대학생의 정신건강 예측구조모형 (Prediction Structure Model of Mental Health of University Students)

  • 본 연구는 Bronfenbrenner의 생태학적 체계이론을 바탕으로 대학생의 정신건강에 영향을 미치는 요인을 구분하고 통합적인 모형 구축과 정신건강 증진을 위한 간호중재 개발의 기틀을 마련하고자 한다. 연구방법은 횡단적 조사연구로 대학생을 대상으로 하였다. 일반적 특성 및 정신건강관련 특성은 SPSS 20.0 프로그램을, 모형의 적합도검증, 가설검증은 Amos 20.0 프로그램을 이용하였다. 연구결과에서 모형의 적합도 지수는 $x^2=614.90$(p=.000), Q값=3.5, GFI=.88, AGFI=.84, NFI=.92, NNFI=.94, CFI=.94, RMR=.02, RMSEA=.08 적합도를 나타내어 연구모형이 가정에 적합한 것으로 나타났다. 연구결과 정신건강에 스트레스가 가장 높은 영향을 미치고 있으며 스트레스 대처 방안과 자아 존중감, 부모의 양육태도가 정신건강에 영향을 미치는 것으로 확인되었다. 대학생의 정신건강을 증진시키기 위해서는 스트레스 관리, 자아존중감 증진, 스트레스 대처 능력 향상을 도모하는 간호중재를 개발하고, 학교현장 및 정신보건 실무에 활용하는 중재가 수행되어야 할 것으로 생각된다.

Feasibility and Effects of a Postoperative Recovery Exercise Program Developed Specifically for Gastric Cancer Patients (PREP-GC) Undergoing Minimally Invasive Gastrectomy

  • Purpose: Exercise intervention after surgery has been found to improve physical fitness and quality of life (QOL). The purpose of this study was to investigate the feasibility and effects of a postoperative recovery exercise program developed specifically for gastric cancer patients (PREP-GC) undergoing minimally invasive gastrectomy. Materials and Methods: Twenty-four patients treated surgically for early gastric cancer were enrolled in the PREP-GC. The exercise program comprised sessions of In-hospital Exercise (1 week), Home Exercise (1 week), and Fitness Improvement Exercise (8 weeks). Adherence and compliance to PREP-GC were evaluated. In addition, body composition, physical fitness, and QOL were assessed during the preoperative period, after the postoperative recovery (2 weeks after surgery), and upon completing the PREP-GC (10 weeks after surgery). Results: Of the 24 enrolled patients, 20 completed the study without any adverse events related to the PREP-GC. Adherence and compliance rates to the Fitness Improvement Exercise were 79.4% and 99.4%, respectively. Upon completing the PREP-GC, patients also exhibited restored cardiopulmonary function and muscular strength, with improved muscular endurance and flexibility (P<0.05). Compared to those in the preoperative period, no differences were found in symptom scale scores measured using the European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life Questionnaire (QLQ-C30) and Quality of Life Questionnaire-Stomach Cancer-Specific Module (QLQ-STO22); however, higher scores for global health status and emotional functioning were observed after completing the PREP-GC (P<0.05). Conclusions: In gastric cancer patients undergoing minimally invasive gastrectomy, PREP-GC was found to be feasible and safe, with high adherence and compliance. Although randomized studies evaluating the benefits of exercise intervention during postoperative recovery are needed, surgeons should encourage patients to participate in systematic exercise intervention programs in the early postoperative period (Registered at the ClinicalTrials.gov, NCT01751880).

일개 종합병원 입원환자의 낙상 실태 및 관련 요인 (Falls in the General Hospital Inpatients: Incidence, Associated Factors)

  • Background : To estimate fall incidence rate and associated factors in inpatients from a general hospital. Method : The data were collected from 104 fall incident reports developed by the patient safety committee in a general hospital in Seoul from 01 January 2007 to 31 December 2008. Information included general characteristics of patients, factors related to fall, types, places, circumstances and outcomes of fall. Result : The incidence rate of fall, which was 4.4 per 1,000 total discharged patients and 0.5 per 1,000 patient-days, was much lower than that of several hospitals in the United States. The difference may reflect the different incidence reporting system of each hospital. Fall-prone patients were, in general, $$\geq_-$$65 years of age, had an alert mental status, were ambulatory with some assistance, and were dependent on and ambulatory device. High incidence of falls was associated with patients with circulatory disease. The majority of fall events usually occurred in bed or at the bedside in the patient's room, and occurred more often during the night than during the day or evening. Risk factors of fall were use of drugs (antihypertensive or neuropsychiatric drugs) and environmental factors (e.g., overly high bed height, surrounding objects, inadequate fitness shoes and slippery floor). Physical injury occurred in 43.3% of fall events, which typically required diagnosis of injury and treatment such as suturing. Risk factors for repeated falls were use of a neuropsychiatric drug (odds ratio=13.9) and gait disturbance (odds ratio=91.2). Risk factors for fall-related injury were alert mental status (odds ratio=3.3 times more likely to fall than those who were drowsy or in a stupor) and general weakness(odds ratio=3.3 times more likely to fall than those who were not generally weak). Conclusion : Medical and nursing staff should be aware of the fall risk factors of hospitalized patients and should intensively pursue preventative strategies. Development of fall prevention education based on these results is recommended.

만성 호흡기 질환자의 건강증진행위 구조 모형 (A Structural Model for Health Promoting Behaviors in Patients with Chronic Respiratory Disease)

  • Purpose: This study was designed to construct a structural model for health promoting behavior in patients with chronic respiratory disease. A hypothetical model was developed based on the literature review. Method: Data was collected by questionnaires from 235 patients with chronic respiratory disease in a General Hospital in Seoul. Data analysis was done using SAS 6.12 for descriptive statistics and the PC-LISREL 8.13 Program for Covariance Structural Analysis. Result: The results are as follows : 1. The fit of the hypothetical model to the data was moderate. It was modified by excluding 2 path and including free parameters and 3 path to it. The modified model with path showed a good fitness to the empirical data($\chi$2=80.20, P=0.05, GFI=0.95, AGFI=0.88, NNFI=0.95, NFI=0.96, RMSR=0.01, RMSEA =0.06). 2. The perceived benefits, self-efficacy, and a plan of action were found to have significant direct effects on the health promoting behavior in patients with chronic respiratory disease. 3. The health perception, self-esteem, and activity related to affect were found to have indirect effects on the health promoting behavior in patients with chronic respiratory disease. Conclusion: The modified model of this study is considered appropriate in explaining and predicting health promoting behavior in patients with chronic respiratory disease. Therefore, it can effectively be used as a reference model for further studies and suggested direction in nursing practice.

전북지역 일부 중년남성의 비만도에 따른 체성분 분석과 식행동에 관한 연구 (A Study on Body Compositions and Food Behaviors of Middle Aged Men Living in Jeonbuk Province by Percentage of Body Fat)

  • The purpose of this study was to compare the body compositions and food behaviors of middle aged men with different obesity indices. The subjects were 62 middle aged men who lived Gunsan city. Heights, body weights, soft/lean masses, fat masses, percentages of body fat, and fat distributions were measured. Food habits and health-related lifestyle habits were evaluated based on questionnaires. The subjects were assigned to one of the following groups based on their percentage of body fat ($\%$Fat) ; normal, overweight and obesity. The results were as follows: their heights, fat masses, percentages of body fat, WHR, RBW, BMI and fitness scores were significantly higher in the obese subjects when compared to the normal and overweight subjects. Self-perceptions of weight and desires of weight control were significantly related with their percents of Fat. The overweight and the obese groups skipped meals, ate supper out, ate snacks, smoked tobacco, and exercised less frequently than the normal group. There were no significant differences in the scores of the dietary habits and drinking of alcohol among the three groups. Therefore, proper nutritional education on regular meals and intervention is required if middle aged men want to be of normal weight and have healthy lifestyles.

파킨슨병 환자의 우울 예측 모형 (A Prediction Model for Depression in Patients with Parkinson's Disease)

  • Objectives: This study investigated how income, duration of illness, social stigma, quality of sleeping, ADL and social participation related to Parkinson's disease(PD) predict depression in a conceptual model based on the International Classification of Functioning(ICF) model. Methods: The sample included 206 adults with idiopathic Parkinson's disease(IPD) attending D university hospital in B Metro-politan City. A structured questionnaire was used and conducted face-to-face interviews. The collected data were analyzed for fitness, using the AMOS 18.0 program. Results: A path analysis showed that the overall model provided empirical evidence for linkages in the ICF model. Depression was manifested by significant direct effects of social stigma(${\beta}=.20$, p<.001), quality of sleeping(${\beta}=-.40$, p<.001), ADL(${\beta}=-.20$, p<.01), and social participation(${\beta}=-.12$, p<.05), indirect effects including income(p<.05), duration of illness(p<.05). These variables explained 45.9% of variance in the prediction model. Conclusions: This model may help nurses to collect and assess information to develop intervention program for depression.

Acupuncture in Sport Recovery: A Brief Review

  • Active and therapeutic sport recovery is becoming a popular and important component in approving performance for pro and recreational athletes alike. It is also helping in the realm of injury prevention. In the search of finding modalities that are widely effective, natural, and safe, acupuncture is a viable and cost-effective treatment for helping athletes achieve this goal. More direct related research is needed, but testimonials from pro athletes and the body of research that currently exists provides powerful evidence on acupunctures ability to help with enhancing recovery. Specializing in acupuncture and exercise science, Chris integrates acupuncture into musculoskeletal rehabilitation therapy or fitness training for pain modulation, speedy recovery, and enhanced performance. Clients can choose to focus on one-on-one corrective exercise therapy, manual and massage therapy, or acupuncture. However, for best results, Chris recommends all three. Other modalities that he uses in therapy are acu-taping, herbal therapy, nutrition supplementation, cupping, guasha, and stretching techniques. The corrective exercise component is one-on-one body balancing management, focusing on strength and conditioning, post physical rehab - exercise therapy, integrative sport specific exercise, weight loss, core strengthening, dynamic lumbar stabilization, active recovery techniques, and myo-fascial release techniques. The acupuncture component focuses on sport injuries, myofascial pain, peripheral neuropathy, arthritis, facial rejuvenation, stress, smoking cessation, addiction detoxification program, weight management, sport recovery and performance.

Accuracy Verification of Heart Rate and Energy Consumption Tracking Devices to Develop Forest-Based Customized Health Care Service Programs

  • This study was carried out to verify the accuracy of fitness tracking devices in monitoring heart rate and energy consumption and to contribute to the development of a forest exercise program that can recommend the intensity and amount of forest exercises based on personal health-related data and provide monitoring and feedback on forest exercises. Among several commercially available wearable devices, Fitbit was selected for the research, as it provides Open API and data collected by Fitbit can be utilized by third parties to develop programs. Fitbit provides users with various information collected during forest exercises including exercise time and distance, heart rate, energy consumption, as well as the altitude and slope of forests collected by GPS. However, in order to verify the usability of the heart rate and energy consumption data collected by Fitbit in forest, the accuracy of heart rate and energy consumption were verified by comparing the data collected by Fitbit and reference. In this study, 13 middle-aged women were participated, and it was found that the heart rate measured by Fitbit showed a very low error rate and high correlation with that measured by the reference. The energy consumption measured by Fitbit was not significantly different from that measured in the reference, but the error rate was slightly higher. However, there was high correlation between the results measured by Fibit and the reference, therefore, it can be concluded that Fitbit can be utilized in developing actual forest exercise programs.

동영상 운동프로그램이 항암 화학요법을 받는 암환자의 피로와 신체기능 및 정서상태에 미치는 효과 (The Effect of a Video Exercise Program on Cancer-related Fatigue, Physical Function and Emotional Status in Patients with Cancer during Chemotherapy)

  • Purpose: The purpose of this study was to evaluate the effects of a home-based video exercise program on cancer-related fatigue, physiological and psychological status in patients with colon and rectal cancers undergoing chemotherapy. Methods: The study design was a non-equivalent control group non-synchronized design. Data were collected from patients with colo-rectal cancers in Yonsei cancer center from July 5th to October 31st in 2011. There were 40 participants; 20 in the experimental group and 20 in the control group. The structured questionnaire was used to measure fatigue, physical function and emotional status. Data were analyzed using SPSS 18.0 and a chi-squre test, Fisher's exact test, Mann-Whitney U test and Wilcoxon signed- rank test were conducted to examine the homogeneity and the research hypotheses. Results: There was a statistically significant difference in White Blood Cell count in the experimental group compared with that of the control group. The exercise group showed a slight decrease of White Blood Cell count compared with that of the control group after 4 week program (z=-2.935, p=.003). However, there were no significant differences in fatigue, physiological and psychological status between the two groups. Conclusion: In this study, the developed video exercise program was effective in markedly slightly decreasing White Blood Cell count in patients with colo-rectal cancers undergoing chemotherapy. Therefore, utilizing the video exercise program can be an useful method to promote health among patients with cancer in clinical practice.

흡연, 음주, 운동과 건강생활양식 (The health lifestyle of adults related to smoking, drinking and exercise)

  • This study is a descriptive survey to explore the health lifestyle of adults. The study subjects are teachers of elementary. middle and high school. and staffs of research institutes located in Chungchung Province and Daejon city. The data was collected from Jan. to march 1997 through self reporte for structured questionnaire. Fantastic check list of Wilson and Ciliska for Health Lifestyle Assessment and DSM-III-R for somatic symptom were used as tools. Data was analysed by frequency, $X^2_test$, t-test and Anova using SAS program. The results are as follows: 1. There were statistically significant differences In drinking(t=7.75, P=.000), exercise(t=-2.99, P=.003)and interpersonal relationship(t=2.22, P=.027) among 10 health lifestyle between smoking group and non-smoking group, in drinking(t=17.98, P=.000), exercise(-4.71. P=.000), and job satisfaction(t=2.22, P=.027) between drinking group and non-drinking group, and in eating habit(t=-2.00, P=.045), drinking (t=4.47, P=000), exercise (t= -16.49, P=000), keeping traffic law(t= -2.68, P=.007), personality (t= -2.05, P=.040) and anxiety/depression(t=-3.47, P=.000) between exercise group and non-exercise group. 2. There was statistically significant difference in cardiovascular symptom(F=4.22, P=.0l) among somatic symptoms of subjects according to exercise level. 3. There was statistically significance difference in lifestyle according to smoking level(F=, 3.33, P=.011), drinking level(F=9.17, P=.0001) and exercise level(F=11.93, P=.000l), and in somatic symptom according to sex(t=-3.93, P=.0001), weight(F=3.83, P=.022), exercise level (F=3.29, P=.03) among general characteristics. 4. There was statistically significant difference between sex in general (t= -3.64, P=.0001), gastrointestinal(t=-2.21, P=.02), musculoskeletal(t=-3.92, P=.001), and total symptom (t= -3.92, P=.0001). 5. There was statistically very highly signigicant difference In weight according to smoking($x^2=25.18,\; P=.001)$ and exercise$(x^2=16.46,\; P=001)$. 6. There was statistically significant difference in frequency between smoking group, drinking group and exercise group$(x^2=24.52,\;P=.001)$. Among a number of habit, smoking, drinking and exercise are important factors of human health to prevent related disease morbidity and death. It is essential for industrial health nurse to committ in this subject considering the influence of those factors and lifestyle on health. There is also a relationship of weight with smoking and exercise, the frequency of overweight/obesiy in smoking/ no-exercise group were high. It is quite necessary for the people having cardiovascular symptom to exercise to lower morbidity and mortality. The industrial health nurse has to keep In mind on this point and consider of time and facilities of fitness of employee. It needs to explore the cause by further research on somatic symptom of women. This research shows that concerning the relationship between smoking, drinking, and exercise, health care provider must take not only management of disease, but health behaviors and lifestyle into consideration.
