Purpose: Internal fixation using compression hip screws (CHS) and traction tables placing patients in the supine position is a gold standard option for treating intertrochanteric fractures; however, at our institution, we approach this treatment with patients in a lateral decubitus position. Here, the results of 100 consecutive elderly (i.e., ${\geq}45$ years of age) patients who underwent internal fixation with CHS in lateral decubitus position are analyzed. Materials and Methods: Between March 2009 and May 2011, 100 consecutive elderly patients who underwent internal fixation with CHS for femoral intertrochanteric fracture were retrospectively reviewed. Clinical outcomes (i.e., Koval score, Harris hip score [HHS]) and radiographic outcomes (i.e., bone union time, amount of sliding of lag screw, tip-apex distance [TAD]) were evaluated. Results: Clinical assessments revealed that the average postoperative Koval score decreased from 1.4 to 2.6 (range, 0-5; P<0.05); HHS was 85 (range, 72-90); and mean bone union time was 5.0 (range, 2.0-8.2) months. Radiographic assessments revealed that anteroposterior average TAD was 6.95 (range, 1.27-14.63) mm; lateral average TAD was 7.26 (range, 1.20-18.43) mm; total average TAD was 14.21 (range, 2.47-28.66) mm; average lag screw sliding was 4.63 (range, 0-44.81) mm; and average angulation was varus $0.72^{\circ}$(range, $-7.6^{\circ}-12.7^{\circ}$). There were no cases of screw tip migration or nonunion, however, there were four cases of excessive screw sliding and six cases of varus angulation at more than $5^{\circ}$. Conclusion: CHS fixation in lateral decubitus position provides favorable clinical and radiological outcomes. This technique is advisable for regular CHS fixation of intertrochanteric fractures.
PURPOSE: This study was to evaluate the muscle activity of gluteus medius, tensor fascia latae, and quadratus lumborum during side-lying abduction exercise in various pelvic tilting positions. METHODS: We measured the activity of three muscles in three pelvic tilt positions for 17 normal subjects with performing the side-lying hip abduction. Three pelvic tilt positions were posterior tilt, neutral tilt and anterior tilt. We used the mean value after participants performed the hip abduction three times each position. RESULTS: The activity of gluteus medius within three pelvic positions showed the highest activity in pelvic posterior tilt position and the lowest in pelvic neutral tilt position (p = .04). The activity of tensor fascia latae showed the lowest in pelvic posterior tilt position and the highest in pelvic posterior tilt position (p = .00). The activity of quadratus lumborum revealed the lowest activity in pelvic neutral tilt position and the highest in pelvic anterior tilt position (p = .00). The activity of selective gluteus medius activation according to pelvic displacement showed the highest activity in pelvic neutral tilt position and lowest in pelvic anterior tilt position (p = .00). CONCLUSION: Hip abduction with Pelvic posterior tilt position may be effective in increasing gluteus medius and may be effective in strengthening exercise program for the gluteus medius. In addition, Hip abduction with pelvic neutral position may have an effect on the selective gluteus medius, which is considered to be effective in the exercise program for muscle reeducation training of the gluteus medius.
Previous studies of kinematic analysis of golf swing usually dealt with variations vertically. The purpose of the study was to examine the horizontal hip joints motion of the fifteen male professional golfers during driver swinging. Kinematic variables were calculated by the Kwon3D motion analysis program. Paired t-tests and one-way ANOVA were used to compare the hip height, distance, displacement, and position differences. Results showed that there were no hip height changes and no hip height differences between left and right hip from address to impact. The axis of the backswing was braced right hip, the axis of the downswing was moving left hip. Hips position at the top of the backswing showed that hips move to target prior to hands, which means the sequential motion of the chain linked body segments. From address to impact, left hip moving distance was longer than right hip(p<.001), but during the whole swing, right hip moving distance was longer than left hip(p<.001). Hip rotation angle to target line was $-48.14{\pm}9.32^{\circ}$ at top of the backswing, $40.88{\pm}8.44^{\circ}$ at impact, and $104.70{\pm}8.14^{\circ}$ at finish.
Background : To assess the effect of a pelvic compression belt on the strength of hip flexor in healthy young individuals. Study design: Pre- and post-treatment measurement design on one factor was used. Methods : 30 healthy volunteers (male: 15, female: 15) participated in this study. Dynamometer was used to measure the strength of hip flexor, and measurements were performed before and after the application of the pelvic compression belt at neutral position of hip joint and at 30 cm raised position from floor with straight leg in supine. Results : After the application of the pelvic compression belt, the strengths of hip flexor measured at both positions were significantly increased when compared with before the application (p<.05). However, at neutral position of hip joint and at 30cm raised position from floor, there were significantly different in the changing patterns in the strengths of hip flexor between men and women (p>.05). Conclusion : The findings suggest that the pelvic compression belt is helpful in strengthening hip muscles. With easy application, it is sufficiently feasible for clinical use.
Purpose: Perforator flap-using ischial sore reconstruction is performed in a prone position. But after the surgery, recurrence frequently occurs in a sitting position. In this sense, we introduce modified flap insetting method which closely resembles patient's sitting position to lessen the flap tension surgically. Materials and Methods: Authors tried to check a skin tension difference between prone position and sitting position in normal people group and to find out the importance of performing flap insetting in hip flexion position. Healthy volunteers were collected (n=20) and designed the same length of 4 divided sections around the ischium. Lengths of each section were measured when hip joint was flexed to 90 degree and when both hip and knee joints were flexed to 90 degree and the statistical evaluation was performed. Twenty cases with ischial sore underwent reconstructive surgery using perforator flap under hip flexion position and followed-up for any recurrences. Results: There was a meaningful difference between the joint flexed skin length and that of the neutral position. Flap showed sufficient thickness over 12 months. Conclusion: It seems that recurrence could be reduced when the reconstructed flap could sufficiently cover in a sitting position regarding its significant length difference in normal people group.
Objective: The purpose of this study was to apply a general linear model in statistics to marker position vectors used to study human joint rotational motion in biomechanics. Method: For this purpose, a linear model that represents the effect of the center of hip joint rotation and the rotation of the marker position on the response was formulated. Five male subjects performed hip joint functional motions, and the positions of nine markers attached on the thigh with respect to the pelvic coordinate system were acquired at the same time. With the nine marker positions, the center of hip joint rotation and marker positions on the thigh were estimated as parameters in the general linear model. Results: After examining the fitted model, this model did not fit the data appropriately. Conclusion: A refined model is required to take into account specific characteristics of longitudinal data and other covariates such as soft tissue artefacts.
The aim of this study was to investigate the characteristics of the flexion withdrawal reflex modulated during Lokomat treadmill walking in people with spinal cord injury. The influence of the limb position and movement were tested in 5 subjects with chronic spinal cord injury. EMG activities from tibialis anterior and moments of the hip joint elicited by the foot stimulation were examined during Lokomat treadmill walking. To trigger the flexion withdrawal reflex during Lokomat treadmill walking, a train of 10 stimulus pulses was applied at the skin of the medial arch. The TA EMG activity was modulated during gait phase and the largest TA reflex was obtained after heel-off and initial swing phase. During swing phase, TA EMG was 40.9% greater for the extended hip position (phase 6), compared with flexed hip position (phase 8). The measured reflex moment of the hip joint was also modulated during gait phase. In order to characterize the neural contribution of flexion reflex at the hip joint, we compared estimated moments consisted of the static and dynamic components with measured moment of the hip joint. The mean static gains of reflex hip moments for swing and stance phase are -0.1, -0.8, respectively. The mean dynamic gains of reflex hip moments are 0.25 for swing, 0.75 for stance phase. From this study, we postulate that the joint moment and muscle response of flexion withdrawal reflex have the phase-dependent modulation and linear relationship with hip angle and angular velocity for swing phase during Lokomat treadmill walking.
In orthopedics, hip arthroplasty is the operation that replaces damaged hip joint to artificial joint. In hip arthroplasty, quite better result can be achieved if robot is applied to machine cavity in bone, especially when cementless stem is used. So several kinds of robots were introduced for hip arthroplasty, but they used MRI, CT Scan, vision analysis and real time tracking of bone position for registration of robot. To overcome shortage of conventional robot surgery, gauge based registration method was proposed and small robot was designed. In this method, small robot is mounted on femur, and its position is determined by gauge registration method. Operation procedure was performed on model femur and result was analyzed. This robotic hip surgery system is expected to more adaptable in operation room.
Purpose : The purpose of this study was to investigate the change of abdominal muscle activation during unilateral leg raising in supine position among three hip abduction angles($0^{\circ}$, $15^{\circ}$, $30^{\circ}$). Methods : Twenty eight able-body volunteers who had no medical history of low back and hip joint were recruited for this study. Abdominal muscle activity was recorded using surface EMG from both sides of the rectus abdominis, internal/external oblique muscle during leg raising in each position. Results : The muscle activations induced under the three different positions were compared and results showed that there was significant difference only in the right external oblique muscle activation. Conclusion : This study suggest that leg raising of different hip abduction angles without trunk control has little influence on abdominal muscle co-activation.
The aim of this study was to identify the characteristics of the flexion withdrawal reflex modulated by the hip angle and hip movement in spinal cord injury (SCI). The influence of the hip position and passive movement were tested in 6 subjects with chronic SCI. Each subject placed in a supine position and lower leg was fixed with the knee at 5 -45 degree flexion and the ankle at 25-40 degree plantar flexion. A train of 10 stimulus pulses were applied at 200 Hz to the skin of the medial arch to trigger flexion reflexes. From results of the regression analysis, static properties of normalized muscle activation of flexor muscles have the linear relationship with respect to hip angle (P< 0.05). In order to verify the neural contribution of flexion reflex, we compared the static and dynamic gains of estimated muscle activations with measured EMG of ankle flexor muscle. Form this study, we postulate that the torque and muscle response of flexion withdrawal reflex have linear relationship with hip angle and angular velocity.
