Discriminant Analysis of Parameter for Cardiac Arrythmia Detection (심전도 부정맥 검출을 위한 변수의 분류 성능 평가)

  • In this paper, the discriminant analysis was performed on parameter for detection of cardiac arrythmia. The parameters used for discriminant analysis was two group. One group consist of 05 width and Heart rate, and the other Morphology and Heart Rate. For this study, we designed data acquisition system for EKG signals. The parameters pre-processed by this system was heart rate, 05 width and Morphology. And then, we analyzed the discriminancy of two group and extracted the quantity of discriminancy. The analysis results showed first that the group with morphology and heart rate is better discriminant than with 05 width and heart rate : next, that it can quantify the discriminany of each group of diseases.

A Quantitative Analysis of Nonlinearity Changes of 24 hour Heart Rate Variability of TOF Children Group and Normal Children Group (TOF 소아 집단과 정상 소아 집단의 24 시간 심박동수 변동량의 비선형성 변화에 대한 정량적 분석)

  • It has been reported that sudden cardiac death and ventricular tachycardia occur after treatment of tetralogy of fallot(TOF). It is regarded that ventricular arrythmia is the main source or the sudden cardiac death, but it is not verified. It is likely that TOF has effect on the heart rate variability because of the ventricular arrythmia. We study how complex and periodic heart rate dynamics change in the normal children (n=13) and TOF children (n=13) throughout 24 hours. We recorded 24-hour holter ECG, and segmented each ECG data into 1 hour length. We analyze each HR time series, and quantify the overall complexity of each HR time series by its correlation dimension. We also calculate the power spectrum of HR, and obtain low-frequency component (0.03-0.15Hz) and high-frequency component (0.15-0.4Hz). We compare the results between normal and TOF groups throughout 24 hours. TOF group have lower correlation dimension ($4.055{\pm}0.4134$ vs. $4.9310{\pm}0.2054$, p<0.05) than the normal group, even though there are no significant differences in the low($0.9864{\pm}0.5598$ vs. $1.5560{\pm}0.8325$, p<0.05) and high($1.1168{\pm}0.1.1448$ vs. $0.9271{\pm}0.6528$, p<0.05) frequency components. It can be concluded that HR time series of TOF group are more regular than that of normal group, and that correlation dimension reveals a nonlinear characteristics of HR time series which is not determined in the frequency domain.

  • Dilated cardiomyopathy in two dogs showing dyspnea was diagnosed at veterinary teaching hospital. In radiographs, cardiomegaly, pleural effusion, pulmonary edema, ascites, and hepatomegaly were seen at case 1. ‘Reversed D’ shape due to right heart enlargement was shown at case 2. In echocardiography, dilatation of chambers was predominant in case 1 and 2. Arrythmia was complicated in case 2.

  • This Paper presents a design of the fuzzy decision maker analyzable of output result of ECG signals. The fuzzy decision maker proposed are divided into two groups whose functions are different each other. The one rules when decision of heart rates, The other decision values for an interval of each points of waveform using of which static state values and abnormal values.   • Acontii Tuber is very toxious material, but In oriental medicine frequently used for emergency state like shock, coma, and severe pain and author study about Acontii Tuber in Bang-Yak-Hap-Pyun(方藥合編). Bang-Yak-Hap-Pyun(方藥合編) is widely used for clinical herbal prescpition book. Acontii Tuber has alkaloid. this chemical compound make toxious state like arrythmia, heart arrest, weakness, numbness, itching. And using this material must be careful. In Bang-Yak-Hap-Pyun(方藥合編) Acontii Tuber is used 1.2g ~ 12g. and it decrease pain, inflammation, increase activity of heart, and Acontii Tuber' commom use is severe pain disease. Raw Acontii Tuber(生附子) is rarely used, mostly roasted Acontii Tuber. In case of Acontii Tuber toxic state, in oriental medicine Gam-du-tang(甘豆湯) & Go-sam(苦蔘) is used, in western medicine atropine & lidocane is used.

  • Most myocardial bridgings are found incidentally without symptoms, but myocardial bridging may induce symptoms such as angina, myocardial infarction, and ventricular arrythmia. In a patient who has symptoms despite of proper medication, stent insertion, supra-arterial myotomy or coronary artery bypass grafting have been applied without a definite guideline of treatment. We report two surgical cases of myocardial bridging with a review of the literature.

  • Cardiac rhabdomyoma is a rare type of benign tumor affecting the heart. There are a few previous reports of intracardiac rhabdomyomas causing ventricular arrythmia. We describe a 1-year-old female tuberous sclerosis patient who was presented with a ventricular tachycardia. Diagnostic echocardiography revealed two masses in the left ventricular outflow tract originating from the ventricular septum. The masses were surgically resected through aortotomy using cardiopulmonary bypass and the masses appeared benign. The pathology was that of a cardiac rhabdomyomas. Postoperative course was uneventful and the ventricular tachycardia was controlled.

  • In this paper, we show that the limitations of Holter monitoring and Wearable Electrocardiogarphy Devices and their arrhythmia detection. Sudden death caused by cardiovascular disease, often referred to as the "silent killer" due to its unpredictable nature, is a major health concern. Electrocardiography (ECG) is a basic diagnostic tool for detecting heart disease, but its limitations make it difficult to detect arrhythmia, a significant indicator of an irregular heart state. To address this limitation, a long-term continuous ECG recording device has been developed, Holter ECG device and wearable device. A significant number of studies have focused on the differences between Holter monitoring and wearable devices. The Holter tests were useful for detecting regularly occurring arrhythmias, whereas wearable patches were better at detecting random and infrequent arrhythmias. Wearable patches were effective in detecting episodes of arrhythmia and myocardial ischemia. Despite the concern, wearable devices had less signal loss than Holter monitoring and patients also preferred wearable devices over Holter monitoring due to convenience. These results could mean that the wearable devices can perfectly replace the Holter test.

Design of Arrhythmia Classification System Based on 1-D Convolutional Neural Networks (1차원 합성곱 신경망에 기반한 부정맥 분류 시스템의 설계)

  • Recently, many researches have been actively to diagnose symptoms of heart disease using ECG signal, which is an electrical signal measuring heart status. In particular, the electrocardiogram signal can be used to monitor and diagnose arrhythmias that indicates an abnormal heart status. In this paper, we proposed 1-D convolutional neural network for arrhythmias classification systems. The proposed model consists of deep 11 layers which can learn to extract features and classify 5 types of arrhythmias. The simulation results over MIT-BIH arrhythmia database show that the learned neural network has more than 99% classification accuracy. It is analyzed that the more the number of convolutional kernels the network has, the more detailed characteristics of ECG signal resulted in better performance. Moreover, we implemented a practical application based on the proposed one to classify arrythmias in real-time.

Effects of Soaansintang(SOAT) on the hemodynamics and electrocardiogram of isolated rat hearts induced by electrical stimulation (소아안신탕(小兒安神湯)이 STRESS를 유발한 흰쥐의 적출심장(摘出心臟)에 미치는 영향)

  • It has long been known that SOAT is effective for sudden palpitation occurring unexpectedly in Oriental Medicine. However, effect of SOAT on the isolated heart has not been studied yet. The purpose of this study is to investigate the effect of SOAT on hemodynamics and ECG of isolated rat hearts induced by electrical stimulation using Langendorff perfusion apparatus for nonworking heart. SOAT extract was manufactured by water-alcohol precipitated method. Sprague-Dawley rats weighting $120{\sim}150g$ were used for the experiments, Subject animals were divided into four groups, which are consisted of 1) control(Group orally administered by normal saline 1ml for 14days), 2) sample A(Group orally administered by SOAT extract 1ml for 14days), 3) sample C(Group injected by SOAT extract 0.5ml after stimulation, 4) sample C(Group injected by SOAT extract 1ml after stimulation. To evluate the effects of SOAT on hemodynamics and ECG of isolated rat heart induced by stimulation, heart rate, left ventricular pressure, systolic power, diastolic power, coronary artery perfusion volume and ECG were measured using Langendorff apparatus in both stimulation mode(5 volts, 450 beats/min) and arrythmic mode(5 volts, 420 beats/min including 60 beats/min) The results obtained are as follows : 1. After receiving stressful electrical stimuli, isolated heart showed the heart rate, left ventricular pressure, systolic power, diastolic power, coronary artery perfusion volume were all decreased temporarily, but perfusion continued longer recovery to the control state appeared. However, the coronary artery perfusion volume diminished continuously. 2. The heart rates did not change significantly with both stimulation mode and arrhythmic mode, among experimental groups. 3. The left ventricular pressure showed with both stimulation mode and arrhythmic mode, the significant changes(p<0.05) especially in the injection sample group. In case of stimulation mode, low concentration injection group(0.5ml) was more significantly increased rather than high concentration group(1ml) and in case of arrhythmic mode, high density group(1ml) was so increased than the other(0.5ml). 4. For the systolic power and diastolic power, no significant changes were noticed in the stimulation mode, but in the arrhythmic mode of injection sample groups, significant change(p<0.05) was noticed in both systolic power and diastolic power. Specially the high concentration group(1ml) showed more significant increase than the low concentration group. 5. For the coronary artery perfusion volume, no significant change difference among sample groups was observed in both the stimulation mode and the arrhythmic mode. 6. For the ECG recordings, arrhythmia was induced by electrical stimulus of arrythmia mode and after the stimulus was removed, irregular wave appeared temporarily, but as perpusion continued, recovery to the control state was abtained like the stimulation mode. According to the above results, SOAT significantly changed the hemodynamic data from the electrically stressed, isolated hearts of connected Langendorff perfusion apparatus and we propose SOAT has the direct effects on the muscular function of heart.
