Molecular and Circulatory Expression of Insulin Growth Factors in Indian Females with Advanced Cervical Cancer

  • Background: Recent studies have demonstrated an association between insulin growth factor (IGF) and insulin growth factor binding protein-3 (IGFBP-III) serum levels and increased risk for various cancers. However, little information is available on clinical implications of the IGF system in Indian patients with cervical cancer. This study explored associations by analyzing their expression profiles in cervical cancer cases. Materials and Methods: Totals of 50 patients with advanced cervical cancer and 40 healthy controls were enrolled. Human papillomavirus (HPV) and cervical biopsy sample were obtained from all participating women. Circulatory levels were estimated by ELISA and the tissue expression was assessed using RT-PCR and Western blotting. Results: Levels of IGF-I and II showed significant increase whereas IGFBP-III showed significant decline in all patients as compared to controls. Spearman correlation analysis between IGFs and HPV status showed significant correlations. Conclusions: We demonstrated elevated circulating levels and tissue expression of IGF-I and IGF-II in advancer cancer cervix patients, as compared with controls, with a converse trend being apparent for IGFBP-III. In future, associations of the IGF system and clinical outcome of cervical cancer patients in post treatment samples might point to significance in disease mapping as a prognostic marker after validation with a larger patient series.

Transforming Growth Factor-β3 Gene SfaN1 Polymorphism in Korean Nonsyndromic Cleft Lip and Palate Patients

  • The nonsyndromic cleft lip and palate (NSCL/P) is a congenital deformity of multifactorial origin with a relatively high incidence in the oriental population. Various etiologic candidate genes have been reported with conflicting results, according to race and analysis methods. Recently, the ablation of the TGF-${\beta}3$ gene function induced cleft palates in experimental animals. Also, polymorphisms in the TGF-${\beta}3$ gene have been studied in different races; however, they have not been studied in Koreans. A novel A $\rightarrow$ G single nucleotide polymorphism (defined by the endonuclease SfaN1) was identified in intron 5 of TGF-${\beta}3$ (IVS5+104 A > G). It resulted in different genotypes, AA, AG, and GG. The objective of this study was to investigate the relationship between the SfaN1 polymorphism in TGF-${\beta}3$ and the risk of NSCL/P in the Korean population. The population of this study consisted of 28 NSCL/P patients and 41 healthy controls. The distribution of the SfaN1 genotypes was different between the cases and controls. The frequency of the G allele was significantly associated with the increased risk of NSCL/P [odds ratio (OR) = 15.92, 95% confidence interval (CI) = 6.3-41.0]. The risk for the disease increased as the G allele numbers increased (GA genotype: OR = 2.11, 95% CI = 0.38-11.68; GG genotype: OR = 110.2, 95% CI = 10.67 - 2783.29) in NSCL/P. A stratified study in patients revealed that the SfaN1 site IVS5+104A > G substitution was strongly associated with an increased risk of NSCL/P in males (p < 0.001), but not in females. In conclusion, the polymorphism of the SfaN1 site in TGF-${\beta}3$ was significantly different between the NSCL/P patients and the control. This may be a good screening marker for NSCL/P patients among Koreans.

Effect of the Short foot Exercise Using an Electromyography Biofeedback on Medial Longitudinal Arch During Static Standing Position (근전도 바이오피드백을 이용한 숏 풋 운동이 정적으로 선 자세 동안 발의 안쪽 세로활 유지에 미치는 영향)

  • Background: Short foot exercise (SFex) is often prescribed and performed in the sport and rehabilitation fields to strengthen intrinsic foot muscles. However, SFex is difficult to perform because of lack of feedback methods. Objects: The aim of this study was to compare the effects of SFex with and without electromyography (EMG) biofeedback on the medial longitudinal arch (MLA) of healthy individuals who maintained a static standing position. Methods: All participants (14 males and 12 females) were randomly divided into two groups (biofeedback and non-biofeedback groups). The EMG activity of the abductor hallucis (AbdH) and tibialis anterior (TA) and the MLA angle on the dominant leg side were measured with the participant in the standing position in the pre- and post-intervention conditions. The intervention session consisted of 15 minutes of SFex with (biofeedback group) or without (non-biofeedback group) EMG biofeedback. The groups were compared using two-way repeated measures analysis of variance. Results: The post-intervention activities of the AbdH muscle (p<.05) and the AbdH/TA ratio (p<.05) were significantly greater in the biofeedback group than in the non-biofeedback group. The activity of the TA (p<.05) and the MLA angle (p<.05) in the biofeedback group were significantly lower in the post-intervention condition than in the pre-intervention condition. Conclusion: The present findings demonstrate that the combination of SFex and EMG biofeedback can effectively facilitate the muscle activity of the AbdH and strengthen the medial longitudinal arch.

Electromyographic Activation of Lumbar Multifidus Muscle Induced by Horizontal Shoulder Extension Exercise: Comparison of Four Intensity Levels of the Exercise (등척성 견관절 수평 신전 운동의 강도에 따른 다열근의 활성도 비교)

  • Purpose : To identify whether isometric shoulder horizontal extension (ISHE) exercise could sufficiently activate the lumbar multifidus muscle, and to determine appropriate exercise intensity to increase muscle strength. Methods : Twenty healthy volunteers (10 males and 10 females) participated in this study. ISHE exercises on the dominant side were performed with $90^{\circ}$ shoulder abduction held in $90^{\circ}$ elbow flexion so that their arms were in the horizontal plane. Electromyographic (EMG) measurements of multifidus activation were performed in standing and supine positions, and were taken under four strength conditions: 75%, 50% and 25% of maximum shoulder horizontal extension strength, and maximum strength. Results : The EMG activations of both lumbar multifidus increased significantly with ISHE exercises of larger % strength (p<.05). In their multifidus EMG data measured in standing and supine positions, men differed significantly in their 75%, 50% and 25% strength for both sides (p<.05), and women differed significantly in their 75%, 50% and 25% strength on their 5th lumbar vertebrae's left side, but 75% only for their 5th lumbar vertebrae's right side (p<.05). The EMG data of 5th lumbar vertebrae's left and 5th lumbar vertebrae's right sides appeared to differ significantly at all strength levels for men while standing, but only at the 25% level for women in the supine position (p<.05). These findings indicate that ISHE exercises can be considered a beneficial method to enhance the multifidus strength. Conclusion : This study provides useful information for further study in this field.

Comparison of the Joint Position Sense at Knee Joint According to Surface Conditions (지지 면 조건에 따른 무릎관절의 관절 위치 재현능력 비교)

  • The purpose of this study was to compare the joint position sense at the knee joint at 3 different surface conditions by using the active knee joint angle reproduction test in the standing position. Twenty healthy volunteers (10 males and 10 females) age 20~29 years were recruited for this study. The knee joint position senses were assessed at three different surface conditions: on the floor (stable condition), TOGU (soft condition), and seat fit (unstable condition) in a closed kinetic chain. Testing orders were selected randomly. The absolute angle error was defined as the absolute difference between target angles ($30^{\circ}{\sim}45^{\circ}$ knee flexion) and subject perceived angle of the knee flexion. One way ANOVA was used to compare the absolute angle of error among 3 different conditions. The Independent t-test was used to compare the absolute angle of error between male and female. The error angles were significantly different among surface conditions ($1.3^{\circ}{\pm}1.2^{\circ}$ for the floor, $2.1^{\circ}{\pm}0.9^{\circ}$ for the TOGU, and $4.4^{\circ}{\pm}1.8^{\circ}$ for the seat fit, p<.05). There was no significant difference in error angle between male and female. In conclusion, the joint position sense of the knee joint in the closed kinetic chain decreased at unstable surface conditions. The result of this study indicates that surface conditions should be considered when assessing and training the joint position sense of the knee joint in clinical setting.

Effects of brown rice-vegetable school meal program on subjective health status, BMI and hematological parameters among high school students (청소년 대상의 현미-채식 급식 프로그램이 주관적 건강상태, BMI 및 혈액지표에 미치는 효과)

  • This study aimed to identify the effect of brown rice-vegetable school meals program on the subjective health status, BMI and hematological parameters of high school students. This is a single group pre-post study design that includes 17 high school boys and 18 high school girls. Students were catered for lunch and dinner with brown rice and a vegetarian diet for 12 weeks at the school. The collected data on subjective health status, BMI levels and hematological parameters were measured in pre and post, and changes were compared by paired t-test. Subjective health status of the participants has improved and BMI was reduced. Blood Hct, Hb, Hct, total protein and albumin have also been reduced. Ca and triglyceride levels decreased in females. However, all levels were reduced within the normal range. These results show that brown rice-vegetable intake of high school students reduces weight and helps them prevent from acquiring cardiovascular diseases in adulthood through the reduction of serum triglyceride and in practicing healthy eating habits.

Effects of a mobile healthcare service provided by public health centers on practicing of health behaviors and health risk factors

  • BACKGROUND/OBJECTIVES: This study evaluated whether a mobile health (mHealth) application can instigate healthy behavioral changes and improvements in metabolic disorders in individuals with metabolic abnormalities. SUBJECTS/METHODS: Participants were divided into an mHealth intervention group (IG), which used a mobile app for 24 weeks, and a conventional IG. All mobile apps featured activity monitors, with blood pressure and glucose monitors, and body-composition measuring devices. The two groups were compared after 24 weeks in terms of health-behavior practice rate and changes in the proportion of people with health risks, and health behaviors performed by the IG that contributed to reductions in more than one health risk factor were analyzed using multiple logistic regression. RESULTS: Preference for low-sodium diet, reading nutritional facts, having breakfast, and performing moderate physical activity significantly increased in the mHealth IG. Furthermore, the mHealth IG showed a significant increase of eight items in the mini-dietary assessment; particularly, the items "I eat at least two types of vegetables of various colors at every meal" and "I consume dairies, such as milk, yogurt, and cheese, every day." The proportion of people with health risks, with the exception of fasting glucose, significantly decreased in the mHealth IG, while only the proportion of people with at-risk triglycerides and waist circumference of females significantly decreased in the control group. Finally, compared to those who did not show improvements of health risks, those who showed improvements of health risks in the mHealth IG had an odds ratio of 1.61 for moderate to vigorous physical activity, 1.65 for "I do not add more salt or soy sauce in my food," and 1.77 for "I remove fat in my meat before eating." CONCLUSIONS: The findings suggest that the additional use of a community-based mHealth service through a mobile application is effective for improving health behaviors and lowering metabolic risks in Koreans.

The Inter-Rater Reliability of Measurements of Active Craniocervical Range of Motion With Smartphone in Patients With Stroke (뇌졸중 환자에서 스마트폰 어플리케이션을 이용한 능동적 머리목관절 가동범위 측정에 관한 측정자간 신뢰도 연구)

  • Background: Numerous studies have used smartphone applications to measure the range of motion in different joints. In addition, studies measuring the active range of motion (AROM) of the craniocervical joint have revealed high reliability. However, the subjects in these studies were all healthy subjects. No study has yet been conducted to measure the inter-rater reliability for the AROM of the craniocervical joint in stroke patients. Objects: The purpose of this study was to investigate the inter-rater reliability of the AROM of the craniocervical joint using a smartphone. Methods: The participants included 21 subjects who had strokes (17 males and 4 females). Two raters evaluated six types of craniocervical AROM, including flexion, extension, lateral flexion to the hemiplegic side, lateral flexion to the non-hemiplegic side, rotation to the hemiplegic side, and rotation to the non-hemiplegic side, using a goniometer and a smartphone to investigate inter-rater reliability. The inter-rater reliability was analyzed by intraclass correlation coefficients (ICC). Results: The inter-rater reliability of the smartphone was good for extension, lateral flexion to the hemiplegic side, lateral flexion to the non-hemiplegic side, and rotation to the hemiplegic side [ICC(2,k)=.86~.88] and excellent for flexion [ICC(2,k)=.95]. The inter-rater reliability for rotation to the non-hemiplegic side was moderate [ICC(2,k)=.72]. Conclusion: These results suggest that the smartphone offers high inter-rater reliability for measurements of the craniocervical AROM in patients with stroke.

Effect of PNF Leg Pattern Application According to Posture on Muscle Activation of the Contralateral Gluteus and Transverse Abdominis: A Preliminary Study (자세변화에 따른 PNF 다리패턴 적용이 반대측 볼기근과 배가로근에 미치는 영향: 예비 연구)

  • Purpose: The purpose of this study was to confirm that the activation of the contralateral muscles changes according to posture after applying the proprioceptive neuromuscular facilitation leg pattern. Methods: Ten healthy adults (four males and six females) with no history of current musculoskeletal/neurological injuries were recruited for this study. Abdominal muscle (transverse abdominis) and hip joint muscle (gluteus maximus and gluteus medius) activation was assessed using surface electromyography (Ultium EMG, Noraxon Inc., USA). We evaluated muscle activation during the application of the PNF leg pattern. The data were analyzed using the SPSS version 21.0 program. Results: The results were found to be statistically significantly different in the 90/90 supine posture and sitting posture for the gluteus maximus (p < 0.05). The results were found to be not statistically significantly different for the transverse abdominis according to posture (p > 0.05). The results were found to be not statistically significantly different for the gluteus medius according to posture (p > 0.05). Conclusion: Application of the PNF leg pattern resulted in a significant change in the muscle activation of the contralateral segment according to posture, and the 90/90 supine posture induced high muscle activation of the gluteus maximus. In addition, the activation of the transverse abdominis was high in all three postures.

Factors Influencing Mental Health Status of Persons with Physical Disabilities (지체장애인의 정신적 건강상태 영향요인)

  • The purpose of this study was to identify the factors affecting mental health status according to gender of persons with physical disabilities and to try their healthy life. Using the 6th Korean Retirement and Income Study, 333 persons with physical disabilities were included in the study. Descriptive statistics, t-test, and regression analysis were conducted to examine the general characteristics, instrumental activities of daily living, material support, and the effects on mental health status. There was no statistically significant difference in mental health status by gender, but there was a difference in Instrumental Activities of Daily Living and life satisfaction. In the case of males, instrumental daily activities performance, material support, depression, and life satisfaction were found to be influential. In the case of females, spouse, depression, and life satisfaction were factors affecting mental health status. Based on the results of this study, we will be able to understand persons with physical disabilities and use them as a basis for developing mental health promotion programs.
