헬스케어 정보기술 분야의 연구 프레임워크 및 연구동향 (A Study on Research Framework and Research Trends in the Healthcare Information Technology Area)

  • 최근의 스마트폰과 정보통신 기술의 발달, 무선센서 영역의 폭발적 성장은 전통적 의료서비스 환경을 급격하게 변화시키고 있다. 이러한 환경 변화에 부응하고자 전통적 의료서비스 체제를 재택생활 영역과 사회공공치료 영역 등으로 그 범위를 확대하기 위한 연구들이 활발하게 진행되고 있다. 본 연구는 최근 활발한 연구 활동이 전개되고 있는 헬스케어 분야에 대한 연구 프레임워크를 개발하여 제시하였고, 이를 바탕으로 해외 연구동향을 분석하였다. 스마트폰을 활용한 재택환자모니터링과 이를 활용한 신의료서비스 분야와 질병의 확대방지와 개인의 건강행동의 촉진과 권장 등을 통한 예방중심의 헬스케어 서비스를 제공하기 위한 사회 공공 헬스케어서비스 체계 구축에 관한 활발한 연구추세를 확인하였다. 본 연구에서 제시한 연구 프레임워크와 연구 동향은 향후 국내의 헬스케어 분야 연구와 헬스케어 정보기술의 실무적 활용에 기여할 것으로 기대된다.

  • 최근 코로나19 바이러스로 인하여 언컨텍트 서비스가 본격적으로 활성화됨에 따라 비대면 접촉 원격 의료 서비스를 제공하기 위한 시스템 개발의 필요성이 증가하게 되었다. 본 연구에서는 원격진료를 지원하기 위한 스마트 헬스케어 시스템인 Rm_She(Remote Medical Smart Healthcare System)을 제안한다. Rm_She는 IoT를 기반으로 생체신호를 감지하는 다양한 헬스케어 제품을 하나의 애플리케이션으로 연결하여 여러 가지의 생체신호 정보를 수집 및 관리할 수 있다. 스마트 폰에 실행되는 헬스체크 앱(HC_app)을 이용하여 여러 종류의 생체신호 측정 장치와 무선랜으로 연결하고, 생체신호 값을 HC_app에서 전송 받아, 사용자에게 측정된 생체신호를 출력하고, 해당 정보를 헬스케어관리서버로 전송한다. 헬스케어 서버에서는 측정값을 전송 받아 데이터베이스에 저장하고, 저장된 측정값은 의료진들이 원격에서 실시간으로 모니터링 할 수 있도록 웹서비스로 제공한다.

범부처 공공보건의료인력 관리조직의 필요성과 과제 (Creation of a central public healthcare human resources management department to coordinate existing agencies)

  • Purpose : The aim of this study is to review the current status of healthcare provision and its human resources administrative management and propose a coordinated human resource management plan for the more efficient operations of healthcare organizations. Methods : We reviewed the literature and held discussions with officials from the United States Department of Health and Human Services to survey United States Public Health Service Commissioned Corps operations. In addition, we surveyed the literature to analyze the current structure and responsibilities of governing bodies involved in public healthcare in Korea. Results : In Korea, there are several administrative offices involved in public health: the Ministry of Health and Welfare, the Ministry of Defense, the Environment Ministry and others. Since these diverse agencies don't integrate their operations, it is difficult to grasp their management of both public healthcare services and their personnel. A potential model is the United States Public Health Service Commissioned Corps, a sub-group of the Department of Health and Human Services and an elite team of highly qualified, public health professionals, which coordinates and manages the overall work and personnel of diverse healthcare organizations. Conclusion : We suggest the establishment of a federal level, public health administrative department of human resource management to centralize and coordinate the existing, disparate healthcare administrative agencies.

Analyzing Service Failure Themes on Online Healthcare Product: Focusing on Online Consumers' Word-of-mouse

  • The emergence of the Internet has provided a new outlet for consumers who experience service failure from products and services, augmenting the traditional options of entry, voice and action. Consumers' negative word of mouth through online (word-of-mouse or eWOM) far exceeds traditional word of mouth (WOM) in respect of its potential effectiveness, speed and spread. This paper tries to figure out the service failure themes in the health care industry by analyzing online word-of-mouse using the critical incidents technique (CIT). Complaint themes in the area of healthcare are identified and analyzed. The results identify that major complaint theme differed according to the site type. Also, the findings indicate that delivery and customer services are critical issues when consumer makes negative WOM.

An U-Healthcare Implementation for Diabetes Patient based on Context Awareness

  • With ubiquitous computing aid, it can improve human being's life quality if all people have more convenient medical service under pervasive computing environment. In this paper, for a pervasive health care application for diabetes patient, we've implemented a health care system, which is composed of three parts. Various sensors monitor both outer and inner environment of human such as temperature, blood pressure, pulse, and glycemic index, etc. These sensors form zigbee-based sensor network. And as a backend, medical information server accumulates sensing data and performs back-end processing. To simply transfer these sensing values to a medical team may be a low level's medical service. So, we've designed a model with context awareness for more improved medical service which is based on ART(adaptive resonance theory) neural network. Our experiments show that a proposed healthcare system can provide improved medical service because it can recognize current context of patient more concretely.

  • This research is studied for investigative purposes of preparation status for healthcare telematics service enforcement via making an analysis of understanding & expectation effect about healthcare telematics introduction. The study is investigated with two groups, professional medical persons (doctors, nurses, pharmacists) and medical demanders (customers), to analyze the recognition difference between two groups. Questions are carried in face to face interviews by using structured questionnaire & Delphi technique. The survey result shows medical demander's expectation level is higher than the other's at all items such as social changes, medical service provider, medical service users, national and government agencies, medical system suppliers.

포괄수가제 확대시행에 따른 의료기관 종사자의 인지도 조사 (The Study on the Recognition of Diagnosis Related Group in Healthcare Workers)

  • This study was conducted in order to survey in healthcare worker's recognition of diagnosis related group(below; DRG) effect from July 1, 2012, to examine their recognition, expectation of the DRG system, and to provide the basic data necessary for the enforcement of the settlement. The subjects of this study were workers at clinics and hospitals sampled from hospital with DRG applying 7 diseases in Busan and Kyung-nam. A questionnaire of survey was conducted with the subjects working at clinics and hospitals from July, 25, 2012 to September 7, 2012, and the subjects were limited to doctors, officers, nurses, medical technicians and nurse assistants, and a total of 618 subjects were enrolled in this study. In the result of this study, generally, the healthcare workers recognized the DRG system. But their knowledge about that was not clear. Expanding enforcement DRG system at the present time, to provide accurate information to the healthcare consumer, workers need to know about DRG system clearly. To this end, for national health policy and medical institutions, workers should be educated constantly about providing medical service as well as the duty of enough explanation about the healthcare consumer's right to know.

대상자가 인지한 의료서비스 질과 재이용 의도와의 관계 (The Relationships Between Patient's Perceived Quality of Healthcare Service and Intention to Re-visit)

  • Purpose: This study was performed to identify the relationship between patient's perceived quality of healthcare services and intention to re-visit. Method: The study subjects were 242 patients from three local clinics. The data were analyzed using SPSS PC+ 12.0 program for descriptive, Pearson's correlation coefficient, and multiple regression analysis. Result: Patients' perceived quality of healthcare services was measured 5.73$\pm$0.71 using the 7-point scale. The patients' perceived quality was moderately correlated with patient satisfaction (r=0.647, p<0.001) and intention to re-visit (r=0.604, p<0.001). Also, patient satisfaction was highly correlated with intention to re-visit (r=0.788, p<0.001). 3) the reliability factor of patients' perceived quality of healthcare services and patient satisfaction were found significant predictors of intention to re-visit. Conclusion: In conclusion, service quality evaluated by patients influenced on their intention of re-visit and especially, reliability was the most significantly influential variable.

  • 자동화된 건강관리 모니터링은 많은 시간이 소요되고, 많은 문제들이 발생할 수 있다. 왜냐하면 정시에 환자들의 건강 상태에 대한 모니터링 정보를 제공받기 어렵기 때문이다 이 논문에서는 임베디드된 건강 관리 서비스가 iSCSI 프로토콜에 의해 제안된다. 이 프로토콜은 자동화된 멀티에이전트와 제한된 자원의 디바이스들(예. PD4 스마트폰, 테블릿PC)의 협력체계에 기반을 두고 있다. 이 둘의 협력체계는 쌍방향 iSCSI 프로토콜에 의해 유지된다. 자동화된 건강관리 제어는 반복되는 방식에서 매우 유용할 수 있다. 완전 집중화된 제어에서는 이 방법이 적용될 수 없다.

  • 헬스케어는 정보통신기술을 활용하여 원거리에서 대상자들의 건강상태에 대한 모니터링과 홈 기반의 보건의료 서비스를 제공하는 것이라 할 수 있다. 따라서 헬스케어 서비스는 응급상황 대처를 위한 경보서비스와, 진료자 및 가족에게 대상자와 댁내 환경을 모니터링 할 수 있는 원격지원 서비스로 분류할 수 있다. 여기서, 일반적인 유선 기반의 고정형 생체계측 시스템에서는 두 가지 서비스를 위해서, 실시간으로 높은 신뢰성을 갖는 데이터를 전송하는데 한계가 있다. 그러므로 대상자가 공간적으로 구속을 받지 않고 자유롭게 이동하면서 생체신호 계측 및 응급경보가 가능하도록, 무선 네트워크 환경에서 구현되는 휴대용 헬스케어 시스템을 구현하고자 한다.
