청각장애인 보청기사용 영향요인에 관한 연구 (Study on Factors to Influence the Hearing-impaired in Relation to Their Using Hearing Aids)

  • 본 연구는 청각장애인의 보청기사용에 영향을 미치는 요인을 탐색하기 위해 4차년도 장애인 고용패널 521명을 대상으로 분석하였다. 분석은 Andersen-Newman의 서비스 이용모델을 기반으로 선행요인, 가능성 요인, 욕구요인으로 구분하여 각각의 요인들이 청각장애인의 보청기 사용에 미치는 영향력을 살펴보기 위해 로지스틱 회귀분석(logistic regression)을 실시하였다. 분석결과 선행요인에서는 연령과 교육수준이 높을수록, 가능성 요인에서는 총소득이 낮고 장애수용정도가 높을수록, 욕구요인에서는 장애등급이 낮을수록 보청기 사용수준이 유의미하게 높아지는 것으로 나타났다. 이러한 결과를 토대로 연구의 제한점과 함의를 제안하였다.

  • 최근의 디지털 신호처리 기술과 집적 회로 설계 기술의 발달은 보청 시스템의 새로운 가능성을 제공하고 있다. 그러나, 배경 잡음은 여전히 많은 난청자가 호소하는 문제로 남아 있다. 본 논문에서는 임상 실험을 통하여 음성 대역 잡음 환경에서 감음신경성 난청자의 음성 인지 능력과 어음 변별력을 측정한 결과를 제시한다. 또한, 보다 향상된 보청 환경을 제공하기 위하여 보청 시스템의 전처리단으로써 음질 향상 기법을 이용하여 배경 잡음을 제거하였다. 음질 향상 기법은 DSP 보드를 이용하여 실시간 시스템으로 구현되었으며, 이를 이용하여 청력 검사를 실시하였다. 임상 실험을 실시한 결과, 음질 향상 기법은 배경 잡음을 제거함으로써 신호의 SNR을 개선시켜 보청 이득과 결합되어 감음신경성 난청자의 음성 변별력을 크게 향상시켰다.

Assessment of Noise Exposure and Hearing Loss Among Workers in Textile Mill (Thamine), Myanmar: A Cross-Sectional Study

  • Background: In a wide range of industries, noise-induced hearing loss remains one of the most prevalent occupational problems. This study aimed to assess the noise exposure level and associated factors of hearing loss among textile workers in Yangon Region, Myanmar. Methods: A cross-sectional study was conducted at a Textile mill (Thamine), Yangon Region, from April to December 2018. In total, 226 workers who were randomly selected from 3 weaving sections participated in face-to-face interviews using a structured questionnaire. A digital sound level meter and pure-tone audiometer were used for the assessment of noise exposure level and hearing loss, respectively. Logistic regression analysis was performed to assess the associated factors of hearing loss. Results: In total workers, 66.4% were exposed to ≥85 dB(A) of noise exposure, and the prevalence of hearing loss was 25.7%. Age ≥35 years, below high school education, hearing difficulty, tinnitus, hypertension, > 9 years of service duration in a textile mill were positively associated with hearing loss. After adjusting confounding factors, age ≥35 years (adjusted odds ratio = 6.90, 95% confidence interval = 3.45-13.82) and tinnitus (adjusted odds ratio = 2.88, 95% confidence interval = 1.13-7.37) were persistently associated with hearing loss. Conclusion: Providing occupational hazard education and enforcement of occupational safety regulations should be taken to decrease the noise exposure level. The regular audiometry test should be conducted for assessment of hearing threshold shift. The employer needs to implement a hearing conservation program in workplace when noise exposure reaches or exceeds 85 dB(A) for 8 hours.

Nationwide Analysis of Mortality Rates and Related Surgical Procedures in Hearing Disability Patients in South Korea

  • Background and Objectives: Hearing loss (HL) and its repercussions are major problems in today's society. There are limited data on the relationship between degree of HL and otologic disorders. The aim of this study is to estimate mortality rates, rates of sudden idiopathic HL and related otologic surgical procedures in hearing disability patients in South Korea. Subjects and Methods: Retrospective medical data for 160,205 patients with hearing disability was extracted. Mortality rates, rates of sudden idiopathic HL and related otologic surgical procedures were compared with a normal control group consisting of 865,475 people; approximately 5 times the number of hearing disability patients. Results: According to the Korean National Disability Registry (NDR), 0.458% of the population in South Korea suffered from hearing disability in 2015. Higher rates of mortality and sudden idiopathic HL were reported in hearing disability patients, increasing up to a maximum of 1.594 times and 1,039.695 times, respectively, compared to the normal control group. Mastoidectomy surgery was 2.5 times more frequently performed and pressure equalizing (PE) tube insertion was about 15 times more frequently performed in hearing disability patients. Conclusions: Hearing disability is related to higher risks of mortality, sudden idiopathic HL and otologic surgical procedures, including mastoidectomy and PE tubing.

측두하악장애와 청력과의 관계 (The Relationship between Temporomandibular Disorders(TMD) and Hearing Acuity)

  • Temporomandibular disorders have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory muscles, and associated structures. Since Dr. James Costen has mentioned about the relationship between TMD and posterior bite collapse, there have been many controversies about the etiology and comorbidities of TMD. The purpose of this study was to investigate the relationship between TMD and hearing acuity in 158 volunteers with or without TMD symptoms. The subjects were examined clinically about TMJ sound, missing teeth, tinnitus, TMJ pain and masticatory muscle pain and the hearing acuity were measured by Audiometer(ITO AE-1000, Japan). The result of this study indicated that there was no significant differences between TMD symptoms and hearing acuity relatively.

  • 청각장애인을 위한 청능 및 언어 훈련은 비장애인과 소통능력을 배양하는 기본적인 훈련이지만 동시에 부모나 지도교사의 경제적부담, 육체적 피로를 수반한다. 이를 해결하기 위해 웹기반의 청능 및 언어 훈련 콘텐츠가 계속해서 개발되어 왔다. 그러나 장애인의 청각상태 및 학습능력의 차이가 존재함에도 불구하고 이런 개인 차이를 고려하지 않은 채 개발되고 있다. 따라서 장애자의 상태와 잔존청력, 학습능력 및 훈련성취도를 정확히 평가하고 분석하여 장애자에게 적절한 훈련진도를 계획하는 것이 중요하다. 본 논문에서 청각장애자를 위해 개인평가에 따라 다양하게 청능 언어 훈련을 기획하고 관리해주는 청능 및 언어 훈련 서비스 모델을 제안한다. 또한 이 모델을 기반으로 하는 기능성 게임콘텐츠를 기획하는 설계방법을 제시한다.

  • Recently, hearing loss patients have been increasing to excessive use of various multimedia devices. One of the hearing rehabilitation systems, bone conduction hearing aid can be used to conductive deafness patients efficiently. However, the conventional bone conduction hearing systems has some problems such as skin diseases, repulsion of patients, and vibration power reduction by skin damping. In this paper, to overcome the conventional problems, we proposed power improvement method by curved beam diaphragm. The proposed method is skin attachment system which is non-implantable, and then the power of transducer is improved by the proposed method. In order to improve the vibration power of diaphragm, variable that has correlation with displacement are extracted, the diaphragm designed by extracted variable. To verify efficient of the proposed method, experiment conducted by finite element analysis. As a result of, the proposed method confirmed improved power to compare with the conventional method and proposed method.

Development of Fully-Implantable Middle Ear Hearing Device with Differential Floating Mass Transducer : Current Status

  • It is expected that fully-implantable middle-ear hearing devices (FIMEHDs) will soon be available with the advantages of complete concealment, easy surgical implantation, and low power operation to resolve the problems of semi-implantable middle-ear hearing devices (SIMEHDs) such as discomfort of wearing an external device and replacement of battery. Over the last 3 years, a Korean research team at Kyungpook National University has developed an FIMEHD called ACRHS-1 based on a differential floating mass transducer (DFMT). The main research focus was functional improvement, the establishment of easy surgical procedures for implantation, miniaturization, and a low-power operation. Accordingly, this paper reviews the overall system architecture, functions, and experimental results for ACRHS-1 and its related accessories, including a wireless battery charger and remote controller.

귀판 합 조구등 추출액이 살리실산 나트륨으로 유발된 와우의 형태학적 변화에 미치는 영향 (Therapeutic Effects of Extract of Uncariae Ramulis and Testudinis Plastrum on Cochlear Morphologic Change Induced by Salicylate Ototoxicity)

  • Background and Objectives: Tinnitus is on the increase due to the increase in the elderly population, industrial pollution and noise pollution. This symptom is especially marked in patients with a hearing problem and the relationship between cause, mechanism and treatment is poorly understood. The characteristics of tinnitus and other hearing problems are well brought out using an animal model with salicylate ototoxicity. Therapeutic effects of Uncariae Ramulis and Testudinis Plastrum were expected in tinnitus and hearing problems; therefore we experimented on an animal model with salicylate ototoxicity. Salicylate is one of the most commonly prescribed drugs, although it has been recognized that salicylate induces hearing loss and tinnitus reversibly. The purpose of this study was to find the therapeutic effects of this by the morphologic study using salicylate ototoxicity. Materials and Methods : Twelve healthy Sprague-Dawley rats were divided into three groups: normal, control and sample. The sample group was treated with the extract of Uncariae Ramulis and Testudinis Plastrum (1cc/100g, once a day for 6 days). Then, to induce the salicylate ototoxicity in the control and sample groups, rats were injected intraperitoneally with sodium salicylate (500mg/kg). We observed the morphologic changes in the cochlea of the rats every 2, 3, 4 and 5 hours after injection. Results : The outer hair cells showed marked changes. Vacuolization formed in the cuticular plate and the endoplasm of the control group. The endoplasm and the cuticular plates of the sample group after 2 hours were similar to the control group, but the cuticular plates of the sample group observed after 3, 4 and 5 hours were not similar. Conclusions : The results suggest that an extract of Uncariae Ramulis and Testudinis Plastrum has therapeutic effects on an animal model with salicylate ototoxicity.
