Background & Objectives: Fixed-dose combinations (FDCs) offer advantages in adherence and cost-effectiveness compared to free combinations (FCs), but they can also complicate the prescribing process, potentially leading to therapeutic duplication (TD). This study aimed to identify the prescribing patterns of FDCs for dyslipidemia and investigate their associated risk of TD. Methods: This was a retrospective cohort study involving drugs that included statins, using Health Insurance Review & Assessment Service-National Patient Sample (HIRA-NPS) data from 2018. The unit of analysis was a prescription claim. The primary outcome was TD. The risk ratio of TD was calculated and adjusted for patient, prescriber, and the number of cardiovascular drugs prescribed using a multivariable Poisson model. Results: Our study included 252,797 FDC prescriptions and 515,666 FC prescriptions. Of the FDC group, 46.52% were male patients and 56.21% were aged 41 to 65. Ezetimibe was included in 71.61% of the FDC group, but only 0.25% of the FC group. TD occurred in 0.18% of the FDC group, and the adjusted risk ratio of TD in FDC prescriptions compared to FC was 6. 44 (95% CI 5. 30-7. 82). Conclusions: Prescribing FDCs for dyslipidemia was associated with a higher risk of TD compared to free combinations. Despite the relatively low absolute prevalence of TD, the findings underline the necessity for strategies to mitigate this risk when prescribing FDCs for dyslipidemia. Our study suggests the potential utility of Clinical Decision Support Systems and standardizing nomenclature in reducing medication errors, providing valuable insights for clinical practice and future research.

We need to establish an informative guideline to increase inter-institutional and inter-observer reproducibility of renal transplant diagnosis, and to improve the diagnostic ability of pathologists in Korea. A first nation-wide survey for renal transplant pathology was conducted by Renal Pathology Study Group of the Korean Society of Pathologists in 2016, to provide the continued excellence in the transplantation pathology laboratory, and to improve the diagnostic ability for the best treatment of transplant patients. This survey revealed the significant variations in scale, work load and biopsy indications for the renal transplant pathology in various institutions in Korea. The Banff classification were used by all institutions for the diagnosis of renal transplant pathology, but different formats were used: most institutions (70%) used the "2013 Banff classification" while the others were using "2007 Banff classification" (20%) or even older formats. In daily diagnostic practice of the renal allografts, difficulties that pathologists encounter were quite diverse due to different environments they work in. Most respondents agreed that standardized diagnostic practice guidelines, regular education on renal transplant pathology and convenient ways of consultation are further needed. We are currently working toward the enhancement of the expertise of renal pathologists and to increase inter-institutional and inter-observer reproducibility by 1) development of a set of virtual slides of renal allograft biopsies for the training, 2) validation and gathering expert's consensus on the core variables of rejection diagnosis by using virtual slides, and 3) continued education by the developed virtual slide atlas.

Due to the multiple ecosystem benefits that iconic large, old growth trees provide, forest managers are applying thinning treatments around these legacy trees to improve their vigor and reduce mortality, especially in the face of climate change and other forest health threats. One objectives of this study was to analyze sub-hourly stem fluctuations of legacy ponderosa (Pinus ponderosa Dougl. Ex P. & C. Laws) and sugar pines (Pinus lambertiana Dougl.) in the mixed-conifer forests of the Sierra Nevada in multiple different radius thinning treatments to assess the short-term effects of these treatments. Thinning treatments applied were: R30C0 (9.1 m radius), R30C2 (9.1 m radius leaving 2 competitors), and RD1.2 (radius equaling DBH multiplied by 1 ft/in multiplied by 1.25). The other objective was to assess climatic drivers of hourly stem fluctuations. Using the dendrometeR package, we gathered daily statistics (i.e. daily amplitude) of the stem fluctuations, as well as stem cycle statistics such as duration and magnitude of contraction, expansion, and stem radial increment. We then performed correlation analyses to assess the climatic drivers of stem fluctuations and to determine which radial thinning treatment was most effective at improving growth. We found an important role that mean solar radiation, air temperature, and relative humidity play in stem variations of both species. One of the main findings from a management perspective was that the RD1.2 treatment group allowed both species to contract less on warmer and higher solar radiation days. Furthermore, sugar pine put on more stem radial increment on higher solar radiation days. These findings suggest that the extended radius RD1.2 thinning treatment may be the most effective at releasing legacy sugar and ponderosa pine trees compared to the other forest management treatments applied.
Purpose: For mothers just delivered of a child, sanhupung(産後風) is a critical disease which can threaten their health. The symptoms of sanhupung were diverse and divided into pain symptoms, general symptoms, and neuropsychiatric symptoms. We tried to search for the relationship between sanhupung and somatization of postpartum depression. Methods: Analysis was carried out with materials concerning the postpartum depression and sanhupung. For the analysis, materials related with the issue of this study were searched at the Korea Education & Research Information Service, and Oriental medicine advanced searching. Results: 1. The range of period and symptoms of sanhupung has been broaden. The symptoms of sanhupung were divided into pain syndrome, general symptoms, and neuropsychiatric symptoms. 2. As the symptoms of postpartum depression might indicate not only physical symptoms but also mental disorder, more attention should be placed on wether the symptoms were involved in somatization of postpartum depression. Conclusion: The authors conclude that obstinate sanhupung that do not healed up and improve in symptoms have a relation to somatization of postpartum depression.
Most advanced countries that are members of the World Physiotherapy have established a 4-year education system or specialized graduate school system for physical therapists based on national standards. They have also expanded their laws and systems to provide physical therapists with the autonomy and independence to offer services in their clinics. However, compared with developed countries in North America and Europe, there are issues with the autonomy and independence of physical therapists in Korea related to national regulations. Social status and recognition of the profession are also lagging. Korea is expected to become a super-aged society by 2025. To reduce the financial burden of healthcare and welfare on the government, it is necessary to extend the time spent by older adults on independent activities and minimize their time spent using medical services. To achieve this goal and maximize the active life of older adults, a plan to efficiently use licensed physical therapists in the country should be prepared. Korea should increase the license utilization rate of physical therapists to reduce waste at the national level and increase the professional hope of the younger generations of physical therapists. To create a healthcare policy focusing on the use of physical therapy personnel, similar to that in advanced countries, it is necessary to unify educational systems and produce excellent physical therapists. Providing professional autonomy can help physical therapists develop a sense of job satisfaction. Outstanding talent will choose physical therapy as a profession if they can see hope for their future careers, and if physical therapy services in Korea are similar to those delivered in advanced countries, physical therapy in Korea can develop into a healthcare service that people desire.
BACKGROUND/OBJECTIVES: Coffee is a complex chemical mixture, with caffeine being the most well-known bioactive substance. The immunomodulatory and anti-inflammatory properties of coffee and caffeine impact health in various aspects, including the respiratory system. The objective is to investigate the effects of coffee and caffeine on airway hyperresponsiveness and allergic reactions, as well as to analyze and compare associated cytokine profiles. MATERIALS/METHODS: BALB/c mice were intraperitoneally sensitized with ovalbumin (OVA) and given OVA inhalation to induce airway hypersensitivity. Two weeks after sensitization, they were intragastrically gavaged with coffee or caffeine, both containing 0.3125 mg caffeine, daily for 4 weeks. Control mice were fed with double-distilled water. Serum OVA-specific antibody levels were measured beforehand and 5 weeks after the first gavage. Airway hyperresponsiveness was detected by whole body plethysmography after gavage. Cytokine levels of bronchoalveolar lavage and cultured splenocytes were analyzed. RESULTS: Coffee effectively suppressed T helper 2-mediated specific antibody response. Airway responsiveness was reduced in mice treated with either coffee or caffeine. Compared to the control, coffee significantly reduced OVA-specific immunoglobulin (Ig) G, IgG1 and IgE antibody responses (P < 0.05). Caffeine also attenuated specific IgG and IgG1 levels, though IgE level was unaffected. Coffee significantly reduced interleukin (IL)-4 and increased IL-10 concentration in spleen cells and bronchoalveolar lavage fluid (P < 0.05). CONCLUSIONS: Coffee effectively attenuated airway hyperresponsiveness and systemic allergic responses induced by OVA food allergen in mice. As a complex composition of bioactive substances, coffee displayed enhanced immunomodulatory and anti-inflammatory effects than caffeine.
Wetlands in the living area are important ecological resources that are the basis for the daily life or farming activities of local residents, and have high ecological value such as ecosystem services and green infrastructure. This study was carried out to develop a functional evaluation methodology optimized for small-scale wetlands(RAMS). Based on on-site evaluation by experts, surveys and in-depth interviews, four functional items, including biodiversity, health, hydrophilic culture and ecology, water circulation, and carbon absorption, and 15 detailed indicators, and the evaluation grade for each detailed indicator, were developed on a 5-point scale. The evaluation methodology optimized for small-scale living areas wetlands (RAMS) proposed as a result of this study can be used as basic data for conservation and restoration and management of small-scale living areas wetlands at home and abroad.

Background: As unpredictable work schedule (UWS) has increased worldwide, various studies have been conducted on the resulting health effects on workers. However, research on the effect of UWS on workers' well-being in Korea is still insufficient. This study aimed to investigate the relationship between UWS and work-family conflict (WFC) using 6th Korean Working Conditions Survey (KWCS). Methods: Both UWS and WFC were measured using self-reported questionnaires, using data from the 6th KWCS conducted between 2020 and 2021, including 31,859 participants. UWS was measured by questions regarding the frequency of changes in work schedules and limited advanced notice. WFC was measured by questions regarding work to family and family to work conflicts. Logistic regression analysis was conducted to investigate the association between UWS and WFC. Results: The prevalence of UWS was higher among men, those under 40 years old, service and sales workers and blue-collar workers, and those with higher salaries. Workplace size also influenced UWS prevalence, with smaller workplaces (less than 50 employees) showing a higher prevalence. The odds ratio (OR) for WFC was significantly higher in workers with UWS compared to workers without UWS after adjusting for gender, age, marital status, occupation, salary, education, weekly working hours, shift work, company size, and having a child under the age of 18 years, employment status (OR: 3.71; 95% confidence interval: 3.23-4.25). Conclusions: The analysis of nationwide data revealed that UWS interferes with workers' performance of family roles, which can lead to WFC. Our findings suggest that it is crucial to implement policies to address unfair work schedule management, promoting a healthier work-life balance and fostering a conducive environment for family responsibilities.

Background: Recently, use of work-related communication technology-smartphones, tablets, and laptops-is increasing rapidly by development of technology with the coronavirus disease 2019 pandemic. Some studies have suggested that work-related communication technology has a significant link with work-family conflict (WFC) but these studies included only limited number of participants and lacked essential covariates. Therefore, this study analyzes this association using large representative data sample and selected waged workers who were married-couples with children. Methods: This study was conducted based on data from the 6th Korean Working Conditions Surveys (KWCS). A total of 17,426 waged workers having a marriage partner and one or more children were selected. Logistic regression analysis was performed to determine whether WFC was associated with communication technology use. The odds ratios (ORs) for WFC were stratified by sex and working hours. Results: In fully adjusted model, WFC was higher those who used communication technology outside regular working hours compared with those who did not use it (OR: 1.66; 95% confidence interval [CI]: 1.39-1.97). When stratified by sex and working hours, the effect was greater in women than in men (OR: 1.79; 95% CI: 1.42-2.26 vs. OR: 1.52; 95% CI: 1.17-1.97) and women who worked over 52 hours per week had the highest OR (3.40; 95% CI: 1.25-9.26). Conclusions: This study revealed that the work-related communication technology use outside regular working hours was associated with WFC. The association were greater among those having longer working hours and female workers. These results suggest that appropriate policy should be implemented to reduce working hours and right to disconnect after work, particularly in female workers.

Background: Computed tomography (CT) scans in the Republic of Korea have spiked, reaching approximately 9 million examinations annually in 2017. CT provides significant medical benefits, but radiation exposure remains a concern. This study aims to analyze CT scans in Korea, as a basis study for radiation safety management. Materials and Methods: The raw data of total CT scans was obtained from the Health Insurance Review & Assessment Service and analyzed by CT scan type, patients' age and sex, and medical facility type. CT scans trends were analyzed considering the disease incidence. Results and Discussion: In 2017, CT scans accounted for 8,977,300. Usage per capita was 0.18 in males and 0.17 in females. CT scans increased with age until the 50- to 59-year-old groups, then decreased. CT scans were high in abdominal/pelvic (35%), chest (26%), and head (22%) regions due to higher disease rates. Head CT was most frequently used for infants and children. Abdominal/pelvic, chest, and spine CT were more frequent for older groups. The CT scans in the upper and lower extremities was high in child and juvenile groups. Chest and abdominal/pelvic CTs were higher in males, whereas spine, whole spine, and CT densitometry were higher in females. The proportion of CT scans of tertiary and general hospitals, hospitals, and clinics accounted for ≥80%, 13%, and 5%, respectively. Abdomen/pelvis, chest, and head/neck CTs were mostly conducted in tertiary and general hospitals, spine CT in hospitals and clinics, extremity CT in hospitals, and CT densitometry in clinics. Conclusion: The trend of CT scans varied based on the incidence rate for each patient's sex and age, and serious illness diagnosis by medical facility type. The results of this study provide data and guidance for evaluating the radiation exposure of the Korean population by CT and developing management policies for medical radiation safety.
