건강가정지원센터의 운영과 사업에 관한 연구 (A Study on Operational Strategies and Programs of Healthy Family Support Centers)

  • This study aims to investigate the Healthy Family Support Centers'(HFSCs) operational patterns and their programs by surveying 46 HFSC employees in order to prepare for the massive increase of the number of HFSCs nationwide. This study might contribute to not only rebuilding the HFSCs' operational strategies but suggesting visible ideas for the HFSCs' programs. Results were as followed: First, most HFSCs consisted of several teams, i.e., educational team, counseling team and cultural team. However, the number of employees in each team was different from one regional community to another. The most difficult task in the HFSCs' operation was publicity work. Citing the issue of system delivery difficulties, HFSC employees insisted that budgetary deficit was the most difficult obstacle in running the HFSCs. Second was in regards to the programs that each team was planning, performing, and evaluating for each program. The HFSCs' programs were mostly structured around family difficulties or problems that arise according to the family life cycle. Based on these study results, more unified and specialized programs for HFSCs should be developed. In order to achieve this related studies should continue to conducted.

상급종합병원의 입원계약 해지권 행사에 대한 검토 -해당 의료기관에서의 치료가 종결된 경우를 중심으로- (A Review of the Right to Terminate a Contract by a Medical Institution - Focusing on the Case that Treatment is Completed -)

  • 우리나라의 의료전달체계는 의료법과 국민건강보험법에 근거하고 있으며, 한정된 의료자원을 효율적으로 운영하기 위하여 질환의 중증도에 따라 의료기관을 이용하도록 구분되어 있다. 상급종합병원에서 이미 중증 질환에 대한 치료가 이루어져 병원급 의료기관으로 전원 또는 자택으로 퇴원이 가능한 경우 의료기관에서 환자에 대하여 의료계약을 해지할 수 있는지가 문제된다. 우리나라 법원의 입장으로는 해당 의료기관에서 더 이상의 입원치료가 불필요한 경우 의료기관의 의료계약 해지권을 인정하는 판결과 그러한 경우에도 의료기관의 의료계약 해지권을 부정하는 판결이 병존하고 있다. 한편 미국 판결 중에는 급성 치료를 담당하는 의료기관에서 입원 중인 환자에게 더 이상 급성 치료가 필요하지 않는 경우에 전문간호시설 등으로 전원을 인정하는 판결들이 있다. 의료자원이 한정되어 있고 의료기관의 계약 해지권이 제한된 취지가 국민의 생명권, 건강권에 대한 위험을 방지하기 위한 목적임을 고려할 때 해당 의료기관에서 치료가 종결되어 환자에게 더 이상 신체적 위해가 없음이 확인된 경우에는 다시 원칙으로 돌아가 의료기관의 계약 해지권을 인정할 필요가 있다.

  • 시맨틱 웹 기반의 서비스를 제공하기 위해서는 특정 도메인 지식에 대한 명시적인 명세화 및 지식의 개념과 개념과의 관계를 정형화하는 온톨로지를 통해서 이루어진다. 그러므로 특정 도메인 지식의 명세화와 정형화를 위해서는 관심 있는 도메인내의 지식을 개념화하는 온톨로지의 생성이 필수적이다. 이 논문에서는 택배 마케팅의 잠재 고객에 대한 정보를 검색하기 위해, 시맨틱 웹 기반의 온톨로지 생성을 위한 구체적인 도메인을 설계하고, 생성된 온톨로지를 이용하는 정보 검색 방법을 제안하였다. 제안된 온톨로지를 기반으로 고객의 정보를 수집하는 데이터 검색 로봇을 구현 하였다. 아울러 온톨로지의 생성과 데이터 검색 로봇이 데이터 검색을 정확하게 수행함을 확인 하였다.

가족정책과 학부모정책의 연계 필요성 및 방안 탐색 (Necessity and Method of Linkage between Family Policy and Parents Support Policy)

  • Family policy and parents support policy are similar in that they are both object-oriented, not function-oriented. An object-oriented policy, different from a function-oriented policy, is demander-oriented in that it combines the existing policies from the object-perspective. Parents support policy and family policy are very similar in that they are both object-oriented policies and emphasize the perspective of the demanders: the parents and family. As is the case for all policies, if the ministries concerned with the policies are different, the projects are promoted from different delivery systems, so it is difficult to link them together. However, it is essential to relate the two policies because a partnership between families and schools, the subjects of the policy, is the basis of healthy family and a healthy society. Therefore, this research analyzes the current family policy and parents support policy, and examines the necessity and possibility of relating the two policies. This analysis is meaningful in that it seeks consilience between the policy areas, and seeks a new demander-oriented paradigm construction system.

소양면 지역사회 환경기초조사 (A Basie Community Health Survey in Rural Korea (Soyang-Myun))

  • 1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. Project area 1) Population of Soyang-Myun was 11,668; male, 5,962 and female, 5,706.   • This study was conducted to assess the effectiveness of service by maternity aids concerning maternal and child health in improving simultaneously infant mortality, contraception and vital registration among expectant mothers in rural Korea, where there is less apportunity for maternal and child health care. It is unrealistic to expect to solve this problem in rural Korea through professional persons considering the situation of medical facilities and the socioeconomic condition of residents. So, we intended to adopt a system of services by maternity aids who were educated formally among indigenous women. After the women were trained in maternal and child health, contraception, and registration for a short period, they were assigned as a maternity aids to each village to help with various activities concerning maternal and child health, for example, registration of pregnant women, home visiting to check for complications, supplying of delivery kits, attendance at delivery, persuasion of contraception, and invitation for registration and so on. Mean-while, four researchers called on the maternity aids to collect materials concerning vital events, maternal child health, contraception and registration, and to give further instruction and supervision as the program proceeded. A. Changes of women's attitude by services of maternity aid. Now, we examined to what extent' such a service system to expectant mothers affected a change in attitude of women residing in the study area as compared to women of the control area. 1) In the birth and death places, there were no changes between last and present infants, in study or control area. 2) In regard to attendants at delivery, there were no changes except for a small percentage of attendance (8%) by maternity aid in study area. But, I expect that more maternity sids could be used as attendants at delivery if they would be trained further and if there was more explanation to the residents about such a service. 3) Considering the rate of utilization of sterilized delivery kit, I am sure that more than 90 percent would be used if the delivery kit were supplied in the proper time. There were significant differences in rates between the study and the control areas. 4) Taking into consideration the utilization rate of the clinic for prenatal care and well baby care, if suck facilities were installed, it would probably be well utilized. 5) In the contraception, the rate of approval was as high as 89 percent in study area as compared to 82 percent in the control area. 6) Considering the rate of pre-and post-partum acceptance on contraception were as much as 70 percent or more, if motivation to use contraception was given to them adequately, the government could reach the goals for family planning as planned. 7) In the vital registration, the rate of birth registration in the study area was some what improved compared to that of the control area, while the rate of death registration was not changed at all. Taking into account the fact that the rate of confirmation of vital events by maternity aids was remarkably high, if the registration system changed to a 'notification' system instead of formal registration ststem, it would be improved significantly compared to present system. B. Effect of the project Thus, with changes in the residents' attitude, was there a reduction in the infant death rate? 1) It is very difficult problem to compare the mortality of infants between last and present infants, because many women don't want to answer accurately about their dead children especially the infants that died within a few days after birth. In this study the data of present death comes from the maternity aides who followed up every pregnancy they had recorded to see what had happened. They seem to have very reliable information on what happened in first few weeks with follow up visitits to check out later changes. From these calculaton, when we compared the rate of infant death between last and present infant, there was remarkable reduction of death rate for present infant compare to that of last children, namely, the former was 30, while the latter 42. The figure is the lowest rate that I have ever heard. As the quality of data we could assess by comparing the causes of death. In the current death rate by communicable disease was much lower compare to the last child especially, tetanus cases and pneumonia. 2) Next, how many respondents used contraception after birth because of frequent contact with the maternity aid. In the registered cases, the respondents showed a tendency to practice contraception at an earlier age and with a small number of children. In a comparison of the rate of contraception between the study and the control area, the rate in the former was significantly higher than that of the latter. What is more, the proportion favoring smaller numbers of children and younger women rose in the study area as compared to the control area. 3) Regarding vital registration, though the rate of registration was gradually improved by efforts of maternity aid, it would be better to change the registration system. 4) In the crude birth rate, the rate in the study area was 22.2 while in the control area was 26.5. Natural increase rate showed 15.4 in the study area, while control area was 19.1. 5) In assessment of the efficiency of the maternity aids judging by the cost-effect viewpoint, the workers in the Medium area seemed to be more efficiency than those of other areas.

유아 어머니의 자녀 성교육 실천정도 (Research on Sex Education of Young Children's Mothers)

  • Purpose: The present study is a descriptive research on the content of sex education conducted at home by the mothers of young children who are attending five kindergartens located in Busan, Korea. This study aims to provide basic materials for systematic sex education at home and in the school. Method: Based on the literature review and prior researches, the writer made a questionnaire consisting of 30 items regarding necessities, contents and difficulties of sex education. The 307 questionnaires were collected out of 480 which were distributed by the kindergarten teachers. Data was analysed with frequency and percentage using the SPSS Win 10.0 Program. The main subjects of the study were mothers in their thirties, who understood the necessity of sex education. However, they were lack of enough sexual knowledge in accordance with the transforming sexual culture and lack of suitable materials. They did not plan to educate their children according to their developmental stages, but just taught them verbally rather than using common materials for the situation. Result Regarding the origin of life that children often asks, mothers did not give a scientific and correct response to satisfy their curiosity about sex, saying that a baby was created after marrying a loved one. While more mothers than the prior research responded correctly to the queries of the path of delivery, some mothers did not. It may cause some children to suppress the questions, which may result a distorted sexual knowledge. That is because mothers understood that sex education was to deliver sexual knowledge, but not realized that sex education was a human education carried out throughout life. Mothers fell short of sexual knowledge, capability, and confidence, comparing with swift social changes and children's capacity. As a result, it is necessary to inspire sexual awareness into mothers and educate them through training. It is also needed to secure a training program similar to a teacher training program. Conclusion: It is essential to develop and to provide evaluation tools as well as proper materials in accordance with the children's developmental stage, so that organic management system among home, education institute and society must lead sex education Further, it is necessary to provide teaching methods and materials to mothers regularly, so that the school and home may conduct sex education together to cope with difficulties.

한국 소아청소년 완화의료의 발전 방안 제언: 국외 제공체계의 시사점을 중심으로 (How to Implement Quality Pediatric Palliative Care Services in South Korea: Lessons from Other Countries)

  • 목적: 소아청소년 호스피스 완화의료(이하 소아완화의료)는 생명을 위협하는 질환을 가진 소아청소년의 삶의 질을 향상시기 위한 총체적 돌봄 철학이자 실무의 표준이다. 국내에서는 2018년 7월 국가 지원의 소아완화의료 시범사업을 도입하였는데, 소아완화의료의 발전 방향을 제시하기 위해 국외 선진국의 소아완화의료 제공체계를 고찰하였다. 방법: 소아완화의료 제공 수준을 검토하여 영국, 미국, 일본, 싱가포르를 대상으로 선정하였다. 소아완화의료 제공체계를 다룬 국내외 학술지 등 문헌을 통합적으로 고찰하고 현지 전문가의 자문과 현지 방문조사를 수행하였다. 1990년 이후 영어, 일본어로 발간된 문헌을 중심으로 PubMed, Google, Google Scholar에서 검색하여 학술지, 정책보고서 등을 참고하였다. 각국의 소아완화의료 발전과정, 정책, 재정 모델, 대상 기준, 전달 체계, 질 관리 체계에 대해 분석하였다. 결과: 영국은 지역사회의 독립형 소아전문 완화의료기관이 일차 의료체계와 협력하며 어린이병원의 전문 소아완화의료 자문팀과 의뢰와 자문을 주고받는다. 미국은 병원기반의 전문 소아완화의료 자문팀을 중심으로 지역사회의 호스피스기관, 가정의료기관이 네트워크를 구축하고 돌봄 코디네이터를 지정하여 소아완화의료를 제공한다. 일본은 완화의료, 재택의료, 장애아동 및 만성질환아동 지원체계에서 완화의료 성격의 서비스를 제공한다. 싱가포르는 소아전문 가정 완화의료 단체가 어린이병원의 전문 자문팀과 협력하여 높은 지리적 접근성을 토대로 중추적으로 완화의료를 제공한다. 결론: 국외의 제공체계를 참고하고 국내의 현장의 요구를 반영하여 소아완화의료의 제공체계를 정비하여 미충족 요구가 발생하지 않도록 한국 소아청소년 호스피스완화의료를 최적화해 나가야 한다.

여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 - (Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

가정간호 시범사업 간호진단 및 간호중재 분석 연구 (A Study Analyzing Nursing Diagnoses and Nursing Interventions used in a Demonstration Home Care Project)

  • As home care in developing and becoming part of the health care delivery system in Korea, it is necessary to examine the use of nursing diagnoses and related nursing interventions with a view to increasing the standardization of nursing recording. This study was done to examine the nursing diagnosis and related nursing interventions used in home care. Data were collected using a chart review of the nursing notes written for the home care given to 38 patients who had pulmonary diseases or traumatic brain or spinal cord injuries and who had received home care as part of a demonstration home care project in a college of Nursing in Seoul. Early on in the project discussions as to format and use to nursing diagnosis was done and a tool was developed based on Gordon's eleven functional catergories with the addition of categories to cover family and environment. This tool was used in the data collection. Data included nursing diagnosis, etiologies and interventions. Real numbers and percentages were used in the analysis. The results show that the most frequently used diagnoses were in the category of physical function (75.6%), followed by the category of emotional and social function (21.8%). The least frequently used category was the one for family and environment (2.6%). The order of the frequency of recorded nursing interventions was the same, 82.3% for physical function, 16.2% of emotional and social function and 1.5% for family and environment. Under the category of physical functioning the most frequently used nursing diagnoses were related to mobility (62.2%), nutrition (23.6%) and elimination (11.9%). The frequencies of nursing interventions for these three diagnostic categories were 69.8%, 16.0% and 10.8% respectively. For emotional and social functioning, the most frequently used diagnoses were for cognition-perception (37.1%), self-perception (30.6%) and perception of health (23.7%). The ordering of the frequency of nursing interventions varied slightly. The most frequently used interventions were for the category of self-perception (31.7%) followed by cognition-perception (24.1%) and perception of health (22.9%). Looking at individual diagnoses, it was found that within the categroy of physical functioning, the most frequently used diagnosis was "impaired physical mobility" (29.5%) and this diagnosis involved 43.9% of the interventions. This was followed by "ineffective breathing pattern" (19.4%) with 17.7% of interventions, and "alteration in nutrition, less than body requirements" (11.2%) with 8.1% of the interventions. For the emotional social category, noncompliance was the most frequently used nursing diagnosis (18.2%) with 19.2% of the interventions. This was followed by "anxiety" (13.4%) with 13.6% of the interventions and by "knowledge deficit" (13.4%) but with only 5.5% of the interventions. The other diagnoses and interventions did not follow this pattern of frequency. Although there were a large number of diagnostic and intervention events, the number of actual diagnoses and interventions used were relatively small ranging from six interventions for "knowledge deficit" to 40 interventions for "imparied physical mobility". From this it can be concluded that the results of this study could be used as basic data for the development of standardized charts with respect to nursing diagnosis and interventions for clients with pulmonary disease and clients with traumatic brain or spinal cord injuries. Interventions that were direct care activities (1178) were much more frequent that education (430), and assessment and observation (148). There were also few diagnoses or interventions related to the family and the environment. This suggests two areas that need to be developed in home care and that need to be considered in the development of standardized records for use in home care.
