 보였다. 선별검사에서 대사증후군의 위험 인자가 많을수록 인슐린 저항성(HOMA-IR, FIGR)이 유의하게 증가하였다. 한편 인슐린 저항성을 가장 잘 설명하는 인자로는 BMI, 중성지방, 총콜레스테롤이었다. 결론: 소아 비만클리닉을 방문하는 비만아에서 이미 대사증후군이 시작되고 있으므로 행동 수정, 식사조절 및 운동 처방뿐 아니라 혈압, 지질, 혈당, 인슐린 농도 등을 정기적으로 검사하여 적극적으로 관리해야 한다.

비만아동의 지질과산화물 형성과 항산화 체계에 관한 연구 (LDL Oxidation, Total Radical Trapping Antioxidant Potential and Plasma Antioxidant Vitamin Systems in Obese School Children)

  • The purpose of this study was to examine the lipid peroxidation, plasma antioxidant status and insulin resistance in childhood obesity. To this end, we measured blood lipid profiles, glucose, insulin concentrations, plasma antioxidant vitamins, baseline conjugated diene formation as a measure of LDL oxidation in vivo and TRAP (total radical trapping antioxidant potential) of 93 school children (58 nonobese, 35 overweight-obese). Insulin resistance was estimated by homeostasis model assessment of insulin resistance (HOMA-IR). The overweight-obese children showed significantly higher levels of leptin (p < 0.0001) and triglyceride (p < 0.05) and significantly lower level of plasma Iycopene (p < 0.001) and $\gamma$-tocopherol (p < 0.05) compared with the normal weight children. Furthermore, the levels of TRAP were significantly lower in overweight-obese children (p < 0.05). Significant positive relationships between plasma leptin and conjugated dienes formation (p < 0.005) and inverse relationship between plasma leptin and lipid corrected levels of $\beta$-carotene (p < 0.05), Iycopene (p < 0.05) were observed. Our results showed an increased lipid peroxidation and decreased antioxidant capacity in childhood obesity which could be involved in the atherosclerotic process.

매생이 추출물 첨가 통밀 쿠키의 마우스 혈당 항상성 개선 효과 (Improvement of blood glucose homeostasis in mice fed with Capsosiphon fulvescens extract-added whole wheat cookie)

  • 본 연구는 일반 쿠키(NC), 통밀 쿠키(WC) 그리고 매생이 추출물이 첨가된 통밀 쿠키(WCFE)를 제조하고 4주 동안 쿠키로만 이루어진 식이를 마우스에게 제공한 뒤 혈당과 관련된 지표들의 변화를 관찰하였다. 4주 섭취 이후 NC를 섭취한 그룹과 비교하여 WC와 WCFE를 섭취한 그룹에서 단기간의 체중 증가의 감소효과를 보였으며, 매생이 추출물의 첨가는 간과 신장의 스트레스 증가에 영향이 없음을 확인할 수 있었다. 공복 혈당 수준은 WC 그룹과 WCFE 그룹에서 NC 그룹에 비해 유의적으로 감소하였다(p<0.001). 혈청 인슐린 농도는 WCFE 그룹에서 유의적인 차이를 보이며 감소하였고(p<0.05), 이를 바탕으로 인슐린 저항성 관련 지표인 HOMA-IR를 산출 시 WCFE 그룹에서 유의적인 차이를 보이며 감소함으로써(p<0.05) 매생이 추출물이 첨가되었을 때 인슐린 저항성을 개선한 것으로 판단된다. 또한, OGTT의 30분대 혈당치에서 NC 그룹 및 WC 그룹과 비교하여 WCFE 그룹에서 혈당 증가 폭이 감소하는 것을 확인할 수 있었고, WCFE 그룹에서 가장 낮은 AUC 면적 값을 나타냈다. OGTT 및 AUC 결과를 종합해볼 때 일반 쿠키와 통밀 쿠키에 비해 매생이 추출물의 첨가로 인해 내당능에 도움을 줄 수 있을 것으로 판단된다. 그러나 혈당 관련 유전자인 GLUT2와 GLUT4의 경우, 통밀에 의해 발현이 증가하는 경향은 관찰되었으나 매생이 추출물 첨가에 의한 효과는 관찰되지 않았다. 또한 IRS-1의 경우 WC 그룹에 비해 WCFE 그룹의 발현이 증가하였고 PI3K p85α 발현의 경우 WC 그룹에 비해 WCFE 그룹의 발현이 감소하는 경향을 보여, 매생이 추출물의 첨가가 혈당 관련 유전자의 발현에는 영향을 미치지 못하는 것으로 보인다. 따라서, 이러한 결과를 종합했을 때 WCFE 섭취 시, 혈당 관련 유전자 발현에는 영향을 미치지는 못하지만, 내당능 개선 효과를 가지며 인슐린 저항성 개선 효과의 가능성을 보여 혈당 항상성 개선에 기여할 것으로 판단된다.

The association between measurement sites of visceral adipose tissue and cardiovascular risk factors after caloric restriction in obese Korean women

  • Quantities as well as distributions of adipose tissue (AT) are significantly related to cardiovascular disease (CVD) risk factors and can be altered with caloric restriction. This study investigated which cross-sectional slice location of AT is most strongly correlated with changes in CVD risk factors after caloric restriction in obese Korean women. Thirty-three obese pre-menopausal Korean women ($32.4{\pm}8.5$ yrs, BMI $27.1{\pm}2.3\;kg/m^2$) participated in a 12 weeks caloric restriction program. Subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) were measured using computed tomography (CT) scans at the sites of L2-L3, L3-L4, and L4-L5. Fasting serum levels of glucose, insulin, triglyceride, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), leptin and homeostasis model assessment-insulin resistance (HOMA-IR) were observed. Pearson's partial correlation coefficients were used to assess the relationship between AT measurement sites and changes in CVD risk factors after calorie restriction. When calories were reduced by 350 kcal/day for 12 weeks, body weight (-2.7%), body fat mass (-8.2%), and waist circumference (-5.8%) all decreased (P < 0.05). In addition, following caloric restriction, serum levels of glucose (-4.6%), TC (-6.2%), LDL-C (-5.3%), leptin (-17.6%) and HOMA-IR (-18.2%) decreased significantly (P < 0.05) as well. Changes in VAT at the level of L3-L4 were significantly greater than those at other abdominal sites, and these changes were correlated with changes in TC (P < 0.05), LDL-C (P < 0.001), SBP (P < 0.001) and HOMA-IR (P < 0.01). These results show that VAT at L3-L4 had a stronger correlation with CVD risk factors than with other AT measurement sites after caloric restriction.
