  • 1. Objectives This case is aimed to verify the relation of Soeurnin Mang-yang Syndrome and Cold Syndrome with pseudo-heat syndrome. 2. Methods The patient has high fever, general body sweating, thirst and constipitation is diagnosed as Soeumin Mang-yang Syndrome. Mang-yang Syndrome one of the symptoms in the Exterior Febrile Disease induced from the Kidney affected by Heat in Soeumin(少陰人) marked by spontaneous sweating and fever with chills. This syndrome is similar to Cold syndrome with pseudo-heat symptoms in general symptoms and pathologic process. Therefore, We medicate Doksampalmul-tang to this patient who diagnosed as Soeumin Mang-yang Syndrome. 3. Results and Conclusions The symptoms that the patient has fever, sweating, thirst and constipitation are solved after the medication. This means recovery of Yang-Energy in Kidney and the spleen.

Changes of Pituitary Hormones after Injection of Naloxone in the Hypotensive Phase of Korean Hemorrhagic Fever (한국형 출혈열 저혈압기에서 Naloxone 투여후 뇌하수체 홀몬의 변동)

  • The opiate antagonist, naloxone, was injected for the reversal of hypotension due to Korean hemorrhagic fever, and the authors observed changes in pituitary hormones. In the hypotensive phase of the Korean hemorrhagic fever, the f-endorphin was high, and normalized granually in the diuretic and convalescent period. The naloxone raised the pulse rate and the blood pressure within 30 minutes without change in the central venous pressure. Around 30 minuted after the injection of the naloxone, the $\beta-endorphin$, ACTH and cortisol rose. The prolactin fell down 60 minutes after the naloxone injection.

  • Malaria caused by Plasmodium species is characterized by paroxysms of fever, chills, fatigue, anemia, and splenomegaly. Vivax malaria has lately re-emerged as an infectious disease and has exhibited high transmission rate in northern Gyeonggi-do province. We encountered a case of malaria in a child presenting with fever and thrombocytopenia who had recently made a school excursion to Pocheon-gun, Gyeonggi-do. The child was diagnosed with Plasmodium vivax malaria and treated with hydroxychloroquine and primaquine. Here, we present this case with a brief review of the literature.

  • A 51-year-old male was admitted to the hospital with complaints of fever and hemoptysis. After evaluation of the fever focus, he was diagnosed with pulmonary valve infective endocarditis. Thus pulmonary valve replacement and antibiotics therapy were performed and discharged. He was brought to the emergency unit presenting with a high fever (> $39^{\circ}C$) and general weakness 6 months after the initial operation. The echocardiography revealed prosthetic pulmonary valve endocarditis. Therefore, redo-pulmonary valve replacement using valved conduit was performed in the Rastelli fashion because of the risk of pulmonary arterial wall injury and recurrent endocarditis from the remnant inflammatory tissue. We report here on the successful surgical treatment of prosthetic pulmonary valve endocarditis with an alternative surgical method.

A Study on the Diversity of Shanghan(傷寒) Concept in Gangpyeong-Sanghanlun(康平傷寒論) (『강평상한론(康平傷寒論)』 내 '상한(傷寒)' 개념의 다양성에 대한 고찰)

  • Objectives : Usually medical terminology of oriental mecidine has a multiple meaning. But concept of Shanghan(傷寒) should be simple, because Shanghanlun(傷寒論) is a clinical guideline book. So I researched to suggest many concept of Shanghan, which are suitable for each chapter of Shanghanlun. Methods : I enumerated provisions including Shnaghan from the original texts of Gangpyeong-Shanghanlun(康平傷寒論). And I translated and reviewed them. Results : 1. Shanghan of Preface(序文) means a disease of high fatality. 2. Shanghan of Shanghanrye(傷寒例) means diseases due to physical damage of cold weather. 3. Shanghan of Diagnosis of Daeyang Disease(辨大陽病) - Neck stiffness(痙), Dampness(濕), Sun stroke(暍) means certain disease names accompanying fever, chill. 4. Shanghan used in Diagnosis of Diseases is a premise of many provisions of Shanghanlun. And Shanghan is made up of finished fever, expected fever, chill, body pain, loss of appetite, image of tension. Conclusions : We can use a appropriate translation on Shanghan of each chapter of Gangpyeong-Sanghanlun. Especially Shanghan used in "Diagnosis of Diseases" should have more accurate meaning.

Seroprevalence of Coxiella burnetii in bulk-tank milk and cattle in Daegu area, Korea (대구지역 집합유와 소에서 큐열 항체 보유율 조사)

  • Seroprevalence of Coxiella burnetii (Q fever) in bulk-tank milk and cattle in Daegu area was analyzed from 2017 to 2018 by ELISA. The prevalence of antibodies in collected bulk-tank milk from 12 dairy cattle farms was 41.7% (10/24) and the seroprevalence of 249 cows reared in the area of Daegu was 3.2% (8/249) By age, the seroprevalence was 1.9% (2/105) in less than 2 years of age, 4.2% (5/119) between 3 and 5 years of age and 4.0% (1/25) in more than 6 years of age. By breed, the seroprevalence of the Hanwoo cattle was 1.2% (2/162) and the seroprevalence of the dairy cattle was 6.9% (6/87). The result suggested that seroprevalence of C. burnetii was relatively high in both bulk-tank milk samples and dairy cattle than the Hanwoo cattle. Based on these data, it is necessary to keep monitoring the prevalence of Q fever in Daegu area.

Hemophagocytic lymphohistiocytosis with recurrent Kikuchi-Fujimoto disease

  • Kikuchi-Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a self-limiting lymphadenitis. It is a benign disease mainly characterized by high fever, lymph node swelling, and leukopenia. Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening disease with clinical symptoms similar to those of KFD, but it requires a significantly more aggressive treatment. A 19-year-old Korean male patient was hospitalized for fever and cervical lymphadenopathy. Variable-sized lymph node enlargements with slightly necrotic lesions were detected on computed tomography. Biopsy specimen from a cervical lymph node showed necrotizing lymphadenitis with HLH. Bone marrow aspiration showed hemophagocytic histiocytosis. The clinical symptoms and the results of the laboratory test and bone marrow aspiration met the diagnostic criteria for HLH. The patient was diagnosed with macrophage activation syndrome-HLH, a secondary HLH associated with KFD. He was treated with dexamethasone (10 mg/m2/day) without immunosuppressive therapy or etoposide-based chemotherapy. The fever disappeared within a day, and other symptoms such as lymphadenopathy, ascites, and pleural effusion improved. Dexamethasone was reduced from day 2 of hospitalization and was tapered over 8 weeks. The patient was discharged on day 6 with continuation of dexamethasone. The patient had no recurrence at the 18-month follow-up.

  • Lithium is mainly prescribed for manic and depressive disorder, also frequently prescribed for the other diseases such as migraine, cluster headache, alcoholism, and obsessive-compulsive disorder. An acute lithium intoxication occurs in cases of patients ingesting large amount lithium at a time, a chronic lithium intoxication occurs in patients on chronic lithium therapy. Acute or chronic lithium poisoning occurs frequently in case of patients on chronic lithium therapy ingesting larger dose than prescribed. Manifestations of lithium poisoning are various. It is possible nervous, cardiovascular, renal, gastrointestinal and endocrine systems to be involved. Due to intracellular high concentration, mortality rate is high in acute lithium intoxication patients on chronic lithium therapy. We report a case of acutely intoxicated 40-year-old male on chronic lithium therapy. His chief complaints were deterioration and high fever. On his arrival to an emergency department, he was in cardiac arrest. He restored return of spontaneous circultion (ROSC) 5 minutes later after cardiopulmonary cerebral resuscitation (CPCR) and referred to department of internal medicine for hemodialysis. Vigorous treatment was given to the patient, but he was expired at 4th hospital day.

Detection of Antibodies to Classical Swine Fever Virus gp55 in Muscle Fluid (Classical Swine Fever Virus gp55 항원에 대한 Muscle Fluid 항체 측정)

  • The objective of the present study was to investigate the use of fluid released from muscle samples as an alternative to serum for ELISA to detect classical swine fever(CSF) virus antibodies in slaughter pigs. The optimal correspondence between serum 1:20 OD values and muscle fluid OD values was achieved at a muscle fluid dilution of 1:2. Significant correlation was found between serum and neck muscle ELISA ($r_s=0.880$, p<0.0001, ${\kappa}=0.82$; specificity of 97.0% and sensitivity 90.6%). The semimembranous muscle showed similar correlation in CSF ELISA($r_s=0.877$, p<0.0001, ${\kappa}=0.75$; specificity of 94.1% and sensitivity 89.1%). High correlation was obtained between serum and mesenteric lymph node in the CSF ELISA ($r_s=0.937$, p<0.0001, ${\kappa}=0.87$; specificity of 97.1% and sensitivity 93.0%). Measmement agreement between serum ELISA and muscle fluid ELISA was calculated and expressed as limits of agreement. The correspondence of ELISA of serum and muscle fluid indicated limits of agreement. Above 95% of all muscle fluid values were distributed within this limits of agreement. Among the samples used for ELISA for detecting CSFV antibodies, mesenteric lymph node had the most correlation and agreement with serum ELISA. F-test for comparison of variances showed no significant difference between the serum and muscle fluid. In conclusion, muscle fluid is a useful postmortem alternative to serum to detect CSFV antibodies.

Endoscopic Findings of Children with Familial Mediterranean Fever

  • Purpose: Familial Mediterranean fever (FMF) is an auto inflammatory disease characterized by periodic fever, synovitis and serositis. Patients may be admitted to gastroenterology units due to gastrointestinal symptoms. In this study; we aimed to analyze endoscopic findings and diagnostic utility of endoscopic procedure in children with FMF. Methods: Patient with FMF that was performed endoscopy for the gastrointestinal symptoms were included to the study (39 of 164 patients, 53 procedure). A control group was randomly designed as age and gender matched four endoscopic procedures per one endoscopic procedure of patients with FMF (n=212). Results: No different was found between the patients and control group in esophagogastroscopy findings. However, the diagnosis of gastrointestinal pathology was made by esophagogastroscopy in 46.2% patients. Colonoscopic examination revealed that the frequency of inflammatory bowel disease (IBD) was higher in undiagnosed patients compared to both the control group (50.0% vs. 6.9%, p<0.05, odds ratio [OR]:13.4 and 95% confidence inteval [95% CI]: 2.1-84.3) and the patients under colchicine treatment (50.0% vs. 8.3%, p<0.05, OR: 11 and 95% CI: 0.8-147.8). Colonoscopic procedure that was made after the diagnosis was found to provide contribution by 16.7% in determining the etiology of the additional symptoms. Conclusion: Patients with FMF may be admitted to pediatric gastroenterology outpatient clinic prior to diagnosis or during the follow-up period. The frequency of IBD is high in undiagnosed patients with FMF. Endoscopic procedures may be helpful in these patients for the diagnosis accompanying mucosal lesions.
