Muscle Strength and Biochemical Markers as Predictors of Depression in Hemodialysis Patients: A Cross-Sectional Study

  • Patients with chronic renal failure, many of which treated with hemodialysis, present a high prevalence of impaired muscle strength which suggest that muscle mass parameters may be used as markers for changes in muscle in these patients. Measurement of handgrip strength (HGS) is a common, simple, and quick measure of muscle function an indicator of overall muscle strength which has been associated with physical activity and several anthropometric traits. Intercellular adhesion molecule-1 (ICAM-1) and insulin-like growth factor-1 (IGF-1) are biochemical markers associated with inflammatory processes which are a common consequence of dialysis. Additionally, hemodialysis patients frequently present signs of malnutrition and depression. This cross-sectional study aimed to evaluate if muscle and biochemical markers could be used to predict the risk of depression in hemodialysis patients. Several anthropometric parameters, nutrient intake, depression state and the serum levels of ICAM-1 and IGF-1 were determined and Pearson's correlation coefficient and/or Spearman's correlation coefficient were used to test the correlation between them. Our results do not show a correlation between HGF, IGF-1 and ICAM-1 with the depression status of the patients, but mid-arm muscle circumference (MAMC) was statistically and positively correlated with depression. Additionally, ICAM-1 levels were negatively correlated with HGS, MAMC, and IGF-1. Overall, the results of the present study suggest that HGS may be used as an indicator of cardiovascular diseases and MAMC may be a good predictor of the level of depression in hemodialysis patients, although further studies are required.

  • Purpose: The purpose of this study was to explore the meaning of the experience of adapting to the dietary treatment for hemodialysis patients. Methods: The data were collected through in-depth interviews of ten hemodialysis patients who have been dialyzed for longer than five months. Giorgi's phenomenological method was used for data analysis. Results: The findings included five constituents and 16 sub-constituents. The five constituents concerning diet therapy were: 'suffering', 'undergoing trial and error', 'understanding the need', 'finding alternatives', and 'realizing the meaning of dietary therapy.' Conclusion: The findings contribute to understanding the process of adapting to diet therapy. The results of this study may be utilized in assisting patients to understand the diet therapy process.

Management of hepatitis C viral infection in chronic kidney disease patients on hemodialysis in the era of direct-acting antivirals

  • The advent of novel, direct-acting antiviral (DAA) regimens for hepatitis C virus (HCV) infection has revolutionized its treatment by producing a sustained virologic response of more than 95% with few side effects and no comorbidities in the general population. Until recently, ideal DAA regimens have not been available to patients with severe renal impairment and end-stage renal disease because there are limited data on the pharmacokinetics, safety, and efficacy of treatment in this unique population. In a hemodialysis context, identifying patients in need of treatment and preventing HCV transmission may also be a matter of concern. Recently published studies suggest that a combination of paritaprevir/ritonavir/ombitasvir and dasabuvir, elbasvir/grazoprevir, or glecaprevir/pibrentasvir successfully treats HCV infection in chronic kidney disease stage 4 or 5 patients with or without hemodialysis.

만성 신부전 환자가 지각한 사회적지지, 자아존중감 및 절망감의 관계 연구 (A Study on the Relationship of Perceived Social Support on Self-esteem and Hopelessness in Patients with Chronic Renal Failur)

  • This study was designed to identify the relation-ship of perceived social support on self-esteem and hopelessness in patients with chronic renal failure who are hemodialysis. The subjects of this study were the 50 patients who were registered in the hemodialysis department of the two hospitals. The data were collected using a questionnaire and The period of the data collection was from August 9 to 16, 1993. The instruments for this study were the perceived social support scale designed by Park Ji-Won, the self -esteem scale designed by Rosenberg and the hopelessness scale designed by Beck et al. The data were analyzed using frequencies, percentages, t-test, ANOVA, and .Pearson correlation coefficient. The results are summarized as follows ; 1. The degree of support according to the type of perceived social support ranged down in the fol-lowing order from high to low the mean emotion-al support 21.12, the mean informational support 19.58, the mean appraisal support 17.00, the mean material support 15.22, the man self-esteem was 32.00 and the mean hopelessness was 60.48. 2. Test for hypothesis ; Hypothesis 1, "The higher the level of perceived social support in patients on hemodialysis, the higher their level of self-esteem will be. " was not supported(r=.05, p=0.74). Hypothesis 2, "The higher the level of perceived social support in patients on hemodialysis, the lower their level of hopelessness will be. " was supported(r=-0.53, p=0.00). 3. The relations between general characteristics and the level of perceived social support, self-esteem, and hopelessness ; (1) The 'Gender'(P=0.04), 'Occupation'(P=0.04), 'Education'(P=0.00), 'Married state'(P=0.00) 'Duration of Hemodialysis'(P=0.00) and 'In-come'(P=0.00) of the subjects were related to perceived social support and showed a statistically significant difference. (2) No general characteristics of the subjects were related self-esteem in a statistically significant way. (3) The 'Education'(P=0.00) , 'Income'(P=0.00) of the subjects were related to hopelessness and showed a statistically significant difference. Thus, it is concluded that social support must be included in nursing interventions for patients with chronic renal failure on hemodialysis.

개별교육프로그램이 역할불이행 혈액투석환자의 투석관련 지식, 이행 및 생리적 지표에 미치는 효과 (Effects of an Individualized Educational Program on Knowledge, Compliance and Physiologic Parameters in Non-Compliant Hemodialysis Patients)

  • Purpose: This study was done to provide effective nursing interventions using an individualized educational program designed to contribute to the knowledge, compliance and physiologic parameters (serum potassium, phosphorus and interdialytic weight gain) of non-compliant hemodialysis patients. Methods: There were 22 participants in the experimental group and 19 in the control group. Nurses with rapport with patients provided education to non-compliant hemodialysis patients for 20 minutes three times a week for six weeks. This education program consisted of individual consulting, telephones conversations regarding hemodialysis, and requests for patient support from patients' families and colleagues. Results: Hypothesis 1; "Knowledge about hemodialysis in the experimental group will be higher than the control. group" was not supported. Hypothesis 2; "Compliance will be higher in the experimental group than in the control group", and Hypothesis 3; "Physiologic parameters of experimental group will improve" were supported as serum potassium and interdialytic weight gain decreased, but results were not consistent for phosphorous. Conclusion: This program for non-compliant patients increased compliance and improved physiologic parameters. Therefore, this educational program should be effective as a nursing intervention.

운동요법이 혈액투석 환자의 체력과 건강관련 삶의 질에 미치는 효과 (Effects of Exercise Intervention on Physical Fitness and Health-relalted Quality of Life in Hemodialysis Patients)

  • Purpose: The purpose of this study was to investigate the effect of stretching, muscle strengthening, and walking exercise on the cardiopulmonary function and health-related quality of life in hemodialysis patients. Methods: Twenty-one patients in the intervention and the control group participated in the exercise respectively on maintenance hemodialysis at four university hospitals. The exercise was composed of 20 to 60 min per session, 3 sessions a week for 12 weeks. The effect of exercise was assessed by cardiopulmonary function (peak oxygen uptake, peak ventilation, peak respiration rate, maximal heart rate, and exercise duration) using a cycle ergometer. Grip strength was measured by dynamometer, and flexibility was measured by sit and reach measuring instrument. Health-related quality of life was measured using Medical Outcomes Study Short Form-36. Results: Peak oxygen uptake, peak ventilation, peak respiration rate, exercise duration, grip strength, flexibility, and physical component scale were significantly improved in the intervention group after 12 week's exercise compared to the control group. Conclusion: These findings indicate the exercise can improve cardiopulmonary function, grip strength, flexibility, and physical component scale of health-related quality of life in hemodialysis patients.

구조화된 상지운동이 혈액투석 환자의 동정맥루 협착에 미치는 효과 (Effects of Structured Arm Exercise on Arteriovenous Fistula Stenosis in Hemodialysis Patient)

  • Purpose: The purpose of this research was to develop and prove the effectiveness of structured arm exercise, which was used to reduce Arteriovenous Fistula (AVF) and Arteriovenous graft (AVG) stricture of hemodialysis patients. Methods: Quasi-experimental research design with non-equivalent control group was applied. 26 Subjects were participated in this study. 12 of hemodialysis patients who do not have a normal range of Static Intra Access Pressure Vein (SIAPV) score in the last three months were assigned to the experimental group and 14 patients who have a normal range of SIAPV score in the last three months to the control group. To analyze the collecting data after structured arm exercise, non parametric method with the repeated measures ANOVA by the Friedman test and Wilcoxon Signed Ranks Test for post-hoc test was performed. Results: Unlike the experimental group after three months, the control group's SIAPV data went over the normal range. The experimental AVF group showed a difference in data after month 2 and month 3. - In AVG group, there were clear differences in each month of the test. Conclusion: This study proved that structured arm exercise therapy could be a simple and effective intervention. It is suggested to be actively utilized for hemodialysis patients.

경혈지압이 혈액투석 환자의 소양증 및 수면에 미치는 효과 (Effects of Acupressure on Pruritus and Sleep in Patients on Hemodialysis)

  • Purpose: This study was done to investigate the effects of acupressure on pruritus and sleep among patients undergoing hemodialysis. Methods: The study design was a nonequivalent control group repeat measures quasiexperimental study. Data were collected from June to September 2015 in the kidney center at a hospital located in B city. Participants were 42 patients; 20 in the experimental group and 22 in the control group. For the experimental group, acupressure on the spots of Sameumgyo (SP6), Zoksamli (ST36), Hyolhae (SP10), and Gokji (LI11) was provided at the time of hemodialysis three times a week for 12 weeks. The numeric rating scale (NRS) was used to measure pruritus, and sleep was measured with the sleep scale developed by Jinju Oh et al. Results: There was a significant decrease in pruritus (F=7.80, p=.008) and increase in sleep (F=80.90, p<.001) in the experimental group compared to the control group. Conclusion: The findings confirmed in the study show that acupressure is an effective nursing intervention to decrease pruritus felt by patients on hemodialysis and to increase the quality of their sleep.

Two Cases of Spinal Epidural Abscess in Hemodialysis Patients

  • While epidural abscesses are rare in hemodialysis patients, they can cause severe neurological complications that can be fatal because only nonspecific symptoms appear in the early stages of the infection. Their incidence increased recently due to intravenous drug abuse, invasive spinal surgery, percutaneous vertebral procedures, and the development of diagnostic modalities. The increased number of cases is related to the use of dialysis catheters in hemodialysis patients. If a patient has fever and back pain, doctors should eliminate the possibility of other common diseases and consider spinal epidural infection. Early diagnosis and proper treatment are important to prevent neurological complications. In this paper, the symptoms, blood work, magnetic resonanceimaging (MRI) findings, and clinical course of two hemodialysis patients who developed spinal epidural abscesses are described.

혈액투석 환자가 지각하는 불확실성과 사회적 지지가 안녕감에 미치는 영향 (The Influence of Uncertainty and Social Support on General Well-being among Hemodialysis Patients)

  • Purpose: The purpose of this study was to explore factors affecting uncertainty and general well-being based on Uncertainty in Illness Theory. Methods: Data were collected from 125 outpatients who had received hemodialysis. The path model among four concepts, such as period of hemodialysis, social support, uncertainty, and general well-being, was tested. Tangible support, positive social interaction, affectionate, and emotional/informational support were measured as social support. Adaptation in the model was operationalized as general well-being which consisted of anxiety, depression, positive well-being, self-control, and general health. Results: All paths were statistically significant at the level of ${\alpha}$=.05. The significant paths were the path from period of hemodialysis to uncertainty (t=-2.86), social support to uncertainty (t=-2.01), uncertainty to general wellbeing (t=-2.85), and social support to general well-being (t=3.55). Conclusion: Patients who perceived low uncertainty and high social support were likely to feel well-being. Therefore, nurses should give patients appropriate information according to their needs and have meaningful interaction with patients to reduce their uncertainty and render social support.
