  • The Korean Society of Heart Failure (KSHF) Guidelines provide evidence-based recommendations based on Korean and international data to guide adequate diagnosis and management of heart failure (HF). Since introduction of 2017 edition of the guidelines, management of advanced HF has considerably improved, especially with advances in mechanical circulatory support and devices. The current guidelines addressed these improvements. In addition, we have included recently updated evidence-based recommendations regarding acute HF in these guidelines. In summary, Part IV of the KSHF Guidelines covers the appropriate diagnosis and optimized management of advanced and acute HF.

Diagnosis of Persistent Right Aortic Arch with an Aberrant Left Subclavian Artery using CT in a Poodle Dog

  • A 6-month-old, female poodle presented with a three-month history of persistent regurgitation immediately after eating. On physical examination, the patient was emaciated and dehydrated. Thoracic radiography showed ventral displacement of the trachea and increased radiopacity in the mediastinum, cranial to the heart base. A severely dilated esophagus was identified cranial to the heart on esophagram. Computed tomography (CT) revealed the esophagus was filled with gas, fluid and a little of contrast and dilated from caudo-cervical to cranio-thoracic part. The esophageal diameter was markedly decreased at the heart base. In addition, the trachea was displaced to the left-ventral side of the right aortic trunk and an aberrant left subclavian artery originating from the aorta was identified. There was no evidence of abdominal vascular anomaly. Based on diagnostic imaging, persistent right aortic arch (PRAA) with an aberrant left subclavian artery was diagnosed. The patient did not undergo surgery and died at 15 days after diagnosis. This report describes imaging diagnosis, including CT and radiography in a weaned dog with regurgitation due to esophageal obstruction by PRAA. When PRAA is suspected and conventional radiography or contrast study is insufficient for diagnosis, CT may be helpful for diagnosing PRAA.

  • Recently heart attack is 80% of the sudden death of elderly. The causes of a heart attack are complex and sudden, and it is difficult to predict the onset even if prevention or medical examination is performed. Therefore, early diagnosis and proper treatment are the most important. In this paper, we show the accuracy of normal and abnormal classification with neural network using weighted fuzzy function for accurate and rapid diagnosis of myocardial infarction. The data used in the experiment was data from the UCI Machine Learning Repository, which consists of 14 features and 303 sample data. The algorithm for feature selection uses the average of weight method. Two features were selected and removed. Heart attack was classified into normal and abnormal(1-normal, 2-abnormal) using the average of weight method. The test result for the diagnosis of heart attack using a weighted fuzzy neural network showed 87.66% accuracy.

  • Two cases of situs inversus totalis with isolated congenital heart disease were treated at department of cardiothoracic surgery, Chungnam National University Hospital. Sixteen months old girl was operated under diagnosis of PDA with mirro-image dextrocardia by triple ligation. Fifteen months old girl was managed under diagnosis of VSD with mirro-image dextrocardia by simple closure of VSD with extracorporeal circulation. The former had smooth postoperative course, but the latter was died of asphyxia due to vomitus on postoperative second day

Cardiac biomarkers for early detection of heart diseases in small animals

  • Background: Heart failure can develop secondary to various heart diseases (e.g. mitral valvular insufficiency, congenital heart defects, heart worm infection). The clinical signs of heart failure can be confused with those of other conditions, such as respiratory disease. Therefore, specific, sensitive, rapid and inexpensive blood tests for heart failure are desirable. Cardiac troponins, natriuretic peptides and cytokines have been more recently used as indicators of heart disease in humans and animals. These peptides are sensitive to changes in vasoconstriction and dilation within the heart and are used for the diagnosis and prognosis of heart failure. Methods: Previously developed and newly developed cardiac biomarkers will be discussed for understating clinical implications and diagnostic values in heart diseases in small animals

Clinical Course of Suspected Diagnosis of Pulmonary Tumor Thrombotic Microangiopathy: A 10-Year Experience of Rapid Progressive Right Ventricular Failure Syndrome in Advanced Cancer Patients

  • Background and Objectives: Several cases involving severe right ventricular (RV) failure in advanced cancer patients have been found to be pulmonary tumor thrombotic microangiopathies (PTTMs). This study aimed to discover the nature of rapid RV failure syndrome with a suspected diagnosis of PTTM for better diagnosis, treatment, and prognosis prediction in clinical practice. Methods: From 2011 to 2021, all patients with clinically suspected PTTM were derived from the one tertiary cancer hospital with more than 2000 in-hospital bed. Results: A total of 28 cases of clinically suspected PTTM with one biopsy confirmed case were included. The most common cancer types were breast (9/28, 32%) and the most common tissue type was adenocarcinoma (22/26, 85%). The time interval from dyspnea New York Heart Association (NYHA) Grade 2, 3, 4 to death, thrombocytopenia to death, desaturation to death, admission to death, RV failure to death, cardiogenic shock to death were 33.5 days, 14.5 days, 7.4 days, 6.4 days, 6.1 days, 6.0 days, 3.8 days and 1.2 days, respectively. The NYHA Grade 4 to death time was 7 days longer in those who received chemotherapy (7.1 days vs. 13.8 days, p value=0.030). However, anticoagulation, vasopressors or intensive care could not change clinical course. Conclusions: Rapid RV failure syndrome with a suspected diagnosis of PTTM showed a rapid progressive course from symptom onset to death. Although chemotherapy was effective, increased life survival was negligible, and treatments other than chemotherapy did not help to improve the patient's prognosis.

  • Achondroplasia and hypochondroplasia are the two most common forms of short-limb dwarfism. They are autosomal dominant diseases that are characterized by a rhizomelic shortening of the limbs, large head with frontal bossing, hypoplasia of the mid-face, genu varum and trident hands. Mutations in the fibroblast growth factor receptor-3 (FGFR3) gene, which is located on chromosome 4p16.3, have been reported to cause achondroplasia and hypochondroplasia. More than 98% of achondroplasia cases are caused by the G380R mutation (c.1138G>A or c.1138G>C). In contrast, the N540K mutation (c.1620C>A) is detected in 60-65% of hypochondroplasia cases. Tests for common mutations are often unable to detect the mutation in patients with a clinical diagnosis of hypochondroplasia. In this study, we presented a case of familial hypochondroplasia with a rare mutation in FGFR3 identified by next generation sequencing.

Factors associated with Maternal-fetal Attachment of Expectant Mothers Whose Fetus Has a Prenatal Diagnosis of Congenital Heart Disease (선천성 심장병 태아를 임신한 임부의 모-태아 애착 및 관련 요인)

  • Purpose: The main purposes of this study were to assess maternal-fetal attachment (MFA) of the expectant mothers of a fetus with a prenatal diagnosis of congenital heart disease (CHD) and to identify factors associated with MFA. Methods: The methodology was a cross sectional survey study using a self-administered questionnaire. Thirty pregnant women carrying a fetus with a prenatal diagnosis of CHD and 30 pregnant women with a normal fetus were enrolled in this study. The MFA Scale and PPS (The Prenatal Psychosocial Profile) were used to collect data. Data were analyzed using SPSS 20.0 Window version. Descriptive statistics, $X^2$-test and t-test were used to compare the two groups. The factors associated with MFA were identified by multiple regression analysis. Results: There was no significant difference between the two groups in MFA and social support from spouse was the only variable showing a significant difference. The model from the multiple regression analysis explained 33.8% of MFA for both groups. Conclusion: MFA of expectant mothers with a prenatal diagnosis of CHD and of mothers with a normal fetus were not significantly different. It is important that health care providers encourage expectant fathers to support the expectant mothers to increase MFA.

  • Medical treatment for animals is very difficult as the opinions of pets' masters take priority over treatment regardless of the seriousness of pets' disease or the needs of medical treatment. In case that a pet has heart disease, especially, it is difficult to get the direct answer from the pet's master on the decision or confirmation of treatment. For those reasons, it is almost impossible to predict and treat the pet before an emergency like the heart failure or an unexpected death happens. Using stethoscope can be the first diagnosis method to check the heart or any kinds of disease inside the body. High-tech equipments like CT, X-ray or Ultrasound can be used, but they can only be used as a second choice of diagnosis method since it requires professional skills and its high price. That's why stethoscope is still the best diagnostic tool when one makes the first diagnosis. In this study, we give a detailed account of digital diagnosis system in which veterinarians can analyze the sound from stethoscope without bringing it to their ears and make a diagnosis wherever they are. And we suggest a new concept of diagnosis system surrounding, which shows the relativeness of disease through Level Crossing Rate(LCR) and energy level from the stethoscope sound made in this system.

A Novel Method to Estimate Heart Rate from ECG

  • Heart rate variability (HRV) in electrocardiogram (ECG) is an important index for understanding the health status of heart and the autonomic nervous system. Most HRV analysis approaches are based on the proper heart rate (HR) data. Estimation of heart rate is thus a key process in the HRV study. In this paper, we report an innovative method to estimate the heart rate. This method is mainly based on the concept of periodicity transform (PT) and instantaneous period (IP) estimate. The method presented is accordingly called the "PT-IP method." It does not require ECG R-wave detection and thus possesses robust noise-immune capability. While the noise contamination, ECG time-varying morphology, and subjects' physiological variations make the R-wave detection a difficult task, this method can help us effectively estimate HR for medical research and clinical diagnosis. The results of estimating HR from empirical ECG data verify the efficacy and reliability of the proposed method.
