보건소 건강증진사업 담당인력으로서 보건교육사 역할의 우선 순위 분석 (Analysis of the priority of roles performed by health educators in charge of health promotion programs at community health centers)

  • Objectives: The purpose of this study was to analyze the roles of workforce required for effective execution of health promotion programs of community health centers in Korea. Methods: Survey was undertaken on 92 people in community health centers and the Analytic Hierarchy Process was employed in order to obtain results regarding the relative importance of role required for health educators. Results: The analysis suggests that of all 5 categories, 'Assess needs for health education' and 'Evaluate health promotion programs and Conduct related research' were relatively more important than the other categories of role. Taking into account the weightings of the main categories and the subcategories, the analysis shows that the order of importance follows, 'Use existing health-related statistical data', 'Collect health-related data', 'Survey method and knowledge and skills related to health statistics', 'Write an evaluation report', 'Understand and apply health education planning theories'. Conclusion: As a health promotion expert of community health center, a health educator is preferentially required to perform 1) the role to analyze the needs of the community and enable the planning for a customized health promotion program, 2) the role to execute evaluation throughout a health promotion programs and disseminate evaluation findings and apply them in following programs, in consideration of higher relative importance of these roles.

  • Background: The industrial revolution that took place in the United Kingdom (UK) between 1760 and 1830 led to profound social change. Occupational medicine was concerned with the diagnosis, treatment, and prevention of occupational diseases, that is, diseases directly caused by exposure to workplace hazards. A similar pattern of development has occurred globally. Methods: A review of relevant literature. Results: The international conceptualization and development of occupational health occurred during the 20th century. A new paradigm for occupational health has emerged that extends the classical focus on what might be termed "health risk management" that is, the focus on workplace hazards and risk to health to include the medical aspects of sickness absence and rehabilitation, the support and management of chronic noncommunicable diseases, and workplace health promotion. Conclusion: The future strategic direction for occupational health will be informed by a needs analysis and a consideration of where it should be positioned within future healthcare provision. What are the occupational health workforce implications of the vision for occupational health provision? New challenges and new ways of working will necessitate a review of the competence and capacity of the occupational health workforce, with implications for future workforce planning.

보건의료인력지원법의 의미와 과제 (The Meaning and Challenges of Health Care Workforce Support Act)

  • 2019년 10월 24일 보건의료인력지원법이 시행되었다. 보건의료인력은 가장 핵심적인 국가보건의료자원으로서, 보건의료인력의 양과 질은 국가의 보건의료 수준과도 연결된다. 본 논문은 보건의료인력지원법의 제정배경 및 주요내용을 살펴보고, 주요쟁점을 분석하여 향후 개선방안을 제시하고자 하였다. 보건의료인력지원법은 보건의료인력의 원활한 확보와 근무환경 개선 등을 지원하기 위해 제정되었다. 이 법은 보건의료인력 종합계획의 수립, 보건의료인력의 수급관리, 보건의료인력의 근무환경 개선, 이들의 지원을 위한 정책기반 조성 등의 사항들을 규정함으로써 보건의료서비스의 질을 제고하고 국민의 건강증진에 이바지하고자 하였다. 보건의료인력지원법은 보건의료환경 및 인력에 관한 사회적 이슈들을 고려하여 발의된 법으로서 보건의료인력의 수급 관리, 근무환경개선 및 복지향상 그리고 우수인력 양성 등에 중점을 두어 보건의료인력을 지원하고자한 데 의미가 있다 할 것이다. 그러나 동법이 가지는 의의를 살리면서 보건의료인력지원을 위한 실효성 있는 법으로 발전하기 위해서는 '입법목적의 달성 및 법의 실효성, 적용 대상자 범위의 적절성, 종합계획과 실태조사의 중복성과 실현가능성, 위원회 설치의 타당성, 보건의료인력지원전문기관의 업무범위와 운영방식의 적절성 측면'에서 제기될 수 있는 쟁점사항들을 검토하여 시급히 개정할 필요가 있다.

  • Objectives: Recently, the rate of death by chronic disease, is increasing steadily. To prevent this, the public health center will have taken a leading role in the local community medical business through an establish to the national health promotion act and an amendment to the law of public health center in Korea. Results: Accordingly this research, using the Pender's health promotion model which is related with subject health behavior who government employees serve at the public health center have taken important position in the local community health promotion, have comprehended the actual condition of health behavior. For increasing the health behavior practice of subject to comprehend the factor which have effect on health behavior practice, which can be a correct role model in the local community health promotion. A survey was performed on 406 government employees who serve at five public health centers in Seoul. The period of survey was from 25th October, 2010 to 15th November, 2010. The results of this study were summarized as below. 1. Work-related stress, perceptible beneficial obstacle, and self-efficacy were composed by 5 points measure. The results show those work-related stress were $3.06{\pm}0.469$, 74perceptible beneficial obstacle were $3.74{\pm}0.471$, and self-efficacy were $3.49{\pm}0.469$. 2. As for the health behavior by general characteristic, the results have specific differences on age, education level, state of marriage, rank of the position, field of the occupation and employment forms in statics analysis. 3. As for the past health behavior by health behavior characteristic, work-related stress have specific differences on the past frequency of drinking (p<.05) in statics analysis, perceptible beneficial obstacle have specific differences on the past frequency of having breakfast(p<.05), having snacks(p<.05) and doing exercise(p<.05) in statics analysis. Self-efficacy have specific difference on the past frequency drinking(p<.01) in statics analysis. 4. According to the correlation between the factors related with health behavior and health behavior, the health behavior have specific differences on perceptible health condition(p<.01), the past health behavior and perceptible beneficial obstacle(p<.05). Work-related stress and self-efficacy don't have specific relation in health behavior practice. 5. The factors, which effect on health behavior practice have specific differences on the past health behavior, employment forms and field of the occupations. The ability of explanation occupied 54.8% what explained of the health behavior practice by general characteristic, perceptible health condition, employment forms and perceptible beneficial obstacle. Conclusions: According to the correlation between the factors related with health behavior, the health behavior have specific differences on perceptible health condition(p<.01), the past health behavior and perceptible beneficial obstacle(p<.05). The factors, which effect on health behavior practice have specific differences on the past health behavior, employment forms and field of the occupations.

지역사회 보건교육 (Community Health Education)

  • Health education aims at behavior change rather than just delivering health knowledge to people. In Korea health education activities in public sector began in 1960 and they were included in the primary prevention program in communities. This article reviewed current health education programs in healthy living practice programs provided by local public health centers in Korea and drew implications for the future role of health education in community setting. Health education has been a core function of the National Health Promotion programs in the nation since the enactment of the National Health Promotion Law in 1995. The National Health Promotion programs are funded by the National Health Promotion Fund which are drawn from tobacco tax. The National Health Promotion programs include healthy living practice programs (smoking prevention and cessation programs, moderate alcohol use programs, physical activity promotion programs, and nutrition programs), chronic disease prevention programs, oral health programs and public hygiene programs. Methods of the National Health Promotion programs include health education, health counseling, health class, health information management, survey and research. Smoking prevention and cessation programs include smoking cessation clinic, smoking cessation education, non-smoking environment program, and non-smoking campaign. Moderate alcohol use programs include alcohol use education, moderate alcohol use campaign, alcohol use counseling, and alcohol free environment programs. Physical activity promotion programs include obesity control, targeted exercise program, and exercise civic group programs. Nutrition programs include nutrition management, obesity management, nutrition education, breakfast eating program, and nutrition counseling and treatment programs. The health education programs in community are not efficient today because there are many overlapping contents and short term goals. Community health education programs needs to be more comprehensive. Workforce development is another big issue at the moment because the National credential program will begin in 2009. Variety of community health education programs should be developed and funded by the national health promotion fund.

보건교육 및 건강증진에 대한 구성주의 방법의 적용 (A Study on utilization of constructivist paradigm in Health Promotion and Education)

  • The National Health Promotion Law passed in 1995 was a milestone for initiating a national health promotion program in Korea, and government officers and health professionals are working on how to approach health promotion issues. The purpose of this study was to analyse methods and use of constructivist paradigm in health promotion and education. The health promotion area needs community empowerment, building community partnerships, and community capacity. To meet these health promotion requirements health promotion workforce should be trained through professional preparation programs that contains communication skills, group process skills, and management of programs in advanced countries such as the United States and Australia. Skills and responsibilities of those who are in charge of providing health promotion services have not yet been clearly defined in Korea because the area of health promotion is a multi-academic field, and needs a different approach, constructivist approach. Constructivist paradigm requires relativism, reasoning skills, collaborating, and motivation. These components are needed for community empowerment. Constructivism also has been applied to the field of education. Problem-based education, outcome-based education, performance-based education came from the constructivism. These educational methods are student-centered method. As the modernizing society becomes more complicated, traditional or conventional teacher-centered education cannot meet the needs of students. Students need to learn skills necessary to make healthy decisions with individual value system. So these interactive, self-learning methods can serve much more to the learner. Constructivist educational methods can be applied to educational programs in computers, too. To expand and differentiate the area of health education and health promotion from other health related fields, it is crucial to devote efforts in application and development of constructivist methods.

Status and Opinions of Public Health Centers and Industrial Dental Offices on the Oral Health Promotion of Korean Adult Workers

  • Background: This study investigated the status of oral health promotion activities for adult workers in public health centers and industrial dental offices and provided basic data for the model development of oral health promotion program for adult workers in Korea. Methods: A questionnaire was developed separately according to the person who in charge of the oral health promotion activities in public health centers nationwide and dental hygienists working in 20 industrial dental offices. This survey was conducted through postal survey and consisted of 29 items and 35 items respectively, including 19 common items for general information, oral health promotion program status and opinion. Statistical analysis was performed using the IBM SPSS ver. 23.0. Results: We analyzed the data of 147 public health centers (57.9%) and 9 industrial dental offices (45.0%). A workforce with a lack of practice was the biggest barrier to oral health promotion activities for adult workers. However, both groups showed high intention for the practice of adult worker's oral health promotion activities. Also, they showed willingness to work together in an organic partnership to perform their roles (94.4% and 77.8%, respectively). Regarding the scope of cooperation in the implementation of the industrial oral health promotion activity linked to the public health center, dental hygienists of industrial dental offices responded that they could coordinate necessary matters and schedule management. Conclusion: The development of an oral health promotion program aided by the relationship between public health centers and industrial dental offices is essential for the oral health promotion of adult workers. The possibility of cooperation between the abovementioned centers was confirmed through this study. In a long-term perspective, it would be necessary to identify a method to institutionalize industrial dental hygienists for the provision of continuous oral health care in workplaces.

Strategies for Worksite Health Interventions to Employees with Elevated Risk of Chronic Diseases

  • Chronic disease rates have become more prevalent in the modern American workforce, which has negative implications for workplace productivity and healthcare costs. Offering workplace health interventions is recognized as an effective strategy to reduce chronic disease progression, absenteeism, and healthcare costs as well as improve population health. This review documents intervention and evaluation strategies used for health promotion programs delivered in workplaces. Using predetermined search terms in five online databases, we identified 1,131 published items from 1995 to 2014. Of these items, 27 peer-reviewed articles met the inclusion criteria; reporting data from completed United States-based workplace interventions that recruited at-risk employees based on their disease or disease-related risk factors. A content rubric was developed and used to catalogue these 27 published field studies. Selected workplace interventions targeted obesity (n = 13), cardiovascular diseases (n = 8), and diabetes (n = 6). Intervention strategies included instructional education/counseling (n = 20), workplace environmental change (n = 6), physical activity (n = 10), use of technology (n = 10), and incentives (n = 13). Self-reported data (n = 21), anthropometric measurements (n = 17), and laboratory tests (n = 14) were used most often in studies with outcome evaluation. This is the first literature review to focus on interventions for employees with elevated risk for chronic diseases. The review has the potential to inform future workplace health interventions by presenting strategies related to implementation and evaluation strategies in workplace settings. These strategies can help determine optimal worksite health programs based on the unique characteristics of work settings and the health risk factors of their employee populations.

보건관리자 제도의 발전과 전망 (Overview of Occupational Health Management System in Korea)

  • Purpose: Regarding workers' health promotion and disease prevention, the law on the health management system has been enacted and implemented since 1961. However, regulations related to health managers do not meet recent demands for changes in fields such as industrial structure, occupational health problems, or the improvement of occupational health skills. This study aimed to suggest improvement strategies regarding the health management system in the workplace. Methods: We summarized past empirical or theoretical literature. Results: This study summarized the overview and history of the Korean occupational health management system, advanced foreign occupational health management system, and the effectiveness of the health management at workplace. Further, this study proposed various strategies to improve the occupational health management system in Korea. Method: We summarized past empirical or theoretical literature. Conclusion: The effective operation of the health management system in the workplace would improve workers' health, in addition to enhancing national competitiveness through securing a healthy workforce.

신 공중보건과 국가공중보건체계 (New Public Health and National Public Health System)

  • The New Public Health(NPH) is a comprehensive approach to protecting and promoting the health status of each individual member and society as a whole. NHP is not so much a philosophy to broaden the understanding of public health as it is an action plan to address current public health system. This paper's objectives include increasing public and professional awareness of the significant changes in the national public health systems of developed countries and contributing to more effective delivery of public health services in Korea. This paper reviews articles and documents concerning NPH and the public health system, and outlines of the achievements in developed countries since NPH movement began. These include the change in the definition and function of public health, expansion of public health networks, strengthening of public health policy, reorientation of public health delivery systems, promotion of workforce capacity, and the implementation of evidence-based management. To overcome the challenges facing the public health system of Korea, we must prioritize the value of population-based approach, expand the notion of a public health system to encompass all sectors that can influence health, promote a "Health in All Policies" approach, focus on an evidence-based health policy and program, develop core competencies for public health workers, and establish performance standards for public health organizations based on the core functions of public health.
