노력성 호흡운동을 통한 편마비환자의 기능적 보행지수 개선 (The improve of hemiplegic patients functional ambulation profile by forceful respiratory exercise)

  • The purpose of this study was to evaluate the change of functional ambulation profile(FAP) and temporal-spatial gait parameters in hemiplegic patient by forceful respiratory exercise. 28 Hemiplegic patients due to stroke was randomized in 3 groups, forceful expiratory training(FET), forceful inspiratory training(FIT) and control group. In the experimental groups, ordinary physical therapy with forceful expiratory training and forceful inspiratory training for 20 minutes duration 3 times per week for 6 weeks were respectively performed. In the control group, only ordinary physical therapy was done. FAP and temporal-spatial gait parameters was measured at before and after experiments. The results of this experimental study were as follows : 1. In comparison of FAP before and after experiment, the FAP was significantly increased in the FET and FIT group (p<.01). In comparison of difference of FAP among 3 groups, there was the significant difference between the FIT group and the control group (p<.05). 2. The results of temporal-spatial gait parameters are as follows : 1) In comparison of gait velocity before and after experiment, the gait velocity was significantly increased in the FET and FIT group (p<.05). In comparison of difference of the gait velocity among 3 groups, there was the significantly difference between the FIT group and the control group (p<.05). 2) In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in FIT group (p<.05). In comparison of the difference of the gait cadence among 3 groups, there was no significant difference between the FIT group and the control group (p>.05). Based on these results, it is concluded that the forced respiratory exercise program for 6 weeks can be improve the FAP and temporal-spatial gait parameters in hemiplegic patients. Therefore, the forced respiratory exercise is useful to improve the walking ability in hemiplegic patients.

편마비 환자가 지각한 가족지지와 불안과의 관계 연구 (A Study on Perceived Family Support and Anxiety in Hemiplegic Patients)

  • The purposes of this study were to identify causal factors of Anxiety and to analyze correlation between perceived family support and anxiety in hemiplegic patients. The subjects of this study were 88 hospitalized hemiplegic patients at Kyung - Hee Oriental medicine Hospital. Data were collected by using interview with questionaire from Jan. 15 to Mar, 31, 1990. The measurement tools used by this researcher were Kang's family support scale, Spielberger's trait anxiety scale and the other anxiety scale which was developed by this researcher, approved it's reliability and validity. For the purposes of the study, the collected data were analyzed by frequency, t-test, ANOVA and the hypothesis was tested by pearson correlation, partial correlation. The results of the study were as follow. 1) The analysis of causal factor of anxiety higher anxiety these three cases ; \circled1 1st causal factor was the delayed recovery of paralyzed upper limbs(2.42$\pm$1.27). \circled2 End causal factor was the dysfunction of physical ability(2.30$\pm$1.29). \circled3 3rd causal factor was difficulty for walking(2.30$\pm$0.83) and the anxiety level(2.02$\pm$0.83) of physical factor was the highest level than any other factors. 2) The hypothesis that the more perceived family support level is the less anxiety level of patient was supported(r=-.29, p=.003). 3) The analysis of the general characteristics exerting influences on anxiety level patients ; (sex, age, marrital status, religious, education level, occupation, economic status, experience of hospitalization, care giver, the period of hospitalizatiot side of paralysis). In this analysis, any factor has not a statistical significance.(p>.05). 4) The analysis of the relationship about effective factors of family support level by the general characteristics of patient (sex, age, marrital status, religious, education level, occupation, economic status, experience of hospitalization, care giver, the period of hospitalization, side of paralysis). In this analysis, there was significant difference on perceived family support between married and bereaved patients(t=-2.68, p= .009) As a result of this study, anxiety level of physical factor was higher than any other factors (psychological factor, social factor) and the delayed recovery of paralyzed upper limbs is the largest causal factor of anxiety of physical factor. Meanwhile, the relationship between the degree of family support and the level of anxiety was negatively correlated but the degree of relationship was low. Therefore, one can infer from this study that sufficient information about recovery of physical problems and family support were effective in preventing and reducing anxiety in hemiplegic patient.

Bioelectrical Impedance Analysis on the Paretic and Non-paretic Regions of Severe and Mild Hemiplegic Stroke Patients

  • For many stroke patients undergoing rehabilitation therapy, there is a need for indicator for evaluating the body function in paretic and non-paretic regions of stroke patients quantitatively. In this paper, the function of muscles and cells in paretic and non-paretic regions of severe and mild hemiplegic stroke patients was evaluated using multi-channel bioelectrical impedance spectroscopy. The paretic and non-paretic regions of severe and mild stroke patients were quantitatively assessed by using bioelectrical impedance parameters such as prediction marker (PM), phase angle (${\theta}$), characteristic frequency ($f_c$), and bioelectrical impedance vector analysis (BIVA). The mean values of impedance vector were significantly discriminated in all comparisons (severe-paretic, severe-non-paretic, mild-paretic, and mild-non-paretic). The bioelectrical impedance parameters were proved to be a very valuable tool for quantitatively evaluating the paretic and non-paretic regions of hemiplegic stroke patients.

편마비 환자의 팔걸이 착용이 하지 근활성에 미치는 효과 (The effects of wearing single strap hemisling of hemiplegic patient on lower extremity muscle activity)

  • Purpose : The purpose of this study was to investigate muscle activity of affected lower extremity by unaffected lower extremity exercise and tried to examine muscle activity of affected lower extremity of hemiplegic patients were caused by stroke according to wearing single strap hemisling or non-wearing. Methods : We measured muscle activity of affected lower extremity when wearing single strap hemisling to affected lower extremity or non-wearing according to unaffected lower extremity used MP 150 Eletromyogram. Results : 1) Muscle activity of gluteus medius muscle was the highest D1 before wearing single strap hemisling. 2) Muscle activity of hamstring muscle was the highest D3 after wearing single strap hemisling. 3) Muscle activity of quadriceps muscle was the highest D5 after wearing single strap hemisling. 4) Muscle activity of tibialis anterior muscle was the highest D1 after wearing single strap hemisling. 5) Muscle activity of soleus muscle was the highest D2 before wearing single strap hemisling. Conclusion : Wearing single strap hemisling of hemiplegic patients had effect on muscle activity of gluteus medius but didn't effect gluteus medius, hamstring, quadriceps, tibialis anterior muscle and soleus muscle.

만성 뇌졸중 환자에게 측방 보행 훈련이 균형과 보행에 미치는 영향 (Effects on Balance and Gait for Chronic Stroke Patients with Side Walking Training)

  • Purpose : The purpose of this study is to investigate the impact on the ability to walk, balance after side walking training of hemiplegic patients caused by stroke. Method : The subjects were training before stroke onset whether more than one year elapsed 15 patients with chronic stroke patients, and Berg balance scale(BBS) and Timed up and go test(TUG), Functional reaching test(FRT), 20m walking time 200m walking time were measured and recorded. Training period, a total of three weeks, and training frequency circuit training times 10 minutes per training, 5-minute break, the 10-minute training total 25-minute training was conducted. Gait line of 3m to be based on the patient's side walking, and the risk of falling compared to the presence of the experimenter trained under was carried out. Result : 1. TUG, 2. 20m walking time, 3. 200m walking time 4. FRT, 5. All showed significant improvement in BBS. Judging from the results, the side walking training conducted three weeks due to chronic stroke hemiplegic patient's ability to balance and showed a positive effect on the improvement of walking ability. Conclusion : Accordingly, it was more effective to train hemiplegic patients with chronic stroke on side walking.

Effect of Visual Biofeedback Training in Real Time on Buttock Pressure and Pelvic Tilting Angles of Hemiplegic Patients During Sitting

  • Background: After a stroke, the control of the trunk muscle may be severely impaired. Due to the importance of trunk control in complex daily postures, the ability to adopt a correct sitting posture is considered a determinant of the recovery of independent function after a stroke. Objects: The purposes of this study were to compare differences in buttock pressure between the left and right sides of hemiplegic patients and differences in their pelvic tilting angles (sagittal and coronal planes) after sitting training with visual biofeedback (VBF) in real time. Methods: Twenty-two individuals with unilateral strokes (11 left-side and 11 right-side hemiplegic stroke patients) participated in this study. Buttock pressure was measured using a pressure mat, and pelvic angles were measured using a palpation meter. Results: The asymmetry of pressure between the right and left (first and third chamber) sides was significantly decreased after the VBF training. The measurements obtained using the palpation meter revealed a significant decrease in the pelvic angles pre- versus post-intervention. Conclusion: VBF training may be distribute a patient's buttock pressure equally while in a sitting posture and increase the length of time a stroke patient can maintain a symmetrical sitting posture. It can also improve pelvic control while sitting in a neutral position.

편마비 환자의 밀기증후군 발생빈도와 신경정신학적 증상과의 관계 (Correlation of Neuropsychological Symptoms and Incidence for Hemiplegia with Pusher Syndrome)

  • The objectives of this study was to investigate the incidence and any correction between hemiplegia with pusher syndrome and neuropsychological symptoms such as hemineglect and anosognosia. Pusher syndrome defines that the patient leans toward the hemiplegic side regardless of the position that patient was placed on and resists any attempt for passive correction of posture that would move his weight toward the midline of the body. The subjects of this study were 69 acute hemiplegia who had been rehabilitated at department of rehabilitation medicine, Asan Medical Center from May 1. 1999 through July 31. 1999. The data were analyzed by researchers who were trained for assessment of anosognosia and hemineglect. 12 subjects were excluded for confusion. The method of statistical analysis used for our study was Fisher's exact test. Results of the study disclosed 21 hemiplegia(38.6%) with pusher syndrome. In conclusion, these hemiplegic patients with pusher syndrome did not have any correlation with hemineglect and anosognosia and also had no preference of stroke side.

  • This study was carried out tn invstigate and compare biomechanical characteristics during free speed gait in hemiplegic patients after stroke who took therapeutic exercise by analyzing kinematic and kinetic data in the sagital plane and electromyographic data. Six patients($41\~69$ years old) and age-matched six volunteers in good health(51-61 years old) wire studied. The patients were sorted into two groups, depending on their self-speed of walking : fast speed group(3 patients) and slow speed group(3 patients). The results were as fellows. : 1. In spatio-temparal parameters, affected and unaffected side of fast group showed symetry but blew group showed asymetry of single limb support, opposite foot contact and stance phase (p<0.05). Compared with normal group, patient group showed slower velocity, shoter stride length and longer double limb support (p<0.05). 2. In the pelvic anterior tilt, patient group showed lower valued than normal group. It. In the ground reaction force-vertical force, fast group showed similar double peak gragh compared with normal group, butvslow group showed lower values without double peak (p<0.05).

경락마사지가 편마비 환자의 손부종과 손기능에 미치는 효과 (The Effect of the Meridian Massage on the Hand Edema and Function of the Hemiplegic Patient)

  • Purpose: The objective of the study was to identify the effects of the meridian massage therapy on hand edema and hand functions in patients with hemiplegia. Method: The experiment was conducted in the Oriental Medical ward of "K" hospital during the period of 2000. 12. 15 - 2001. 03. 15. Fifty five subjects with hemiplegia following a stroke participated in the study. Volumetric size of the hand and the circumference of the index finger of each patient was measured and functional points were recorded for pre and post experiment. Result: 1. Apparent decrease in volumetric size of the hand and the circumference of the index finger(p=.022. p=.021), and higher functional points(p=.003) in the experimental group in comparison to the control group. 2.Volume of hands & their functions appeared to be irrelevant(r=-.195, p=.154). 3. Circumference of the index finger & their functions also turned out to be irrelevant (r=-.110, p=.424). Conclusion: Meridian massage is an effective nursing interventional therapy to relieve hand edema and improve hand functions in hemiplegic patients and has great potential for use in a wide range of medical fields as an efficient supplementary treatment for stroke rehabilitation.

체중이동 훈련을 통한 관절가동화기법이 편마비환자의 균형 및 보행속도에 미치는 영향 (The Effect of Weight Shift Training With Joint Mobilization on Balance and Gait Velocity of Hemiplegic Patients)

  • The purpose of this study was to determine the effects of weight shift training with joint mobilization on the ankle joint passive range of motion (PROM), balance capacity and gait velocity in hemiplegic patients. Fourteen subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with seven subjects in each group. The EG received weight shift training with joint mobilization in the paretic leg's subtalar joint in order to increase ankle dorsiflexion. The CG received general physical therapy training. Both groups received training five times a week over a period of two consecutive weeks. The figures for PROM of ankle dorsiflexion on the paretic leg, the functional reach test (FRT), the timed up and go (TUG) test, and gait velocity were recorded both before and after the training sessions for both groups. The EG's results in gait velocity, the FRT and the TUG test improved after training (p<.05). The PROM of ankle dorsiflexion improved both in the EG and the CG (p<.05), the EG demonstrated a significantly higher increase (p<.05) than that of the CG. The results of this study suggest that increased joint mobilization positively affects balance and gait velocity of hemiplegic patients. Further studies with a greater sample size are necessary in order further prove the accuracy of the results of this study.
