ㆍ These are not accidents ㆍ They are “unplanned incidents” that had multiple, obvious causes(both minor and egregious) ㆍ Prevention would have involved no more than good safety and good management practices ㆍ The outcome was predicable, and included loss of life, loss of prduction capacity, civil lawsuits, and OSHA citations.
The purpose of this study was focus on development of educational objectives by using oral health education in the elementary school from now on. Based on the results of analysis compared with a model of educational objectives and contents by reflecting the programs for analysis were the oral health education programs of the Ministry of health and social Affairs in Korea, and the programs of two states in u.s. in order to construct goals and contents of oral health education in school. The results in this study can be arranged as follows: First, an analysis on the educational objectives of dental health education program resulted that, from a viewpoint of Bloom's taxonomy of educational objectives, in case of domestic, most learning objectives expressed in the handbook of Ministry of Health and social Welfare were given too much emphasis to the cognitive domain of 37 an items in all. Affective domain were appeared the lack of balance of the development domain. Second, the result of analysis for the programs of two states of America, the cognitive domain were the most abundant of 48 an items in all, and were not significantly different to those of Korea in the form of objectives domain or statement. However a big different was shown that they organized the same objectives domain as a pattern of repeating and deepening at every grade. Third, in this study, 10 educational objectives of dental health education were established, based on the result of analyzing those existing educational programs as central figure. Those are oral cavity, brushing, nutrition of teeth, fluoride, flossing, teeth injury, dental decay, periodontal disease, malocclusion, routine dental visits. And general objectives and specific objectives of each learning area were stated and arranged grade by grade.
Purpose: This study was considered in order to identify the factors affecting healthcare utilization by Asian immigrants in the United States. Methods: From February to April 2011, a descriptive survey study was conducted in a convenience sample of 250 Korean and Asian Indian immigrants aged between 40 and 64 in the Triangle area of North Carolina. An author-developed instrument was used to assess predisposing, enabling, and need factors according to Anderson's Behavioral Model of Health Services. Utilization Data analysis was performed by $X^2$-test, t-test, and binary logistic regression. Results: Participants' healthcare services experiences were significantly different when they had a longer stay in the U.S., had been employed, had higher income, were Asian Indians, had better English-speaking skills, better health status, more knowledge of health system and health insurance, had higher satisfaction with the healthcare system, and when they were taking prescribed medications and having health insurance. The strongest association with experience of healthcare services was having health insurance with an adjusted odds ratio (OR) of 15.37 (95% CI 4.95-47.71, p<.001) and self-reported English proficiency (OR=1.99, 95% CI 1.00-3.96, p=.05). Conclusion: Intervention strategies to increase accessibility to healthcare services should focus on these significant predictors.
선행연구들은 어머니들의 적극적인 건강정보 탐색자로서의 역할, 가족들의 건강을 돌보는 매니저로서의 역할에 주목해왔다. 그러나 미국 내에 거주하는 한인 이민자 어머니들의 건강정보 필요성이나 탐색 행동에 관한 연구는 미비하다. 본 연구는 커뮤니티를 기반으로 한 설문 연구로서, 한인 이민자 어머니들의 건강정보 필요성과 탐색에 대한 행동을 알아보고자 하였다. 만0세부터 10세의 한인 어머니들 사이에서 가장 중요하다고 여겨지는 건강 주제들은 예방접종, 치아건강, 육아, 영양/식단, 그리고 운동 등으로 밝혀졌다. 또한 특정 주제들에 한하여 아이들의 성별과 나이에 따라 어머니들의 건강 정보 필요성이 다르다는 것을 알 수 있었다. 마지막으로, 미국뿐만 아니라 한국의 정보 전문가들이 건강 정보 서비스를 개발하는데 있어서 염두에 둬야 할 점들을 제시하고 있다.
이 연구는 미국 에너지 산업의 두 환경 정책 적용 아래 이산화황 농도를 평가하기 위하여 계산가능한 부분적 균형(computable partial equilibrium) 모형을 이용한다. 배출 최고한도(emission caps) 정책과 교환기능 허가(tradable allowances) 정책은 동일한 방출 총계를 산출하기 위해 선택되었다. 두 정책을 통해 상당히 다른 지역간 농도차를 확인할 수 있다. 이러한 편차는 개별 주 및 전국적으로 건강 손상의 총계로 있는 수 억의 달러의 다른 형태의 손실로 변환된다. 배출 최고한도 정책이 교환가능 허가 정책을 4억 5,200만 달러만큼 능가한다. Caps는 다른 정책보다는 8억 4,000만 달러 적은 피해를 전달하는 남서지역 중남부와 남동 국가를 선호한다. 그러나 북부와 북동 국가에 3억 9,000만 달러 더 높은 피해를 전달한다.
The purpose of study was to examine the effect of nonnutritive sucking(NNS) on behavioral state in preform infants. Fourteen infants (gestational age 28-35 weeks, M=31.86 ; birth weights 1095-2275g, M=1694g) admitted to NICU, serving as their controls were randomly administered 5 min of nonnutritive sucking and a control condition. Behavioral state was measured using the Anderson Behavioral State Scale(ABSS). Heart rates were obtained for each infant before and during NNS and bottle feeding. Data collecction was done by 3 experienced nurses in NICU 2 times a day for conseculive 4 days for each subject. Interrater reliability were .80-.90. The findings were as follows : 1. Preform infants averaged active restless states 58.93% in the prointervention phase and 27.32% in the NNS phase. Inactive awake states were 3.57% in the preintervention phase and 34.64% in the NNS phase. 2. There were significantly more positive changes to inactive awake states for the NNS intervention, Z=-2.35, p=.01. 3. There was no statistically significant difference in heart rate change between NNS intervention and control condition, z=-1.15, p=.24. 4. The rates of feeding success determined by finishing prescribed amount of milk by bottle feeding within 15 minutes were 83.94% in NNS intervention and 89.29% in control condition. The findings from this study confirmed that NNS is effective for behavioral state modulation and inducing optimal state for feeding in preform infant. The NNS intervention in nursing practice may help the transition of preform infants for nipple feeding.
The effects of indigestible dextrin on serum lipid and glucose concentrations, large intestinal functions, feces states, and gastrointestinal transit time were studied with 90 male Sprague-Dawley rats. Diets contained 0.5% cellulose(0.5CL control), 10% cellulose(10CL), and 10% indigestible dextrin I (10ID-I), respectively and were fed to the rats for 3 weeks. Serum cholesterol and LDL-cholesterol concentrations were lower in rats fed the 10ID-I diet. Serum triglyceride concentration was lower in rats fed the 10CL diet. Cecal content and cecum weight significantly increased in the rats fed the 10ID- I diet. Cecal pH of the rats fed the 10ID-I diet was lower than that of the rats fed the 0.5CL diet. Changes of fecal output were not observed in case of 0.5CL and 10ID-I diets but a significant increase was observed in the case of 10CL. Gastrointestinal transit time of 10ID-I was the longest among the three diets. These results demonstrate that indigestible dextrin delivered to the cecum was not digested and increased the cecal mass. Indigestible dextrin improves colonic health of rats by inducing low pH of the cecum. (Korean J Nutrition 31(6) 991-998, 1998)
Purpose: The purpose of this study was to identify predictors of drinking problems in Korean international students in the United States. Methods: The subjects involved this study were 190 Korean international students in the United States. Data were measured using a drinking problems questionnaire, an acculturative stress scale for international student, and the CES-D. The collected data were analyzed using the SPSS/WIN 17.0 program to calculate mean, standard deviation, ANOVA, Sheff$\acute{e}$ test and pearson's correlation coefficient. and stepwise multiple regression. Results: 1) The mean score of the degree of perceived acculturative stress was $2.15{\pm}0.63$, of depression was $17.67{\pm}9.85$, and of drinking problems was $15.03{\pm}15.52.2$) There were significant correlations between acculturative stress and drinking problems (r=.506, p<.001), between depression and drinking problems (r=.6726, p<.001), and between acculturative stress and depression (r=.453, p<.001). 3) In stepwise multiple regression analysis the most powerful predictor of drinking problems were acculturative stress (${\beta}$=.243, p<.001), and depression (${\beta}$=.562, p<.001). Conclusion: Based on the results, practical implications for reducing the acculturative stress and improving the psychological well-being of Korean students were discussed.
It has been 5 years since evaluation systems were introduced to the social welfare field in Korea. More than half of social welfare agencies (617 out of 1,267) were evaluated in the past two years. Most areas of the agencies were the target of an extensive evaluation in 2000. As "the Evaluation Era" has come, problems were raised especially regarding the Accreditation system of the Ministry of Health and Social Welfare. More specifically, the problems were related to operation and management of the evaluation systems including the issue of evaluators' objectivity and fairness. In addition, evaluation costs have been too high to the government. The evaluation systems of OECD nations including the United States were quite different. These nations have used the Performance Measurement in order to secure the objectivity and the fairness. Although the quantity of information was limited compared to that of the Accreditation system, these nations have taken the lead in the governmental evaluation. In this context, this study compared social welfare evaluation systems between Korea and the United States. It presented the current status and problems of the Accreditation system of the Ministry of Health and Social Welfare, discussed the American Performance Measurement and its limitations, and compared the strengths and weaknesses of the two evaluation systems. In addition, this study suggested the strategies of Korean evaluation systems in the long term as well as in the short term.
The tripartite mission of 'academic medicine' is education, research, and patient care. Academic medical centers (AMCs) are carrying out the mission and ultimately aiming to improve the health of people and communities. Globally, AMCs are facing a tremendous financial risk stemming from the changes in health insurance reimbursement plans and a shortage of human resources. Innovative AMCs in the United States are trying to transform their physician-centered, and siloed structure into a patient-centered, and integrated structure. They are also building integrated systems with primary healthcare groups to provide continuous patient care from primary to tertiary levels and making strategic networks based on value-based payment and the patient-centered model. These changes have been proven to improve outcomes of patient care and increase fiscal revenues, which are both crucial in supporting education and research. To address the shortage of human resources, programs are being built to develop newly appointed faculty for the future. AMCs have different approaches to bringing changes into their organizations; however, there is a common emphasis on 'a patient-centered approach,' which helps them set more explicit organizational values and make strategic decisions based on their values. Korean AMCs are facing similar challenges to AMCs in the United States in spite of many differences between the countries' healthcare systems. The innovative efforts of AMCs in the United States to address the challenges will be helpful, well-worked examples for Korean AMCs with similar challenges.
