Frequency, Clinical Characteristics and Correlates of Premenstrual Syndrome in High School Students (여고생에서 월경전 증후군의 빈도와 임상적 특징 및 관련요인)

  • Objectives : To investigate the frequency and clinical characteristics of premenstrual syndrome(PMS)/premenstrual dysphoric disorder(PMDD) in high school students, and determine the correlates of PMS/PMDD in association with comorbid depression and anxiety. Methods : A total of 1688 students were recruited from 5 high schools in Seoul, Korea. Subjects completed the questionnaire composed of scales to measure premenstrual symptoms, depression, and anxiety, as well as sociodemographic and reproductive variables. Subjects were categorized into 3 groups by using the Premenstrual Symptom Screening Tool(PSST) to determine the frequency and clinical characteristics of PMS/PMDD. Multivariate logistic regression was used to identify the correlates of PMS/PMDD. Results : The frequency of moderate to severe PMS and PMDD was 20.1% and 6.4%, respectively. Irritability(78.8%), fatigue(76.4%), and emotional sensitivity(69.8%) were common premenstrual symptoms, and functional impairment in academic performance(67.1%) was dominant. Dysmenorrhea[odd ratio(OR)=3.68, 95% confidence interval(CI) 2.45-5.55], family history of PMS(OR=1.91, 95% CI 1.35-2.71), and use of oral contraceptive (OR=1.85, 95% CI 1.16-2.94) were associated with the increased risk of PMS/PMDD after adjustment for depression and anxiety. Negative attitude to menses(OR=15.60, 95% CI 3.61-67.42) was associated with the increased risk of PMS/PMDD, particularly in subjects without depression and anxiety. Conclusions : PMS was common, as the frequency of PMS more than moderate severity including PMDD exceeded 25%, and disrupted daily functioning in adolescents. PMS is associated with various sociodemographic and menstrual characteristics, and these associations are affected by comorbid depression and anxiety.

Study on the Association of DSOM Pathogenic Factor in the Insomnia Patients (불면환자의 수면의 질 척도와 한방진단시스템 병기(病機)와의 연관성 연구)

  • Objectives: This study was to investigate the relation of Pittsburgh Sleep Quality Index(PSQI) and Diagnos system of oriental medicine(DSOM) in the Insomnia Patients. Methods: For this study, we carried out PSQI and DSOM of 92 patients with insomnia who have come to Kijang public health center and Donguei oriental hospital of Donguei university from November 2008 to October 2009. We verified the association of PSQI and DSOM using crosstabulation analysis, independent sample T-test, one-way ANOVA and correlation analysis. Results: 1. Blood-deficiency(血虛) is the most frequent pathogenic factor and frequency of nightmare has positive correlation with that factor including heart(心) in total insomnia patients. 2. Female patients have a higher percentage of chronic insomnia than male patients. 3. As patients aged, maintaining sleep gets harder, sleep time is lessening, taking hypnotics is increasing and quality of sleep gets worse. 4. There is high frequency of chronic insomnia in 40~50 years and over 60 years old. Also, the people who are over 60 years of age tend to sleepless and urinated frequently during the middle of the night. The people with the age of 20~30 years tend to take less sleeping pills and frequency of sleeping during daytime is higher than the other age groups but the quality of sleep is quite good. 5. The patient with over 6 months of chronic insomnia tend to be older in age than the patient with less than 6 months. And the patient with chronic sleeping disorder has frequent pain and the quality of sleep is very poor. 6. The patient who took hypnotics tend to be older than hypnotics non-user and frequency of urination was relatively high. 7. The patient who visited oriental medical hospital were tend to be older and get difficult in maintaining sleep. And frequency of taking hypnotics and urinating during night were tend to be higher and the quality of sleep was poor compared with the patient who visited public health center. Conclusions: This study provides insights into the complicated relations of the associated symptom of insomnia with Diagnos system of oriental medicine. And especially this study showed apparent correlation between nightmare in insomnia patients and pathogenic factor of blood-deficiency and heart. Specific correlations and characteristics in this study could be connect develop of new questionaire of insomnia.

Evaluation of Clinical Efficacy of Korea Red Ginseng for Erectile Dysfunction by International Index of Erectile Function(IIEF) (국제 발기능측정설문지(IIEF)를 이용한 발기부전 환자에서의 홍삼 효능 평가)

  • International index of erectile function (IIEF) has a high sensitivity and specificity for the evaluating treatment of erectile dysfunction. IIEF was used to evaluate on an international level the effectiveness of red ginseng on patients with erectile dysfunction and determine its possible development as a natural drug. 50 patients with erectile failure who were 20 years or older and without organic cause were randomly divided into two groups. Group 1(25 patients) received red ginseng 600mg three times a day and group 2 (25 patients) received placebo in the same fashion for 8 weeks. Each group was evaluated with IIEF at the start and end of therapy. Of the 50 patients 47 (group 1 24, group 2 23) who completed the 8 week therapy were evaluated. The mean age and symptom duration of the patients was 45.7$\pm$8.7 (27-68) and 6.2$\pm$5.6(1-29) years, respectively. Patient age, symptom duration. marriage status, marriage duration, partner age were not statistic significance between the two groups (p>0.05). Blood chemistry, urinalysis, and hormonal assay did not reveal difference between the two groups. primary efficacy (erectile rigidity and its maintenance) and all domains in secondary efficacy were significantly better after therapy in group 1 compared to those of group 2 (p<0.05) except for frequency of sexual desire, degree of sexual desire, and frequency of intercourse trial (p>0.05). Group 1 patients were significantly more satisfied after treatment compared to group 2 patients with 58.3% (14/24)and 26.1%(6/23) satisfaction rates, respectively. There were no adverse reactions or complications with ginseng adiminstration. Further study into the effects of saponin and ginsenoside that are the main substances in ginseng for erectile dysfunction is needed. In addition, its possible additive effect with ginseng and sildenafil should be elucidated in the future.

Patients' Experiences of Sensations After Breast Cancer Surgery in Korean Women (유방암 수술 환자의 감각 변화에 대한 연구)

  • Purpose: The purpose of this study was to obtain information about the experiences of sensations after breast cancer surgery characterized by prevalence, frequency, and severity, distresses, and disturbances in ADL using the BSAS (Breast Sensation Assessment Scale) and to develop the standardized Korean BSAS. Methods: Thirty two women from 3 to 100 days after breast cancer surgery (BCS) completed Roberta's BSAS. The 18 sensations of BSAS were translated using the references of Lee's Korean Pain Rating Scale and English-English, English-Korean dictionaries and the consultation from two native Americans, one bilingual permanent resident of the States, and one nursing professor. The Korean-translated BSAS has high reliability in test-retest. Likert type 4-point scale and 100 mm VAS were used for assessment. Results: Certain sensations remained prevalent (tender, pull, pain), frequent (numb, nag, throb), severe (throb, shoot, numb), causing distress (throb, penetrate, shoot), and influencing on ADL (throb, penetrate, nag). The most frequently experienced sensation other than BSAS was itching. There was little difference in the prevalence of symptom experiences between sentinel lymph node biopsy and axillary lymph node dissection. Conclusion: The women after BCS are suffering from neuropathic sensations. The Korean- translated BSAS could be used in effectively assessing breast sensations after BCS in Korean women.

Effect of physical health conditions on suicidal ideation in Korean adolescents (우리나라 청소년의 신체적 건강상태가 자살생각에 미치는 영향)

  • Objectives: This study investigated the relationship between adolescents' suicidal ideation and their physical health conditions to contribute to lower suicide rates among adolescents. Methods: Secondary analysis of 2108 high school students of $10^{th}$ grade was conducted using the Korean Child and Youth Panel survey in 2013. The association between suicidal ideation and socio-demographic and health characteristics were analyzed using Chi-square tests and regression models. Results: Adolescents' physical health conditions were significantly related to suicidal ideation. In particular, rhinitis and various symptoms such as feeling feverish, low appetite, feeling of nausea, stomachache while studying, and sleeping problem showed statistically significant relationships with suicidal ideation (P < .05). Furthermore, the higher frequency of diseases and body symptoms increases the rate of suicidal ideation. Considering socio-economic characteristics, family structure was also important. Conclusions: Physical health condition is a major contributor to adolescent's suicidal ideation. results suggest that improving social support system for adolescents' physical health conditions across family and school may be important in suicide prevention efforts.

Analysis of Awareness and Requirement of Home Care Nursing in Cancer Patients by Experienced Symptoms and Nursing Items (암 환자의 증상경험과 가정간호항목별 가정간호 인식도 및 가정간호 요구도)

  • Purpose: This study sought to provide basic information for the establishment of home care nursing system in cancer patients. Methods: Data were collected by the descriptive questionnaires consisting of 42 articles from five fields of nursing services. Patient's symptoms were investigated by the Korean version of EORTC QLQ-C30. Data were analysed using SPSS-PC 12.0. Results: Among the cancer patients (n=182), 40.1% had serious limitations on their usual lives. 74.7% had serious economic burdens. 79.7% agreed strongly with the necessity of home care nursing system, 74.2% were willing to use home care nursing, and 91.2% felt that home care nursing should be mandatory in cancer centers. There was no correlation between the frequency of symptoms or nursing items and the degree of home care nursing requirements. Digestive symptoms, symptoms requiring procedures, and symptoms to meet educational help displayed a high degree of requirement. Conclusion: Home care nursing should be activated for cancer patients as a bridge between hospital-based acute care and community-based chronic care which could increase the quality of care and reduce insurance related payments.

A Comparative Study on the Effects on Urinary Incontinence between Pelvic Floor Muscle Exercise and Magnetic Stimulation Therapy (지역사회 요실금 여성의 골반저근운동과 자기장자극 효과비교)

  • Purpose: To Perform a randomized comparative study investigating the effects of Pelvic Floor Muscle Exercise(PME) and Magnetic Stimulation Therapy(MST) and to identified the problems in each of PME & MST Method: Forty-nine patients with mild stress incontinence were randomly assigned to either of two treatment groups (24 patients in the PME group and 25 in the MST group). The PME group had a video exercising program for 40 times every day during 6 weeks. The MST group was treated with BioCon-2000TM, 2times/ week for six weeks. Pre-test and post-test were performed by Prineometer, 1-hour pad test. and Jackson's BFLUTS questionnaire of Jackson. Collected data were analysed using SAS 9.1 by frequency, Kolmogorov-Smirnov Z. Chi Square-test, t-test, Fisher Exact probability test, Paired t-test, and Wilcoxon's rank sum test. Results: In comparison between before and after PME and MST, statistically significant difference was observed in maximal vaginal pressure, duration of vaginal contraction, amount of urine, and symptom of urination. In the comparison of the effectiveness between PME & MST, only the maximal of vaginal pressure (Z=2.58, p= .010) was significantly different. Conclusion: The factor more effective in the MST group than in the PME group was high compliance.

The Relationship between Psychosocial Factors and Symptoms of Patient with Rhematoid Arthritis (류마티스 관절염 환자의 심리사회적 요인과 증상과의 관계)

  • This study was done to Identify the relations between psychosocial factors and symptoms of patient with rheumatoid arthritis. Its subjects were 150 outpatients selected at H. Rheumatism clinic from July to August in 1998. Data were collected by means of questionnaires using interview. Those data were analyzed by Frequency, t-test, ANOVA, pearson's correlation using SPSS window 8.0. The results were as follows : 1) The level of family support was relatively high and the level of self-efficacy and performance of daily living activities were average. Among their symptoms, fatigue and pain were frequent problems. 2) Family support was different by educational level, self-efficacy and physical, psychological symptoms were different by performance of regular exercise. 3) The family support was inversely related to physical symptoms especially ADL, while it was not related to psychological symptoms except a reverse correlation between depression and anxiety, and family support. Self-efficacy held a reverse correlation with both physical and psychological symptoms. And there was a close relationship between symptoms. In conclusion it was found that the promotion of family support, self-efficacy and exercise-performance holds a key post in reducing symptoms of rheumatoid arthritis.

Distribution of Sasang Constitution and Prescriptions of Patients with Coldness in Hands and Feet (일개병원에서의 수족냉증환자에 대한 사상체질분포 및 처방)

  • Objectives This study was implemented to investigate the distribution of Sasang constitution and prescription of the patients with coldness in hands and feet.Methods Sixteen patients with coldness were analyzed in terms of age, Sasang constitution, season and prescriptions.Results The distribution of Sasang constitution of the patients with coldness in hands and feet was like Taeeumin 50%, Soeumin 43.8% and Soyangin 6.2%. And the date of consultation of the patients is different seasonally, 50% of the patients visited in winter, 18.8% in spring and fall respectively and 6.3% in summer. The frequency by age group is different was like 31.3% of the patients visited in 20s, 18.8% in 10s, 30s and 50s, 12.5% in 40s. In terms of Exterior and Interior diseases, Soyanging and Soeumin patients were categorized in Interior diseases while Taeeumin patients were categorized in Exterior disease.Conclusions Patients with coldness in hands and feet were mostly classified into Taeeumin and Soeumin and the prevalence of its symptom was high in their 20s. Patients were willing to visit clinics in winter. The Exterior/Interior diseases and mild/severe diseases were different according to Sasang constitution.

The Influence of Predisposing Factors and Sign of Hypoadrenia on Autonomic Nervous System Activity and Salivary Cortisol (부신기능저하증의 예후인자와 증후가 자율신경 활성도와 타액 코티졸에 미치는 영향)

  • Objectives : This study was performed to identify the influence of salivary cortisol and HRV(Heart rate variability) on hypoadrenia predisposing factors and sign. Methods : 30 patients with chronic fatigue symptom belonging to Shen xu(腎虛) were recruited for investigation from March to April 2007. The participants were divided into HS(High score) group(n=15) and LS(Low score) group(n=15) in predisposing factors of adrenal questionnaire. At the same time equal patients separated into OH(+)(Orthostatic hypotension positive) group(n=12) and OH(-)(Orthostatic hypotension negative) group(n=18). Salivary cortisol and HRV made use of evaluating hormonal imbalance and autonomic nervous system of hypoadrenia. Results : Salivary cortisol at P.M.4 in HS group was significantly(p=0.011) lower than LS group. And LF(Low frequency) of OH(+) group was considerably(p=0.014) lower than OH(-) group. Conclusions : Shen xu bian zheng(腎虛辨證), measure of orthostatic hypotension, predisposing factors of adrenal questionnaire, salivary cortisol and HRV deserve clinical application for management of subclinical hypoadrenia.
