Structural and Mechanistic Insights into the Tropism of Epstein-Barr Virus

  • Epstein-Barr virus (EBV) is the prototypical ${\gamma}$-herpesvirus and an obligate human pathogen that infects mainly epithelial cells and B cells, which can result in malignancies. EBV infects these target cells by fusing with the viral and cellular lipid bilayer membranes using multiple viral factors and host receptor(s) thus exhibiting a unique complexity in its entry machinery. To enter epithelial cells, EBV requires minimally the conserved core fusion machinery comprised of the glycoproteins gH/gL acting as the receptor-binding complex and gB as the fusogen. EBV can enter B cells using gp42, which binds tightly to gH/gL and interacts with host HLA class II, activating fusion. Previously, we published the individual crystal structures of EBV entry factors, such as gH/gL and gp42, the EBV/host receptor complex, gp42/HLA-DR1, and the fusion protein EBV gB in a postfusion conformation, which allowed us to identify structural determinants and regions critical for receptor-binding and membrane fusion. Recently, we reported different low resolution models of the EBV B cell entry triggering complex (gHgL/gp42/HLA class II) in "open" and "closed" states based on negative-stain single particle electron microscopy, which provide further mechanistic insights. This review summarizes the current knowledge of these key players in EBV entry and how their structures impact receptor-binding and the triggering of gB-mediated fusion.

핵산 유도체들에 대한 in vitro 항바이러스 약효검색

  • 바이러스 치료제 개발을 위하여 합성된 핵산유도체 11개에 대한 in vitro 항바이러스 약효검색을 수행하였다. 검색대상 바이러스로서 외피보유 DNA 바이러스인 human herpesvirus에 속하는 herpes simplex virus type 1과 type 2에 대해서는 Vero 세포체계에서 3일 후 CPE 저해정도를 MTT 검색법으로 cytomegalovirus에 대해서는 HEL 세포체계에서 7일 후 Giemsa 염색법으로 약효를 측정하였다. 외피비보유 RNA 바이러스인 picornavirus에 속하는 poliovirus type 1과 type 3과 coxsackie B virus type 3에 대한 약효를 HeLa 세포체계에서 2일 후 CPE 저해정도를 MTT 검색법으로 측정하였다. 아울러 selectivity index를 구하기 위하여 Vero와 HeLa 세포에 대한 약물자체의 독성인 cytocidal effect를 MIT 검색법으로 측정하였다. 그 결과 항 herpesvirus 약효는 어떤 물질에서도 발견되지 않았으나 한 물질이 poliovirus type 1과 3에 대하여 selectivity index 10정도 (CC$_{60}$ 38 ug/ml, EC$_{50}$ 1-4 ug/ml)를 나타내었고 자세한 기작은 좀 더 조사할 필요가 있다.

Current scenario and future applicability of antivirals against herpes zoster

  • Herpes zoster (HZ) is a common disease in the aging population and immunocompromised individuals, with a lifetime risk of 20%-30% that increases with age. HZ is caused by reactivation of the varicella-zoster virus (VZV), which remains latent in the spinal dorsal root ganglia and cranial sensory ganglia after resolution of the primary VZV infection. The main focus of HZ management is rapid recovery from VZV infection as well as the reduction and prevention of zoster-associated pain (ZAP) and postherpetic neuralgia (PHN). The use of antivirals against VZV is essential in the treatment of HZ. However, limited antivirals are only licensed clinically for the treatment of HZ, including acyclovir, valacyclovir, famciclovir, brivudine, and amenamevir. Fortunately, some new antivirals against different types of Herpesviridae have been investigated and suggested as novel drugs against VZV. Therefore, this review focuses on discussing the difference in efficacy and safety in the currently licensed antivirals for the treatment of HZ, the applicability of future novel antivirals against VZV, and the preventive or therapeutic effects of these antivirals on ZAP or PHN.

인간 내인성 레트로 바이러스와 신경학적 장애 (Human endogenous retroviruses and neurologic disorders)

  • 인간 내인성 레트로 바이러스는 약 3~4 천만 년 전 인간게놈에 통합되기 시작했던 화석바이러스로서 인간게놈의 약 8%를 구성하고 있다. 이러한 고대의 레트로 바이러스는 연속된 유전자변이로 인해 본래의 기능을 잃게 됨으로써 "쓸모없는 DNA (junk DNA)"로 여겨져 왔다. 그러나 최근 연구는 모든 인간 내인성 레트로 바이러스가 단지 침묵을 지키는 승객만이 아님을 보여준다. 현재까지는 이들 바이러스가 인간 질병의 직접 원인이 된다는 것은 밝혀지지 않았으나, 다발성 경화증, 정신 분열증 및 근 위축성 측삭경화증을 포함한 신경학적 장애와 인간면 역결핍바이러스 (HIV)나 헤르페스 등의 바이러스 감염, 여러 유형의 암과 같은 질환을 가진 환자에서 인간 내인성 레트로 바이러스의 DNA가 과도발현 됨을 보여주는 연구들이 다양하게 이루어지고 있다. 이 리뷰논문은 신경학적 질환에 대한 인간 내인성 레트로 바이러스의 가능한 관련여부를 다루고 있다.

소아에서의 조직구성 괴사성 림프절염 : human herpesvirus 8과 Epstein Barr virus와의 연관성 (Histiocytic necrotizing lymphadenitis in children : association of human herpes virus 8 and Epstein Barr virus)

  • 목 적 : KD는 주로 동양의 젊은 성인 여성에게서 경부 림프절 종대로 발현하는 것이 특징이며 수개월 내에 자연 치유되는 양호한 경과를 가지는 질환이다, 저자들은 KD로 진단된 소아환아의 임상적 특징을 알아보고, 환아들의 림프절 조직에서 바이러스 검출을 시도하여 KD와 HHV 8, EBV와의 연관성을 알아보기 위하여 본 연구를 시행하였다. 방 법 : 인제대학교 상계백병원에서 1998년 1월부터 2005년 12월까지 KD로 진단되어 치료받은 17세 이하 소아 26례를 대상으로 임상적 특징을 고찰하였다. 병력지 고찰을 통하여 후향적으로 분석하였고 추적 조사는 외래 병력 기록지와 전화 방문을 통하여 분석하였다. KD 환아의 림프절 조직으로부터 DNA를 추출하여 HHV 8 DNA를 검출하기 위해 PCR, EBV RNA를 검출하기 위해 ISH가 시행되었다. 결 과 : KD로 진단된 26명 중 남아 11례, 여아 15례로 성비는 1:1.4였고, 평균 연령은 13세였다. 환아의 연도별 분포는 2000년에 7례로 가장 많았으며, 여름에 가장 많이 진단되었다. 주증상은 발열(8/26)과 림프절의 동통(11/26)으로 발열의 기간은 평균 7.3일 이었다. 목빗근 뒷부위의 림프절 종대가 72%(18/24)였고 1례에서 경부 이외의 림프절 종대로 나타났다. 림프절의 크기는 $1cm{\times}1cm$에서부터 $6cm{\times}6cm$까지로 다양하게 나타났다. 백혈구 감소가 46%(6/13)에서 있었고 적혈구 침강 속도상승(>20 mm/hr)이 62%(8/13)에서 나타났다. 검체 확보가 가능하였던 20례 모두에서 HHV 8 DNA는 검출되지 않았으며, ISH를 이용한 EBV RNA 검사 결과도 음성이었다. KD 26례 모두 임상 경과는 양호하였으며, 1례(4%)만이 추적 중 발열과 함께 재발하였다. 전신성 홍반성 루프스를 포함한 결체 조직 질환으로의 이환은 관찰되지 않았다. 결 론 : KD는 소아에서 드물지 않게 발생하므로, 림프절 종대를 호소하는 환아에서 감별 진단에 포함되어야 한다. HHV 8과 EBV는 소아에서 진단된 KD의 원인 병원체로 작용할 가능성이 적을 것으로 생각된다.

Idiopathic multicentric Castleman disease presenting progressive reticular honeycomb infiltration of lung and immunoglobulin G and immunoglobulin G4 dominant hypergammaglobulinemia: a case report

  • Multicentric Castleman disease (MCD) is an uncommon systemic lymphoproliferative disorder that may cause multiple organ damage. Castleman disease-associated diffuse parenchymal lung disease (DPLD) has not been well studied. A 32-year-old man was referred to our hospital for progressive generalized weakness, light-headedness, and dyspnea on exertion for more than one year. Laboratory evaluations showed profound anemia, an elevated erythrocyte sedimentation rate, and an increased C-reactive protein level with polyclonal hypergammaglobulinemia. Chest radiography, computed tomography (CT), and positron emission tomography-CT scan demonstrated diffuse lung infiltration with multiple cystic lesions and multiple lymphadenopathy. In addition to these clinical laboratory findings, bone marrow, lung, and lymph node biopsies confirmed the diagnosis of idiopathic MCD (iMCD). Siltuximab, an interleukin-6 inhibitor, and glucocorticoid therapy were initiated. The patient has been tolerating the treatment well and had no disease progression or any complications in 4 years. Herein, we report this case of human herpesvirus-8-negative iMCD-associated DPLD accompanied by multiple cystic lesions, multiple lymphadenopathy, and polyclonal hypergammaglobulinemia with elevated immunoglobulin G (IgG) and IgG4 levels. We recommend a close evaluation of MCD in cases of DPLD with hypergammaglobulinemia.

  • To evaluate in vitro anti-HIV efficacies of nucleoside derivatives, MT-4 cell line was infected with HIV-1 and HIV-2 respectively and treated with various compounds and the formerly approved drugs such as AZT, d4T, ddC and ddI. CPE method was used to evaluate their antiviral activity. Most dideoxynucleosides, AZT, d4T, ddC and ddI, showed anti-HIV activities against both viruses but no other compounds including anti-herpesvirus drugs did any. Further experiments were carried out to study their inhibitory mechanism of viral adsorption. The results showed no inhibition of syncytium formation due to an interaction between the gp120 expressed in HIV -infected cell surface and CD4 receptor on the uninfected cell surface in the presence of AZT. AZT showed no activity up to $100\;{\mu}g/ml$. Inhibition of reverse transcriptase (RT) in the presence of AZT-triphosphate was tested by using RT expressed in E. coli and purified and its $IC_{50}$ was 4.5 nM.

  • Varicella, more commonly known as chickenpox, is caused by the varicella-zoster virus. It is a common benign childhood illness. In adults, Varicella is uncommon but is more severely associated with complications including pneumonia, hepatitis, disseminated intravascular coagulation, encephalitis and myocarditis. A serious and life-threatening complication is the development of varicella-zoster virus pneumonia (VZVP). Although VZVP is well described in immunocompromised hosts, it is rarely seen in immunocompetent adults. The VZVP in healthy adults is more prevalent in cigarette smokers and during pregnancy. However, reports of VZVP in healthy adults are scarce in Korea. The authors report here a case of VZVP in an immunocompetent adult and present a literature review.

  • Lymphoepithelioma-like carcinoma (LELC) of the lung is a very rare tumor. Originally described in the nasopharynx as lymphoepithelioma, this carcinoma has also been found in the stomach, esophagus, thymus, cervix, urinary bladder, skin, and salivary glands. Histologically, it is an undifferentiated carcinoma that has a syncytial appearance with tumor cells and is infiltrated by numerous lymphocytes, macrophages, and plasma cells. LELC of the lung occurs more commonly in Asians, particularly Chinese. Many studies have reported the association between Epstein-Barr virus (EBV) and LELC of the lung in Asian patients. A 45-year-old man had a solitary pulmonary nodule on a routine chest X-ray examination. As a malignant tumor was suspected, surgical resection was performed to establish the correct diagnosis. The pathology of the excised tumor demonstrated LELC of the lung. This is the first report of LELC of the lung in Korea.

Human Cytomegalovirus Inhibition of Interferon Signal Transduction

  • Cytomegalovirus (CMV), a beta-herpesvirus with worldwide distribution, exhibits host persistence, a distinguishing characteristic of all herpesviruses. This persistence is dependent upon restricted gene expression in infected cells as well as the ability of productively infected cells to escape from normal cell-mediated anti-viral immunosurveillance. Type I (IFN-α/β) and type II (IFN-γ) interferons are major components of the innate defense system against viral infection. They are potent inducers of MHC class I and II antigens and of antigen processing proteins. Additionally, IFNS mediate direct antiviral effects through induction effector molecules that block viral infection and replications such as 2′, 5-oligoadenylate synthetase (2, 5-OAS). IFNS function through activation of well-defined signal transduction pathways that involve phosphorylation of constituent proteins and ultimate formation of active transcription factors. Recent studies have shown that a number of diverse viruses, including CMV, EBV, HPV mumps and Ebola, are capable of inhibiting IFN-mediated signal transduction through a variety of mechanisms. As an example, CMV infection inhibits the ability of infected cells Is transcribe HLA class I and II antigens as well as the antiviral effector molecules 2, 5-OAS and MxA I. EMSA studies have shown that IFN-α and IFN-γ are unable to induce complete signal transduction in the presence of CMV infection, phenomena that are associated with specific decreases in JAKl and p48. Viral inhibition of IFN signal transduction represents a new mechanistic paradigm for increased viral survival, a paradigm predicting widespread consequences in the case of signal transduction factors common to multiple cytokine pathways.
