Motor Paresis Caused by Herpes Zoster in Patients with Rotator Cuff Tear -A report cases- (회전근 개 파열환자에서 대상포진에 의한 운동신경 불완전마비 -증례보고-)

  • Segmental zoster paresis is characterized by focal, asymmetric motor weakness in the myotome corresponding to the dermatome of the rash. A 73-year-old man, who presented with severe right shoulder pain and shoulder girdle muscle weakness, was diagnosed with segmental zoster paresis involvement of the C5⁣-C6 motor roots as a complication of herpes zoster. Girdle muscles (supraspinatus, deltoid and infraspinatus) atrophy had developed in his right shoulder. An MRI showed rotator cuff tearing in his right shoulder; therefore, an arthroscopic rotator cuff repair was performed. Herein, this case is presented to emphasize the importance of considering post-herpetic segmental motor paresis in the differential diagnosis of acute painful motor weakness of the upper extremities.

  • 면역 결핍 바이러스와 허피즈 바이러스 -1,2에 대한 한약의 항바이러스 작용을 관찰하기 위하여, 세포 바이러스 검색에 초기 독성과 감염 바이러스의 생존 세포 수를 비교적 단기간내 볼 수 있는 96-well plate를 이용한 MTT assay로 측정하였다. 본 실험은 예비 실험 단계에서 사용된 총 85가지의 한약중 단미제 6가지와 복합처방 44가지를 선정하였으며 복합처방은 유사한 치법으로 분류하였고 한약의 시료 추출은 순수하게 물로 전탕하여 여과하였다. 실험 결과 파두와 맥아가 면액 결핍 바이러스에 대해 감염초기 유의성이 있었으며 호장근, 갈근우방자탕과 형방패독산이 허피즈 바이러스-1,2에 대해 감염초기 유의성이 있었으나 실험의 시간 경과에 따른 지속적인 약효 안정성을 보이지는 못하였다. 이 실험을 통하여 한약을 이용한 우수한 바이러스 치료를 개발하기 위해서는 단미제나 복합 처방에서 항바이러스 작용이 큰 유효성분의 대량 분리와 세포 실험에 있어서 오차를 줄 일 수 있는 세포면역학적 실험의 도입 등이 필요할 것으로 사료된다.

Antiherpetic Activities of Flavonoids against Herpes Simplex Virus Type 1 (HSV-1) and Type 2 (HSV-2) In Vitro

  • Flavonoids, a group of low molecular weight phenylbenzopyrones, have various pharmacological properties including antioxidant, anticancer, bactericidal, and anti-inflammatory. We carried out anti-herpetic assays on 18 flavonoids in five classes and a virus-induced cytopathic effect (CPE) inhibitory assay, plaque reduction assay, and yield reduction assay were performed. When flavonoids were applied at various concentrations to Vero cells infected by HSV-1 and 2, most of the f1avonoids showed inhibitory effects on virus-induced CPE. Among the flavonoids, EC, ECG (flavanols), genistein (isoflavone), naringenin (flavanone), and quercetin (flavonol) showed a high level of CPE inhibitory activity. The antiviral activity of flavonoids were also examined by a plaque reduction assay. EC, ECG, galangin, and kaempferol showed a strong antiviral activity, and catechin, EGC, EGCG, naringenin, chrysin, baicalin, fisetin, myricetin, quercetin, and genistein showed moderate inhibitory effects against HSV-1. In these experiments, flavanols and flavonols appeared to be more active than flavones. Furthermore, treatment of Vero cells with ECG and galangin (which previously showed strong antiviral activities) before virus adsorption led to a slight enhancement of inhibition as determined by a yield reduction assay, indicating that an intracellular effect may also be involved.

Antiherpetic Activities of Natural Naringenin Alone and in Combinations with Acyclovir and Vidarabine (천연 나린게닌의 항허피스바이러스작용과 아시클로버 및 비다라빈과의 병용효과)

  • To search for less toxic antiherpetic agents, the inhibitory effects of natural naringenin on the plaque formation of herpes simplex virus type 1 (HSV-1) and type 2 (HSV-2) in Vero cells were examined by the plaque reduction assay in vitro. Naringenin inhibited plaque formations of HSV-l and HSV-2 in a dose dependent manner. It also exhibited more potent antiherpetic activity on HSV-l with selectivity index (SI) of 19.1 than on HSV-2 with SI of 5.7 The combined antiherpetic effects of naringenin with nucleoside antiherpetic agents, acyclovir and vidarabine, were examined on the multiplication of these two strains of herpesviruses in Vero cells by the combination assay. The results of combination assay were evaluated by the combination index (CI) that was calculated by the multiple drug effect analysis. The combinations of naringenin with acyclovir on HSV-l and HSV-2 showed more potent synergism with CI values of 0.28∼0.81 for 50%, 70%, 90% effective levels than those with vidarabine with CI values of 0.86-3.28.

Zoster sine herpete: a review

  • Zoster sine herpete (ZSH) is one of the atypical clinical manifestations of herpes zoster (HZ), which stems from infection and reactivation of the varicella-zoster virus (VZV) in the cranial nerve, spinal nerve, viscera, or autonomic nerve. Patients with ZSH display variable symptoms, such as neuralgia, however, different from HZ, ZSH show no zoster, which makes clinical diagnosis difficult. ZSH not only causes initial symptoms, such as neuropathic pain in the affected nerve, Bell palsy, and Ramsay Hunt syndrome, but also postherpetic neuralgia and fatal complications such as VZV encephalitis and stroke. The misdiagnosis of ZSH and tardy antiviral treatment may lead to severe ZSH sequelae. We review the publications related to ZSH, especially its diagnosis with VZV DNA and/or anti-VZV immunoglobulin (IgG and IgM). More work about ZSH, especially ZSH epidemiological survey and guidelines for its diagnosis and treatment, are needed because most of the present studies are case reports.

Susceptibility of Koi, Koi×Red Common Carp, and Red Common Carp×Koi to Koi Herpesvirus (KHV)

  • The disease-causing koi herpes virus (KHV), also known as cyprinid herpesvirus-3 (CyHV3), causes mass mortality of koi and carp. Koi (Cyprinus carpio) is a host for KHV, one of 12 virus species in the Alloherpesviridae family. We examined the effects of KHV disease koi (KK), and on koi×red common carp (KR) and red common carp×koi (RK) cross, using a virus challenge test. The infected fish had clinical signs that included gill necrosis and skin lesions. The RK and KR were highly more resistant (cumulative mortality: RK; 6% and KR; 8%) to KHV infection than KK fish (cumulative mortality: 28%). KHV DNA was confirmed in the tissues of all dead fish in groups by use of polymerase chain reaction (PCR), and the presence of the KHV protein in kidney was confirmed by immunohistochemistry. Histological analysis showed severe gill lesions and fusion of the lamellae in KK fish, but less severe damage in RK fish. In immunohistochemistry analysis, the KHV protein localized in the cytoplasm of infected kidney cells of KK, but the cross groups had lower levels of KHV antigen. Our data indicate that the cross groups had increased resistance to KHV disease.

  • Zoster-associated pain (ZAP) in patients with herpes zoster (HZ) may persist for a long time, occurring even years after the rash has healed. In this case, the patient is diagnosed as having postherpetic neuralgia (PHN). Prodromal symptoms can present with constant or intermittent pain, and are often accompanied by other symptoms, resulting in misdiagnosis and/or inappropriate treatment. The aim of this study is to investigate the characteristics of the prodromal symptoms of ZAP through a multicenter study in Korea.

  • Kaposi's sarcoma (KS) is a cancer that causes patches of abnormal tissue to grow under the skin. It also occurs in the immunosuppressive population. KS is currently believed to be caused by infection with human herpes virus-8 (HHV-8) in non-human immunodeficiency virus patient. A 79-year-old female visited the outpatient clinic presenting with increasing number and size of palpable masses on both upper and lower extremities. She was first diagnosed as drug-erupted dermatitis and stopped her medications, but the symptoms got worse. We did partial biopsy, and KS with HHV-8 was diagnosed histopathologically. She planned to undergo further evaluations and proper treatments. This rare case suggests the need to consider a classic type of KS in the differential diagnosis of specific dermatologic symptoms such as macular, nodular, and darkish patches of upper or lower extremities in elderly patients. It is believed that this case helps to strengthen awareness of this rare disease.

A Recent 10-Year Clinical and Epidemiological Study on Zoster under 20 Years Old (최근 10년간 경인지역 20세 이하 소아 청소년에서 발생한 대상포진의 역학적 변화와 임상적 고찰)

  • Objective : The purpose of this study is to investigate the epidemiology and clinical characteristics of herpes zoster in children and adolescents in Korea and to delineate the effects of varicella vaccine on the incidence of zoster and its complication. Methods : We evaluated the clinical records of 201 patients younger than 20 years admitted for herpes zoster at four hospitals located in Kyung-Gi provine during Jul. 1995 to Jun. 2005. Results : The admitted patients for herpes zoster have increased during the past 10 years. The effects of varicella vaccine on the incidence of herpes zoster remain inconclusive in this study. Only twelve percent of the study patients had underlying diseases. In 34(17%) of 201 patients, complications were confirmed during hospitalization. Of 34 patients, meningitis occurred most frequently. Irrespective of varicella vaccination, occurrence of complications was higher in patients who had experienced varicella previously(=overt varicella infection) than patients who had not(=subclinical varicella infection). In case of no previous varicella history, we found complications to be much lower in those who had received the varicella vaccine than those who had not. Conclusion : We cannot conclude whether the varicella vaccine has an effect on the increasing incidence of zoster. We can conclude that the subclinical varicella infection or vaccination for varicella might lead to a decreased incidence of zoster complications.

  • Background : One of the important mechanisms responsible for a tumor escaping the immune response is an absence of the tumor associated antigen (TAA) on the cancer cell surface. To overcome this, combination gene therapy using a herpes simplex thymidine kinase (HSTK) gene, prototype of drug sensitizing gene, was conducted to enhance T AA release by cell destruction, as well as the cytokine genes for immune cell attraction. Methods : We investigated whether or not transduction with the adenovirus-HSTK (Ad-HSTK) enhanced the sensitivity of Lewis lung carcinoma (LLC) to ganciclovir (GCV) and induced a bystander effect. A Tumor vaccine trial was performed using LLC with ad-HSTK$\pm$ad-GM-CSF$\pm$ad-IL-2 to determine if they exhibit some antitumor effect on established lung cancer xenografts. Results : LLC with ad-HSTK revealed a much higher sensitivity to ganciclovir (GCV). LLC transduced with ad-HSTK and/or ad-IL-2, ad-GM-CSF showed a lower in vivo tumorigenicity. In the treatment experiment, vaccination with LLC transduced with ad-HSTK, ad-IL-2, or ad-GM-CSF alone modestly suppressed the growth of an established tumor. Combined transduction with HSTK and GM-CSF induced stronger growth suppression of a established lung cancer, while HSTK and IL-2 combination transduction did not have any antitumor effect on individual transduction. Vaccination with LLC-HSTK-GM-CSF increased the infiltration of dendritic cells in the spleen. Conclusion : It was concluded that a tumor vaccine transduced with HSTK and GM-CSF induces strong antitumor immunity by activating the dendritic cells.
