  • Purpose: This study was conducted to identify the status of health management and to evaluate the need for health professionals in day care centers in the South Korea. Method: Data were obtained from 360 child care providers in 16 provinces from May to August, 2007. Data was analyzed using SPSS WIN 14.0. Results: Safety education (traffic. 83.8% general safety. 71.6%) was provided more often than the other topic of education. All health related education was provided by child care providers. When minor health problems occurred, the most common management strategy was to send the child home (36.1%). During emergency conditions, majority of child care providers preferred taking the children to medical centers without giving any emergency care and 19.4% reported sending the child home without any immediate care. Child care providers recognized the need for health care professionals. Conclusion: The results of this study suggest the need for services of health professionals in day care centers to satisfy the needs and expectations for health management in these centers. As health professionals, public health nurses or pediatric nurse practitioners may play an important role in health management of children in day care centers.

  • By the rural area health care special law in 1980, Primary health care posts were established in rural areas as fundamental elements of the national health system. Nurses have been deployed to the posts after taking an education course mandated by the special law. However, health care posts have confronted environmental changes over the past 30 years such as an aging and decreasing rural population and advanced traffic systems, which make it necessary to reshape their form and role. Therefore, some guidelines are suggested for future role enlargement of health care posts by analyzing their current management and duties. The guidelines are as follows: 1) enlarging the portion of prevention and management of chronic degenerative diseases, 2) development and practice of diverse health promotion programs, 3) extension of primary health care for the increasing older population, 4) development of health programs for married immigrants, 5) practice of timely maternal child health programs, 6) development of adequate health care posts for low-income people in rapidly urbanizing rural areas and in poor areas in big cities, and 7) revision of laws and institutional arrangements for the role enlargement of health care posts to match social changes and customer needs.

  • Purpose: The purpose of this study was to explore practice-based evidence for health promotion in vulnerable populations with hypertension in primary health care settings. Methods: Two methodological procedures were adopted for this triangulation study. In the first phase, the sample was obtained from the computerized clinical data repository of a community nursing center. A total of 286 clients were assessed for hypertension as an actual circulation problem as coded in the Omaha System. In the second phase, a qualitative focus group was surveyed through semi-structured interviews conducted by nine advanced practice nurses who had been serving the hypertensive patients. Results: The community nurses provided essential primary healthcare services including health teaching guidance and counseling, and surveillance to vulnerable populations living in medically underserved community. There was a significant positive correlation between knowledge and behavior (r=.53, p<.01), between knowledge and health status (r=.40, p<.05), and between behavior and health status (r=.48, p<.01). Conclusion: This triangulation study encompassed not only quantitative findings from the computerized records of clients but also other information acquired from advanced practice nurses. This study contributes to understanding the importance of health promotion nursing interventions even with populations already diagnosed with chronic diseases such as hypertension.

  • As people get broader access to health information through the internet, there is a greater need for measure to maximize the social advantages of the internet and to minimize negative side-effects. With this concern, this paper classifies internet health information services sites into : on-line supply of health information, on-line consultation, on-line diagnosis, and on-line sales. As well this paper analyzes domestic laws supporting and/or regulating these services. The efficient provision of internet health services requires comprehensive laws on individual privacy protection, prevention and handling of medical accidents, an electronic prescription form for internet diagnosis, electronic signing, payment for medical expenses, qualifications for internet medical practitioners. Additional laws are required to establish internet pharmacies and internet health goods stores. These new laws can be prepared either separately or through revision of existing laws governing medical practice, pharmacies, and public health promotion. However, as the legal control by the government on cyber processes and entities has a fairly minimal effect, consumers should be encouraged to improve their own capacity for wisely using internet health services and health-service providers should be encouraged to promote voluntary supervision and control of their own services and practices.

Data was collected from August 16, to August 25, 1990. In order to identify the health needs of the community people, research within the last five years was reviewed and analyzed. The data on 1, 842 communities gathered by the WHO Nursing Collaborations Center of the College of Nursing, Yonsei University was utilized to identify community problems related to health and the self - confidence in job performance of the CHPs. Psase II of the study consisted of a workshop with 13 professionals including Community Health Practitioners to evaluate the existing education program and a conceptual framework of the curriculum for the job education of CHPs. The results of the study are Summariged below : 1. The only 26 among 45 content items of the education program related to job skills was used by 80% of the responding CHPs. The knowledge of $\ulcorner$Networking community organization$\lrcorner$ was used by only 53.7% of the respondents. Educational content about $\ulcorner$Mental disease$\lrcorner$ was used by less than 50% of CHPs because of a knowledge deficit. 2. The CHPs reported that their activities concentrated on clinical services during the last six months. The survey showed that they seemed to neglect the activities for health promotion and disease prevention. Thus, $\ulcorner$Education for community loaders$\lrcorner$(15.9%), $\ulcorner$Activity for eavironmental health$\lrcorner$(16.3%) and $\ulcorner$Social work for needey people$\lrcorner$(23.3%) were done by less than 30% of CHPs. 3. More than 90% of CHPs reported being self - confident for the activities of $\ulcorner$Health education and counselling$\lrcorner$, $\ulcorner$Medicine prescription$\lrcorner$ and $\ulcorner$Immunization$\lrcorner$. But 50% of CHPs reported that they were not have self - confident in $\ulcorner$Management of water and environmental health$\lrcorner$ and only 25.6% of CHPs could insert an IUD independently. 4. It was identified that respiratory diseases and the gastrointestinal diseases were most common problems for the community people, followed by musculoskeletal and skin problems. 5. The community problems were classified into eight categories : physical environmental problems, environmental hygiene, health problems, health behavior, social problem, lack of resources, financial problem and the problems of the cultural and value system. 6. • Background: Occupational end users of pesticides may experience bodily absorption of the pesticide products they use, risking possible health effects. The purpose of this paper is to provide a guide for researchers, practitioners, and policy makers working in the field of agricultural health or other areas where occupational end use of pesticides and exposure issues are of interest. Methods: This paper characterizes the health effects of pesticide exposure, jobs associated with pesticide use, pesticide-related tasks, absorption of pesticides through the skin, and the use of personal protective equipment (PPE) for reducing exposure. Conclusions: Although international and national efforts to reduce pesticide exposure through regulatory means should continue, it is difficult in the agricultural sector to implement engineering or system controls. It is clear that use of PPE does reduce dermal pesticide exposure but compliance among the majority of occupationally exposed pesticide end users appears to be poor. More research is needed on higher-order controls to reduce pesticide exposure and to understand the reasons for poor compliance with PPE and identify effective training methods.

  • This study examines the meaning of medical-text publication in the Joseon Dynasty by applying anthropological notions of "health care system" and "popular health care sector" to the social and political contexts of the pre-modern state. The present study focuses on the social network of senders and receivers in which medical knowledge is communicated and shared. Exploring the multi-layered structure of the network among the state, the author-practitioners, and populace, this study argues that the network of knowledge sharing system by publication of medical texts itself is a core structure in the health care system of the Joseon Dynasty. This pre-modern health care system aimed to vitalize and reinforce the "popular health care sector" by sharing medical knowledge with populace through the book-publication system. Foucault's notion of "biopolitics" provides a comparative window between the modern health care system and the health care system of the Joseon period, articulating the particularity of the pre-modern health care system.

  • Purpose: This study was done to learn from the development of rural health nursing in developed countries. Method: A literature search was done using keywords such as "rural and nursing" or "rural and health" from the database CHINAL and website of institutions related to rural health and nursing. Results: In Australia, the type of rural health institution was categorized according to the population size and nursing service was done differently according to area. It is unique to Canada to have the Canadian Association of Rural and Remote Nursing, which published the survey report "the Nature of Nursing Practice in Rural and Remote Canada". In the U.S., the role of nurse practitioners in rural communities was relatively well developed and a diverse nursing care model was demonstrated. To improve health care access in rural communities, financial and political support by the governments has been part of long-term plans in the three countries. Conclusion: It is very informative to identity the difference and similarity in rural health nursing in three developed countries. For the future development of Korean rural nursing, suggestion can be made in terms of research, education and policy development.
