Helicobacter pylori에 감염된 위상피세포에서 Skp2의 변화 (Changes in Skp2 in Helicobacter pylori-Infected Gastric Epithelial Cells)

  • It has been suggested that Helicobacter pylori(H. pylori) infections can promote the development and progression of gastric cancer through the modulation of cell cycle regulators such as $p27^{Kip1}$ and Skp2. $p27^{Kip1}$ is a cyclin-dependent kinase (CDK) inhibitor that blocks the G1/S transition necessary for cell cycle progression. Skp2 is a component of the ubiquitin ligase complex called $SCF^{Skp2}$(SKP1-Cullin-F-box), which specifically binds and promotes the degradation of $p27^{Kip1}$. A low level of $p27^{Kip1}$ and a high level of Skp2 have been reported in many types of cancers, including gastric cancer. In addition, a decrease in $p27^{Kip1}$ has been reported in H. pylori-infected specimens. However, data on Skp2 in H. pylori infections are limited. This study examines the changes in the status of Skp2 in H. pylori-infected gastric epithelial AGS cells. For this, we stimulated AGS cells with H. pylori(NCTC 11637) at the ratio of 300:1(bacterium:cell) for 6 hours. The results of an immunoprecipitation analysis, followed by a western blot, indicate that the interaction between Skp2 and 14-3-3 was elevated 3 hours after the H. pylori treatment. In addition, there was an increase in cytoplasmic Skp2 after 3 hours, whereas there was no change in the nuclear level. Since it has been reported that interaction with 14-3-3 and the subsequent cytoplasmic translocation of Skp2 can increase its protein stability, increases in the interaction with 14-3-3 and the cytoplasmic Skp2 after the H. pylori treatment can increase the level of Skp2 in AGS cells. This phenomenon may explain, at least to some extent, the mechanism underlying the relationship between H. pylori infections and gastric carcinogenesis.

개의 위내 Helicobacter 균속 감염에 대한 삼중요법의 효과 (Effect of Triple Therapy on Eradication of Gastric Helicobacter Species Infection in Dogs)

  • 위내 Helicobacter 균속에 자연 감염된 개에서 삼중요법의 효과를 알아보기 위해 Helicobacter 균속에 자연 감염된 것으로 판명된 7두의 개를 대상으로 amoxicillin, metronidazole을 14일간 경구투여하였다. 삼중요법 실시에 따른 Helicobacter 균속 감염상태 변화를 평가하기 위해 투여 7일째, 14일째 그리고 투여 중지후 30일째의 위생검 조직에 대한 요소검사, Helicobacter 균속에 대한 중합효소 연쇄반응 검사와 함께 분변시료를 이용한 중합효소 연쇄반응 검사를 실시하였다. 삼중요법 14일째 위생검 조직의 요소검사에서는 7두중 6두가 음성이었으나 위생검조직과 분변시료의 중합효소 연쇄반응 검사에서는 각각 3두와 4두만이 음성이었다. 삼중요법 중지후 30일째 검사에서는 각 검사법이 모두 동일한 결과를 나타내었는데 7두중 3두에서 음성으로 나타났다. 이상의 결과를 바탕으로 위내 Helicobacter 균속에 자연 감염된 개에서 amoxicillin, metronidazole과 omeprazole을 이용한 삼중요법은 Helicobacter 균속 감염 정도를 낮추거나 때로는 완전 박멸할 수 있을 것으로 보여지기에 만성위염의 임상증상을 나타내는 개의 Helicobacter 균속 감염증에 적용할 수 있으리라 사료된다.

Helicobacter pylori의 한약 치료에 대한 연구 (A Study of Oriental Herbal Medicine on the Treatment of Helicobacter Pylori Infections)

  • Objectives : Many studies have shown that helicobacter pylori (H. pylori) infection is associated with gastroduodenal diseases. The purpose of this report was to evaluate recently published research on the influence of oriental herbal medicine on H. pylori infections. Methods : Recently published literature were systematically compared with their findings of how oriental herbal medical treatment affects H. pylori-associated disease. Results : The eradication rate of H. pylori in oriental herbal medicine groups was 66.93% while it was 66.02% in western medicine groups. In oriental herbal plus western medicine groups, interestingly, the rate increased to 84.78%. On the other hand, the total treatment efficacy rate of H. pylori in oriental herbal medicine groups was 91.27%. The treatment efficacy rate in oriental herbal plus western medicine groups rose to a record 93.22%, which was 15.34% higher than the rate in western medicine groups. In addition, the rate of adverse effects was 2.71%, 4.85%, 15.80% in oriental herbal medicine, western medicine, and oriental herbal plus western medicine groups, respectively. Diarrhea was most frequently observed in oriental herbal medicine groups, while nausea was most frequently observed in the other groups. Conclusions : The results of this study showed that herbal medicinal treatment can increase the rate of H. pylori eradication and improve H. pylori-related gastrointestinal symptoms. These findings suggest that herbal medicine can solve the problems including side effects due to antibiotic resistance of standard triple therapy.

  • Accurate diagnosis of Helicobacter pylori (H. pylori) infection is mandatory for the effective management of many gastroduodenal diseases. Currently, various diagnostic methods are available for detecting these infections, and the choice of method should take into account the clinical condition, accessibility, advantage, disadvantage, as well as cost-effectiveness. The diagnostic methods are divided into invasive (endoscopic-based) and non-invasive methods. Non-invasive methods included urea breath test, stool antigen test, serology, and molecular methods. Invasive methods included endoscopic imaging, rapid urease test, histology, culture, and molecular methods. In this article, we provide a review of the currently available options and recent advances of various diagnostic methods.

Phenotypic Differences of Gastric Cancer according to the Helicobacter pylori Infection in Kor ean Patients

  • Purpose: Infection with Helicobacter pylori is an important risk factor for gastric cancer in humans. We compared the clinicopathologic features of gastric cancer patients based on H. pylori infection. Materials and Methods: We prospectively studied 155 patients who had gastric cancer and underwent gastrectomies in 1 hospital in Korea. We examined H. pylori infections using the rapid urease test (RUT) with gastrectomy specimens and collected clinical and pathologic data. Results: The number of H. pylori infections based on the RUT was 137 (88%). The H. pylori-negative group was significantly associated with AGC and tumor histology. H. pylori infection was significantly correlated with type I/IIa in EGC and type III/IV/V in AGC. AGC was significantly correlated with larger tumor size, lymphatic invasion, perineural invasion, and H. pylori infection based on univariate and multivariate analyses. Conclusions: We report the prevalence of H. pylori based on the RUT in gastric cancer patients. H. pylori infection influences the tumor histology, progression, and growth type of gastric cancer.

구강 편평태선 환자의 타액에서 Helicobacter pylori의 검출 (Detection of Helicobacter pylori in Saliva of Patient with Oral Lichen Planus)

  • 편평태선은 피부와 점막에 발생하는 흔한 만성 염증성 질환으로 정확한 원인은 잘 알려져 있지 않으나 종종 감염과 관련 되어있다. 다양한 박테리아 중 Helicobacter pylori(H. pylori)는 위염, 위 십이지장 궤양 그리고 위암과 관련되어 있다. 위궤양과 구강 궤양들의 조직학적 특징의 유사성을 고려할 때 H. pylori는 구강 점막궤양의 발생과 관련 있음을 추론할 수 있다. 따라서 미란성 구강편평태선의 발생에 H. pylori가 관련 있는지를 조사하기위해 이 연구를 수행하였다. 미란성 구강편평태선을 가진 환자의 타액을 중합효소연쇄반응에 의해 분석한 결과 21명의 환자 중 16명(76.2%)에서 H. pylori가 검출되었고, 대조군은 44명 중 11명(25%)에서 H. pylori가 검출되어 통계적 유의성을 나타내었다(P>0.001). 이상의 결과를 종합해 볼 때 타액내 H. pylori는 미란성 구강편평태선의 발생에 원인이 될 수 있음을 추론할 수 있었다.

Co-Infection with Cytomegalovirus and Helicobacter pylori in a Child with $M\acute{e}n\acute{e}$trier's Disease

  • $M\acute{e}n\acute{e}$trier's disease is a rare protein-losing gastropathy characterized by hypertrophic gastric fold, foveolar hyperplasia, and hypoproteinemia with resulting peripheral edema. It is clinically evident as nonspecific gastrointestinal symptoms, including abdominal discomfort, nausea and vomiting, abdominal pain, weight loss, diarrhea, and edema. Pediatric $M\acute{e}n\acute{e}$trier's disease usually has an insidious onset and progressive, chronic clinical course and it spontaneously resolves in weeks or months. The pathogenesis of $M\acute{e}n\acute{e}$trier's disease is not clearly understood. $M\acute{e}n\acute{e}$trier's disease is thought to be associated with some gastric infections. But the cause of $M\acute{e}n\acute{e}$trier's disease is unknown, an association with cytomegalovirus (CMV) and Helicobacter pylori has been suggested. In Korea, We present the first a case of pediatric $M\acute{e}n\acute{e}$trier's disease with positive evidence of CMV and H. pylori.

Lack of Association Between Helicobacter pylori Infection and Oral Lichen Planus

  • Oral lichen planus is a premalignant chronic inflammatory mucosal disorder with unknown etiology. It is a multifactorial disease and in addition to genetic background, infections, stress, drug reactions are suggested as risk factors. Helicobacter pylori which is involved in development of many gastrointestinal lesions may also be implicated in oral lichen planus induction. This is of clear importance for cancer prevention and the present study was performed to determine any association between H. pylori infection and oral lichen planus in southwestern Iran. Anti H. pylori IgG levels were determined in 41 patients and 82 sex-age matched controls. The results showed no association between H. pylori infection and oral lichen planus (51% in patients vs. 66% in control). or any of its clinical presentations.

Comparative Transcriptome Analysis of Caryophyllene-Treated Helicobacter pylori

  • Helicobacter pylori (H. pylori) establishes long-term infections associated with severe gastric diseases such as peptic ulceration and gastric cancer. Exposure to an antibacterial agent can help regulate the expression levels of its pathogenic genes. In this study, we analyzed the transcriptional changes in H. pylori genes induced by β-caryophyllene. We used next-generation sequencing (NGS) to analyze RNA expression changes, and reverse transcription-polymerase chain reaction (RT-PCR) was performed as required to verify the results. The NGS results showed that 30 out of 1,632 genes were expressed differentially by β-caryophyllene treatment. Eleven genes associated with DNA replication, virulence factors, and T4SS components were significantly downregulated. RT-PCR confirmed that treatment reduced the expression levels of 11 genes. RT-PCR showed the reduced expression of 11 genes (dnaE, dnaN, holB, gyrA, cagA, vacA, secA, flgE, virB2, virB4, and virB8) following β-caryophyllene treatment. These results suggest that β-caryophyllene can modulate various H. pylori pathogenic determinants and be a potential therapeutic agent for H. pylori infection.

Mouse Models of Gastric Carcinogenesis

  • Gastric cancer is one of the most common cancers in the world. Animal models have been used to elucidate the details of the molecular mechanisms of various cancers. However, most inbred strains of mice have resistance to gastric carcinogenesis. Helicobacter infection and carcinogen treatment have been used to establish mouse models that exhibit phenotypes similar to those of human gastric cancer. A large number of transgenic and knockout mouse models of gastric cancer have been developed using genetic engineering. A combination of carcinogens and gene manipulation has been applied to facilitate development of advanced gastric cancer; however, it is rare for mouse models of gastric cancer to show aggressive, metastatic phenotypes required for preclinical studies. Here, we review current mouse models of gastric carcinogenesis and provide our perspectives on future developments in this field.
