치매노인 배우자의 적응경험에 관한 근거이론적 접근 (A Grounded Theory Approach to the Adjustment Experiences in Spouses of Elderly People with Dementia)

  • Purpose: This study explored the adjustment experiences of spouses of elderly people with dementia in Korea. Methods: A grounded theory method was used. The data were collected by individual in-depth interviews with 11 spouses of elderly people with dementia. Results: In the analysis, 17 categories were derived. A core category emerged: Adjusting to daily life of providing love-based care. Eight major strategies for dealing with the psychosocial problems faced by spouses of elderly people with dementia: Feeling one's own responsibility in caring; halting personal activities; adopting a spouse-oriented daily lifestyle; caring with love; efforts for maintaining one's own health; efforts for maintaining spouse's health; depending on religious belief; and trying to have time for oneself. The consequences that resulted from applying these strategies were identified as "feeling worthwhile for caring," "maintaining a peaceful life," and "being grateful of the present." Conclusions: This study presents a holistic view of the adjustment experiences of spouses of elderly people with dementia. This study can also provide directions for developing interventions for enhancing the quality of life of spouses of elderly people with dementia.

생명현상의 다양성과 특징에 관한 고찰 - 보건의료에 미치는 영향을 중심으로 - (Diversity and its Characteristics of Life Phenomenon)

  • This research was intended to delve into the diversity of life phenomenon and its characteristics. First of all, this research gave real examples to compare the differences in men's health, disease, and longevity in order to confirm the existence of diversity of life phenomenon. In addition, it also studied the process and mechanism of manifestation of life phenomenon, as well as the influence and problems of existing studies' results and implications. The results are as follow. 1. Differences in health, diseases, and longevity were very big and diverse in researches on different races, nations, ages, socioeconomic status, positions, and even (monozygotic) twins. 2. The basic foundation of all organisms is DNA, and environmental factors change DNA methylation and the structure of chromatin by constantly influencing DNA. Due to this, the manifestation, control, and phenotype of DNA change, resulting in diversified life phenomenon. Therefore, it is the environmental factors, not DNA, that has more influence on the diversity of life. 3. Looking at available studies, the most reasonable perspective on human requires focusing on the diversity of life phenomenon, holistic thinking, and reversible change instead of irreversible determinism. Considerable differences in life phenomenon between entities require a change in malformed perspective on life. Public health and medicine deals with live human beings, a more precise and accurate perspective on life is very important. Because management methods of health and disease, such as structure and approaches of medical research, prevention and cure, must be different by life perspectives.

우리나라 토양환경보전 정책의 패러다임 전환 (Paradigm Shift in Policy of Soil Environment Conservation in Korea)

  • This paper reviews the soil conservation policies (SCP) in the global community and suggests the improved options in SCP in Korea. Soil Environment Conservation Act in Korea states soil is a valuable natural resource and it's value should be enhanced to provide the benefits that soil ecosystem can offer to people. However, SCP in Korea limits its application to not only the scope of soil environment but also the issues on soil pollution. The SCPs in the advanced countries have shifted their scopes from soil environment to soil ecosystem, put emphasis on the conservation of soil health rather than soil quality, and set the goals to optimize the soil ecosystem services to people while minimizing the soil threats. In this context, the soil security initiative was recently proposed to accomplish this goal while employing the nexus concept to bridge the soil ecosystem services with water, atmosphere, climate and biodiversity. Therefore, the key policies in soil conservation in Korea should expand the scope from soil environment to soil ecosystem, focus on soil health management, and develop the holistic governance among diverse stakeholder to maximize the soil ecosystem services. Soil ecosystem should be secured by national soil policies for human health.

한국 노인의 주관적 구강건강평가와 삶의 질 관련성 연구 (Association between Perceived Oral Health and Quality of Life in Korea Older Population)

  • 본 연구는 한국 노인의 주관적 구강평가와 일반건강평가, 삶의 질의 관련성 연구를 위하여 국민건강영양조사 제 7기 1차년도 자료 중 60세 이상 노인 1,866명(69.5세)의 자료를 SPSS 23.0으로 위계적 로지스틱 회귀분석하였다. 대상자의 87.7%가 구강상태를 보통 이하로 인식하였으며, 여성, 낮은 교육수준 일 경우 구강상태를 부정적으로 평가하였다(p<.05). 일반건강평가가 긍정적일수록 구강평가도 긍정적이었으며(F=19.04, p<.001), 공변량 통제 후 로지스틱 회귀분석에서 일반건강평가가 부정적일수록(OR: 2.86, 95% CI: 1.5-5.5), 영구치 우식이 있을수록(OR: 2.68, 95% CI: 1.67-4.32), 우울 문제가 있을수록(OR: 1.45, 95% CI: 1.42-2.57) 구강평가는 부정적이었다. 따라서, 고령자의 주관적 구강평가와 일반건강평가, 삶의 질 사이에는 유의한 관계가 있으므로 삶의 질 향상을 위하여 통합적 정책이 이루어져야 할 것으로 사료된다.

Theoretical evaluation of Cox's interaction model of client health behavior for health promotion in adult women

  • This study aimed to evaluate Cox's interaction model of client health behavior (IMCHB) as used in studies on women's health. Using keyword combinations of "women" and "IMCHB" or "interaction model of client health behavior," we searched the PubMed, MEDLINE, Embase, and RISS databases for studies on the promotion of women's health published from January 2009 to April 2019. Finally, 11 studies were selected and evaluated according to seven criteria for theory evaluation, which combined Fawcett's theory evaluation criteria and Chinn and Kramer's criteria. We found that the IMCHB corresponds to a verifiable practical level of a middle-range theory, although it may be partially abstract. It contains all four concepts of the metaparadigm of nursing, in terms of a holistic philosophical approach. A theoretical evaluation demonstrated that the IMCHB has significance, generality, testability, empirical adequacy, and pragmatic adequacy for nursing practice and research. However, the lack of clear conceptual definitions and the presence of complex relationships among concepts resulted in a lack of internal consistency and parsimony. According to an in-depth verification through a review of the literature, the IMCHB has been used as a health promotion intervention strategy for various populations of women and has led to useful results in nursing practice. The IMCHB was confirmed to be a suitable theory for experimental and clinical research. Future research can build on this middle-range theory for women's health research and practice.

Spiritual Care for Cancer Patients in Iran

  • Background: Studies have shown that a return to spirituality is a major coping response in cancer patients so that therapists can adopt a holistic approach by addressing spirituality in their patient care. The present study was conducted to develop a guideline in the spiritual field for healthcare providers who serve cancer patients in Iran. Materials and Methods: Relevant statements were extracted from scientific documents that through study questions were reviewed and modified by a consensus panel. Results: The statements were arranged in six areas, including spiritual needs assessment, spiritual care candidates, the main components of spiritual care, spiritual care providers, the settings of spiritual care and the resources and facilities for spiritual care. Conclusions: In addition to the development and preparation of these guidelines, health policy-makers should also seek to motivate and train health service providers to offer these services and facilitate their provision and help with widespread implementation.

당뇨병 노인환자의 구강보건교육 시 고려사항 분석: 포커스 그룹 인터뷰 (Analysis of considerations in oral health education for elderly diabetic patients: Focus group interview)

  • 본 연구는 65세 이상 노인 중 일반인과 당뇨병 진단을 받은 자를 대상으로 그룹 인터뷰를 통해 구강건강과 구강보건교육 시 고려사항을 파악하고자 위함이다. 연구 대상자는 G시 노인종합복지관을 방문하는 65세 이상 건강인 5명과 당뇨병 환자 5명을 2그룹으로 나누어 각 그룹 당 60-70분의 인터뷰를 진행하였다. 연구결과 핵심 주제는 '당뇨병과 구강질환과의 연관성에 관한 지식', '구강건강상태', '구강건강관리 방법', '구강보건교육 시 고려사항' 4가지의 주제모음이 도출되었다. 본 연구에 참여한 건강인들은 구강보건교육이 필요하다고 생각하는 내용이 치주관리방법, 임플란트, 전체적 구강관리, 전신건강이라고 대답하였고, 당뇨병 환자들은 당뇨병, 치주관리방법, 구강관리실천의 중요성이라고 대답하였다. 본 연구결과를 토대로 추후, 당뇨병 노인환자을 대상으로 맞춤형 구강보건교육 프로그램 개발이 필요할 것으로 사료된다.

여성건강과 성 (Women's Health and Sexuality)

  • The purpose of this study is to describe how what influence sexuality has on women's health. Sex is determined by the sex chromosome: but sociocultural norms have much influence on the sex role of a woman or man. Women's sexuality has had a negative impact on them in a male-dominated society, which destroyed women's health, put women in a powerless position and forced them to live as dependent persons. Sociocultural perception of the sex role has not been very open, and very strict rules have controlled those perceptions; but currently these perceptions have been changing dramatically. Especially, women's sex role has changed, bringing about many problems: the number of women engaging in premarital sex, the number of unwed mothers, the number of pregnancies without marriage, the divorce rate, and the number of dysfunctional families have all increased. Those kinds of problems have negative effects on women, children and members of the whole family. Sexually transmitted disease because of free sex is a serious health issue for women: the number of women with AIDS has increased rapidly. Another big issue is sexual abuse, which is insulting to women, decreases women's self-esteem, increases depression, puts women in a powerless position and eventually causes women to get sick. Male-preference (among newborns) ideology raises health issues for women, such as artificial abortion. In the area of sex differentiation, therefore, we have to change people's thinking from male-preference ideology to equal sex preference. Finally, we have to use a holistic approach for women's health and increase awareness of the fact that the sex role and women's health are very important for the family, society and nation. Women's health is the nation's power.

Korean Adolescents' Experience of Yoga Class: "Healthy Habits Beyond Exercise"

  • Purpose: This study was designed to explore adolescents' experience with yoga class in South Korea. Methods: Qualitative data were collected by focus group interviews from February to March 2014. Eleven adolescents who experienced yoga class were recruited from two different schools. The interview questions investigated adolescents' experience with yoga regarding expectations before yoga class, positive changes after yoga, and advice for future yoga classes. Results: Three primary themes from the qualitative content analysis include 1) motivation to join a yoga class, 2) perceived benefits after class and 3) suggestions for school-based yoga. We found that high school students had intrinsic and extrinsic motivations to join yoga classes, and the perceived benefits included physical, psychological, cognitive and social aspects. The students also indicated the need for support by person, yoga with fun and information, yoga tailored by individual goal, simple and easy class. Conclusion: These results suggest that yoga could be an appropriate intervention for holistic health care, and school-based yoga should be applied by focusing on the various situational needs of adolescents'. Furthermore, when yoga leads to healthy habits compared to just simple exercise, a balanced development of adolescents can be achieved.

Infant-rearing experiences of parents during the COVID-19 pandemic in South Korea: a mixed-methods approach

  • Purpose: This study aimed to identify the infant-rearing experiences of parents during the coronavirus disease 2019 (COVID-19) pandemic and provide foundational data for the development of infant-rearing support programs during pandemic situations. Methods: Convergent mixed methods were used to better understand the research outcomes by converging both quantitative and qualitative data. A total of 149 parents with infant-rearing experiences during the pandemic responded to a self-report survey, and 10 parents participated in the interviews. Data were analyzed using Colaizzi's method, descriptive statistics, t-test, one-way analysis of variance, the Scheffé test, Pearson correlation coefficients, and hierarchical regression. Results: Analysis of qualitative data yielded the following three categories: five theme clusters, ten themes, and thirty-nine subthemes. The factors influencing infant-rearing behavior were nuclear family (β=.34, p<.001) and rearing stress (β=-.39, p<.001). The explanatory power of the regression equation was 26.6%. Conclusion: Infectious disease disasters, such as the COVID-19 pandemic, can quickly alter infant-rearing conditions, causing heightened parental anxiety. This may affect infant-rearing behaviors and hinder healthy infant development. Future research should develop a comprehensive tool to measure holistic health-related parenting behaviors across the different stages of child development. Additionally, pediatric nurse practitioners can play an active role in educating parents, supporting parenting, and promoting healthy infant development in their communities, making pediatric nurse practitioners a highly relevant and necessary healthcare profession during infectious disease disasters. Thus, there is a need to improve institutions and build infrastructure at the national level to support them.
