Newly Developed Urinary Retention and Motor Weakness of Lower Extremities in a Postherpetic Neuralgia Patient

  • During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

  • Herpes zoster (HZ) is a common disease in the aging population and immunocompromised individuals, with a lifetime risk of 20%-30% that increases with age. HZ is caused by reactivation of the varicella-zoster virus (VZV), which remains latent in the spinal dorsal root ganglia and cranial sensory ganglia after resolution of the primary VZV infection. The main focus of HZ management is rapid recovery from VZV infection as well as the reduction and prevention of zoster-associated pain (ZAP) and postherpetic neuralgia (PHN). The use of antivirals against VZV is essential in the treatment of HZ. However, limited antivirals are only licensed clinically for the treatment of HZ, including acyclovir, valacyclovir, famciclovir, brivudine, and amenamevir. Fortunately, some new antivirals against different types of Herpesviridae have been investigated and suggested as novel drugs against VZV. Therefore, this review focuses on discussing the difference in efficacy and safety in the currently licensed antivirals for the treatment of HZ, the applicability of future novel antivirals against VZV, and the preventive or therapeutic effects of these antivirals on ZAP or PHN.

  • In this case, patient diagnosed as herpes zoster was given Famciclovir for 5 days but did not respond. The patient was then confirmed as Soyangin and given combined medication of Yangdokbackho-tang and Famciclovir and showed remarkable improvement in vesicles and his pain. Most of symptoms were gone after 10 days of combined medication. We used Visual Analogue Scale(VAS) and pictures of the affected part for the assessment and report this case.

  • Finger infections are a common problem often caused by viruses, bacteria, or fungi. Similarly, toe infections can present with similar clinical symptoms. Prompt identification of the cause of an infection is crucial for preventing disease progression to a state necessitating immediate and appropriate medical or surgical intervention. Herpetic whitlow is characterized by erythema and painful, non-purulent vesicles and typically results from a herpes simplex virus type 1 or 2 finger infections. However, while herpes whitlow of a finger is common, cases involving a toe are rare. Consequently, a lack of experience of herpetic whitlow of the toe could lead to a misdiagnosis as a bacterial infection and potentially result in unnecessary surgical treatment. Herein, we present a case of herpetic whitlow affecting a great toe that was initially misdiagnosed as a bacterial infection and subsequently treated surgically.

대상포진 환자의 건강 관련 삶의 질 영향 요인 (Health-related Quality of Life and Its Affecting Factors among the in Patients with Herpes zoster)

  • 본 연구는 대상포진 환자의 건강과 관련 삶의 질에 영향을 미치는 요인을 확인하여 건강한 삶의 증진을 돕기 위한 간호중재 프로그램을 개발하는 기초자료를 제공하고자 수행되었다. 대상포진 환자 141명을 대상으로 2016년 9월 1일부터 12월 31일까지 수집하였고 SPSS/WIN 23.0을 이용하여 수집된 자료를 다중회귀분석으로 분석하였다. 그 결과 대상포진 환자의 현재 통증정도는 5.23±2.11점, 지각된 건강상태는 3.35±1.04점, 자기효능감은 66.36±10.79점, 가족지지는 42.093±8.68점, 사회적 지지는 59.80±20.42점, 그리고 건강관련 삶의 질은 74.12±5.73점이었다. 연구 대상자의 건강 관련 삶의 질 영향요인은 지각된 건강상태, 동거가족여부로 확인되었으며 이들 변수의 설명력은 39.0%로 나타났다. 따라서 지각된 건강상태가 긍정적일수록, 동거가족이 있을 때 대상포진 환자의 건강 관련 삶의 질이 높은 것으로 나타났다. 이러한 연구를 토대로 대상포진 환자의 건강 관련 삶의 질을 향상시키기 위해서는 간호사가 대상자에 맞는 긍정적인 건강상태 지각 형성을 도울 수 있는 중재방안 마련과 본 연구에서 확인된 영향요인들을 고려하여 효율적인 간호중재 프로그램 개발이 되어야 한다고 사료된다.

궤양성 질환의 증상과 처치 (Symptoms & Treatment of Ulcerative Conditions)

  • 구강내에서 발생하는 연조직 질환중 궤양을 수반하여 환자로 하여금 통증을 호소하게 하는 질환으로는 급성 괴사성 궤양성 치간염 (Acute Necrotizing Ulcerative Gingivitis), 급성 수포성 구내염(Acute Herpetic Gingivo Stomatitis), 재발성 구순포진(Recurrent Herpes Labialis), 재발성 아프타성 구내염(Recurrent Aphthous Stomatitis), 대상성 포진(Herpes Zoster) 등이 있으며, 다른 질환보다도 그 발생빈도가 높아 임상에서 접하게 되며 그 치유속도가 늦어 가끔 당혹감을 느끼는 수가 있다. 이들 질환의 임상적 소견과 그 질환에 대한 치료법의 예를 간략하게 생각해 보고져 한다.

  • For screening of the antiviral agent from soil, about 520 strains of microorganisms were evaluated for their antiviral activity, About 6.9% of strains showed more than 95% antiviral activity against Herpes Simples Virus (HSV)-1. Two strains among 30 strains active against HSV-1 virus showed a quite strong activity against human immunodeficiency virus.

  • Several acyclonucleoside analogues of pyrimidine base and N$^1$-derivatives of 5-fluorouracil have been synthesized and evaluated for their biological effects. When tested with in vitro Lekemia L1210 cells, the 5-fluorouracil derivatives exhibited slightly higher antitumor activity than the parent 5-fluorouracil. When tested against Herpes Simplex Virus type 1 and type 2 cultured in the Vero cell, the 5-fluorouracil derivatives showed weak antiviral activity.

  • 1-Dodecylazacycloheptan-2-one (Azone) is a new agent that enhances the percutaneous penetration of a number of different chemicals. BVDU and FEAU were evaluated for their potential efficacy in the treatment of cutaneous herpes simplex virus infections by in vitro studies through hairless mouse skin. This study demonstrates the value of penetration enhancing agent (Azone) and the need for a predictable evaluations in the development of topical antiviral agents.

세포교정영양요법(OCNT)을 이용한 대상포진 후 신경통 개선 사례 (Improvement of Symptoms in Patients with Postherpetic Neuralgia Using Ortho-Cellular Nutrition Therapy)

  • Objective: To improve patients' postherpetic neuralgia (PHN) symptoms using Ortho-Cellular Nutrition Therapy (OCNT). Methods: A Korean woman in her forties suffering from neuralgia due to herpes zoster was treated with OCNT for approximately six months. Results: After initiating OCNT, the patient gradually experienced reduced neuralgia symptoms in the chest, flank, and back areas. Approximately six months later, she no longer felt discomfort from these symptoms. Conclusion: OCNT can be beneficial in alleviating symptoms of neuralgia in patients suffering from PHN.
