국내 지진동 특성에 대한 기기 용접 정착부의 비탄성에너지 흡수계수를 고려한 지진취약도 평가 (Seismic Fragility Analysis of Equipment Considering the Inelastic Energy Absorption Factor of Weld Anchorage for Seismic Characteristics in Korea)

  • In Korea, most nuclear power plants were designed based on the design response spectrum of Regulatory Guide 1.60 of the NRC. However, in the case of earthquakes occurring in the country, the characteristics of seismic motions in Korea and the design response spectrum differed. The seismic motion in Korea had a higher spectral acceleration in the high-frequency range compared to the design response spectrum. The seismic capacity may be reduced when evaluating the seismic performance of the equipment with high-frequency earthquakes compared with what is evaluated by the design response spectrum for the equipment with a high natural frequency. Therefore, EPRI proposed the inelastic energy absorption factor for the equipment anchorage. In this study, the seismic performance of welding anchorage was evaluated by considering domestic seismic characteristics and EPRI's inelastic energy absorption factor. In order to reflect the characteristics of domestic earthquakes, the uniform hazard response spectrum (UHRS) of Uljin was used. Moreover, the seismic performance of the equipment was evaluated with a design response spectrum of R.G.1.60 and a uniform hazard response spectrum (UHRS) as seismic inputs. As a result, it was confirmed that the seismic performance of the weld anchorage could be increased when the inelastic energy absorption factor is used. Also, a comparative analysis was performed on the seismic capacity of the anchorage of equipment by the welding and the extended bolt.

RUSLE 기법을 이용한 경주지역의 토양침식 위험도 평가 (Risk Assessment of Soil Erosion in Gyeongju Using RUSLE Method)

  • The purpose of this study is to present the raw data for establishing the plan of top soil conservation in soil environment and preventing the soil loss by establishing the potential amount of soil loss using RUSLE. The results are as follows. To apply the RUSLE model, we calculated the potential amount of soil loss by using 5 factors; rainfall erosion factor(R), topographical factor(LS), soil erosion factor(K), land cover factor(C) and erosion control factor(P). The assessment map of soil loss was drawn up by classifying 5 grades. According to the soil loss estimation by the RUSLE, it showed that approximately 83.9% of the study area had relatively lower possibility of soil loss which was the 1 ton/ha in annual soil loss. Whereas, the 7.0% of the study area was defined as high risk area which was the 10 ton/ha in annual. Therefore, this area was needed that there was environment-friendly construction of farm land, improvement of cultivation environment and so forth. In future, if we will analyze the amount of soil loss of Gyeongju national park and Hyeongsan river watershed, we will offer the help to establishing the conservation plan of soil environment in Gyeongsangbuk-do.

Prostatectomy Provides Better Symptom-Free Survival Than Radiotherapy Among Patients With High-Risk or Locally Advanced Prostate Cancer After Neoadjuvant Hormonal Therapy

  • Purpose: The purpose of this study is to compare the radiation therapy (RT) and radical prostatectomy (RP) of high-risk or locally advanced prostate cancer (PC) patients after neoadjuvant hormonal therapy (NHT). Materials and Methods: This retrospective study evaluated patients underwent RT (42 patients) or RP (152 patients) after NHT at a single center during 2003-2014. Times to biochemical recurrence (BCR), pelvic local recurrence (PLR), metastasis, clinical painful symptom progression (CPSP), castration-resistant PC (CRPC), and overall survival were compared between the RT and RP groups, after adjustment for TN stage, using the Kaplan-Meier method and log-rank test. Results: Significant inter-group differences were observed for age, Gleason score, initial PSA, and clinical and pathological T stages (all p<0.05). During a median follow-up of 71.7 months, the overall incidences of BCR, PLR, metastasis, CPSP, CRPC, and death were 49.5%, 16.5%, 8.3%, 7.7%, 7.7%, and 17.5%, respectively. The median times to BCR were 100 months for RT and 36.2 months for RP (p=0.004), although the median times were not reached for the other outcomes (all p>0.05). The independent predictor of CPSP was RP (hazard ratio, 0.291; p=0.013). Conclusions: Despite significantly different baseline parameters, RP provided better CPSP-free survival than RT among patients with localized high-risk or locally advanced PC.

Diet and Cancer Risk in the Korean Population: A Meta-analysis

  • Many studies have found links between diet and cancer. The summary estimates of the association between dietary factors and cancer risk were investigated using previously reported studies of the Korean population. Gastric cancer risk was inversely associated with the high intake of soy foods [OR (95% CI): 0.32 (0.25-0.40) for soybean, 0.56 (0.45-0.71) for soybean curd, and 0.67 (0.46-0.98) for soymilk], allium vegetables [OR (95% CI): 0.37 (0.26-0.53) for green onion, 0.54 (0.40-0.73) for garlic, and 0.54 (0.35-0.85) for onion], fruits [OR (95% CI): 0.61 (0.42-0.88)], and mushrooms [OR (95% CI): 0.43 (0.21-0.88)]. Salt and Kimchi were associated with an increased gastric cancer risk [OR (95% CI): 1.92 (1.52-2.43) and 2.21 (1.29-3.77), respectively]. Colorectal cancer risk was positively associated with meat intake [OR (95% CI): 1.25 (1.15-1.36)]. Total soy products, soybean curd, and soymilk showed an inverse association with breast cancer risk [OR (95% CI): 0.61 (0.38-0.99), 0.47 (0.34-0.66), and 0.75 (0.57-0.98), respectively]. Green/yellow and light colored vegetables were associated with a reduced risk of breast cancer [OR (95% CI): 0.34 (0.23-0.49) and 0.44 (0.21-0.90), respectively]. Mushroom intake was inversely associated in pre-menopausal women only [OR (95% CI): 0.47 (0.26-0.86)]. In conclusion, soy foods, fruits and vegetables might reduce cancer risk in the Korean population. High salt food might be risk factor for gastric cancer, and intake of high amount of meat might cause colorectal cancer.

  • Purpose: Although obesity is a well-established risk factor for many cancers, the effect of body mass index (BMI) on oral cancer risk remains controversial. We therefore investigated the effect of BMI on oral cancer risk in a case-control study in Korea. Methods: Overall, 364 patients with oral cancer and 439 community controls were enrolled. Odds ratios (OR) and 95% confidence intervals (CI) were estimated using logistic regression models, adjusted for age, smoking status and alcohol consumption. Results: We found no overall significant evidence of an association between oral cancer risk and BMI in either gender. However, when the relationship between BMI and oral cancer risk was examined according to female age groups (<50 and ${\geq}$50 years), there was a significant association between oral cancer risk and high BMI in female subjects younger than 50 years of age (OR=3.92, 95% CI 1.03~14.9, $P$ for trend=0.04), but not in older (${\geq}$50 years) female subjects (OR=1.11, 95% CI 0.55~2.24, $P$ for trend=0.76). There was no significant relationship between BMI and oral cancer risk in any of the male age subgroups. Conclusion: Our study provides the first epidemiological evidence supporting an association between obesity and an increased risk of oral cancer.

TCD를 이용한 정상군과 중풍원인질환군의 혈류측정에 관한 비교연구 (Case-control Study : Cerebral Blood Flow as Measured by TCD in Patients with Risk Factors for Stroke)

  • Objectives : The purpose of this study was to evaluate the blood flow using doppler ultrasound of the MCA, ACA, PCA, BA, ICA in the patients with risk factor for stroke. Methods : 110 patients with risk factor for stroke were selected who had hypertension, diabetes mellitus, hyperlipidemia, or heart disease, as well as 89 healthy adults who did not have any symptoms of those diseases. To evaluate the blood flow, the Vs and Vm of the MCA, ACA, PCA, BA, and ICA in the two groups were measured. Result : In normal healthy adults, subjects showed a decrease in the Vs and Vm: with advancing in age, there was a significant difference in the Vs of MCA, PCA, BA, and ICA. There was a significant difference in the Vm of MCA, PCA, BA, and ICA. In normal healthy adults, females showed high velocities in all examined vessels. There was a significant difference in the Vs of BA. There was a significant difference in the Vm of BA, and ICA. There was a decrease in the Vs, and Vm of all examined vessels of patients with risk factors for stroke in comparison with normal healthy adults. There was no significant difference in the Vs. There was a significant difference in the Vm of MCA, ACA, and PCA. Results were the same between patients aged under 50 and total patients in the Vs and Vm of examined vessels. There were differences between patients aged over 50 and total patients in the Vs of MCA, PCA, BA and for patients aged over 50; patients with risk factor for stroke who had hypertension, diabetes mellitus, hyperlipidemia, or heat disease has higher Vs than healthy adults. Conclusions : There was a significant difference in the blood flow velocity between patients with risk factors for stroke and healthy adults. However, this result was different from results of comparison of TCD findings between patients and healthy individuals by age. Therefore, more detailed studies about aged patients are needed.

  • Objectives : The purpose of this study was to determine the influence of workers health behaviorals, oral health behaviorals and metabolic syndrome risk factors on oral health and to identify the relationship between these. and then, a basis data propose for integrative health promotion programs development and effective Management measures. Methods : The subjects were 4,600 workers working at a industrial place in North Gyeongsang Province, data were collected from July 13, 2010 to September 12, 2010. using the results of the subjects medical check-ups and Oral examinations, this study was performed. collected data included workers general characteristics, job characteristics, and smoking, drinking, exercise behavior as lifestyle factors, and waist measurement, fasting blood sugar level, systolic blood pressure, diastolic blood pressure, the level of Triglyceride and HDL-cholesterol as metabolic syndrome risk factor indicator. Data were analyzed by descriptive statistics, chi-square test, correlation analysis, logistic regression using SPSS 18.0. Results : In results of the subjects medical check-ups, 14.1% were above the criterion value of waist measurement, 2.5% fasting blood sugar level, 8.5% hypertension, 16.8% Triglyceride level and 4.0% HDL-cholesterol respectively. according to oral examination results, showing that 43.3% inflammation of the gums. The inflammation of the gums was correlated with gender, age, dental clinic visit, scaling management, smoking, exercise behavior and high triglyceride level and hypertension of metabolic syndrome risk factor indicators. In addition, this result was statistically significant. Conclusions : Based on this study, the workers health should be managed actively and effectively by using periodical workers health check-ups. At a corporate level, the institutional supports were achieved and arranged for activation of regular oral health education programs, and the prevention plan of metabolic syndrome were needed for changing exercise behavior by conducting suitable exercise programs.

환자-대조군 연구를 통한 뇌경색 위험인자에 대한 통계적 고찰 (Statistical Study on Risk Factor of Cerebral Infarction by Case-Control Study)

  • The purpose of this case-control study was done to examine the relationship among the cerebral infarction, blood lipids and homocysteine. We compared the components of blood lipids and homocysteine between cerebral infarction patients group (n=127) and controls group (n=158). We performed Pearson's chi-square test and Student's t-test for univariate analysis, Binary logistic regression for multivariate analysis to evaluate risk factors of cerebral infarction and Pearson's correlation analysis to investigate correlation between blood lipids and homocysteine. The results were as follows. The blood levels of High density lipoprotein cholesterol(HDL-Chol) and Low density lipoprotein cholesterol (LDL-Chol) were significantly lower in patients group, while age, the blood levels of Triglyceride(TG) and homocysteine were significantly higher in patients group. Hypertension had a 4.62 odds ratio, $TG{\geq}150\;mg/d{\ell}$ had a 2.33 odds ratio, HDL-Chol ${\leq}40\;mg/d{\ell}$ had a 6.85 odds ratio, but sex, DM, T-Chol, LDL-Chol had no direct relationship with odds ratio(non significant). In addition, among T-Chol and TG , HDL-Chol, LDL-Chol, they had positive correlation each other. Between TG and HDL-Chol had negative correlation each other. Homocysteine was not correlated with blood lipids adjusted for age and sex. These results suggest that low HDL-Chol and high TG may be risk factor of cerebral infarction. The correlation between homocysteine and blood lipids was not proven.y urther reserch on the subject is needed.

The association of PBX1 polymorphisms with overweight/obesity and metabolic alterations in the Korean population

  • Pre-B-cell leukemia transcription factor 1 (PBX1), which is located on chromosome 1q23, was recently reported to be associated with type 2 diabetes mellitus. We examined whether single nucleotide polymorphisms (SNPs) of the PBX1 gene are associated with overweight/obesity in a Korean population. We genotyped 66 SNPs in the PBX1 gene and investigated their association with clinical phenotypes found in 214 overweight/obese subjects and 160 control subjects using the Affymetrix Targeted Genotyping chip array. Seven SNPs (g.+75l86C>T, g.+78350C>A, g.+80646C>T, g.+138004C>T, g.+185219G>A, g.+191272A>C, and g.+265317T>A) were associated with the risk of obesity in three models (codominant, dominant, and recessive) (P=0.007-0.05). Haplotype 1 (CAC) and 3 (TAC) of block 3 and haplotype 2 (GGAAT) of block 10 were also strongly associated with the risk of obesity. In the control group, subjects that had homozygote for the major allele for both g.+185219G>A and g.+191272A>C showed lower high density lipoprotein-cholesterol (HDL-C) level compared to those possessing the minor allele, suggesting that the association between the homozygote for the major allele for both g.+185219G>A and g.+191272A>C and HDL-C is attributable to the increased risk of obesity. This study suggests that the PBX1 gene is a possible risk factor in overweight/obese patients.

Temporal Variations of Dietary Habits in a High-Risk Area for Upper Gastrointestinal Cancers: a Population-Based Study from Northern Iran

  • Background: Nutrition transition is a global health problem, especially in developing countries. It is known as an important factor for development of different types of health conditions including cancers. Objectives: We aimed to assess the pattern of nutrition transition in a high-risk area for upper gastrointestinal cancers in Northern Iran during the last decade. Materials and Methods: This cross-sectional study was conducted on households of Golestan province, Iran. Data on household food consumption between 2001 and 2010 were obtained from the Statistical Center of Iran. The proportions of households with medium/high consumption of main foods were calculated for each year. Joint point software was used for assessing trends. Annual percent changes (APCs) and 95%CIs were calculated. Results: In total, 12,060 households were recruited. The APCs (95%CI) of the proportion of households medium/high consumption of cereals, vegetables, legumes, fish, dairy products and meats were -3.1 (-4.1 to -2.2), -2.9 (-3.8 to -2.1), -2.3 (-3.2 to -1.4), -2.8 (-3.3 to -2.4), -1.9 (-3.0 to -0.9) and 2.7 (1.2 to 4.3), respectively. Conclusions: We found significant increase in meat consumption among our population between 2001 and 2010. Our results also suggested significant decreasing trend in consumption of so-called healthy foods including, plant foods, fish, and dairy products. Regarding its correlation with health conditions including cancers, nutrition transition should be considered as a priority in health policy making in our region as well as other high-risk populations. It is recommended to conduct community level interventions to increase consumption of plant foods, fish, and dairy products.
