Working Environments and Working Conditions Affecting Workers' Stress Symptoms (작업환경과 조건이 근로자의 스트레스에 미치는 영향)

  • There has been a rapid industrial progress in Korea since 1962 by the success of 5-year economic development plan, and the number of industrial work has also made a rapid increase. Consequently, the management of the occupational health for the purpose of promoting the health of industrial workers and improving the working environment is badly needed in these days. Health services on industrial noisy environment have been provided only for noise-induced hearing loss management until now. But gradually, modem diseases and death have come to be related to the stress and mental health, therefore noise-induced mental disorder, like a stress became very important. Thus, this study has been carried out to analyze the relationship between workers' stress symptoms and the perceived working environment and the perceived working conditions. This study included 786 industrial workers selected from II factories in Buchun. The results were as follows: 1. For demographic characteristics, most of the workers were males(75.7%), the 20~29 years old were 33.8% and those who graduated from high school were 56.1%. The workers whose monthly income ranged from 700,000 to 1,500,000 won were 37.9% and who has a religion were 49.0%. 2. For occupational characteristics, workers who had worked 5~10 years in the factories were 35.8%. Those who felt much for them workload were 42.7% and who worked more than 8 hours a day were 73.7%. Those who were dissatisfied with their pay and job were 51.1 % and 31.2%. The workers who responded ventilation condition of their worksites were bad were 50.4% and the dissatisfied with working environment of their worksites were 43.8%. 3. For the noise exposure level in worksite, workers who were exposed to 70∼90㏈ were 37.4%, 90∼100㏈ were 25.2% and 50∼70㏈ were 18.8%. 4. Workers∼ stress symptoms were significantly related to marital status and their monthly income(P〈0.05). Workers who were single and had lower monthly income showed higher PSI(Psychiatric Symptom Index) scores than those who were married and had higher monthly income. Higher PSI scores were also significantly related to the night-work, workload, dissatisfaction with their job, and bad relationship with their bosses or co-workers. 5. The higher noise exposure level in worksite from 80㏈ was, the more severe stress symptoms including PSI subparts were reported; Anxiety, Anger, Depression, and Cognitive disorder symptom(P〈0.001). 6. According to the results of stepwise multiple regression analysis, factors affecting workers' PSI scores were noise exposure level in worksite(R2=0.150), relationship with coworkers, amount of workload, monthly income and relationship with bosses orderly and the total R2 of this 6 factors was 29.7%.

A Statistical Analysis of Outpatients in Ophthalmology and Otolaryngology of Korean Medicine at Kyung Hee University Korean Medicine Hospital in 2012-2016 (2012-2016년 경희대학교 한방병원 한방안이비인후과 외래환자에 대한 통계적 분석)

  • Objectives : The purpose of this study is to research on the characteristics of outpatients and distributions of diseases in ophthalmology, otolaryngology of Korean medicine in 2012-2016 and compare the result with previous studies. Methods : This study is based on the analysis of medical records of total 2,319 outpatients in ophthalmology, otolaryngology of Korean medicine at Kyung Hee Korean medicine hospital from January 1, 2012 to December 31, 2016. We searched on the demographic characteristics of outpatients and the distributions of diseases in order of frequency by age and sex. Then, we compared these results to previous results of papers at same hospital in 2000 and 2007. Results : The common diseases in 2012-2016 were dry eye syndrome in ophthalmology(32.14%), tinnitus and hearing loss in otology(72.69%), rhinitis in rhinology(82.20%) and laryngopharyngitis in laryngology(67.77%). The most increasing diseases in the distribution between 2004-2006 and 2012-2016 were retinopathy in ophthalmology(+366.77%), etc including the eustachian tube disorder in otology(+151.55%), olfactory disorder in rhinology(+167.50%) and laryngopharyngitis in laryngology(+126.20%). Conclusions : If we accumulate these kinds of epidemiologic studies on ophthalmology, otolaryngology of Korean medicine, it will be great help to make Korean medical preparations for the increasing diseases on these fields.

Acoustic Characteristics of Korean Spoken by the Women Immigrants from Japan and Philippine (여성 결혼이민자들의 한국어 조음에 나타나는 음향음성학 특성 연구 - 일본과 필리핀 출신 여성 결혼이민자들을 대상으로)

  • The number of Asian women immigrants in Korea is getting bigger and it's important to note that their communication problem in Korean causes not only the difficulty of adapting to Korean society but their children's speech-language disorder. To date there is little research on their acoustics characters and articulatory errors. Therefore, this study focuses on acoustic characters and articulatory error patterns of the women immigrants from Japan and Philippine based on the theory of "contrastive analysis". The subjects were 16 Japanese women immigrants(age: 42.5$\pm$4.4) and 14 Philippine women immigrants(age: 31.64$\pm$6.7) and control group consisted of 10 Korean women(age: 28.3$\pm$1.2). Speech and hearing of all subjects and control group were within normal limits. Speech samples were analyzed in a computer using CSL and data analysis was done on FFT widow for F1, F2, F3 of vowels and on wideband spectrogram for VOT of plosives and africatives. The results of this study were like this; For Japanese women immigrants, they had different articulatory patterns of /e/, /a/, /u/, /o/, /$\varepsilon$/, /m/ from those of Koreans and showed articulatory errors on the fortis and aspirated sounds. The reason is Japanese has only two distinctive characters for plosives and affricates; voicing and voiceless. The Philippine women immigrants also showed the same error patterns as the Japanese women immigrants. Especially the errors on aspirated sounds were prominent because their mother tongue has no distinctive characters about aspirated sounds. For vowels, they showed errors of /a/, /o/, /c/.

Psychiatric Characteristics According to Tinnitus Severity (이명 심각도에 따른 정신의학적 특징)

  • Objectives The purpose of this study was to examine the psychiatric characteristics of outpatients with tinnitus. Methods A total of 88 subjects were included in this study. According to Tinnitus Handicap Inventory (THI) scores, the subjects were classified into two group ; a mild tinnitus symptoms (mild-tinnitus) group and a severe tinnitus symptoms (severe-tinnitus) group. A questionnaire was used for an assessment of demographic characteristics, and the THI, the Visual Analogue Scale (VAS) about tinnitus, the Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI) are applied for evaluation of other clinical psychiatric characteristics. Results Higher THI scores were positively correlated with tinnitus course, the number of accompanying symptoms, and the VAS. BDI total scores, BDI factors, and BAI total scores were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Also suicidal ideation, interpersonal problems, sleep problems, occupational impairment, and fatigue were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Conclusions Tinnitus is a common disorder of hearing which is associated frequently with psychiatric problems. This study suggests that psychiatric interventions should be taken into consideration in the treatment of patients suffering from tinnitus.

TREATMENT OF THE CHILD WITH ROBINOW SYNDROME UNDER GENERAL ANESTHESIA : A CASE REPORT (Robinow 증후군 환아의 전신마취를 이용한 치료증례보고)

  • Robinow syndrome or fetal face syndrome is a rare inherited disorder characterized by short stature, mesomelic brachymelia, hypoplastic genitalia, and a typical facial appearence ("fetal face") with frontal bossing, hypertelorism, ear abnormalities, a short upturned nose, long philtrum, micrognathia, and macrocephaly. Intraoral features have included quite a few dental cavities, crowding, hypoplastic uvula, cleft lip or/and cleft palate, gingival hyperplasia, alveolar hyperplasia, enamel hypoplasia, delayed eruption, and congenital missing of the permanent teeth. We report on a 10 years old girl with Robinow syndrome. The patient had most of the typical anomalies of the syndrome and negative family history but, in addition, had mental retardation, hearing loss, and serous otitis media. Intraoral findings included dental cavities, crowding, hypoplastic uvula, repaired cleft palate, and mouth breathing. Dental treatment and V-tube insertion(by dept. of ENT) were performed under general anesthesia. In all cases of Robinow syndrome, thorough evaluation and united treatments with medical specialists should be performed.

Variable expression observed in a Korean family with Townes-Brocks syndrome caused by a SALL1 mutation

  • Townes-Brocks syndrome (TBS) is a rare genetic disorder characterized by the classic triad of congenital anomalies of the anus, thumbs, and ears, with variable expressivity. Additionally, renal malformations, cardiac anomalies, and endocrine and eye abnormalities can accompany TBS, although less frequently. TBS is inherited in an autosomal dominant fashion; however, about 50% of patients have a family history of TBS and the remaining 50% have de novo mutations. SALL1, located on chromosome 16q12.1, is the only causative gene of TBS. SALL1 acts as a transcription factor and may play an important role in inducing the anomalies during embryogenesis. Clinical features of TBS overlap with those of other multiple anomaly syndromes, such as VACTERL syndrome, Baller-Gerold syndrome, Goldenhar syndrome, cat eye syndrome, and Holt-Oram syndrome. Consequently, there are some difficulties in differential diagnosis based on clinical manifestations. Herein, we report a Korean family with two generations of TBS that was diagnosed based on physical examination findings and medical history. Although the same mutation in SALL1 was identified in both the mother and the son, they displayed different clinical manifestations, suggesting a phenotypic diversity of TBS.

Clinical Study on Gadolinium-DPTA enhanced MRI of Bell's palsy (구안와사(口眼喎斜) 환자(患者)의 Gadolinium-DPTA enhanced MRI 소견(所見)에 대한 임상적(臨床的) 고찰(考察))

  • This study is designed to evaluate the clinical implications of Gd-DPTA (Gadolinium-diethyl enetriamine pentacetic acid) enhanced MRI(Magnetic resonance imaging) in Bell's palsy and find it's usefulness in Oriental Medicine In this study, 25 outparients with Bell's palsy were studied that MRI was performed. To evaluate degree of facial palsy, H-B(House-Brackmann) Grade was used. In Oriental Medical therapy, Acupuncture and Herbal medicine were treated. Subjective cause was divided into exposure to chill, fatigue, stress, mixed cases. Enhanced site was compared with symptoms which were disorder of eye, hearing, taste, and facial muscle palsy. Also, Relation between time which was performed MRI and enhancement was analyzed. The enhanced lesion in MRI was divided into five segments; Internal audiitory canal, Labyrinthine segment, Geniculate ganglion, Tympanic segment, Mastoid segment. In Bell's palsy, 20 of 25 patients(80%) had abnormal contrast enhancement of the facial nerve. The H-B grade and interval performed MRI from onset were directly proportionate to enhancement. That is to say, Severe facial palsy short interval show high possibility of enhancement. There was no relation between subjective causes and enhanced site of facial nerve in MRI. Also Clinical symptoms didn't coincide with MRI findings.

A Statistics Study of Surgery, Ophthalmology & Otolaryngology (외관과(外官科) 외래환자의 통계적 고찰)

  • We analyzed statistics study in 1695 patients, who had visited to the Dept. of surgery, ophthalmology & otolaryngology Hospital of Downtown Orirental Medicine Kyung Hee University from March, 1996 to July, 1998. The results were as follows. 1. Distribution of ophthalmology, otolaryngology and dermatology classification was $43.48\%\;in\;ophthalmology,\;16.70\%\;in\;rhinology,\;5.72\%\;in\;laryngology,\;17.94\%\;in\;dermatology\;and\;16.17\%$ in otiatrics. 2. Ophthalmology group were $31.93\%$ males and $47.90\%$ females in 10 years old within. In treatment term, opthalmology patients who had treated for 1 month within was $59.70\%$. Strabismus and visual disorder was the most common ophthalmo-disease. 3. Rhinology group were $30.00\%$ males in 10 years old within and $21.56\%$ females in 21-30years old. In treatmentm, rhinology patients who had treated for 1 month within was $53.71\%$. Rhinitis and sinusitis was the most common rhinio-disease. 4. Otiatrics group were $30.54\%$ males in 41-50 years old and $31.78\%$ females in 51-60 years old. In treatment term, otiatries patients who had treated for 1 month within was $53.28\%$. Tinnitus, hearing loss and otitis media was the most common otiatrics-disease. 5. Laryngology group were $29.27\%$ males in 31-40 years old and $28.57\%$ females in 51-60 years old. In treatment term. Laryngology patients who had treated for 1 month within was $91.75\%$ Epipharyngitis and abnormal sensation pharyngolarylgeal region was the most common laryngo-disease. 6. Dermatology group were $39.82\%$ males and $39.79\%$ females in 21-30 years old. In treatment term, dematology patients who had treated for 1 month within was $83.55\%$. Acne and Pruritus was the most common ophthalmo-disease.

  • Duplication of chromosome 12p has been rarely reported and are thought to be associated with congenital malformations and impaired development. We report a baby boy born with multiple dysmorphic features and congenital malformations. His karyotype was 46,XY, add(12)(p13.3). He has suffered from intrauterine growth restriction at birth. He showed abnormal cranio-facial findings such as microcephaly, hypognathia, clepft palate and low set ear. He presented with absence of uvula, micropenis and rocker bottom features of both feet, congenital heart disease, poor corticomedullary differentiation of kidney, and sensorineuronal hearing loss. We have been follow up him for seizure disorder and delayed development at out patient department.

Nonfatal injuries in Korean children and adolescents, 2007-2018

  • Background: Injury is the leading cause of death or disability in children and adolescents. Rates of deaths from injuries have recently declined, but studies of the occurrence of nonfatal injuries are lacking. Purpose: This study aimed to investigate nonfatal injuries in children and adolescents younger than 20 years based on data from the Korean National Health and Nutrition Survey, 2007-2018. Methods: A questionnaire survey was conducted to determine whether children and adolescents had experienced an injury requiring a hospital visit in the previous year. We investigated each injury's risk factors and characteristics. Results: Of a total of 21,598 children and adolescents, 1,748 (weighted percentage, 8.1%) experienced one or more injuries in the previous year. There was no yearly difference in the proportion of injuries experienced. Among the male subjects, 10.0% had an injury experience; among the female participants, 6.1% had an injury experience (P<0.001). The highest rate was 9.0% in children aged 1-4 years. In multivariate logistic regression analysis, male sex; having an urban residence; having restricted activity due to visual, hearing, or developmental impairment; and attention deficit/hyperactivity disorder were significant risk factors for injury experience. The characteristics of up to 3 injuries per patient were investigated, and 1,951 injuries were analyzed. Falls and slips accounted for 34.9%, collisions for 34.1%, and motor vehicle accidents for 11.3% of the total injuries. Ninety-six percent of injuries were unintentional, 20% caused school absences, and 10% required hospitalization. Conclusion: Among Korean children and adolescents, 8.1% experienced injuries at least once a year with no significant differences in incidence over the past 12 years. Greater attention and effort to prevent injuries are needed.
