평폐탕(平肺湯)이 Oleic acid로 유발(誘發)된 가견(家犬)의 급성폐수종(急性肺水腫)에 미치는 영향(影響) (Effects of Pyengpaetang Extracts on the Acute Pulmonary Edema induced by Oleic acid in dogs)

  • Certain oriental medication have been shown to be effective in decreasing pulmonary vascular resistance and increasing cardiac output in primary pulmonary artery pressure secondary to pulmonary edema. So oleic acid was administered in 14 dogs in order to induce acute lung injury. And we studied the hemodynamics and blood gas changes of Pyengpaetang(50mg, 100mg) with continuous postive pressure was ventilation in pulmonary edema. The pulmonary edema group, arterial oxygenation was improved after 5 and $10cmH_2O$ PEEP(positive end expiratory pressure), but cardiovascular system was depressed. Blood pressure and cardiac output were decreased, and CVP, MP AP, PCWP were increased. In Pyengpaetang(50mg) group, mean aortic pressure was decreased and PCWP(pulmonary capillary wedge pressure) was decreased remarkably, while there was a significant increase in cardiac output. And there was improvement in $PaO_2$ and $PaCO_2$ without hemodynamic changes after applying 5cm $H_2O$ PEEP, but arterial blood gases$(PaO_2,\;PaCO_2)$ were improved, while cardiovascular effects were depressed after cm $H_2O$ PEEP. In Pyengpaetang(100mg) treated group, there was no significant hemodynamic change. But mean pulmonary arterial pressure was significantly increased, and cardiac output was decreased significantly after applying the more degree of PEEP. And blood gases were not changed significantly after applying the more degree of PEEP. The above results suggest that the effects of Pyengpaetang(50mg) group is superior to those of Pyengpaetang(100mg) group on the effects of hemodynamics and gas exchanges in acute lung injury in dogs. So we can conclude that lower degree PEEP 5cm $H_2O$ is more beneficial in Pyengpaetang(50mg) treated group.

관류 강조 자기공명 영상을 이용한 소아 모야모야 환아의 뇌 혈역학 분석 (Cerebral Hemodynamic Analysis in Pediatric Moyamoya Patients using Perfusion Weighted MRI)

  • Objective: Classically, single photon emission tomography is known to be the reference standard for evaluating the hemodynamic status of patients with moyamoya disease. Recently, T2-weighted perfusion magnetic resonance(MR) imaging has been found to be effective in estimating cerebral hemodynamics in moyamoya disease. We aim to assess the utility of perfusion-weighted MR imaging for evaluating hemodynamic status of moyamoya disease. Methods: The subjects were fourteen moyamoya patients(mean age: 7.21 yrs) who were admitted at our hospital between Sep. 2001 to Sep 2003. Four normal children were used for control group. Perfusion MR imaging was performed before any treatment by using a T2-weighted contrast material-enhanced technique. Relative cerebral blood volume(rCBV) and time to peak enhancement(TTP) maps were calculated. Relative ratios of rCBV and TTP in the anterior cerebral artery(ACA), middle cerebral artery(MCA) and basal ganglia were measured and compared with those of the posterior cerebral artery(PCA) in each cerebral hemispheres. Using this data, we analysed the hemodynamic aspect of pediatric moyamoya disease patients in regarding to the age, Suzuki stage, signal change in FLAIR MR imaging, and hemispheres inducing symptoms. Results: The mean rCBV ratio of ACA, MCA did not differ between normal children and moyamoya patients. However the significant TTP delay was observed at ACA, MCA territories (mean = 2.3071 sec, 1.2089 see, respectively, p < 0.0001). As the Suzuki stage of patients is advanced, rCBV ratio is decreased and TTP differences increased. Conclusion: Perfusion MR can be applied for evaluating preoperative cerebral hemodynamic status of moyamoya patients. Furthermore, perfusion MR imaging can be used for determine which hemisphere should be treated, first.

스텐트 공극률의 뇌동맥류 모델 내부 유동장 영향 수치해석 (Effect of Stent Design Porosity on Hemodynamics Within Cerebral Aneurysm Model: Numerical Analysis)

  • 본 연구에서는 상대적으로 얇은 두께의 스트럿을 가진 스텐트를 적용하여 스텐트의 공극률(80%, 74% 및 64%)에 따른 뇌동맥류 내부 유동 특성 변화를 이해하고자 CFD 해석을 수행하고, 기존의 발표된 실험 결과와 비교하였다. 수치해석 방법으로는 이차의 사면체 요소(quadratic tetrahedral element) 기반의 유한요소해석(FEM) 코드를 이용하였다. 뇌동맥류 내부 평균유속 감소비의 정량적인 면에서는 실험결과와 약간의 차이를 보였으나, 스텐트 공극률에 따른 뇌동맥류 내부 유동장 패턴 및 평균유속 감소의 상대적 특성 등은 비교적 잘 일치하는 것을 확인할 수 있었다. 또한 본 연구에서 고려한 가장 높은 80% 공극률을 가지는 스텐트의 경우에도 비교적 우수한 뇌동맥류 유입 유속 감소 효과를 가짐을 확인할 수 있었으며, 이보다 더 낮은 공극률을 가진 스텐트의 경우에는 약간의 추가적인 뇌동맥류 유입 유속 감소 효과를 가지나, 유속 및 벽전단응력 등의 혈류역학적 특성은 큰 변화가 없음을 알 수 있었다.

뇌동맥류 모델에 대한 혈류역학 해석 (Computational Hemodynamics in the Intracranial Aneurysm Model)

  • 뇌동맥류모델은 CT 영상을 기반으로 추출하여, ANSYS-FLUENT를 사용해 전산 유체유동해석을 수행하였다. 본 연구를 통해 뇌동맥류에서 최소 벽전단응력은 동맥류가 발생한 영역에서 일어나는 것을 알 수 있다. 또한 뇌동맥류 모델에서 우측중뇌동맥 안쪽벽면에 작용하는 벽전단응력의 크기는 동맥류 전부와 후부의 벽면에 작용하는 벽전단응력의 크기에 비해 20 배 더 크게 발생하는 것을 알 수 있다. 그러나 동맥류 영역에서의 전단응력의 크기는 매우 작게 나타났다. 혈관 수축이 일어나는 동안 동맥류의 영역에서 매우 복잡한 이차유동이 발생하는 것을 볼 수 있다. 동맥류 내부에서의 혈류유동은 나선형 유동형태를 보이며, 본 연구의 혈류역학적 특성 분석을 통해 뇌동맥류의 파열을 예견할 수 있을 것으로 판단한다.

Effects of Protein Kinase C Modulation on Hepatic Hemodynamics and Glucoregulation

  • This study evaluated the effects of PKC activation using phorbol 12-myristate 13-acetate (PMA) and PKC inhibition using the isoquinoline sulfomide derivative H-7 on hemodynamics and glucoregulation in the isolated perfused rat liver. Livers were isolated from fed male Holtzman rats and perfused with Krebs Ringer bicarbonate solution under a constant flow of 50 ml/min at $35^{\circ}C.$ Portal vein pressure, glucose and lactate concentrations in the medium and oxygen consumption rates were continuously monitored by a Grass polygraph, YSI glucose and lactate monitors, and a YSI oxygen monitor, respectively. PMA at concentration of 2 to 200 nM increased the portal vein pressure, glucose and lactate production, but decreased oxygen consumption rate in a dose-dependent fashion. H-7 $(200\;{\mu}M)$ attenuated PMA (50 nM)-induced vasoconstriction $(15.1{\pm}1.36\;vs\;10.56{\pm}1.17\;mmHg),$ glucose production rate $(91.3{\pm}6.15\;vs\;71.8{\pm}2.50\;{\mu}moles/g/hr),$ lactate production rate $(72.4{\pm}6.82\;vs\;53.6{\pm}4.82\;{\mu}moles/g/hr)$ and oxygen consumption rate $(33.7{\pm}1.41\;vs\;27.9{\pm}1.75\;{\mu}l/g/min).$ The effects of PMA were blocked either by addition of verapamil $(9\;{\mu}M)$ or perfusion with $Ca^{2+}-free$ KRB. These results suggest that the hemodynamic and glucoregulatory changes in the perfused rat liver are mediated by protein kinase C activation and require $Ca^{2+}$ influx from the extracellular fluid.

Natriuresis Induced by Intracerebroventricular Diazepam in Rabbits

  • The renal function is under regulatory influence of central nervous system (CNS), in which various neurotransmitter and neuromodulator systems take part. However, a possible role of central GABA-benzodiazepine system on the central regulation of renal function has not been explored. This study was undertaken to delineate the renal effects of diazepam. Diazepam, a benzodiazepine agonist, administered into a lateral ventricle (icv) of the rabbit brain in doses ranging from 10 to 100 ${\mu}g/kg,$ elicited dose-related diuresis and natriuresis along with improved renal hemodynamics. However, when given intravenously, 100 ${\mu}g/kg$ diazepam did not produce any significant changes in all parameters of renal function and systemic blood pressure. Diazepam, 100 ${\mu}g/kg$ icv, transiently decreased the renal nerve activity (RNA), which recovered after 3 min. The plasma level of atrial natriuretic peptide (ANP) increased 7-fold, the peak coinciding with the natriuresis and diuresis. Muscimol, a GABAergic agonist, 1.0 ${\mu}g/kg$ given icv, elicited marked antidiuresis and antinatriuresis, accompanied by decreases in systemic blood pressure and renal hemodynamics. When icv 0.3 ${\mu}g/kg$ muscimol was given 3 min prior to 30 ${\mu}g/kg$ of diazepam icv, urinary flow and Na excretion rates did not change significantly, while systemic hypotension was produced. These results indicate that icv diazepam may bring about natriuresis and diuresis by influencing the central regulation of renal function, and that the renal effects are related to the increased plasma ANP levels, not to the decreased renal nerve activity, and suggest that the effects may not be mediated by the activation of central GABAergic system.

Korean Red Ginseng Induced Cardioprotection against Myocardial Ischemia in Guinea Pig

  • This study was designed to evaluate the protective effect of Korean red ginseng (KRG) against ischemia/reperfusion (I/R) injury in isolated guinea pig heart. KRG has been shown to possess various ginsenosides, which are the major components of Panax ginseng. These components are known naturally occurring compounds with beneficial effects and free radical scavenging activity. The heart was induced to ischemia for 60 min, followed by 120 min reperfusion. The hearts were randomly allocated into five groups (n=8 for each group): normal control (N/C), KRG control, I/R control, 250 mg/kg KRG group and 500 mg/kg KRG group. KRG significantly increased hemodynamics parameters such as aortic flow, coronary flow and cardiac output. Moreover, KRG significantly increased left ventricular systolic pressure (LVSP), the maximal rate of contraction (+dP/$dt_{max}$) and maximal rate of relaxation (-dP/$dt_{max}$). Also, treatment of KRG ameliorated electrocardiographic index such as the QRS, QT and RR intervals. Moreover, KRG significantly suppressed the lactate dehydrogenase, creatine kinase-MB fraction and cardiac troponin I and ameliorated the oxidative stress markers such as malondialdehyde and glutathione. KRG was standardized through ultra performance liquid chromatograph analysis for its major ginsenosides. Taken together, KRG has been shown to prevent cardiac injury by normalizing the biochemical and oxidative stress.

점탄성 유체해석용 RANS 기반 난류 모델 개발 및 검증 (Development and Evaluation of RANS based Turbulence Model for Viscoelastic Fluid)

  • 대동맥이나 협착된 경동맥에서는 심장수축기에 간헐적으로 난류현상이 발생하고 있으며, 혈액의 점성특성으로 인해 기존 난류모델로는 정확한 해석이 어려운 실정이다. 혈류는 점탄성 유체의 성질을 가지고 있어 유체의 전단 변형률 증가에 따라 점도가 감소하는 점탄성 유체이며, 이러한 점탄성 유체는 난류 유동시 저항 감소 현상이 발생한다. 기존의 난류해석 모델들은 점성변화가 없는 뉴턴 유체에 적합한 모델들이 대부분이기 때문에, 점탄성 유체의 저항 감소 현상을 고려한 비뉴턴 유체 해석에 적합한 난류 모델개발이 필요하다. 본 논문은 난류 모델 가운데 수렴성이 좋고 해석시간이 짧은 표준 $k-{\varepsilon}$ 모델을 기반으로 저항 완충 함수를 이용하여 비뉴턴 유체의 저항감소 현상을 해석할 수 있는 수정된 난류모델을 제시하였으며, 이를 기존 난류모델들과 비교하여 제시된 난류 모델을 검증하였다. 새로 제시된 수정된 난류모델은 벽함수 및 점성저층을 고려하지 않았기 때문에 해석시간이 대폭적으로 감소하였으며, 적은 격자수를 이용하여 효율적으로 비뉴턴 유체의 난류 현상을 해석할 수 있기 때문에 향후 혈류해석 및 점탄성유체 해석에 적용할 예정이다.

Monitoring Differences in Vaginal Hemodynamic and Temperature Response for Sexual Arousal by Different Anesthetic Agents Using an O ptical Probe

  • The selection of anesthetic agent is important in preclinical studies, since each agent affects the systemic hemodynamics in different ways. For that reason, we hypothesized that different anesthetic agents will result in different vaginal hemodynamic response and temperature during sexual arousal, in an animal model. To validate the hypothesis, animal experiments were performed using female rats with two anesthetic agents widely used in preclinical studies: ketamine and isoflurane. Our previously developed near-infrared-spectroscopy-based probe was used to measure the changes of oxyhemoglobin (OHb), deoxyhemoglobin (RHb), and total hemoglobin (THb) concentrations along with temperature from the animal vaginal wall. As a control, saline was administered to both isoflurane- and ketamine-anesthetized animals, and did not show any significant changes in OHb, RHb, THb, or temperature. However, an administration of apomorphine (APO, 80 ㎍/kg) induced increases of OHb (63 ± 28 μM/DPF), RHb (35 ± 20 μM/DPF), and THb (98 ± 49 μM/DPF) in ketamine-anesthetized animals, while decreases of OHb (52 ± 76 μM/DPF) and THb (38 ± 30 μM/DPF) and an increase of RHb (28 ± 51 μM/DPF) were found in isoflurane-anesthetized animals. The vaginal temperature decreased from the baseline in both ketamine-(0.42℃) and isoflurane-(1.22℃)anesthetized animals. These results confirmed our hypothesis, and suggest that a preclinical study monitoring hemodynamic responses under anesthesia should employ an appropriate anesthetic agent for the study.

30분의 수술전 가온이 고관절 전치환술 노인 환자의 수술중 심부체온, 수축기압, 심박동수, 수술후 전율 및 염증반응에 미치는 효과 (The Effects of 30-Minutes of Pre-Warming on Core Body Temperature, Systolic Blood Pressure, Heart Rate, Postoperative Shivering, and Inflammation Response in Elderly Patients with Total Hip Replacement under Spinal Anesthesia: A Randomized Double-blind Controlled Trial)

  • Purpose: This study was designed to determine the effects of pre-warming on core body temperature (CBT) and hemodynamics from the induction of spinal anesthesia until 30 min postoperatively in surgical patients who undergo total hip replacement under spinal anesthesia. Our goal was to assess postoperative shivering and inflammatory response. Methods: Sixty-two surgical patients were recruited by informed notice. Data for this study were collected at a 1,300-bed university hospital in Incheon, South Korea from January 15 through November 15, 2013. Data on CBT, systemic blood pressure (SBP), and heart rate were measured from arrival in the pre-anesthesia room to 3 hours after the induction of spinal anesthesia. Shivering was measured for 30 minutes post-operatively. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were measured pre-operatively, and 1 and 2 days postoperatively. The 62 patients were randomly allocated to an experimental group (EG), which underwent pre-warming for 30 minutes, or a control group (CG), which did not undergo pre-warming. Results: Analysis of CBT from induction of spinal anesthesia to 3 hours after induction revealed significant interaction between group and time (F=3.85, p=.008). In addition, the incidence of shivering in the EG was lower than that in the CG ($x^2=6.15$, p=.013). However, analyses of SBP, heart rate, CRP, and ESR did not reveal significant interaction between time and group. Conclusion: Pre-warming for 30 minutes is effective in increasing CBT 2 and 3 hours after induction of spinal anesthesia. In addition, pre-warming is effective in decreasing post-operative shivering.
