Research on the Health Status of the Life-Insureds by the THI (생명보험 가입자의 THI 건강조사)

  • This study is a series of reviews in connection with the application of the Questionnaire THI, in order to intend to provide such datum to be of a help as the screening-test by means of getting through comparative analysis, and finding out existence of any disease appeared as a result of the general medical examinations, and the psychosomatic symptoms, through the questionnaire THI. This study had been researched through the 507 insureds of the life insurance residing in Seoul and in the suburbs of Seoul, during the period 5 Jan. 1987 to 27 Feb. through the questionnaires. Exception of the 88 insincere respondents out of the total objectives, the 419 questionnaires were nanlized through percentage, Mean, T-test, ANOVA test, and Discriminant Analysis. 1. The Subjective Health Problem of the Objectives: The average of the health problem appliation quorum appeared to be 1. 81, the articles of both nervousness and aggressiveness appeared to be each 2.25 and 2.04. It tells that the state of appliation is heavy. It was very significant statistically the relation between the elements of sex, ages, educational backgrounds and occupations. 2. The result of the General Examination of the Objectives: The number of the persons with abnormal findings was 300 persons among the total of 419 persons as a result of the general medical examinations, and it was applicable to $73.5\%$. By each age and marital state were statistically significant. 3. Comparison of the result of the General Examination and Subjective Health of those Objectives: As a result of discriminant analysis of the independent variables through the result of the general examination, the greatest element for influence appeared to be the age, and then, regular examination, sex, and the somatic complaints of the THI in order. Accordingly, it is considered to be able to function as the screening test of the general examination, the THI. In case of a greater unit of group required a medical examination, It is considered to be of a help for the health management service for more effective and precise in quality and economic, only if a medical examination is carried out after selecting the somatic subjective problem and research in advance to the application of the THI prior to the examination.

Factors Influencing the Health Examination in Unmarried Women (고연령 미혼여성의 건강검진에 영향을 미치는 요인)

  • Purpose: This study was to investigate factors that influence health examination for unmarried women. Methods: A correlation study was carried out with 144 unmarried women with an age range of 35~49 years through selfadministered questionnaires. This questionnaire included Health Belief Model modifying factors (demographic.sociological, structural, and cues to action variables). Data were analyzed using descriptive statistics, $x^2$-test, multiple logistic regression. Results: The mean age of participants was $37.2{\pm}2.67$ years. Rates for breast cancer, cervical cancer and comprehensive health examination were each 34.7%, 38.2% and 94%. In multiple logistic regression analysis, influencing factors on physical examination were age(OR=0.06, 95% CI: 0.02~0.26), personal medical insurance (OR=6.30, 95% CI: 1.60~24.82), housemate (OR=7.63, 95% CI: 2.58~22.52), exercise (OR=3.72, 95% CI: 1.37~10.12) in breast cancer examination, and age (OR=0.08, 95% CI: 0.02~0.34; OR=0.07, 95% CI: 0.01~0.85), personal medical insurance (OR=14.17, 95% CI: 2.94~68.23), sexual experiences (OR=3.38, 95% CI: 1.28~8.91), drinking (OR=2.92, 95% CI: 1.14~7.49) in cervical cancer examination. Conclusion: The results emphasize the necessity of preparing nursing education and intervention in consideration of associated factors which influence on the health examination in unmarried women.

Determinants of the Use and Type of Comprehensive Medical Examination Services (건강검진 수검 및 검진유형 선택의 결정요인)

  • Objectives : This study analyzed the factors that determine the use and type of medical examination services (MES) to develop a model explaining the use behavior of MES which could consequently contribute to policy implications for medical examinations. Methods : Based on Anderson's healthcare utilization model, the MES utilization model was developed by including the perceived needs for MES. The data were collected from an online survey of a population aged 20-39 years and from a telephone survey of a population aged 40 years or older, respectively. Chi-Square tests and hierarchical logistic regression analyses were done with SAS version 9.3. Results : Generally, as health status became lower, the use of MES increased. However, patients with two or more chronic diseases were less likely to use private MES compared to patients with one chronic disease. The perceived needs for MES were only related to the use of service and not to the choice of the MES type. Conclusions : There were different results for the significant determinants between the use of the MES and the choice of the MES type. The healthcare industry needs to aware of consumer needs to provide MES based on empirical findings.

Health Examination Data Based Medical Treatment Prediction by Using SVM (SVM을 이용한 건강검진정보 기반 진료과목 예측)

  • Nowadays, living standard is improved and people have high interest to the personal health care problem. Accordingly, people desire to know the personal physical condition and the related medical treatment. Thus, there is the necessary of the personalized medical treatment, and there are many studies about the automatic disease diagnosis and the related services. Those studies focus on the particular disease prediction which is based on the related particular data. However, there is no studies about the medical treatment prediction. In our study, national health data based medical treatment predictor is built by using SVM, and the performance is evaluated by comparing with other prediction methods. The experimental results show that the health data based medical treatment prediction resulted in the average accuracy of 80%, and the SVM performs better than other prediction algorithms.

  • The medical fitness of pilots is part of the civil aviation safety scenery. This study aimed to analyze the health problems occurring among Korean commercial pilots. Three data sources were used to identify the health problems: 7,574 aviation medical examination data, 5,400 sick leaves and 39 cases of a decrease in medical fitness during flight duty period and layover of Korean commercial pilots who have been working for K airline from Jan. 1, 2016 to Dec. 31, 2018. This study was retrospectively analyzed using SPSS 22 statistical program. Waiver of medical certificates was an average 11.9% of total issuance for 3 years, with a denial of 0.1%. The leading cause of denial of medical certificates was predominantly of cardiovascular cause (55.6%). Mild respiratory and digestive disease accounted for 82% of total sick prevalence and 68% of total sick days. The psychiatric and cardiovascular disease were ranked high according to the number of days lost per case. The most common cause of decrease in medical fitness during flight duty period was acute abdominal pain (36.4%) and musculoskeletal disorder (40%) when staying abroad. Aeromedical emphasis on minimizing cardiovascular risk remains appropriate. Major pilot health problems identified in this study should be considered in establishing pilot health promotion programs for aviation safety.

The Influences of Health Insurance on the Contents of Medical Services for Selected Hospitalized Patients (의료보험 실시가 입원환자의 진료내용에 미치는 영향 -한 병원의 정상분만산모와 충수절제술환자를 통한 사례연구-)

  • This study was written to discover the changes that may exist in the contents of medical services after introduction of health insurance system, and to identify the net-effect of health insurance system on medical services. Uncomplicated nornmal delivery and appendectomy patients were divided into 4 groups, the non-insured in pre-NHI periods(group A), the insured of health insurance for employees in pre-NHI periods(group B), the insured of regional health insurance for city residents in post-NHI periods(group C) and the insured of health insurance for employees in post-NHI periods(group D). The mehtod of matching was applied to control for major demographic differences among these 4 groups of each disease. In pre-NHI period, the medical services and the variation of medical services of the non-insured were compared with those of the insured. The difference between the change of medical services from group A to those of group C, and the change of medical services from group B to those group D is defined as the net-effect of health insurance. The results are as follows. First, in length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients, frequency of sitz bath in delivery patients, there was net-effect of health insurance in increasing direction. Second, length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients and frequency of sitz bath in delivery patients were significantly more in the insured than in the non-insured group in pre-NHI period. Third, the variation of medical services of post-NHI period was not less then those of pre-NHI period. Fourth, antenatal care on which the third party does not pay and the patient pays for all, was diffrerent by socioeconomic and educational level of patients.

A Study on the Knowledge, Attitude and Practice about Industrial Health Care Services of workers -Kwang-ju City and Chonnam Province- (근로자의 산업보건관리에 대한 지식, 태도, 실천 조사연구 -광주, 전남지역-)

  • The purpose of this study was to identify the degree and the relationship of the Knowledge, Attitude and Practice of industrial health care services of workers. This information will provide useful data for an effective industrial health care service. The was collected from 600 data employees in Kwang-ju city & Chonnam province. The data was gathered by questionnaire from Dec 20, 1992 to Jan 20, 1993. The questionnaire was developed by the Academic Affairs of Community Health Nursing Academy. Data was analyzed by using the statistical computer package, SPSS to manipulate the data along with percentages, means, standard deviations, modes, t-test and ANOVA. The results in this study were summarized as follows: 1. General characteristics of workers: Distributions of workers was laborer(76.2%). in the 25-34 age group(50.2%), married(63.3%), males comprised(77.5%), the educational level with the highest percentage was high school graduates (71.1%), with a monthly income of 300-700 thousand won(43.8%), and has been working in the present career over seven years (31.5%). 2. The Knowledge, Attitude and Practice levels of employees about industrial health care services The levels were measured according to a five point scale. The total mean score of knowledge was 2.92 points out of 4. The following are the scores of Knowledge of special medical examination and location of industrial clinic(3.48), periodic medical examination (3.18), occupational disease(3.08), personal protective equipment (2.92), and health education(1.37). The total mean score of Attitude was 2.77 point out of 4. The following are the scores of Attitude in order of working environment (3.35), necessity of periodic medical examination and health education(3.15), the worker's perception influence on the working environment to health was high(3.11). But, the level of satisfaction in the content of periodic medical examination was low(2.19). The total mean score of Practice was 2.70 points out of 4. The scores of Practice in order were, practice of periodic medical examination(3.70), utilization of industrial clinic(2.92), and to participate in health education(1.47). 3. The relationship of general characteristics to Knowledge, Attitude, and Practice of workers: Knowledge had a significant difference by sex (P<.01), marital status(P<.01), education level (P<.05) and monthly income(P<.01). Attitude was significantly different with sex(P<.05) and Practice was significantly related to monthly income (P<.01).

Factors Affecting the Satisfaction and Revisit Intension of Health Promotion Center In A University Hospital (한 대학병원 부속 건강검진센터 이용자의 만족도와 재이용 의사에 미치는 요인)

  • The purpose of this study was to serve as a basis for providing quality medical service and mapping out consumer-centered marketing strategies to successfully cope with the rapidly changing medical environment and meet consumer needs, by examining what affected the satisfaction and revisit of health Promotion center Client. The subjects in this study were 186 of visitor to health Promotion center in a university hospital in the city of Busan. A survey was conducted with structured questionnaire from March 2 to 30, 2001. The collected data were analyzed with SPSS for Windows (ver10.0). For more statistical analysis, frequency analysis, component analysis, t-test, ANOVA and correlation analysis procedures were utilized. Results of the study can be summarized as follows: 1. Regarding demographic characteristics, 51.6% of the Client investigated were male, and 48.4% were female. The greatest number of them were in their 40s(38.9%). 86.5% were married, and 34.2% were self-employed. 44% were a high-school graduate, and the monthly mean income of 59.4% was one to three million Won. And, the residential area of 46.8%, the largest percentage, was a half-an-hour distance from health Promotion center. 2. The most common motivation of their selection of the health Promotion center was a recommendation by Staff and neighborhood(59.7%), followed by excellent facilities and services(17.2%), the tradition and reputation of the hospital(7.5%), and its publicity pamphlets(7.5%). 3. 45.9%, the largest percentage, acquired health-related information from their friends or acquaintances. 43.8%, the greatest percentage, visited there because they felt there's something wrong with their body. 53.4% worried about the possibility of being attacked by cancer, and 57.5% wanted to take a precise cancer examination. For health maintenance, 50.1% got regular exercise. Regular exercise was considered most crucial for health maintenance or promotion. 4. The largest reason they used that examination center again was the kindness of employees(52.7%), followed by the tradition and reputation of the hospital(21%) and excellent examination setting and equipment(10.8%). 5. By demographic factor, there was no significant difference between the man and women in satisfaction level with examination and expenses. The women expressed more satisfaction at facilities, and the high-school graduate group were more contented with expenses. 6. As a result of investigating their satisfaction level according to the motivation of selecting that examination center, the group that chose the center due to excellent facilities and services were more satisfied with examination and expenses. 7. As for the relationship of total examination satisfaction to revisit. intention, there was a higher correlational relationship between total health examination satisfaction and expense satisfaction. Especially, the more they were satisfied with examination, the more they were willing to revisit there for another examination.

A study on some workers' effective oral health behavior and workplace oral examination (일부 근로자의 유효구강건강행태 및 직장구강검진에 관한 연구)

  • Objectives : The direction for qualitative improvement of our country's workers' by arranging data necessary for improving oral health program and system aiming to promote workers' oral health in the future is suggested in this study. Methods : The questionnaire by self-administrated method, which was selected by convenience sampling method, was carried out from October 4th to December 31th, 2011 targeting 424 workers from 6 working places in Jellabuk-do Province. Results : 1. It was indicated that workers' interest in oral health is higher than moderate and that workers are perceiving oral health as one of critical problems and thinking that the level of their oral health knowledge is low and oral health status is under moderate. 2. It was indicated that the rate of workplace oral screening is high, but the dissatisfaction ratio with workplace oral examination is high. Non-screening of medical treatment was indicated to be the highest after oral examination. 3. Factors of having influence upon the workplace oral examination included age, marital status, appearance, educational level, work-life term, and average monthly household income. Conclusions : It is judged to be likely important to make them have positive attitude toward oral health care through the accurate publicity of workplace oral examination and education on prevention of oral disease by reflecting workers' high interest in oral health.

Is Health Locus of Control a Modifying Factor in the Health Belief Model for Prediction of Breast Self-Examination?

  • Background: Breast cancer is one of the most common cancers among women in the world. Early detection is necessary to improve outcomes and decrease related costs. The aim of this study was to assess the predictive power of health locus of control as a modifying factor in the Health Belief Model (HBM) for prediction of breast self-examination. Materials and Methods: In this cross- sectional study, 400 women selected through the convenience sampling from health centers. Data were collected using part of the Champion's HBM scale (CHBMS), the Health Locus of Control Scale and a self administered questionnaire. For data analysis by SPSS the independent T test, Chi square test, logistic and linear regression modes were appliedl. Results: The results showed that 10.9% of the participants reported performing BSE regularly. Health locus of control did not act as a predictor of BSE as a modifying factor. In this study, perceived self-efficacy was the strongest predictor of BSE performance (Exp (B) =1.863) with direct effect, while awareness had direct and indirect influence. Conclusions: For increasing BSE, improvement of self-efficacy especially in young women and increasing knowledge about cancer is necessary.
