Relationship between Physical Function Factors and Discrimination Experiences of the Elderly in Korea: The 2014 and 2017 National Survey on Living Conditions and Welfare Needs of Korean Older Persons (우리나라 노인의 신체기능 요인과 차별경험의 관련성 연구: 노인실태조사(2014, 2017) 자료 분석)

  • Background: Supporting the elderly population is presented as a social issue, and it affects age discrimination, which forms a negative perception and avoids the elderly. Since age discrimination lowers the quality of life of the elderly and hinders social unity, it may be important to research related factors. This study examined the physical function as a factor that influences discrimination experiences and aimed to identify the relationship between physical function factors and discrimination experiences. Methods: In this study, we analyzed 20,225 elderly from the 2014 and 2017 survey of living conditions and welfare needs of Korean older persons, conducted nationwide among older than 65 years. Physical function factors are activities of daily living, instrumental activities of daily living, sight discomfort, hearing discomfort, chewing discomfort, and leg strength as factors of interest. We performed multivariable logistic regression that reflected survey characteristics, adjusting for socio-demographic factors, health related factors, and social-support factors. Results: The number of elderly who faced discrimination experiences was 1,175 (5.8%). The results showed that the odds ratio of facing a discrimination experience in the dependent group was significantly higher compared to the independent group when being transferring out of the room (4.05; 95% confidence interval [CI], 1.50-10.88) and difficulty in hearing (1.25; 95% CI, 1.05-1.49). Even with respect to chewing ability, which was significant in models 1 and 2, they face more discrimination experiences (1.30; 95% CI, 1.11-1.53). Conclusion: These research results indicate that an important strategy for preventing age discrimination is to understand the physical function conditions of the elderly and promote the physical functions of the elderly related to transferring, hearing, and chewing; it is necessary to develop a practical intervention plan that considers these aspects.

Changes of Acoustic Reflex Thresholds and Speech-In-Noise Perception Using Personal Listening Device Under Subway Interior Noise (지하철 실내 소음 하에서 음향기기 사용에 따른 음향반사역치와 소음 속 어음인지 변화)

  • Although it is well-known that environmental noise can lead to hearing loss in individuals, the true extent of subway noise effects in the general population remains poorly understood. The purpose of the present study is to see changes of acoustic reflex thresholds and speech perception scores when passengers listen to music presented from their personal listening device in the subway. Forty subjects with normal hearing participated being divided into two groups, experimental and control groups. As a baseline, all subjects were measured by acoustic reflex thresholds in five test frequencies and Korean speech perception in noise (KSPIN) test at 0 and -5 dB SNR. In the experiment, the control group read newspaper or magazine in the subway noise, whereas the experimental group listened to music presented from their smartphone under the subway noise through speakers at 73.45 dBA for 60 min. After completing the experiment, two groups also conducted both acoustic reflex thresholds and KSPIN tests in the same condition as the baseline. The results showed that there was a significant difference of correct percent in speech-in-noise test between experimental and control groups and of that between two signal-to-noise ratios, which means the double noise exposure of 60 min might cause some degradation of speech perception when noise increases compared to only subway noise condition that was not statistically significant difference. We concluded that a risk of some degraded speech perception ability would be expected when passengers have a habit of listening to music in the subway noisy situation for a long duration.

  • The language of the newborn, like that of adults, is one of gesture, posture, and expression(Lewis, 1980). Helping parents understand and respond to their newborn's cues will make caring for their baby more enjoyable and may well provide the foundation for a communicative bond that will last lifetime. Infant state provides a dynamic pattern reflecting the full behavioral repertoire of the healthy infant(Brazelton, 1973, 1984). States are organized in a predictable emporal sequence and provide a basic classification of conditions that occur over and over again(Wolff, 1987). They are recognized by characteristic behavioral patterns, physiological changes, and infants' level of responsiveness. Most inportantly, however, states provide caregivers a framework for observing and understanding infants' behavior. When parents know how to determine whether their infant is sleep, awake, or drowsy, and they know the implications, recognition of states has for both the infant's behavior and for their caregiving, then a lot of hings about taking care of a newborn become much easier and more rewarding. Most parents have the skills and desire to do what is best for their infant. The skills 7373parents bring to the interaction are: the ability to read their infant's cues: to stimulate the baby through touch, movement, talking, and looking at: and to respond in a contingent manner to the infant's signals. Among the crucial skills infants bring to the interaction are perceptual abilities: hearing and seeing, the capacity to look at another for a period of time, the ability to smile, be consoled, adapt their body to holding or movement, and be regular and predictable in responding. Research demonstrates that the absence of these skills by either partner adversely affects parent-infant interaction and later development. Observing early parent-infant interactions during the hospital stay is important in order to identify parent-infant pairs in need of continued monitoring(Barnard, et al., 1989).

  • In implementing a computerized virtual training system, the auditory element is responsible for the human cognitive ability following visual elements. Especially, the improvement of hearing ability is closely related to the performance of the training, and it contributes to improvement of the training effect. In this paper, we propose a sound system that is necessary for constructing such a virtual training system as a test system that can use a sound source using a head related transfer function (HRTF). Functional and auditory tests were performed to evaluate system performance.

The Effects of Toe-Tap Exercise on Dynamic Balance and Muscle Activity of Gluteus Maximus (토탭 운동이 동적균형과 큰볼기근의 근활성에 미치는 영향)

  • Background: This study hypothesized that increased muscle activity and balancing ability of the gluteus maximus during toe-tap exercise. Design: Cross sectional Study. Methods: After hearing the explanation of the experiment, the subjects performed a Star excursion balance test and measured the Maximum Voluntary Isomeric Contraction (MVIC). After toe-tap exercise, the MVIC was measured again and the Star excursion balance test was measured. Results: There was no significant difference in activity of gluteus muscle before and after the toe-tap exercise. There was a significant difference in the balance ability in the lateral, posterolateral, and posterior sides of the non-superior foot, but there was no significant difference in the anterior, anterolateral, posteromedial, medial, and anteromedial sides. In the case of superior foot, there were significant differences in six directions, with the exception of anterolateral and lateral sides. Conclusion: The pelvic stabilizing "gluteus maximus" exercises for balance, which is currently on the table, has many high-level exercises that are hard to do unless you're an athlete. But the Toe-tap exercise is also possible for the elderly and weak women.

Alveolar Fricative Sound Errors by the Type of Morpheme in the Spontaneous Speech of 3- and 4-Year-Old Children (자발화에 나타난 형태소 유형에 따른 3-4세 아동의 치경마찰음 오류)

  • Korean alveolar fricatives are late-developing speech sounds. Most previous research on phonemes used individual words or pseudo words to produce sounds, but word-level phonological analysis does not always reflect a child's practical articulation ability. Also, there has been limited research on articulation development looking at speech production by grammatical morphemes despite its importance in Korean language. Therefore, this research examines the articulation development and phonological patterns of the /s/ phoneme in terms of morphological types produced in children's spontaneous conversational speech. The subjects were twenty-two typically developing 3- and 4-year-old Koreans. All children showed normal levels in three screening tests: hearing, vocabulary, and articulation. Spontaneous conversational samples were recorded at the children's homes. The results are as follows. The error rates decreased with increasing age in all morphological contexts. Also, error percentages within an age group were significantly lower in lexical morphemes than in grammatical morphemes. The stopping of fricative sounds was the main error pattern in all morphological contexts and reduced as age increased. This research shows that articulation performance can differ significantly by morphological contexts. The present study provides data that can be used to identify the difficult context for articulatory evaluation and therapy of alveolar fricative sounds.

Development of GPS based Self Triage App for Disaster Vulnerable Populations (재난 취약계층을 위한 GPS 기반 Self Triage 앱 개발)

  • Purpose: The purpose of this study was to develop a self triage application for rescue requests by disaster vulnerable populations. Literature was reviewed in order to define application trends and needs. Methods: Development of the self triage application was conducted in six stages as a hybrid model (analysis, design, development, implementation, evaluation, modification) of the ADDIE (Analysis, Design, Development, Implementation, Evaluation) model and Driscoll & Alexander model. Application system function and contents were tried with 6 experts and 4 people with hearing impairments. Results: Expert assessment of the application for self triage showed that reliability at 5 points was the highest, followed by utility at 4.8 points. Scores for quickness and expressiveness were low at 4.6 and 4.2 points respectively. User acceptability assessment of the application was measured at 66.73 points. Conclusion: The results show that the application for self triage is helpful to disaster vulnerable populations by providing relief in disaster situations. It is expected that use of this application as a self rescue ability can be made available for disaster situations. However, it will be necessary to establish policies for communication strategies with rescuers and public relations to improve the access rate of disaster app service.

  • With international urgence of terror situation, many methods of terror techniques and skills are appearing and more intense and threatening terror is braking out not only in the air(plane), but also in the sea(ship). Korea is surrounded by 3 sides of sea and is a proud maritime nation that should advance to foreign country through the sea because South Korea and North Korea are confronting. Korea depend on the maritime transportation, 99.7% of exporting, importing material resources. Therefore, acquisition of the marine safety has the great affect on national security and economical life. On the high tension of situation about threat and possibility of maritime terroism, the potential ways of the improvement policy for counter-tactics against big maritime terror are the following. First, we should construct clear and well-organized network for accurate information about maritime terrorism Second, we should have the information of all domestic, foreign passenger's ships Third, national important facilities such as atomic energy plant, thermo-electric power plant, shipyard need policies to manage all emergency situation Fourth, government authorities should improve tactic abilitities by competing with support and budget inside of nation as well as outside of nation. To develop big maritime terror of our country, we will have to hold cooperative training & tactics contest of big maritime terror by cooperating educational industry organization with similar institute, improve the ability of members of big terror and acquire tactics information by excavating and exchanging a new technique through tactics seminar and public hearing.

  • The maximum sound pressure level in a ship is restricted by the international standard such as MIL-Std. 740-1 in order to protect hearing ability for the crew on a ship. Especially, the noise of a fan system is restricted by the maximum level of the sound power according to the military specification such as MIL-F-1900A. In this paper, the reduction of tonal noise for the fan-motor system on a ship is described related to the electrical as well as structural problems. For the electrical problems, the difference of the magnetic flux caused by the variation of the air-gap is described related to the bad concentricity of the motor housing and the shaft-bearing. And for the structural problem, the resonance of the fan motor system is described.

Prevalence and Risk Factors of Unmet Healthcare Needs among Korean Adults with Hypertension (한국인 고혈압 환자의 의료접근성 및 미 충족 의료실태와 위험요인 분석)

  • Purpose: The purposes of this secondary analysis study was to examine prevalence, risk factors and unmet healthcare needs among adults with hypertension. Methods: A sample of 3,386 adults over the age of 40 with hypertension were drawn from the Korea Health Panel Study (2013). Using SPSS 22.0 version, descriptive statistics including frequency, percentage, chi-square and logistic regression were performed. Results: Results showed that 18.9% of the sample reported unmet healthcare needs with the most frequently cited one was financial burdens (43.2%). The reported experiences of unmet healthcare needs differed by gender, marital status, vision or hearing impairment, memory problem, impaired mobility, subjective health status, total family income, depressive episode and the difficulty in making decisions. The sample participants were more likely to report unmet healthcare if they had vision impairment, low income and perception that their health status as moderate to poor. Those without vision impairment were less likely to report unmet healthcare needs. Conclusion: The identified risk factors of unmet healthcare needs should be addressed which would enhance access both to health care and to resolution of unmet healthcare needs. Since visual ability seems to impact perception of unmet healthcare needs, it may be useful to find ways to address this factor.
