The Clinical Effects of Acupuncture and Acupotomy Therapy for HIVD (요추 추간판 탈출증 환자의 침도요법의 효과에 대한 임상적 고찰)

  • Objectives : The study on Acupotomy therapy has recently become a popular academic field as proven to be an great alternative to the limitation of Western medical treatment. However, there has been little study examining acupotomy therapy on treating HIVD. Thus, this study aims to investigate the sense of improvement and satisfaction from the HIVD patients treated with acupotomy therapy. Methods : A total of 63 subjects that consists of HIVD patients were participated in this study. The experimental group(33subjects) was treated with acupotomy therapy and acupuncture, and the control group (30subjects) was treated with acupuncture. All the subjects were asked to answer VAS(visual analogue scale) and ODI(Oswestry disability index) before and after the treatment. Results : The results of the VAS and ODI of comparison analysis between two groups, show that the improvement of HIVD in acupotomy and acupuncture treatment group is more effective than that in acupuncture treatment group

The Effects of Miniscalpel Acupuncture on Cervicogenic Headache: A report of three cases

  • Objectives : This study examined the effects of miniscalpel acupuncture (MA) in three patients with cervicogenic headache. Methods : Patients were treated with MA, which was performed once per week for three weeks. A headache score, visual analogue scale (VAS), and neck disability index (NDI) were used for the evaluation of treatment effects. Results : In cases 1, 2, and 3, the headache score decreased from 4 to 1, 3 to 0, and 3 to 1, respectively. The VAS score decreased from 8.5 to 1.9, 5 to 0.4, and 5.3 to 2 in cases 1, 2, and 3, respectively. The NDI score decreased from 28 to 4, 50 to 4, and 38 to 16 in cases 1, 2, and 3, respectively. Conclusion : MA appears to be effective for the treatment of cervicogenic headache. Further data should be collected and a comparative study using other treatment methods should be performed.

The Clinical Study of 35 Admission Patients to Oriental Medical Hospital due to Thoraco-Lumbar Compression Fracture (흉요추부 압박골절 진단을 받고 한방병원에 입원한 환자 35명에 대한 임상적 고찰)

  • Objective : The purpose of this study is to investigate the results of korean traditional conservative therapy for thoraco-lumbar compression fracture and it's recovery rate and different result in accordance with sex, age, medical history. Method : We studied about 35 cases who were admitted to Jaseng Hospital of Oriental Medicine with diagnosis of 'Thoraco lumbar compression fracture' with X-ray examination. Result : Out of 35 patients 31 patients discharged with satisfying results. Most of the compression fracture occurred in age of $60{\sim}70$. And the single fractured patients recovered more easily than the multi-level fractured patients. It didn't affect the results whether the patient had another vertebral disease(such as Herniated intervertebral disc or osteoporosis) or not. And most of the patients who had vertebral compression fracture visited the western medicine hospital first. Conclusions : We analyzed the tendency of the patients who had vertebral compression fracture, and concluded that the conservative Korean traditional therapy is an effective means of treatment for the patients who have thoraco-lumbar compression fracture.

A Case Report of 2 Failed Back Surgery Syndrome Patients Treated by Chuna Cranio-Sacral Therapy with Korean medical treatments (두개천골추나요법을 한의학적 치료에 병행하여 호전된 척추수술실패증후군 환자 치험 2례)

  • Objectives : The purpose of this study is to show the effectiveness of Chuna Cranio-Sacral Therapy for remaining pain and muscle strength decrease after herniated intervertebral disc surgery. Methods : Two patients with Failed Back Surgery Syndrome(FBSS) were hospitalized and treated by Korean medical treatments with Chuna Cranio-Sacral Therapy. The Range of Motion(ROM) of the lumbar spine and Manual Muscle Test(MMT) were adopted to measure the resulting motor recovery after treatment. And the Numerical Rating Scale(NRS) was adopted to measure changes of pain level. Two patients both had Chuna Cranio-Sacral Therapy category III disorder with short right leg. We applied category III blocking technique everyday for 20 minutes with Korean medical treatments. Results : In both cases, the range of motions were improved and the pain level decreased. And in case 1, muscle strength also showed improvements. Conclusions : We suggest Chuna Cranio-Sacral Therapy is effective for pain decrease and motor recovery of FBSS patients. Further studies are needed to set up an Korean medical protocol for FBSS.

A Case-control Study of The Effect of Cotreatment with Sinseon Moxibustion on Low Back Pain of HIVD Patients (요추추간판탈출증 환자의 요통에 대한 신선구법 병행치료 효과의 대조군 연구)

  • Objectives : The purpose of this study is to investigate the clinical effects of Sinseon moxibustion for HIVD patients with low back pain. Methods : From March 1st, 2011 to May 31st, 2011, 95 HIVD patients who were admitted to Bu-Cheon Jaseng Oriental Medicine Hospital were divided into two groups. One group(control group) was treated with acupuncture and herbal medicine. The other group(experimental group) was treated with acupuncture, herbal medicine and Sinseon moxibustion. We evaluated the treatment effect of each group with numerical rating scale(NRS) and Oswestry disability index(ODI). The evaluations were performed four times(admission day, 5th day after admission, 9th day after admission, 13th day after admission) Results : 1. In both control group and experimental group, NRS and ODI decreased significantly in statistics as treatment was performed. 2. Between 9th day and 13th day after admission, experimental group showed significant reduction in NRS and ODI compared to control group. 3. From admission day to 13th day after admission, experimental group showed significant reduction in NRS and ODI compared to control group. Conclusions : We recommend that cotreatment of Sinseon moxibustion as a useful therapy to HIVD patients with low back pain.

Comparative Study of Clinical and Radiological Outcomes of a Zero-Profile Device Concerning Reduced Postoperative Dysphagia after Single Level Anterior Cervical Discectomy and Fusion

  • Objective : This study analyzed clinical and radiological outcomes of a zero-profile anchored spacer (Zero-P) and conventional cage-plate (CCP) for single level anterior cervical discectomy and fusion (ACDF) to compare the incidence and difference of postoperative dysphagia with both devices. Methods : We retrospectively reviewed our experiences of single level ACDF with the CCP and Zero-P. From January 2011 to December 2013, 48 patients who had single level herniated intervertebral disc were operated on using ACDF, with CCP in 27 patients and Zero-P in 21 patients. Patients who received more than double-level ACDF or combined circumferential fusion were excluded. Age, operation time, estimated blood loss (EBL), pre-operative modified Japanese Orthopaedic Association (mJOA) scores, post-operative mJOA scores, achieved mJOA scores and recovery rate of mJOA scores were assessed. Prevertebral soft tissue thickness and postoperative dysphagia were analyzed on the day of surgery, and 2 weeks and 6 months postoperatively. Results : The Zero-P group showed same or favorable clinical and radiological outcomes compared with the CCP group. Postoperative dysphagia was significantly low in the Zero-P group. Conclusions : Application of Zero-P may achieve favorable outcomes and reduce postoperative dysphagia in single level ACDF.

Taping Therapy for the Low Back Pain based on Postural Reflexes (자세반사에 기초한 요통의 테이핑 치료)

  • The muscles relating to movement of painful low back was analyzed kinematically, by method of applying elastic tapes and putting non-elastic tapes on muscles involving those movements which cause pain and limitation of range of motion (ROM) in low back and trunk. Taping therapy which is effective for improvement of painful low back and which is supposed to facilitate the total and continual movement based on the postural reflexes will be presented in this paper. Fifty cases who had painful low back were investigated. Patients with painful low back were at first asked pain point and direction of painful movement, and then tested the muscles which are cause of those pain and limited motion. Before attaching tape, all subjects were divided into two groups, flexor pattern and extensor pattern, according to direction of increasing pain. Elastic tapes were applied from origin to insertion of objective muscles and non-elastic tapes were put on effective points of the muscles which were associated with respect to pain and limited ROM. As a result of this study, all subjects with low back pain significantly improved in pain and ROM at the low back. The longest treatment duration group for the low back pain cases was herniated lumbar disc(10.4 days) group. Low back pain have been treated by so many ways. muscles factor should be emphasized to be distinguish1y important to reduce low back pain. The muscles relating to make directly painful motion in low back and trunk should be confirmed by means of taping and painful motion should be analyzed by point of view of postural reflexes.

Symptomatic Post-Discectomy Pseudocyst after Endoscopic Lumbar Discectomy

  • Objective: The objectives of this study were to determine the frequency of symptomatic postdiscectomy pseudocyst (PP) after endoscopic discectomy and to compare the results of surgical and conservative management of them. Methods: Initial study participants were 1,503 cases (1,406 patients) receiving endoscopic lumbar discectomy by 23-member board of neurosurgeons from March 2003 to October 2008. All patients' postoperative magnetic resonance imaging (MRI) scans were evaluated. On the postoperative MRI, cystic lesion of T2W high and T1W low at discectomy site was regarded as PP. Reviews of medical records and radiological findings were done. The PP patients were divided into two groups, surgical and conservative management by treatment modality after PP detection. We compared the results of the two groups using the visual analogue scale (VAS) for low back pain (LBP), VAS for leg pain (LP) and the Oswestry disability index (ODI). Results: Among 1,503 cases of all male soldiers, the MRls showed that pseudocysts formed in 15 patients, about 1.0% of the initial cases. The mean postoperative interval from surgery to PP detection was 53.7 days. Interlaminar approach was correlated with PP formation compared with transforaminal approach. (p=0.001).The mean VAS for LBP and LP in the surgical group improved from 6.5 and 4.8 to 2.0 and 2.3, respectively. The mean VAS for LBP and LP in the conservative group improved from 4.4 and 4.4 to 3.9 and 2.3, respectively. There was no difference in treatment outcome between surgical and conservative management of symptomatic PP. Conclusion: Although this study was done in limited environment, symptomatic PP was detected at two months' postoperative period in about 1% of cases. Interlaminar approach seems to be more related with PP compared with transforaminal approach.

Clinical Analysis of Chemonucleolysis for Treatment of Lumbar Disc Herniation (요추간판 탈출증치료에서 화학적 수핵용해술의 임상적 고찰)

  • During last 48 months, in our department 51 patients with single or multiple herniated lumbar discs have received chemonucleolysis. Forty-one of 51 cases have single level herniation and remaining cases have multiple level. Age distribution were from 19 to 59 years old. Over 4 months(longest case: 18 months) follow up revealed as follows: 40 cases(78%) revealed excellent results: 4 cases(7.8%) received discectomy 3-6 months after chemonucleolysis because of recurred symtoms : no anaphylaxis and no major complications were noted.

Comparative Analysis of the Independent Medical Examination Reports and Legal Decisions in Pain Medicine

  • Background: An independent medical examination (IME) is a critical process for awarding reparation for injury. However, conducting an IME in pain medicine is very difficult, not only because pain is a subjective symptom, but also because there are no proper objective methods to demonstrate it. This study was conducted to compare IME reports and the court decisions on the disability status of the patients. Methods: We analyzed 79 IME reports and 25 corresponding court decisions on the disability status of patients. The diagnoses, causal relationships between the patients' status and the trauma, McBride's degree of disability, the American Medical Association's impairment ratings, the estimated annual cost for future treatment, and the necessity of care-giving were compared and analyzed. Results: The diagnoses in the 79 cases were complex regional pain syndrome (CRPS) type I (58 cases), CRPS type II (7 cases), peripheral neuropathy (5 cases), myofascial pain syndrome (4 cases), herniated intervertebral disc (2 cases), and fibromyalgia (1 case). The types of accidents were road traffic accidents (50 cases), military injuries (14 cases), industrial accidents (11 cases), and others (4 cases). The IME reports and the court decisions stated considerably different McBride's degrees of disability (P = 0.014). However, there was no significant difference in the estimated cost for future treatment between the IME reports and the court decisions (P = 0.912). Conclusions: IME reports should be accurate, fair, and based on objective findings. Feedback on IMEs from the court decisions is helpful for reference use.
