Influencing Factors for and Medical Expenditures of Metabolic Syndrome among Public Officials (공무원의 대사증후군 관련 요인 및 대사증후군에 따른 의료비 지출)

  • Purpose: This study examined the influencing factors for metabolic syndrome and the annual medical expenditures of metabolic syndrome among public officials. Methods: The National Health Insurance data in 2009 were collected for 364,932 public officials and the heath examination results and annual medical expenditures were analyzed using PASW 18.0 program. Results: The prevalence of metabolic syndrome is 17.6%, and it was higher in male officials than that of females in all age groups. In men, the influencing factors for metabolic syndrome were: age, family history of stroke, cardiovascular disease, hypertension, and diabetes mellitus, smoking, alcohol consumption, exercise, and obesity. However, in women, health-related behaviors such as smoking, alcohol consumption and exercise did not affect metabolic syndrome. People who had metabolic syndrome showed significantly higher medical expenditures than those without metabolic syndrome. The odds ratios of having the highest quartile in medical expenditures were 1.372 (95% CI 1.252~1.504, p<.001) in women with metabolic syndrome and 1.213 (95% CI: 1.184~1.243, p<.001) in men. Conclusion: The results implied that health-related behaviors were associated with metabolic syndrome, and resulted in higher medical expenditures. In order not only to decrease the risk of metabolic syndrome but also reduce medical expenditures, nurses should plan health promotion strategies to educate public officials about healthy life strategies.

  • With the continuous acceleration of economic and social development, people gradually pay attention to their health, improve their living environment, diet, strengthen exercise, and even conduct regular health examination, to ensure that they always understand the health status. Even so, people still face many health problems, and the number of chronic diseases is increasing. Recently, COVID-19 has also reminded people that public health problems are also facing severe challenges. With the development of artificial intelligence equipment and technology, medical diagnosis expert systems based on big data have become a topic of concern to many researchers. At present, there are many algorithms that can help computers initially diagnose diseases for patients, but they want to improve the accuracy of diagnosis. And taking into account the pathology that varies from person to person, the health diagnosis expert system urgently needs a new algorithm to improve accuracy. Through the understanding of classic algorithms, this paper has optimized it, and finally proved through experiments that the combined classification algorithm improved by latent factors can meet the needs of medical intelligent diagnosis.

  • The National Competency Standard (NCS) is being implemented on the basis of a college of higher education. The college offers intensive education by organizing three areas of knowledge, skills and attitudes so that students can perform their full capacity at the same time as graduating in industrial field. Health Department at the College has been training for a variety of medical personnel, including medical technician regions. Especially, the medical technician and medical sector must be licensed to work in the medical field. Therefore, passing the national examination license is a prerequisite for employment. In this study, the health law scores of the third grade students (about 200 students) of the radiology department of D College were analyzed at weekly intervals. The weekly score acquisition can be predict the pattern of student class achievement of individual students. Furthermore, these results can predict the possibility of passing the radiation license examination for individual students.

Study on Quality Improvement Activities in Korean Hospitals (국내 의료기관의 질 향상 사업주제)

  • Background : the hospitals in Korea are in a situation of a severe competition than the past. This situation was resulted from the increase in the number of hospitals and also from the government policy controlling the medical insurance fee. Moreover, consumer's desire for the high quality medical service g\has been significantly increased. Many programs to improve the quality of medical services are being performed in hospitals since the middle of 1990's. Studies up to now reported that more than 10 programs are being performed per hospital in Korea. So far studies have been performed to measure only the number of such programs in a hospital. The purposes of this study are to examine a specific area involved in the programs designed to improve the medical service quality and to suggest a future direction of the such programs. In addition, we hope that the results from this study could assist the programs for the medical service quality. Methods : A mailed questionnaire survey of the QI staffs at hospitals with 400 beds or more was conducted between September 15 and October 30, 2000. Of the 108 hospitals eligible for inclusion in our study, 69 participated, yielding a response rate of 63.9%. Excluding 7 hospital which are not responsed about activities of hospital then 62 hospitals were used for the analysis. Result : The total number of programs was 1,081 from the 62 hospitals participated in the survey. The highest number (24.8 programs) was found in the hospital having more than 800 beds and performing the programs more than 5 years. The 1,081 programs were consisted of 445 from the medical examination area, 343 from the medical examination support area, and 296 from the management area. Conclusion : This study showed the present situation of hospitals in Korea regarding to the quality improvement programs. The results from this study suggest that the pattern of the program for the medical service improvement is being changed to service process and result-centered programs from the structural area.

  • The National Health Insurance Corporation has been retrieving from health care providers the payments made to them by insured patients as a result of the health care providers' arbitrary denial of coverage under the National Health Insurance, and has been disbursing such retrieved monies back to the patients, pursuant to Article 57, Sections 1 and 4 of the National Health Insurance Act. However, such practice is an application of the law that lacks legal exactitude. Another problem with such practice is that there is no legal provision under any laws or notices that expressly prohibits arbitrary denial of coverage. A legislative solution, therefore, is called for to address these issues.

A Study on the Types and Characteristics of Ancient Medical Facilities (고대 의료시설의 유형 및 특성에 관한 연구)

  • This study is to understand the architectural types and characteristics of the medical facilities during ancient period. The study is based on the research of the medical and architectural history. The medical or healthcare facilities are influenced by their social, cultural and conceptual idea, especially how they think about 'disease', 'cure' and the 'medicine'. As the results of the examination of this study can be summarized as followings. Firstly, Ancient medical facilities are classified into four types according to the ideas of 'disease' and 'cure' ; 1) God oriented facility 2) health welfare facility 3) treatment oriented facility 4) practice & educational facility. Secondly, there are three typical types of the spatial characteristics what modern hospitals have ; 1) nursing ward 2) treatment ward 3) hostel or hospice. And they are all assembled around the courtyard in common. Thirdly, their architectures are not 'designed' but 'derived' plan by reasonable other building types, and transformed by medical function. Fourthly, a consideration of the surrounding circumstances is the most important point to make the medical health facilities during ancient period.

A Study on the Dietary Habits of People in Gyeonggi Area by Age Groups - Based on the Nutrition Counseling Data of Medical Health Examination - (경기 지역 종합 검진 영양 상담자의 연령별 식습관 비교)

  • The purpose of this study was to analyze the dietary habits by age groups, based on the nutrition counseling data of medical health examination. The subjects were 5,811 adults(3,258 males and 2,553 females) who took comprehensive medical test in Gyeonggi area, and they were evaluated from January 1 to December 31 in 2005. Survey samples were divided by age groups 30>, $30{\sim}39,\;40{\sim}49,\;50{\sim}59,\;60{\sim}69,\;70{\leq}years$. The subjects were composed of 56.1% male and 43.9% female. The results showed that the dietary habits of $50{\sim}59$ group were relatively good. On the other hand, in 30>, $30{\sim}39\;and\;40{\sim}49$ age groups, since they had irregular meals and high ratio of skipping breakfast. and they had meals out frequently, nutritional unbalance in these age groups was expected for the future. Therefore the correct nutritional facts should be taught to them so that they have a healthy dietary habit.

Relationship Between Life Style and Serum Lipid Levels in Adults using Data from Health Examination (건강검진자료에 의한 일반 성인의 생활습관과 혈청지질치와의 관련성)

  • The purpose of this study was to obtain the serum lipid levels according to the lifestyles, and examine the influence of lifestyles on the serum lipid levels among adults who examined the health checkup in an university hospital. The subjects for this study were 4,112 adults who underwent medical examinations at the health center of a university hospital in Daejeon city from Jan 2012 to Dec 2013. The lifestyles and serum lipid levels of study subjects were obtained from self-recorded questionnaires and medical examination charts of the hospital. As a result, the mean values of the serum lipid levels (TC, HDL-C. LDL-C and TG) and atherogenic index (AI) of the study subjects showed a significantly difference according to the lifestyle, such as age, alcohol consumption, smoking, regular exercise, overeating and meat consumption in both sexes. The TC, HDL-C. LDL-C, TG and AI showed a positive correlation with age, AUDIT score, but the HPI score showed a negative correlation in both sexes. In the age-adjusted odds ratio, the risk ratio of an abnormality of TC, HDL-C. LDL-C, TG and AI increased significantly because there was an increase in the group of everyday overeating and meat consumption, smoking group, no exercise group, and low HPI group than their respective counterparts in both sexes. The above results suggested that the serum lipid levels of the subjects was closely related to increasing age, and lifestyles, such as alcohol consumption, smoking, regular exercise, overeating, and meat consumption.

Health Beliefs and Breast Cancer Screening Behaviors among Iranian Female Health Workers

  • Background: Breast cancer is a serious health problem. Early detection is crucial for optimal treatment and reducing mortality. Objective: The aim of this study was to evaluate health beliefs concerning performance of breast self- examination (BSE) and mammography in a sample of Iranian female health workers. Materials and Methods: This cross-sectional study was performed among 441 female health care workers (physicians=88, nurses=163, midwives=38, officers=68, and others=84) in 3 different health centers in Yazd, Iran. Data were collected using a self administered questionnaire which included demographic characteristics and thenPersian version of the Champion's Health Belief Model Scale (CHBMS). Results: The mean age of the participants was $34.7{\pm}13.7$. It was found that 41.9% of the workers performed BSE in the past and 14.9% of them regularly, but only 10.6% of them had undergone a mammogram. Perceived barriers to BSE (F=6.351, P=0.021) and mammography (F=5.214, p=0.022) were significantly higher in officers than physicians, nurses or midwives. Perceived barriers were lower among those who had performed BSE and mammography, but not significant (p=0.34 and p=0.56, respectively). Furthermore, perceived susceptibility and perceived benefits of the workers who had BSE and mammography were significantly higher than who did not (p<0.05). Perceived seriousness was not a significant variable in BSE and mammography (p=0.71 and p=0.43, respectively). Conclusions: The health beliefs of health workers concerning the perceived susceptibility of breast cancer and the perceived benefits BSE and mammography significantly impact their screening practices.

Factors Affecting Colorectal Cancer Screening Behaviors : Based on the 4th Korea National Health and Nutrition Examination Survey (대장암 조기 검진 행위에 영향을 미치는 요인 -제4차 2기(2008년) 국민건강영양조사 자료를 중심으로-)

  • Objectives: This study aims to identify the factors associated with colorectal cancer screening behaviors. Methods: The nation-wide representative samples of 2,928 adults aged ${\geq}50$ years for colorectal cancer screening were derived from the fourth Korea National Health and Nutrition Examination Survey (KNHANES IV). This study investigated socio-demographic, health behavioral and contextual factors associated with colorectal cancer screening using descriptive statistics and multivariate logistic regression analysis. Results: In terms of socio-demographic factors, gender, age, marital status, occupation, monthly income, and resident region were significantly different between screening group and non-screening group. Among health behavioral and contextual factors, regular physical checkup, weight control, physical activity, smoking, drinking and having other cancers were significantly different. From the multivariate logistic regression analysis, marital status, education level, regular physical checkup and weight control were associated with colorectal cancer screening behavior. Conclusions: It is necessary to understand the importance of early detection and cancer screening. Appropriate health education and active promotion about the cancer screening should be developed based on the study findings in order to motivate people to have cancer screening. Also, these findings should be reflected in the health policy.
