A study on development of suitable healthcare equipment for aged people at their homes - Toward efficiency and reduction of the homehelpers' labor in healthcare

  • Concerning the development of suitable healthcare equipment for a aged people at their homes in order to reduce the labor and increase the efficiency in the homehelpers' work, we conducted a study on the actual conditions of healthcare in Kyoto city (healthcare in taking a meal and excretion, moving and taking a bath by using a hand-powered lift, moving by a wheelchair, shower carrier healthcare, assistance in body care and housework) by digital video filming, interviews and layout investigations. On the base of those investigations by using methods for analysis of video pictures, we divided the healthcate work into scenes and e exctracted the main problems in terms of health care equipment, information, environment, healthcare skills, mental burden, social s system, family burden, ete. Furthermore the questionnaire survey of home helpers (18 persons - 50% answers) clarified that more than half of them have lumbago pains and the biggest load in the everyday healthcare c comes from moving the patients or getting them into or out of t the equipment for moving. Finally we considered concrete proposals appropriate for each h healthcare scene image, based on evaluation toward efficiency (design guidelines) for suitable healthcare equipment for aged people at their homes: 1) Design toward a proper posture 2) Design toward self-help possibility 3) Design toward patient active participation 4) Design toward rationality of the whole process 5) Design toward place-appropriate healthcare equipment 6) Design toward healing of mind 7) Good level of healthcare skills 8) Economic, systematic rationality

  • U-healthcare is real-time monitoring of personal biometric information using by portable devices, home network and information and communication technology based healthcare systems, and fused together automatically to overcome the constraints of time and space are connected with hospitals and doctors. As u-healthcare gives health service in anytime and anywhere, it becomes to be a new type of medical services in patients management and disease prevention. In this paper, recent changes in prevention-oriented care is analyzed in becoming early response for Healthcare Information System by requirements analysis for technology development trend. According to the healthcare system, PACS, OCS, EMR and emergency medical system, U-healthcare is presenting the design of a patient-centered integrated client system. As the relationship between the meaning of the terms is used in the ontology, information models in the system is providing a common vocabulary with various levels of formality. In this paper, we propose an ontology-based system for patient-centered services, including the concept of clustering to clustering the data to define the relationship between these ontologies for more systematic data.

  • A smart home provides services that its inhabitant needs or wants, by integrating and simultaneously controlling various devices and sensors. In this study, we focused on a smart-home system for people with disabilities and for elderly people. We introduced a new type of system for real-time analysis of body posture of the inhabitants of a smart home. The system includes the concept that offers remote healthcare or medical services by using a 3D tilt sensor for recognizing the static and dynamic postures of inhabitants in real time. It consists of a smart-home server and a 3D tilt sensor, and it uses wireless technology to communicate with the inhabitants and thus enhance their mobility. The smart-home server includes the inference engine that differentiates the dynamic postures from the static ones. Finally, we also demonstrate the usefulness of the proposed system by applying it to a real environment.

  • The home network, contributing to improve the quality of life in dwelling, has expanded its installation and use in past years, but the elderly care system supporting self-reliance is not fully prepared. Although the aging society in Korea is progressing rapidly, inducting and spreading home network system is still required supporting of 'Aging in Place' for the elderly. This research is based the survey of spatial characteristics of housing of the elderly. Also, the research studied the technical level and policy of home network in domestic and abroad and analyze the professional view of experts in the home network field, as a result, suggested methods of building and activating the home network system.

  • Purpose: Child safety seats (CSS) are critical for the protection of children, in case of motor vehicle accidents. Although the national legislation mandates that all newborns must be placed in an appropriately installed CSS during transportation, people often do not perceive the importance of CSS and do not use it as recommended. The purpose of this survey was to understand the use of CSS for the safe transport of newborns from hospital to home. Methods: We interviewed parents of newborn infants, using a structured questionnaire, at the time of their discharge from Cheil General Hospital & Women's Health Care Center, between May 2014 and July 2014. Results: A total of 403 participants were interviewed. The rate of CSS use was only 14.9%. Overall, 76.4% of the families interviewed were not aware about the recommendations on CSS use for newborns when travelling in a car. The provision of education on using CSS significantly influenced their rate of use. Parents who were educated about mounting the CSS in a car used it more as compared with others (25.7% vs. 12.2%) (P=0.002). Furthermore, if parents had heard about the importance or necessity of CSS, they used it more than others did (19.5% vs. 10.6%, P=0.032). Conclusion: Despite the legal regulation, most parents transport their newborn infants without a CSS while traveling from hospital to their home. The rate of CSS use was influenced by parental education and their knowledge about its necessity. Education programs for parents must be reinforced to increase the CSS use.

Influence of Job Stress, Professionalism, Job Satisfaction, and Intention- to- Retention of Visiting Nurses: A Descriptive Study (보건소 방문간호사의 직무스트레스, 전문직업성, 직무만족도과 재직의도)

  • Purpose: The purpose of this study was to establish baseline data for the development of employment policies to improve the efficiency and stability of visiting healthcare services. It identifies factors affecting visiting nurses' intention-to-retention at healthcare centers in Seoul. Methods: This descriptive study investigated subjective health perception, job stress, professionalism, job satisfaction, and intention-to-retention of 269 nurses with more than one year of work experience as a visiting nurse. These factors were analyzed using t-tests, chi-square, partial correlation, and stepwise multiple regression. Results: The results indicate that satisfaction for professional status in job satisfaction, educational level, autonomy in professionalism, and visiting nursing career were significant factors that impacted the retention of visiting nurses. Conclusion: It is necessary to frame policies and provide support to enhance the satisfaction and autonomy for visiting nurse as a profession for the efficiency and stability of visiting healthcare services.

Awareness and Performance of Healthcare-Associated Infections Control and Psycological Empowerment of Intensive Care Unit Nurses (중환자실 간호사의 심리적 임파워먼트와 의료관련감염 관리의 인지도 및 수행도)

  • Purpose: This study was conducted to examine psychological empowerment and awareness and performance of Healthcare-Associated Infections(HAIs) control among Intensive Care Unit (ICU) nurses. Method: The data for this study were collected using structured questionnaires from 178 nurses working in the ICUs of four hospitals with more than 500 beds. Results: The subjects'psychological empowerment, awareness and performance of HAIs control were mean over. The subjects'psychological empowerment showed statistically significant differences depending on age, marital status, academic background, clinical career, ICU career, and position and experience in infection control among their general characteristics. Awareness of HAIs control showed statistically significant differences depending on age, gender, marital status, clinical career, and ICU career. Performance showed statistically significant differences depending on age, gender, marital status, and, ICU career. Regarding the subjects' performance of HAIs control, there was a statistically significant positive correlation between psychological empowerment and awareness. Conclusion: It was found that it is necessary to strengthen differential customized training according to the subjects'characteristics for effective HAIs control, and to strengthen the nurses'awareness of HAIs control through systematic evaluation, monitoring, and feedback, as well as continuous education and training.

The Effect of Job Characteristics on Emotional Empowerment in Visiting Health Personnel (방문간호서비스의 직무특성이 방문보건인력의 심리적 임파워먼트에 미치는 영향)

  • Purpose: The aim of this study was to analyze the effects of job characteristics on emotional empowerment in visiting health personnel. Methods: Subjects were recruited in ten community health care centers in one directorial area. Data collection was done using a self-report questionnaire. Job characteristics of visiting healthcare personnel were measured using the questionnaire developed by Kang (2006), based on Hackman & Oldham (1975). Emotional empowerment was measured using the questionnaire developed by Kang (2006), based on Spreitzer (1995). Results: First, the score of job characteristics was revealed to be 3.51 points the task significance was high, and the feedback was low. Second, the level of emotional empowerment was revealed to be 3.78 points the meaning was high, and the impact was low. Third, the prediction power of job characteristics on emotional empowerment was 34% autonomy, task identity, and task significance were identified as statistically significant predictive factors. Conclusion: The job characteristics of visiting healthcare personnel are highly correlated with emotional empowerment. Autonomy, task identity, and task significance are predictive factors of emotional empowerment. These results can be used to develop more effective job planning for increasing organizational effectiveness in visiting healthcare personnel.

  • In the current legal system, establishing aging friendly smart home based on healthcare for the senior people over 65 seems hard to be achieved. For these reasons, this study is intended to explore the improvements in the legal system using the comparative analysis in the domestic and foreign legal systems. The related legal system is divided into three sections - 'healthcare,' 'aging friendly,' and 'smart home' - providing that telemedicine would be implemented. According to the analysis of the legal system for healthcare, telemedicine is executed by the medical law. Thus, we need to investigate the concept and all the matters of telemedicine on the basis of foreign cases. As the result of the analysis of the legal system for 'Aging Friendly,' the definition of the elderly is ambiguous and we find the improvement of health and medical system. In addition to these, the definite bounds of the healthcare equipment have to be set. From these results, we are aware of the necessity of the improvements of the legal system, and suggest plans for these problems. That is to organize the legal system and make a new law through revising the current specific identification. This study focuses on suggesting the improvements of the legal system with the comparative analysis of the domestic and foreign legal systems.

Home Care Nursing: An Economic Analysis (가정간호의 경제성 평가에 관한 문헌분석)

  • Purpose: The purpose of this study was to examine the economic effectiveness of by conducting a literature review of published articles, masters theses, and doctoral dissertations. Method: Prior to the literature review, inclusion-exclusion criteria were established. We then reviewed 1,029 CINAHL, MEDLINE, and Cochrane DB papers, and 153 RISS papers collected between Results: A total of 12 studies met the inclusion-exclusion criteria.-effectiveness: 6 cost-effectiveness studies, 1 cost-utility studies, and 5 cost-benefit studies. Each of the 12 reviewed studies concluded that home care nursing had greater economic benefits than other compared healthcare services. Conclusion: Home care nursing has significant economic benefits in multi health care service settings and for various patient groups. Therefore these results will be used a critical evidence for the development of economically effective home based health care systems for future policy making.
