Changes in Healthcare Utilizations of Cancer Patients since the Launch of KTX (KTX 도입 이후 암환자의 의료이용 변화)

  • We examine the change in healthcare utilization pattern of cancer patients since the launch of KTX in 2004. It is found that during the year of 2004 and 2005 the rapid increase in the ratio of cancer patients' healthcare utilizations for the tertiary hospitals in Seoul to the total healthcare utilizations has accompanied the rapid decrease in the corresponding ratio for the tertiary hospitals in the local metropolitan areas under the influence of KTX while the corresponding ratio for the tertiary hospitals in the local cities with little influence of KTX on them has exhibited mild change. Since healthcare consumers' choice of hospitals can be characterized by "foot voting" action in the healthcare service market in Korea, such a phenomenon may have strong implication that the introduction of KTX may have affected substantially the inter-area healthcare utilization pattern by cancer patients by reducing various sorts of long-distance travel costs. Therefore, considering the potential contribution of KTX to the increase of cancer patients' accessibility to hospitals with higher qualities, support policies such as fare discount for low-income cancer patients in local areas may need to be taken into consideration where the well-designed fare discount program for low-income cancer patients may increase utilizations of KTX by lowincome cancer patients without incurring additional costs to running of KTX.

The Association between Social Support and Health Behaviors for Metabolic Syndrome Prevention among University Students: The Mediating Effect of Perceived Stress (대학생 집단에서 사회적 지지와 대사증후군 예방 건강 행동 간의 상관관계: 지각된 스트레스의 매개효과)

  • Purpose: Health behaviors for metabolic syndrome (MetS) prevention should be emphasized from early adulthood. There is little information on psychosocial factors associated with health behaviors for MetS prevention. The aim of this study was to determine whether there would be a mediating effect of perceived stress on the association between social support and health behaviors for MetS prevention among university students. Methods: This cross-sectional and correlation study was conducted with 502 university students in South Korea. Social support, perceived stress, and lifestyle evaluation for metabolic syndrome scales were used. Online questionnaire survey was conducted between November and December 2019. The mediating effect of social support on health behaviors for MetS prevention was analyzed using PROCESS macro program with bootstrapping method to test our hypotheses. Results: Social support directly influenced perceived stress (β=-.35, p<.001) and health behaviors for MetS prevention (β=.14, p=.002). Health behaviors for MetS prevention was indirectly influenced by perceived stress (β=-.25, p<.001). The size of indirect effect of social support on health behaviors for MetS prevention was 0.06. Conclusions: The association of social support and health behaviors for MetS prevention was partially mediated by perceived stress among university students. Therefore, a university-based nursing intervention should comprise social support strategies with stress management to promote health behaviors for MetS prevention.

A Study on Designation and Management of Seoul-Type Elderly Care Facilities (서울형 노인요양시설 지정 및 운영을 위한 정책 연구)

  • Purpose: The purpose of this study to assess long-term care facilities in Seoul and certificate Seoul-type elderly care facilities. After certification, Seoul city can provide certificated facilities with budget and support to improve service quality. Methods: Questionnaires to elderly care facilities experts and field surveys to facilities in Seoul have been conducted for the data collection. Results: The result of this study can be summarized into three points. The first one is that a direction of operation for Seoul-type elderly care facilities is to improve manpower and provide special program for enforcing quality assurance. The second one is that this study provide index of evaluation for Seoul-type elderly care facilities. The indexes include management, facility environment and safety, service delivery process, and service results. The third one is that management plan for this policy should be carried out. The management plan include demonstration project, opinion research. Implications: Seoul-type elderly care facilities can have positive effects on quality of service. For the introduction of this policy, Seoul city and autonomous Gu have to make careful preparations for this policy.

Environmental Affordance of a Well Recognized Senior Center of Japan (일본의 노인종합복지관 성공사례의 환경지원성 분석 연구)

  • According to the 2005 Korean census, the 65 and over population now exceeds 9.5% of the total population and is growing rapidly. Meeting elderly environment and care needs of this rapidly growing segment of the population becomes a major challenge for public policies and planners. Since great deal amount of elderly will reside in their houses, aging in place concept becomes important. For the success of aging in place, the quality of individual house unit, community support systems, and quality of senior center of the community are crucial. Since elderly environments and facilities serve not only medical and/or care programs but also social activity program in aging society, senior centers need to promote social activities and other care programs. The purpose of this study is to analyze characteristics of environmental affordance of a well received senior center in Tokyo, Japan. In order to analyze the characteristics, Murtha & Lee user benefit criteria and Lawton's environmental affordance approaches were used. As results, design characteristics and interior elements which provide environmental affordance were enumerated by type of space. Based on needs and user benefit criteria, those features were analyzed. This study shows design characteristics, elements, and attributes which are well received and utilized by elderly users.

Influences of Working Conditions and Health Status on Absence due to Sickness in Health and Medical related Workers (보건·의료 관련직 근로자의 근로환경과 건강상태가 질병결근에 미치는 영향)

  • Purpose: This study was to examine the influences of working conditions and health status on absence due to sickness in health and medical related workers. Methods: Data from the Third Korean Working Condition Survey was used. Study participants included 929 workers who have been working in the medical and healthcare fields. Descriptive analysis, independent t-test, ${\chi}^2$ test, Fisher's exact test and logistic regression analyses were performed using SPSS 21.0 statistical program. Results: Absence due to sickness was found in 7.8% of health and medical related workers. Absence due to sickness differed significantly depending on monthly income, company size, work-related musculoskeletal risk factors, job control, physical health problems and subjective health status would be important factors affecting absence. Conclusion: This study provides evidences that predictors of absence due to sickness in workers of medical and healthcare fields are identified. Therefore, we need the active support for health care service promotion and prevention for predictors of absence due to sickness.

A Study of School Doctor Program for Dysmenorrhea in Korean Medicine (월경통 한의 교의 사업에 관한 연구)

  • Objectives This study aims to investigate the effect of the Korean medicine in treating dysmenorrhea as a part of school health program (SHP) and satisfaction rate of the participated students in this program. With this study, we tried to contribute to the development of health promotion program of Korean medicine. Methods As part of the 'Korean Medicine Health Promotion Project', we conducted a dysmenorrhea program at one of many high schools in Eumseong-gun. From April to June 2018, we conducted the promotion program a total of 7 times and analyzed the questionnaire regarding satisfaction and the monthly experience of menstruation. Results The study participants' grade distribution was $2^{nd}$ grade 32, $3^{rd}$ grade 23 with a total 55 students. Mean dysmenorrhea VAS was $6.07{\pm}2.10$, and the mean number of visits to the dysmenorrhea program was $3.61{\pm}1.47$. There were no significant differences in the average number of visits between grades. Overall satisfaction was $4.06{\pm}0.97$, teacher satisfaction was $4.29{\pm}1.10$, and lecture satisfaction was $4.18{\pm}0.95$. Dysmenorrhea decreased but was not statistically significant. Conclusions Korean medicine is appropriate for the health promotion program. With cooperation and support from local schools, it will be very possible to carry out a successful business. It is also necessary to organize the program according to the viewpoint of youth and to interact with the students.

A Study on the Importance of Works Perceived by Quality Improvement Coordinators and Their Current Work Performances (질 관리 실무자가 인지하는 업무중요도와 수행업무에 관한 연구)

  • Background : This is an investigative study to analyse the importance of works perceived by Quality Improvement(QI) Coordinators and to evaluate their current work performances using the questionnaires developed by the investigator. Methods : The data were collected from 37 subjects over two weeks period from Oct. 1 to Oct. 17, 1998 and analysed by the descriptive statistics of SPSS program. The items of questionnaire are consisted of 13 work domains including 73 activities based on Quality Management(QM) Coordinator's job description of National Association for Healthcare Quality:data collection & analysis, communication, monitoring, evaluation, accreditation, tool development, policy development, program development, self development, education & trainning, system design, planning, and consultation & support. Results : 1) Of the performances in 13 work domains, the frequencies of the work performed were accreditation(89%), planning(88%), communication(83%), data collection & analysis(82%), monitoring(76%), policy development(72%), consultation & support (71%), education & trainning(70%), self development(68%), evaluation(63%), tool development(61%), program development(44%) and system design(43%) in order. 2) For the importances (1=not important, 5=very important), the policy development(4.46) scored highest then monitoring(4.42), planning(4.41), education & trainning(4.38), communication(4.35), evaluation(4.34) tool development (4.30), data collection & analysis(4.29), program development(4.22), consultation & support(4.22), accreditation(4.15), self development(4.05) and system design(3.98) in order. 3) There was a difference between the work performance and the perceived importance. The results showed the low performances in policy development, monitoring, education & trainning and evaluation which ranked high by the perceived importance and the high performances in accreditation, data collection & analysis, self development, communication and consultation & support which ranked middle to low by the perceived importance. 4) The reasons for low performances of QI Coordinators were the lack of clear assignment for the responsibility and allowed authority for work to QI Coordinators(30.8%), insufficient member of QI Coordinators(13.9%), lack of hospital director's interest(11.5%), low motivation of QI Coordinators (10.6%) and insufficient knowledge & experience of QI Coordinators (8.8%). Conclusion : Most works were perceived important by QI Coordinators, but there was a difference in the work performance. The works performed over 70% were related with accreditation, data collection & analysis, communication, planning and monitoring, on the other hand under 50% in performances were related with system design, program development, tool development and evaluation.

Impact of a 'Proactive Self-Audit Program of Fraudulent Claims' on Healthcare Providers' Claims Patterns: Intravenous Injections (KK020) (부당청구 예방형 자율점검제가 의료기관의 청구행태에 미치는 영향: 정맥 내 일시주사(KK020)를 중심으로)

  • Background: This study aims to examine changes in fraudulent claim counts and total reimbursements before and after enhancements in counterfeit claim controls and monitoring of provider claim patterns under the "Proactive self-audit pilot program of fraudulent claims." Methods: This study used the claims data and hospital information (July 2021-February 2022) of the Health Insurance Review and Assessment Service. The data was collected from 1,129 hospitals assigned to the pilot program, selected from the providers who filed a claim for reimbursement for intravenous injections. Paired and independent t-tests, along with regression analysis, were utilized to analyze changing patterns and factors influencing claim behaviors. Results: This program led to a reduction in the number of fraudulent claims and the total amount of reimbursements across all levels of hospitals in the experimental groups (except for physicians below 40 years old). In the control group, general hospitals and hospitals demonstrated some significant decreases based on the duration since opening, while clinics showed significant reductions in specified subjects. Additionally, a notable increase was observed among male physicians over the age of 50 years. Overall, claims and reimbursements significantly declined after the intervention. Furthermore, a positive correlation was found between hospital opening duration and claim numbers, suggesting longer-established hospitals were more likely to file claims. Conclusion: The results indicate that the pilot program successfully encouraged providers to autonomously minimize fraudulent claims. Therefore, it is advised to extend further support, including promotional activities, training, seminars, and continuous monitoring, to nonparticipating hospitals to facilitate independent improvements in their claim practices.

CDISC Transformer: a metadata-based transformation tool for clinical trial and research data into CDISC standards

  • CDISC (Clinical Data Interchanging Standards Consortium) standards are to support the acquisition, exchange, submission and archival of clinical trial and research data. SDTM (Study Data Tabulation Model) for Case Report Forms (CRFs) was recommended for U.S. Food and Drug Administration (FDA) regulatory submissions since 2004. Although the SDTM Implementation Guide gives a standardized and predefined collection of submission metadata 'domains' containing extensive variable collections, transforming CRFs to SDTM files for FDA submission is still a very hard and time-consuming task. For addressing this issue, we developed metadata based SDTM mapping rules. Using these mapping rules, we also developed a semi-automatic tool, named CDISC Transformer, for transforming clinical trial data to CDISC standard compliant data. The performance of CDISC Transformer with or without MDR support was evaluated using CDISC blank CRF as the 'gold standard'. Both MDR and user inquiry-supported transformation substantially improved the accuracy of our transformation rules. CDISC Transformer will greatly reduce the workloads and enhance standardized data entry and integration for clinical trial and research in various healthcare domains.

Derivation of Rural Service and Infrastructure Factor Considering Urban-to-rural Migration - Focus on Welfare and Culture - (도시민 유입을 고려한 농촌 서비스 및 인프라 개선 주요 항목 도출 - 복지문화 요인을 중심으로 -)

  • This study was attempted to grasp the push-pull factors of urban to rural migrants in relation to services and infrastructure of rural welfare and culture. Online and offline survey were conducted for urban residents who were willing to return to the rural area and those who were already returning to the rural area. In the fields of health care, social welfare, and education, it was found that the satisfaction level of urban-to-rural migrants was relatively higher than those of latent urban-to-rural migrants. In the field of culture, leisure and sports, the level of satisfaction and concern were similar, so it was found that the expectations for the field were relatively high before returning rural area. As for the agreement degree to expand support, the demand for emergency medical facilities and dementia care program was the highest, and the demand for health care was found to be relatively high. The results of the survey on the top priority items indicate that latent urban-to-rural migrants require support of facility and space item and expansion of program item, and urban-to-rural migrants have high demand for expansion of program item in all field except healthcare field. The results of this study are expected to provide useful information for establishing the direction of the rural area development project in connection with the revitalization of policy of people return to rural area.
