A clinical review of community acquired methicillin resistant staphylococcal scalded skin syndrome (지역사회 획득 메치실린 내성 포도알균에 의한 포도알균 열상 피부 증후군의 임상적 고찰)

  • Purpose : Staphylococcal scalded skin syndrome (4S) is a well known disease defined by clinical, microbiological and histological criteria caused by Staphylococcus aureus. This disease is uncommon but has been increasingly recognized. We investigated the clinical features of staphylococcal scalded skin syndrome. Methods : We reviewed retrospectively medical records of 53 patients diagnosis of staphylococcal scalded skin syndrome who were admitted to Changwon Fatima hospital from February 2002 to December 2005. These patients were divided into 3 clinical types; generalized type, intermediate type, abortive type. Age, sex ratio, clinical manifestations, laboratory findings, response to therapy and prognosis were investigated. Result : 1)The mean age of patients was 2.8 years, ranging from 20 days to 7 years. Male-to-female ratio was 1.9:1. 2) By clinical types, 6 patients were in the generalized type (11%), 29 patients in the intermediate type (55%), 18 patients in the abortive type (34%). The coexisting diseases were variable, including conjunctivitis (25 cases), atopic dermatitis (11 cases), otitis media (1 case). On laboratory findings, most of patients didn't have leukocytosis or increased C-reactive protein. 4) A total of fifteen Methicillin Resistant Staphylococcal Aureus (MRSA) strains were isolated from September 2003 through December 2005. Fourteen strains were positive for exfoliative toxin B gene by PCR and negative for enterotoxin, toxic shock syndrome toxin and Panton-Valentine leukocidin genes. 5) The mean duration of admission was 7.3 days. Patients were treated with vancomycin or amoxacillin/clavulanate or ampicillin/sulbactam or cefuroxime without significant sequelaes. Conclusion : Recently, Staphylococcal scalded skin syndrome caused by exfoliative toxin B produced by MRSA in the Changwon area has been increasing.

Synergistic Inhibition of Aronia melanocarpa and Moringa oleifera Seed Extract on Experimental Atopic Dermatitis (아로니아 및 모링가 종자 복합물의 항아토피 상승효과)

  • Atopic dermatitis is a chronic, relapsing inflammatory skin disease. This study aimed to investigate the therapeutic benefits of Aronia melanocarpa (AM) and Moringa oleifera seed extract (MO) on experimental atopic dermatitis. We examined the effects of AM or MO and their combination on 2,4-dinitrochlorobenzene (DNCB)-induced atopic dermatitis in BALB/c mice as well as tumor necrosis factor $(TNF)-{\alpha}$ and interferon $(IFN)-{\gamma}-stimulated$ HaCaT keratinocytes. Mice were orally treated with extract during repeated application of DNCB to shaved dorsal skin. Our results show that treatment with AM and MO in combination reduced histological manifestations such as epidermal hyperplasia and inflammatory cell infiltration. Furthermore, it significantly decreased skin thickness and serum immunoglobulin E (IgE) level compared to the AM or MO alone treated group. Combined extract of AM and MO suppressed expression of $TNF-{\alpha}/IFN-{\gamma}-induced$ T helper 2 (Th2) chemokines such as thymus and activation-regulated chemokine and macrophage-derived chemokine. To sum up, combination of AM and MO suppressed the inflammatory response and serum IgE as an indicator of several allergic diseases in DNCB-induced experimental atopic dermatitis and Th2 chemokine expression in HaCaT cells. This result suggests that combination of AM and MO could be a valuable strategy to improve atopic dermatitis.

  • Squamous cell carcinoma (SCC) in head and neck show a variability in the response to chemotherapy, even when it present with similar histological tumor type, grade, and clinical stage. The purpose of present study it to identify predictive bio-marker for the sensitivity or resistance to conventional chemotherapeutic agents, 5-fluorouracil (5-FU) and Cisplatin Oral cancer cell lines were used in present study. MTT assay was performed to evaluate the sensitivity and/or resistance to 5-FU and Cisplatin. And RT-PCR was carried out for evaluation of the mRNA expressions of various genes associated with mutation, inflammation (COX pathway), cell cycle, senescence and extracellular matrix (ECM). The molecules which are correlated with the sensitivity to 5-FU are XPA, XPC, OGG, APEX, COX-2, PPAR, Cyclin E, Cyclin B1, CDC2, hTERT, hTR, TIMP-3, TIMP-4 and HSP47. And the molecules are correlated with the sensitivity to Cisplatin are COX-1, iNOS, eNOS, PCNA, collagen 1 and MMP-9. Taken together, when choosing the appropriate chemotherpeutic agents for patients, considering the molecules which are correlated or reversely correlated is helpful to choose the resonable agents for cancer patients.

Effect of Exercise Training on Aging Atrophyin Rat Skeletal Muscle III. Effect of Short Term Exercise Training for Senile Rat (흰쥐 골격근의 노화성 위축에 대한 운동훈련의 영향 III.노화 흰쥐에 적용한 단기간의 운동훈련의 영향)

  • The present study was designed to examine effect of short term treadmill and weight-training on aging arophy in the rat skeletal muscle. Male rats of 24 months old were used. Each groups included control, treadmill and weight-training for 4 weeks by using treadmill apparatus and body press apparatus. The histo and cytochemical, ultrastructural and stereological changes in senile skeletal muscles of the rat were observed in the present study. During the training period the body weight and muscular weight in all groups remained constant. The volume density of muscle fiber type IIC and IIB were increased, that of type IIA was decreased, but type I remained constant in treadmill-training group. In weight-training rat, the density of type IIA and IIB were increased, both those of type IIC was decreased. But, all changes of muscle fiber type is not significant. Senile control group some usual formation of mild contraction band, liposuscin pigment and muscular splitting were observed. After treadmill-training, histological and ultrastructural changes occurred in the muscle fiber, such as irregularity of the sarcolemma, interfibrillar vacuolization, longitudinal splitting, and widened I-bond. After weight-training, the changes occurred in the trained muscle fiber, such as appearances of many lysosomes and autophagic vacuoles, severe contraction band, and breakup of myofibrils. Histo and cytochemical studies showed that the activities of succinic dehydrogenase and acid phosphatase remained constant, activities of $Mg^{++}$-ATPase decrease with training. Stereological changes were not observed in the volume and numerical density of all subject component, but the surface density of mitochondrial inner membrane was increased with treadmill-training. These experimental results suggested that endurance training during short-term may result in the adaptible response in senile skeletal muscles. On the other side, weight-training is bad for senile skeletal muscle.

Anti-Inflammatory Effect of Chondrus ocellatus Holmes Ethanol Extract on Lipopolysaccharide-induced Inflammatory Responses in RAW 264.7 Cells (Lipopolysaccharide로 유도된 RAW 264.7 세포와 마우스모델에 대한 진두발 에탄올 추출물의 항염증 효과)

  • This study aimed to investigate the anti-inflammatory effect of the ethanol extract from Chondrus ocellatus Holmes (COHEE) in RAW 264.7 cells and in a mouse ear edema model, by measuring the production of lipopolysaccharide-induced inflammatory response mediators. There were no cytotoxic effects on the proliferation of macrophages treated with COHEE compared with the control. COHEE inhibited the production of nitric oxide and pro-inflammatory cytokines [interleukin (IL)-6, tumor necrosis factor-α, and IL-1β]. The extract also reduced the expression of inducible nitric oxide synthase, cyclooxygenase-2, nuclear factor-κB p65, and phosphorylated mitogen-activated protein kinase in a dose-dependent manner. In the croton-oil-induced ear edema model, COHEE decreased the formation of mouse ear edema at the highest dose compared with the control, and histological analysis revealed that the epidermal/dermal tissue thickness and mast cell numbers were reduced. Therefore, these results suggest that COHEE may be a promising topical anti-inflammatory therapeutic material through its action of modulating NF-κB and the MAPK signaling pathway.

Changes of Glutamate and Polyamine Levels of Hippocampal Microdialysates in Response to Occlusion of Both Carotid Arteries in Mongolian Gerbils (뇌허혈 손상에 있어서 해마-세포외액내 Glutamate와 Polyamine 농도의 변동에 관한 연구)

  • Reversible brain ischemia was produced by occluding both common carotid arteries for 5 min, and the effects of aminoguanidine (AG), $DL-{\alpha}-difluoromethylornithine$ (DFMO), MK-801, and nimodipine (NM) on the ischemia induced changes of the polyamine, glutamate and acetylcholine levels in the hippocampus CA1 subfield and the specific $[^3H]\;MK-801$ binding to the hippocampus synaptosomal membranes were studied with a histological reference of the cresyl violet stained hippocampus. The basal putrescine level $(PT:\;74.4{\pm}8.8\;nM)$ showed a rapid increase (up to 1.7 fold) for 5 min of ischemia, remained significantly increased for 6 h, and then resumed the further increase to amount gradually up to about 3 fold 96 h after recirculation. However, the level of spermidine was little changed, and the spermine level showed a transient increase during ischemia followed by a sustained decrease to about 40% of the preischemic level after recirculation. The increase of PT level induced by brain ischemia was enhanced with AG or MK-801, but it was reduced by DFMO or NM. The basal glutamate level $(GT:\;0.90{\pm}0.l4\;{\mu}M)$ rapidly increased to a peak level of $8.19{\pm}1.14\;{\mu}M$ within 5 min after onset of the ischemia and then decreased to the preischemic level in about 25 min after recirculation. And NM reduced the ischemia induced increase of GT level by about 25%, but AG, DFMO and MK-801 did not affect the GT increase. The basal acetylcholine level $(ACh:\;118.0{\pm}10.5\;{\mu}M)$ did little change during/after brain ischemia and was little affected by AG or NM. But DFMO and MK-801, respectively, produced the moderate decrease of ACh level. The specific $[^3H]\;MK-801$ binding to the hippocampus synaptosomal membrane was little affected by brain ischemia for 5 min. The control value (78.9 fmole/mg protein) was moderately decreased by AG and MK-801, respectively but was little changed by DFMO or NM. The microscopic findings of the brains extirpated on day 7 after ischemia showed severe neuronal damage of the hippocampus, particularly CA1 subfield. NM and AG moderately attenuated the delayed neuronal damage, and DFMO, on the contrary, aggravated the ischemia induced damage. However, MK-801 did not protect the hippocampus from ischemic damage. These results suggest that unlike to the mode of anti-ischemic action of NM, AG might protect the hippocampus from ischemic injury as being negatively regulatory on the N-methyl-D-aspartate (NMDA) receptor function in the hippocampus.

Changes in Biochemical Components of Several Tissues in Sinonovacula constricta, in Relation to Gonad Developmental Phases (가리맛조개, Sinonovacula constricta의 생식소 발달단계에 따른 일부 조직의 생화학적 성분 변화)

  • We investigated the reproductive cycle with gonad developmental phases of Sinonovacula constricta by histological observations, and seasonal changes in biochemical component of the adductor muscle, visceral mass, foot muscle and mantle were studied by biochemical analysis, from January to December, 2004. The reproductive cycle of this species can be classifed into five successive stages: early active stage (March to May), late active stage (May to July), ripe stage (July to September), partially spawned stage (August to October) and spentfinactive stage (October to March). Total protein content was the highest in the following order: adductor muscle, visceral mass, foot muscle, and mantle. Except for mantle, it was generally higher during the ripe and spawning stages, while lower during the spent/inactive stage. There were positive correlations in total protein contents among adductor muscle, foot muscle, and visceral mass. However, the correlations were not statistically significant. Total lipid content was the highest in the visceral mass; it was more than 5 or 6-fold higher than those in the adductor muscle, foot muscle, or mantle. The monthly change was also most dynamic in the visceral mass. It first Increased during the early active stage (March to May), decreased during late active stage (May to July), and then increased again rapidly during the spawning stage (September). There were a strong negative correlation in total lipid contents between foot muscle and adductor muscle (r = -0.634, p = 0.027), and a strong positive correlation between adductor muscle and mantle (r = 0.665, p = 0.018). Glycogen contents showed more or less similar pattern to total lipid contents in the adductor muscle, foot muscle, and visceral mass. It was higher during the early active and spawning stages, while lower during the late active and spent/inactive stages. There was no statistically significant correlation in glycogen contents among different tissues. Especially, total lipid content showed a negative correlationship between the foot muscle, adductor muscle, visceral mass and mantle. Therefore, these results indicate that the nutrient content of the foot muscle, adductor muscle, viseral mass and mantle changed in response to gonadal energy needs.

바지락, Ruditapes philippinarum의 생식소 발달 단계에 따른 폐각근 조직과 내장낭 조직의 생화학적 성분 변화

On the whole, changes in total lipid contents showed a negative correlationship between the adductor muscle and visceral mass tissues (r = -0.699, p<0.05). Changes in glycogen contents in the adductor muscle tissues reached the maximum in the late active and ripe stages (April) and rapidly decreased in the partially spawned stage (May to October), while their contents in the visceral mass tissues reached the maximum in the early and late active stages (February) and rapidly decreased in the late active stage (March). Thereafter, their levels gradually increased in the ripe and partially spawned stages (April to July). On the whole, changes in glycogen contents appeared no correlationship between the adductor muscle and visceral mass tissues (r = 0.062, p > 0.05). These results indicate that the adductor muscle and visceral mass tissues are an important energy storage and nutrient supply organ in the Manila clams, and the nutrient contents of the adductor muscle and visceral muscle tissues change in response to gonadal energy needs.

Docetaxel as Second-line Monotherapy for Advanced Non-small Cell Lung Cancer (진행성 비소세포폐암의 이차항암화학요법으로서 Docetaxel 단독요법의 성적)

  • Background : The survival benefit associated with first-line chemotherapy in lung cancer has led to the need for second-line chemotherapy, for which Docetaxel ($Taxotere^{(R)}$) has proven efficacy in both settings. This study evaluated the safety and efficacy of docetaxel in patients with non-small cell lung cancer who had failed first-line platinum-based chemotherapy. Methods : Thirty one patients with non-small-cell lung cancer, who had failed first-line platinum-based chemotherapy, between March 1999 and August 2003, were enrolled in this study. Patients received intravenous docetaxel, either $75mg/m^2$ or $100mg/m^2$, with routine premedication every three weeks. Results : Fourteen patients (45.2%) had a partial response. The median survival and progression-free survival times were 12.5 months (95% CI 7.3-17.6) and 3.0 months (95% CI 1.6-4.5), respectively. This study showed 2 factors gave different survival benefits; the age (< 60 years: 20.1 months vs. ${\geq}60years$: 6.6 months, p = 0.0105) and the histological type (adenocarcinoma: 25.6 months vs. others: 7.9 months, p=0.0055). The predominant toxicity was neutropenia, which occurred as WHO grade 3 or 4 in 38.7 % of patients. One treatment-related death was also reported. Non-hematological toxicity was minor and easily controlled. There were no significant statistical differences in the survival benefit and toxicity between the two doses. Conclusion : Docetaxel, as second-line monotherapy, was well tolerated and effective in patients with non-small-cell lung cancer who failed first-line platinum-based chemotherapy.

The Study of Bone Healing Response According to the Various Surfaces of the Implant in the Mandible of the Mature Dog (성견 하악골에 식립한 다양한 임플란트 표면에서의 골 치유 양상에 관한 연구)

  • There are many reports that these implant surface treatments may affect to cellular reaction in the surface of implant. This study was done by installing the 8 type implant with the variable surface treatment, used or developed, in the mandible of the mature dog to evaluate how the method of surface treatment of the implant can affect to the bone healing by analyzing histologically and histomorphometrically and find out bone healing appearance periodically after installing implant. By using the 8 type implants which have the different surface treatment, 72 implants were installed on the mandible of 9 mature dogs, and 3 dogs were sacrificed on every 2, 4, 8 weeks. After making bone fragment by cutting and managing, we analyzed histologically, then compared with BIC(Bone to implant contact) for the histomorphometrical analysis. In the result of histological analysis, there was large amount of bone formation in good state on the adjacent area of implant in the 2 weeks testing group. At 4 weeks, although there was general bone formation, the new bone was separated with the basal bone. At 8 weeks, the new bone became matured and connected tightly to the basal bone. There was no difference in the each surface of 8 implants. In the result of histomorphometrical analysis, 2 weeks group had considerably lower value than 4 and 8 weeks group, and there was no difference between 4 and 8 weeks group. There was no difference in the each surface treatment of implants.
