신경원성 종양으로 오인된 이소성 골 형성: 증례 보고 (Heterotopic Ossification Mimics Neurogenic Tumor: A Case Report)

  • 이소성 골 형성은 수술 후 또는 특정 선행요인 없이 연부조직에 생기는 골 형성을 말한다. 고관절 및 슬관절 등 큰 관절에서 호발하며, 위험인자로 수술 및 수술 후 빠른 관절운동 등이 있다. 저자들은 슬관절 근위 경골 절골술 후 발생한 신경원성 종양으로 오인된 이소성 골 형성 1예를 경험하였기에 보고하는 바이다.

  • Heterotopic ossification (HO) is the formation of mature bone in soft tissue where bone normally does not exist. Although HO is among the most common complications after orthopedic surgery, it is not familiar to oral and maxillofacial surgeons. Here we report rare cases of HO. When a patient presents atypical osseous lesions, HO as well as similar lesions such as osteoma, osteochondroma should be considered in the provisional diagnosis. Three-dimensional reconstruction of preoperative computerized tomography imaging improves surgical success.

  • Heterotopic ossification is formation of bone in atypical extra-skeletal tissues and usually occurs spontaneously or following neurologic injury with unknown cause. We report a 46-year-old female with right shoulder pain and restricted range of motion (ROM) for 3 months without history of trauma. Magnetic resonance imaging (MRI) showed a lesion within the rotator cuff supraglenoid. Excisional biopsy from a previous institution revealed a heterotopic ossificans (HO). Following repeat MRI and bone scan, histopathology from arthroscopic resection confirmed an HO. The patient demonstrated improved pain and ROM at follow-up. Idiopathic HO rarely occurs in the shoulder joint, and resection of HO should be delayed until maturation of the lesion to avoid recurrence. The current case showed that arthroscopic HO resection provides an excellent surgical view to ensure complete lesion removal and minimize soft tissue damage at the supraglenoid area. Furthermore, the minimally invasive procedure of arthroscopy may reduce rehabilitation time and facilitate early return to work.

Surgical Resection of Neurogenic Heterotopic Ossification around Hip Joint in Stroke Patients: A Safety and Outcome Report

  • Purpose: Resection remains the most reliable treatment for established heterotopic ossification, despite questions regarding its effectiveness due to the potential for complications. This study evaluated the clinical outcomes and complications of neurogenic heterotopic ossification (NHO) resection in stroke patients' ankylosed hips. Materials and Methods: We retrospectively analyzed nine hip NHO resections performed on seven patients from 2010 to 2018. The pre- and postoperative range of motion of the operated hip were compared. Analysis of postoperative complications, including infection, recurrence, iatrogenic fracture, and neurovascular injury was performed. Results: The mean operative time was 132.78±21.08 minutes, with a mean hemoglobin drop of 3.06±0.82 g/dL within the first postoperative week. The mean duration of postoperative follow-up was 52.08±28.72 months for all patients. Postoperative range of motion showed improvement from preoperative. Flexion and external rotation (mean, 58.89±30.60° and 16.67±18.03°, respectively) showed the greatest gain of motion of the operated hip joint. Postoperative infections resolved in two cases through surgical debridement, and one case required conversion to total hip arthroplasty due to instability. There were no recurrences, iatrogenic fractures, or neurovascular injuries. Conclusion: Resection is a beneficial intervention for restoring the functional range of motion of the hip in order to improve the quality of life for patients with NHO and neurological disorders. We recommend performance of a minimal resection to achieve a targeted functional arc of motion in order to minimize the risk of postoperative complications.

외상성 하퇴 절단지에 발생한 이소성 골화증에 대한 체외충격파 치료 (Extracorporeal Shock Wave Therapy for Painful Heterotopic Ossification after Traumatic Transtibial Amputation)

  • The incidence of heterotopic ossification (HO) was reported to be higher in combat-injured patients than in civilian trauma patients. HO is often considered a possible cause of residual limbs pain in amputee. Here, we report the case of a 21-year-old male, who underwent a traumatic right transfemoral and left transtibial amputation with two segments of painful HO around his left amputation site. We report the effect of extracorporeal shock wave therapy (ESWT) on size and pain associated with HO. After ESWT, the visual analog scale score decreased from 5~6 to 0~1 and the size of two masses decreased from 13.1 × 6.7 mm and 12.5 mm to 11.9 × 4.7 mm and 12.2 mm, respectively. To the best of our knowledge, this is the first case that has reported on the treatment of HO using ESWT for a traumatic transtibial amputation patient. The case suggests that ESWT could serve as a complementary treatment for HO in traumatic amputation patient.

이소골화로 오진된 복막의 일차성 모래종암종: 증례 보고 (Primary Peritoneal Psammocarcinoma Misdiagnosed as an Heterotopic Ossification: A Case Report)

  • 복막의 모래종암종은 장액성 암종의 드문 한 형태로서 많은 양의 모래종 형성과 인접한 주변 장기로의 침범을 특징으로 하고 있다. 이전 자궁절제술의 과거력이 있는 55세 여자 환자가 심한 복부 통증을 주소로 응급실로 내원하였고 시행한 조영증강 CT에서 복강내 석회화 종괴를 발견하였다. 양쪽 난소는 정상적으로 관찰되었으며 환자의 이전 수술력으로 인하여 이소성 골화로 생각되었다. 진단을 위해 시행한 내시경적 그물막절제를 통하여 최종적으로 일차성 복막 모래종암종으로 진단되었고 부피감량수술을 연속적으로 시행하였다. 복막의 모래종암종은 비록 드물지만 복강내 석회성 종괴 혹은 석회성 결절을 동반한 환자에게서 감별 진단에 포함되어야 할 것이다.

  • Heterotopic ossification (HO) around the hip joint is not uncommon following neurological injury. Often, surgical treatment is performed in patients with restricted motion and/or refractory pain due to grade III or IV HO according to Brooker classification. The major complication that occurs as a result of surgical HO removal is perioperative bleeding due to hyper-vascularization of the lesion. Here, we report a case of preoperative embolization in a 51-year-old male patient presenting with restricted bilateral hip range of motion (ROM) due to HO following a spinal cord injury. In the right hip without preoperative arterial embolization, massive bleeding occurred during surgical removal of HO. Thus, the patient received a transfusion postoperatively due to decreased serum hemoglobin levels. For surgery of the left hip, preoperative embolization of the arteries supplying HO was performed. Surgical treatment was completed without bleeding complications, and the patient recovered without a postoperative transfusion. This case highlights that, while completing surgical removal for ROM improvements, orthopedic surgeons should consider preoperative arterial embolization in patients with hip HO.

방사선 영상장치 모니터링하 이소성 석회화 절제술 (C-arm Guided Surgical Excision of Heterotopic Calcification)

  • Purpose: Heterotopic calcification is the abnormal deposition of calcium salts in tissues other than bone and enamel, and it occurs in the form of dystrophic calcification or metastatic calcification. This deposition can occur under many conditions, but in some rare cases, it may develop in burns and nonhealed scars. It is difficult to treat the combination of heterotopic calcification and ulceration in scar tissues by using conservative therapy and to determine the margin of excision in such cases. Our study proposes the use of intraoperative C-arm-guided mapping of lesions with heterotopic calcification, and adequate excision of ulcers in chronic scars where heterotopic calcification is also observed. Methods: This study included 2 patients and was conducted from January 2010 to July 2010. The first patient was a 63-year-old woman who presented with atypical calcium deposits and chronic ulceration in the lower one-third region of the right leg. The second patient was a 38-year-old man who presented with a nonhealing ulcer that had developed on the right leg 3 months earlier he had a history of 40% scalding burns on the entire body. Surgery is the most reliable method for treating heterotopic calcification therefore, both patients were treated using intraoperative C-arm-guided marginal mapping of heterotopic calcification, followed by release of contracture, and eventually split-thickness skin grafting. Results: Plain radiographs of the leg showed spotty radiopaque areas in the hard part of the scar well superficial to the underlying bones. Histopathological analysis revealed multiple foci of calcified deposits, increased fibrosis, and inflammation in the scar tissue. Surgery-related complications were not observed. Conclusion: C-arm guided excision of calcified scars and the release of contracture can cure nonhealing ulcers and may therefore prevent recalcification.
