농업인을 위한 학습자 참여형 안전보건 교육 프로그램 설계 (A Study on Developing the Participatory Educational Program of Agricultural Safety and Health for Farmers)

  • The purpose of this study was to develop a participatory education program of farmer's safety and health. After investigating the concept and status of agricultural safety and health to identify the problem of farmer's safety and health, we made the model in 3 steps, consist of analysis, design, and development, based on the instructional system design and the participatory instructional method. We also applied the participatory instructional method to the program to facilitate a change in practice by making farmers understand the contents. The findings of this study revealed the followings: 1) we developed the education program focused on characteristics of farmers as learners; 2) the education program was developed based on the model for developing education programs and the specific procedures that we made up; 3) we designed the program in modules to increase the availability; 4) we raised the content validity of the learning contents in the safety and health education program targeting farmers; 5) manuals and supplementaries for teachers were developed from learner's textbooks.

Low-Molecular-Weight Collagen Peptide Ameliorates Osteoarthritis Progression through Promoting Extracellular Matrix Synthesis by Chondrocytes in a Rabbit Anterior Cruciate Ligament Transection Model

  • This study examined whether the oral administration of low-molecular-weight collagen peptide (LMCP) containing 3% Gly-Pro-Hyp with >15% tripeptide (Gly-X-Y) content could ameliorate osteoarthritis (OA) progression using a rabbit anterior cruciate ligament transection (ACLT) model of induced OA and chondrocytes isolated from a patient with OA. Oral LMCP administration (100 or 200 mg/kg/day) for 12 weeks ameliorated cartilage damage and reduced the loss of proteoglycan compared to the findings in the ACLT control group, resulting in dose-dependent (p < 0.05) improvements of the OARSI score in hematoxylin & eosin (H&E) and Safranin O staining. In micro-computed tomography analysis, LMCP also significantly (p < 0.05) suppressed the deterioration of the microstructure in tibial subchondral bone during OA progression. The elevation of IL-1β and IL-6 concentrations in synovial fluid following OA induction was dose-dependently (p < 0.05) reduced by LMCP treatment. Furthermore, immunohistochemistry illustrated that LMCP significantly (p < 0.05) upregulated type II collagen and downregulated matrix metalloproteinase-13 in cartilage tissue. Consistent with the in vivo results, LMCP significantly (p < 0.05) increased the mRNA expression of COL2A1 and ACAN in chondrocytes isolated from a patient with OA regardless of the conditions for IL-1β induction. These findings suggest that LMCP has potential as a therapeutic treatment for OA that stimulates cartilage regeneration.

Activity Level and Body Mass Index as Predictors of Physical Workload During Working Career

  • The increasing prevalence of inactivity and obesity, along with aging, has implications on work capacity of labor force. This study reports the relationships between activity level and BMI by age with objectively measured physical workload. Data were examined from a sample of 19 481 Finnish employees using an estimate of minute-to-minute oxygen consumption based on R-R interval recordings. The mean estimated %VO2max during the working day was 12.1 (±3.6) and 15.1 (±4.5)% for men and women, respectively. Based on a linear model, the mean %VO2max increased by 1.5%-unit per 10-year increase in age, by 2.1%-unit per 5 kg/㎡ increase in BMI, and decreased by 1.6%-unit if improving physical activity class by two (p < 0.001 for all). Overweight and obesity, together with inactivity, notably increases workload throughout the career, even though at young adulthood, the daily workload is almost the same for each person regardless of the BMI, activity level, or gender. This study highlights the importance of regular physical activity and normal weight in protecting the worker from excessive physical (cardiovascular) workload during the whole working career.

Prostate cancer in workers exposed to night-shift work: two cases recognized by the Korean Epidemiologic Investigation Evaluation Committee

  • Background: In 2019, the International Agency for Research on Cancer re-evaluated the carcinogenicity of night-shift work and reported that there is limited evidence that night-shift work is carcinogenic for the development of prostate cancer. Therefore, in 2020 and 2021, the Korean Epidemiologic Investigation Evaluation Committee concluded that 2 cases of prostate cancer were occupational diseases related to the night-shift work. Here, we report the 2 cases of prostate cancer in night-shift workers which were first concluded as occupational diseases by the Korean Epidemiologic Investigation Evaluation Committee. Case presentation: Patient A: A 61-year-old man worked as a city bus driver for approximately 17 years, from 2002 to 2019, and was exposed to night-shift work during this period. In March 2017, the patient was diagnosed with high-grade prostate cancer through core-needle biopsy after experiencing stinging pain lasting for 2 months. Patient B: A 56-year-old man worked as an electrician and an automated equipment operator in a cement manufacturing plant for 35 years from 1976 to 2013 and was exposed to night-shift work during this period. In 2013, the patient was diagnosed with high-grade prostate cancer through core needle biopsy at a university hospital because of dysuria that lasted for 6 months. Conclusions: The 2 workers were diagnosed with high-grade prostate cancer after working night shifts for 17 and 35 years respectively. Additionally, previous studies have reported that high-grade prostate cancer has a stronger relationship with night-shift work than low or medium-grade prostate cancer. Therefore, the Korean Epidemiologic Investigation Evaluation Committee concluded that night-shift work in these 2 patients contributed to the development of their prostate cancer.

초등학교의 학교보건정책 지침개발 및 수행정도 분석 - WHO의 건강증진학교 지침을 중심으로 - (Development and Performance Analysis of Elementary School Health Promotion Policy Guideline)

  • Purpose: This study examined the development and performance of the school health promotion policy guideline that is applicable to Korean school health. Methods: 1. The WHO's guideline was translated into Korean and modified by school health experts 2. A questionnaire was conducted to assess the performance of school health policy on 207 nurses in elementary schools in Seoul and Cheonbuk. Result: The Korean version of the school health promotion policy guidelines were developed including 9 components and 36 checkpoints. Compared to that of WHO, the component 'school nursing teacher' was added and three components were eliminated: 'equitable access to school resources', 'control of helminth', and 'sun protection policy'. The performance degree of first-aid, closure of school in a calamity, and control of HIV/AIDS was very low. The performance degree of a healthy diet, and regular physical examination was quite high. This study was the first that developed the school health promotion policy guideline and examined the performance. Further research is needed to validate the guidelines developed by this study.

한의 신의료기술 평가 활성화 방안 제언 (A study on the current status and development of the new health technology assessment of Korean medicine field)

  • Objectives: The purpose of this study is to examine the current status of Korean medicine health technology assessment and explore realistic plans to activate it. Methods: We investigated all the applications for new health technology assessment related to Korean medicine from 2007 to 2016. The several expert meetings were held to draw out the barriers and improvement strategies of the new health technology assessment of Korean medicine field. Results: There were 31 cases in total except for duplications or reapplies falling into 3 main types. First, 19 of them were to try to enter a medical market and be covered by National Health Insurance. Eight cases were to apply western medicine technology as new health technology in Korean medicine area. The rest was 4 cases, which were totally not appropriate for the purpose of new health technology assessment system. According to the expert opinion, the obstacles of activation in new health technology assessment of Korean medicine were application of unstandardized technology, lack of understanding and experience, lack of clinical trial supporting system for Korean medicine, lack of committee members within the nHTA(new Health Technology Assessment) review board, ambiguous definition of medical practice and sharp conflict between western medicine and Korean medicine. Conclusions: Several suggestions were derived. First of all, to activate Korean medicine in the nHTA system, the existing system should be used sufficiently, and multifaceted efforts are needed to upgrade the system, if necessary. Also, self-help efforts, Korean medicine clinical trial supporting system and increasing R&D investment, establishing extra-committee for Korean medicine in nHTA could be needed. Finally, long-term strategy for improving collaboration between Korean medicine and western medicine should be considered.

산모의 우울이 생후 4개월 영아의 기질, 건강과 성장발달에 미치는 영향 (The Impacts of Postpartum Depression on the Temperament, Health and Development of 4-month-old Infants)

  • Purpose: This study was conducted to determine the impacts of maternal depression on the temperament, health problems, and development of 4-month-old infants. Methods: For this longitudinal comparative study, data collected at one month postpartum and at the 4th month of follow-up were evaluated from sixty five pairs. The Edinburgh Postnatal Depression Scale (EPDS) was used to assess the symptoms of depression. Results: Eighteen point 5 percent and15.4% of mothers were diagnosed with postpartum depression at one month post-partum and at the 4th month of follow-up, respectively. The temperament of the infants between the two groups was not different. However, the infants of the depressed mothers had more health problems after 4 months. Depressed mothers had a lower score for the acceptance of Home Observation for Measurement of the Environment (HOME), and the development score of their infants was lower than that of the non-depressed mothers. Conclusion: Maternal depression affects the health and development of infants. Women should be screened for potential risk factors and for symptoms of depression during postpartum periods so that appropriate interventions including emotional support can be initiated in a timely manner.

Development of a Health Education Modification Program Regarding Liver Flukes and Cholangiocarcinoma in High Risk Areas of Nakhon Ratchasima Province Using Self-Efficacy and Motivation Theory

  • A quasi-experimental study was conducted to develop a health education modification program based on self-efficacy and motivation regarding liver flukes and cholangiocarcinoma development in Keang Sanam Nang district, Nakhon Ratchasima province, Thailand. A total of 36 individuals were invited to participate in the program and were screened for population at risk of liver fluke infection and cholangiocarcinoma using SUT-OV-001 and SUT-CCA-001. Development of health education modification program regarding liver fluke and cholangiocarcinoma prevention included 3 steps: (1) preparation, (2) health education program, and (3) follow-up and evaluation. The study was implemented for 10 weeks. Pre-and-post-test knowledge was measured with questionnaires, Kuder-Richardson-20: KR-20 = 0.718,and Cronbach's Alpha Coefficient = 0.724 and 0.716 for percection and outcome expectation questionnaires. Paired and independent t-tests were applied for data analysis. The majority of the participants were female (55.6%), aged between ${\leq}50$ and 60 years old (36.1%), married (86.1%), education level of primary school (63.9%), agricultural occupation (80.6%), and income <4,000 Baht (44.4%). The results revealed that after the health education program, the experimental group had a mean score of knowledge, perception, and outcome expectation regarding liver fluke and cholangiocarcinoma prevention significantly higher than before participation and in the control group. In conclusion, this successful health education modification program for liver fluke and cholangiocarcinoma, therefore may useful for further work behavior modification in other epidemic areas.

An Analysis of Trainers' Perspectives within an Ecological Framework: Factors that Influence Mine Safety Training Processes

  • Background: Satisfactory completion of mine safety training is a prerequisite for being hired and for continued employment in the coal industry. Although training includes content to develop skills in a variety of mineworker competencies, research and recommendations continue to specify that specific limitations in the self-escape portion of training still exist and that mineworkers need to be better prepared to respond to emergencies that could occur in their mine. Ecological models are often used to inform the development of health promotion programs but have not been widely applied to occupational health and safety training programs. Methods: Nine mine safety trainers participated in in-depth semi-structured interviews. A theoretical analysis of the interviews was completed via an ecological lens. Each level of the social ecological model was used to examine factors that could be addressed both during and after mine safety training. Results: The analysis suggests that problems surrounding communication and collaboration, leadership development, and responsibility and accountability at different levels within the mining industry contribute to deficiencies in mineworkers' mastery and maintenance of skills. Conclusion: This study offers a new technique to identify limitations in safety training systems and processes. The analysis suggests that training should be developed and disseminated with consideration of various levels-individual, interpersonal, organizational, and community-to promote skills. If factors identified within and between levels are addressed, it may be easier to sustain mineworker competencies that are established during safety training.

Integrative Multi-Omics Approaches in Cancer Research: From Biological Networks to Clinical Subtypes

  • Multi-omics approaches are novel frameworks that integrate multiple omics datasets generated from the same patients to better understand the molecular and clinical features of cancers. A wide range of emerging omics and multi-view clustering algorithms now provide unprecedented opportunities to further classify cancers into subtypes, improve the survival prediction and therapeutic outcome of these subtypes, and understand key pathophysiological processes through different molecular layers. In this review, we overview the concept and rationale of multi-omics approaches in cancer research. We also introduce recent advances in the development of multi-omics algorithms and integration methods for multiple-layered datasets from cancer patients. Finally, we summarize the latest findings from large-scale multi-omics studies of various cancers and their implications for patient subtyping and drug development.
