  • The roots and rhizome of Acanthopanax koreanum are used as folk medicine to ameliorate hepatitis in Korea. The ability of A. koreanum to protect mice from fulminant hepatitis induced in mice by D-galactosamine and lipopolysaccharide was evaluated. Preparations of A. koreanum used were an ethanol extract, and the ethanol-soluble and ethanol-insoluble components of the water extract. (omitted)

Large-Scale Culture of Hepatitis A Virus in Human Diploid MRC-5 Cells and Partial Purification of the Viral Antigen for Use as a Vaccine

  • A large-scale culture of hepatitis A virus in human diploid MRC-5 cells was conducted. In a roller bottle culture, the virus was grown to a maximum titer in 3 weeks after infection. Over 95% of the cell-associated virus was excreted after culturing the infected cells in suspension media without fetal bovine serum for 3 days. The cultured virus was inactivated with formalin, concentrated by ultrafiltration, and partially purified by ultracentrifugation in a non-ionic gradient medium of Renocal. Two separate peak fractions showing high anti-HAY ELISA titer were pooled and about 40% of HAV antigen was recovered by this purification procedure. Of the partially purified vaccine, the protein pattern in SDS-PAGE and immunogenicity in mice were compared with a commercial HAV vaccine. In SDS-PAGE, the purified vaccine in this study and the commercial vaccine showed almost the same protein pattern. The seroconversion rate of the purified vaccine in mice was not different from that of the commercial vaccine. Therefore, we could prepare a good grade of HAV vaccine by a simple purification procedure although the purification itself was not completed.

Transient and stable expression of hepatitis B surface antigen in tomato (Lycopersicon esculentum L.)

  • Cotyledonary leaves of tomato cv. Megha were transformed with the hepatitis B virus 's' gene, which encodes surface antigen. Six plant expression cassettes (pHBS, pHER, pEFEHBS, pEFEHER, pSHER and pEFESHER) were used to assay the possible expression levels by agroinfiltration. The maximum transient expression level of 489.5 ng/g D.W. was noted in pEFEHER-infiltrated cotyledonary leaves. Transgenic tomato plants with pEFEHBS and pEFEHER expression cassettes were regenerated and characterized by molecular analysis. The expression of the antigen in the fruits was confirmed by RT-PCR and ELISA analysis. This is the first report on the expression of hepatitis B surface antigen in tomato.

Molecular and Structural Characterization of the Domain 2 of Hepatitis C Virus Non-structural Protein 5A

  • Hepatitis C virus (HCV) non-structural protein 5A protein (NS5A), which consists of three functional domains, is involved in regulating viral replication, interferon resistance, and apoptosis. Recently, the three-dimensional structure of the domain 1 was determined. However, currently the molecular basis for the domains 2 and 3 of HCV NS5A is yet to be defined. Toward this end, we expressed, purified the domain 2 of the NS5A (NS5A-D2), and then performed biochemical and structural studies. The purified domain 2 was active and was able to bind NS5B and PKR, biological partners of NS5A. The results from gel filtration, CD analysis, 1D $^1H$ NMR and 2D $^1H-^{15}N$ heteronuclear single quantum correlation (HSQC) spectroscopy indicate that the domain 2 of NS5A appears to be flexible and disordered.

페그-인터페론 치료를 받는 C형 간염 환자의 우울 예측요인 (The Depression Predictors among Patients with Peg-interferon Treated Hepatitis C)

  • Purpose: This study was to identify factors predicting the occurrence of depression among patients with hepatitis C treated with Peg-interferon. Methods: The subjects were 134 patients treated with Peg-interferon after the diagnosis with hepatitis C at the D University Hospital and 3 hospitals located in Busan, South Korea. The predictors of depression were identified by measuring the general characteristics of the subjects and their disease-related characteristics, social support, sleep disorders, and degree of side effects by interferon. The data were analyzed with a t-test, ANOVA, and logistic regression using the SPSS/WIN 18.0 program. Results: Sixty-six point four percent of the subjects suffered from mild to more severe depression. Fatigue, hair loss, itching, and sleep disorders turned out to be significant predictors. In addition, the explanatory power of these variables for the occurrence of depression was 70%. Conclusion: Sleep disorders and drug side effects were determined to have significant influence on the development of depression in hepatitis C patients treated with Peg-interferon. Therefore, the development of nursing intervention programs aimed to prevent depression by effectively managing sleep disorders and side effects is mandated in future to help the given population.

CCR5 Polymorphism as a Protective Factor for Hepatocellular Carcinoma in Hepatitis B Virus-Infected Iranian Patients

  • The CC chemokine receptor 5 (CCR5) delta 32 allele results in a nonfunctional form of the chemokine receptor and has been implicated in a variety of immune-mediated diseases. $CCR5{\Delta}32$ may also predispose one to chronic liver disease or be linked with resistance to HBV infection. This study was undertaken to investigate any association between CCR5 polymorphism with resistance to hepatitis B or susceptibility to HBV infection. A total of 812 Iranian individuals were enrolled into two groups: HBV infected cases (n=357), who were HBsAg-positive, and healthy controls (n=455). We assessed polymorphisms in the CCR5 gene using specific CCR5 oligonucleotide primers surrounding the breakpoint deletion. Genotype distributions of the HBV infected cases and healthy controls were determined and compared. The CCR5/CCR5 (WW) and $CCR5/CCR5{\Delta}32$ (W/D) genotypes were found in (98%) and (2%) of HBV infected cases, respectively. The $CCR5{\Delta}32/{\Delta}32$genotype was not found in HBV infected cases. Genotype distributions of CCR5 in healthy controls were W/W genotype in (87.3%), W/D genotype in (11.2%) and D/D genotype in (1.5%). Heterozygosity for $CCR5/CCR5{\Delta}32$ (W/D) in healthy controls was greater than in HBV infected cases (11.2% vs 2%, p < 0.001). W/D and D/D genotypes were more prominent in healthy controls than in HBV infected cases. This study provides evidence that the $CCR5{\Delta}32$ polymorphism may have a protective effect in resistance to HBV infection at least in the Iranian population.

담즙정체성 간염의 임상적 양상 (Clinical Features of Cholestatic Hepatitis)

  • 담즙정체성 간염의 원인과 임상양상, 검사실 소견 및 경과를 알아보고자 1991년에서 2000년까지 영남대학교 의과대학 부속병원에서 혈액검사 및 간생검으로 담즙정체성 간염이 확진된 14명의 환자를 대상으로 후향적 연구를 시행하여 다음과 같은 결과를 얻었다. 담즙정체성 간염에서 비정상 간기능검사의 기간은 1개월에서 30개월까지 다양하게 나타났고, 항결핵제, 항생제에 의한 담즙정체성 간염이외에도 한약제, 건강식품에 의한 경우 검사실 소견과 임상경과가 중하게 나타나는 경우가 있으므로, 이들 약물을 사용한 병력이 있는 경우 정기적인 간기능 검사가 필요하다. 담즙정체성 간염이 만성 간내 담즙정체를 보이는 경우 바이러스에 의한 담즙정체성 간염과 담관소멸 증후군으로의 진행 유무, 원발성 담즙성 간경변증, 자가면역성 간염과의 감별이 필요하며, 지속적인 간기능 검사이상을 보일 경우에는 연속적인 간생검이 필요할 것으로 생각된다.

Development of a Rapid Automated Fluorescent Lateral Flow Immunoassay to Detect Hepatitis B Surface Antigen (HBsAg), Antibody to HBsAg, and Antibody to Hepatitis C

  • Background: Accurate, rapid, and cost-effective screening tests for hepatitis B virus (HBV) and hepatitis C virus (HCV) infection may be useful in laboratories that cannot afford automated chemiluminescent immunoassays (CLIAs). We evaluated the diagnostic performance of a novel rapid automated fluorescent lateral flow immunoassay (LFIA). Methods: A fluorescent LFIA using a small bench-top fluorescence reader, Automated Fluorescent Immunoassay System (AFIAS; Boditech Med Inc., Chuncheon, Korea), was developed for qualitative detection of hepatitis B surface antigen (HBsAg), antibody to HBsAg (anti-HBs), and antibody to HCV (anti-HCV) within 20 minutes. We compared the diagnostic performance of AFIAS with that of automated CLIAs-Elecsys (Roche Diagnostics GmbH, Penzberg, Germany) and ARCHITECT (Abbott Laboratories, Abbott Park, IL, USA)-using 20 seroconversion panels and 3,500 clinical serum samples. Results: Evaluation with the seroconversion panels demonstrated that AFIAS had adequate sensitivity for HBsAg and anti-HCV detection. From the clinical samples, AFIAS sensitivity and specificity were 99.8% and 99.3% for the HBsAg test, 100.0% and 100.0% for the anti-HBs test, and 98.8% and 99.1% for the anti-HCV test, respectively. Its agreement rates with the Elecsys HBsAg, anti-HBs, and anti-HCV detection assays were 99.4%, 100.0%, and 99.0%, respectively. AFIAS detected all samples with HBsAg genotypes A-F and H and anti-HCV genotypes 1, 1a, 1b, 2a, 2b, 4, and 6. Cross-reactivity with other infections was not observed. Conclusions: The AFIAS HBsAg, anti-HBs, and anti-HCV tests demonstrated diagnostic performance equivalent to current automated CLIAs. AFIAS could be used for a large-scale HBV or HCV screening in low-resource laboratories or low-to middle-income areas.

Studies on the development of enzyme linked immuno-sorbent assay (ELISA) for hepatitis B surface antigen (HBsAg) by monoclonal antibodies of different affinity constants

  • Mouse monocolonal antibodies to Hepatitis B surface antien (HBsAg) were prepared and their functional capabilities tested by the method of solid phase enzyme linked immuno sorbent assay (ELISA). HBsAg binding studies inicated that one monoclonal antibody 6E-1-1 bound more HBsAg at a faster rate than the other monoclonal antibodies. Also, for the binding inhibition studies with the selected monoclonal antibody 6E-1-1, one monoclonal antibody 8D-3-6 didn't exhibit binding inhibition for HBsAg. Then, a simultaneous ELISA method was developed for the immunodiagnosis of HBsAg. Different combinations of two monoclonal antibodies as solid phase and horseradish peroxidase (HRPO) labeled phase were studied. The combination of monoclonal antibody of higher affinity constant (6E-1-1) immobilized in a solid phase and monoclonal antibody of lower affinity constant (8D-3-6) as a HRPO laeled phase was more sensitive when two monoclonal antibodies of different affinity constants for HBsAg were prepared.

소아 자가면역성 간염: 임상적 특성, 조직 소견 및 예후 (Clinical Characteristics, Histology and Prognosis of Autoimmune Hepatitis in Korean Children)

  • 목적: 자가면역성 간염은 원인 불명의 진행성 염증성 간질환으로 혈중 자가항체의 출현과 고감마글로불린혈증, 그리고 조직학적으로 문맥주위 염증을 특징으로 한다. 소아에서는 산발적인 증례 보고가 있었으나 국내에서는 아직까지 체계적인 연구가 없었다. 이에 저자들은 소아에서 자가면역성 간염의 임상적, 조직학적 소견과 치료 경과 및 예후에 대해 알아보고자 본 연구를 시행하였다. 방법: 1990년부터 2004년까지 서울대학교 어린이병원에서 자가면역성 간염으로 진단되었던 14명의 환자들의 임상적 특징, 혈액 검사 소견 및 간 조직 검사 소견을 분석하였다. 또한 이들 환자들에게 시행된 치료 약제 및 치료에의 반응, 그리고 재발 여부 등 치료 경과를 조사하였다. 결과: 대상 환자 14명의 진단시 연령은 평균 $9{\pm}3$세였다. 여자가 11명으로 전체의 78%를 차지하였고, 6명에서 급성 발병을 보였다. 임상 증상으로는 황달이 11명에서 호소하여 가장 흔하였으며, 이 외에도 피로감, 식욕부진, 진한색 소변, 상복부 불쾌감, 잦은 코피, 오심, 소양감 및 설사가 동반되었다. 신체 검진 소견에서는 간비종대가 흔하게 나타났다. 6명의 환자에서 다른 자가면역성 질환이 동반되었고 1명에서는 일등친 중 그레이브스병의 병력이 있었다. 혈중 자가항체 검사 결과 13명에서 항핵 항체가 양성으로 나왔으며 항핵 항체가 음성으로 나왔던 1명은 항평활근 항체가 양성으로 나와 대상 환자 모두가 제1형 자가면역성 간염에 해당되었다. 7명에서는 항핵 항체와 항평활근 항체가 모두 양성으로 나왔다. 혈청 IgG는 평균 $2971{\pm}2147mg/dL$였고, AST는 평균 $469{\pm}402\;U/L$, 총 빌리루빈은 평균 $4.9{\pm}5.5mg/dL$였다. 간 조직 검사 소견으로는 문맥 주위의 염증(10명), 간세포의 로제트 형성(3명), 소엽의 염증(6명)이 흔하게 나타났고 2명에서는 진단 당시 이미 조직학적으로 간경변이 동반되었다. 대상 환자 14명 중 6명은 UDCA만 사용하며 관찰 중 간수치가 호전되었고 나머지 8명에게는 스테로이드 치료가 시행되었다. 이중 7명이 초기 스테로이드 치료시 관해를 보였으나 스테로이드를 감량하거나 중지시 재발하였다. 스테로이드 재치료가 시행되었고 3명은 스테로이드 감량 중 관해 유지 중이고 2명은 azathioprine으로 관해 유지 중이다. 결론: 자가 면역성 간염은 원인 불명의 소아 만성 간 질환의 원인 중 하나이며, 적절한 치료로 예후가 개선될 수 있는 간질환이다. 그러므로 특히 여아에서 고감마글로블린혈증을 보이거나 다른 자가면역성 질환이 동반될 경우 자가면역성 간염을 의심하여 자가항체 선별 검사를 시행하고 적절한 진단 및 치료를 시행하는 것이 중요하리라고 생각된다.
