Herpes Zoster Ophthalmicus in Minor Facial Burn (눈대상포진이 병발된 경미한 얼굴의 화상)

  • Purpose: Many conditions can mimic the presentation of burns. Herpes zoster is one of them. The characteristic features of herpes zoster such as vesicles, pustular lesions and crusts can also be found in burns. Herpes zoster ophthalmicus is a disease caused by recurrent infection of varicella - zoster virus in the ophthalmic division of the trigeminal nerve. This virus frequently affects nasociliary branch and serious ocular complications can occur. Thus, early diagnosis and proper treatment of this disease is important to prevent further ocular manifestations. We report a man who sustained minor facial burn injury that was complicated with herpes zoster ophthalmicus. Methods: A 66 - year - old man visited emergency room with multiple whitish vesicles with serous discharge on right forehead, right medial canthal area and nose. At first he was thought to have a secondary infection of facial burn injury. The vesicles on his face began to form crusts on the next day. Since his skin lesion was located on the ophthalmic division of trigeminal nerve, we also suspected herpes zoster ophthalmicus. He was referred to dermatologist and ophthalmologist. Results: We used antiviral agent (Acyclovir) and NSAIDs for treatment. The patient had no ocular complications. His skin lesion was almost healed after 1 month and remained scars. We treated a patient with minor facial burn complicated with herpes zoster ophthalmicus with antiviral agent. Conclusion: In this work, we describe a case of old patient with herpetic infection and emphasize the need for careful examination to diagnose accurately.

Trends of Korean Medicine Treatment for the Acute Stage of Herpes Zoster : A Review for the case reports (대상포진의 급성기 치료에 대한 국내 연구 현황 : 증례 보고 분석)

  • Objectives : The purpose of this study is to analyze the trend of Korean medical treatments on acute stage of herpes zoster. Methods : Domestic databases(OASIS, KTKP, RISS, NDSL, KISS) were used to search case study papers related to the acute herpes zoster. Total 33 studies were selected and analyzed. Results : According to clinical type, Ramsay Hunt syndrome was the most frequent type, followed by herpes zoster genralisatus, facial herpes zoster. Manual acupuncture was the most frequently used treatment modalities, followed by Korean herbal medicine. LI4(合谷), LR3(太衝), ST36(足三里), GB20(風池) were the major acupoints used for the treatment of acute herpes zoster. Yongdamsagan-tang(龍膽瀉肝湯) was the most frequently used prescription followed by Ligigeopoong-san(理氣祛風散). As principal herb, Gentianae Scabrae Radix et Rhizoma(龍膽) and Bupleuri Radix(柴胡) were the most frequently used herbs followed by Alismatis Rhizoma(澤瀉). Conclusions : Though several limits remains, this is the first study to analyze the trends of Korean medicine treatment for the acute stage of herpes zoster, it may helpful for the clinical practitioner.

  • Herpes simplex virus [HSV] esophagitis is a rare disease and most of cases are reported in the immunocompromised patients. We report a case of fatal HSV infection started from herpes labialis and esophagitis. She had initially suffered from perioral ulcer, esophagitis, and interstitial pneumonitis later and eventually died of respiratory insufficiency one month later after the aneurysmal surgery. She did not have any immunocompromised medical history and also had no evidence of herpes encephalitis in laboratory study and magnetic resonance image [MRI]. With the availability of effective agents for the treatment of HSV infection, early recognition of HSV esophagitis is important because it may be clinically confused with Cushing ulcer and may be complicated with a fatal pneumonitis.

  • Objectives : These 2 case studies were made to evaluate and observe the treatment for the Herpes Zoster through the Hwangryunhaedoktang Herbal Acupuncture. Methods : Clinical observation analysis about 2 cases of Herpes Zoster had been done for the patients of the DongSeo Medical Center. These cases were mainly treated with subcutaneous injection of Hwangryunhaedoktang Herbal Acupuncture at the vesicles. Results : After the treatment, the vesicles were cleared and the pain was decreased. Conclusions : Based on the clinical results, Hwangryunhaedoktang Herbal Acupuncture can be one of a good remedy for Herpes Zoster.

  • An electron microscopic study was carried out on the morphogenesis of herpes simplex type 2 virus in BHK-21 cells BHK-21 cells was found susceptible to infection and replication of herpes simplex type 2 virus cytopathic effects of the herpes type appeared at approximately 1 day postinoculation. Foci consisting of rounded refractile cells and syncytia were observed. Projection of the nuclear membrane in the infected cells was also seen, Several infected cells showed a track-shaped structure which apparently consisted of multiple layered membranes of the nucleus.

  • Objectives : The purpose of this study is to investigate the efficacy of Bee venom Pharmarcopuncture on facial palsy in Herpes Zoster otiucus. Methods : We used the acupuncture and Bee venom Pharmarcopuncture complex therapies on facial palsy in Herpes Zoster otiucus occurred in pregnancy. The improvement of the symptom and facial movement was evaluated by VAS and Yanagihara scale. Results & Conclusions : After treatment, both VAS and Yanagihara's scale were significantly improved. In this case, we found that the acupuncture and Bee venom Pharmarcopuncture complex therapy is effective for facial palsy in Herpes Zoster otiucus.

  • Herpes zoster is a relatively common disease caused by Varicella-Zoster virus and characterized by a vesicular dermatomal rash and a variety of neurologic manifestations. These neurologic symptoms include herpes zoster neuralgia, myelitis, encephalitis, cranial arteritis, segmental and rarely polyradiculitis. This report is a case of a 57-year-old female with herpes zoster paresis affecting the arm and hand. Herpetic pain was much relieved after continuous cervical epidural blockade. However, fifty days after onset of ailment, she complained motor paralysis. EMG of muscles innervated by the left $C_8$ and $T_1$ roots revealed high frequency denervation potentials and large amplitude polyphasic motor units. We recommended physiotherapy and aggressive exercise therapy of the hand. After eighty days of therapy, denervated spontaneous activities disappeared on EMG. Clinically, strength of muscles had nearly recovered to 75% of normal strength in left hand intrinsics.

The clinical study on the cased of Herpes Zoster Treated with Korean Oriental Medicine (대상포진 치험 증례 임상보고)

  • Objective : These case study were made to evaluate and observe the treatment for the Herpes-Zoster through the korean oriental medicine. Method : Clinical observation and analysis about 4 cases of Herpes-zoster including the Ramsay's-hunt syndrome had been done the patients of the Sang-Ji Oriental Medicine Hospital. These cases were mainly treated with oriental medicine using the Herb medication, Korean Bee-Venom therapy, Acupuncture and Electro-acupuncture therapy. Acupuncture technic was mainly used Sa-Am acupuncture(Four needle technique). Result : After treatment, all of cases were completely cured without any complication. Conclusion : Based on the clinical results, Korean Oriental Medicine is believed to be effective for treating Herpes-zoster, and further studies should be conducted to provide more valuable information.

The Efficacy of Epidural Blockade on Acute Herpes Zoster (대상포진 급성기에 시행한 경막외 차단술의 효과)

  • Background: The efficacy of nerve block on the progression of herpes zoster still remains controversy. The purpose of this study was to evaluate the efficacy of epidural blockade on the pain course of acute herpes zoster. Methods: Among 75 patients admitted with acute herpes zoster, 40 patients were treated with acyclovir and epidural block (epidural group), 35 patients were treated with acyclovir only (no block group). Follow up with patients was done for up to 3 months, either at the outpatient department or by telephone. The days required for pain relief were compared. We also investigated the factors associated with outcome. Results: Though statistically insignificant, patients in the epidural group tended to have faster pain relief. Patients with shorter symptom duration experienced significantly faster pain relief. But other factors such as sex, age, involved dermatome and other coexisting diseases (cancer, diabetes mellitus) were not associated with the outcome in this study. Conclusions: Though patients with epidural block leaded to have taster pair relief, more studies with larger patient population may be needed to confirm the efficacy of epidural block on herpes zoster.

  • Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.
